Electromyographic Analysis of Gluteus Maximus, Gluteus Medius, Hamstring and Erector Spinae Muscles Activity During the Bridge Exercise With Hip External Rotation in Different Knee Flexion Angles in Healthy Subjects

  • Background: The bridge exercise targets the gluteus maximus (Gmax) and gluteus medius (Gmed). However, there is also a risk of dominant hamstring (HAM) and erector spinae (ES) muscles. Objects: To analyze the muscle activity the of Gmax, Gmed, HAM and ES during the bridge exercise with and without hip external rotation in different degrees of knee flexion. Methods: Twenty-three subjects were participated. The electormyography (EMG) activity of the Gmax, Gmed, HAM and ES muscles was recorded during the exercise. The subjects performed the bridge exercise under four different conditions: (a) with $90^{\circ}$ knee flexion, without hip external rotation (b) with $90^{\circ}$ knee flexion, with hip external rotation (c) with $135^{\circ}$ knee flexion, without hip external rotation (d) with $135^{\circ}$ knee flexion, with hip external rotation. Results: There was no significant interaction effect between the degree of knee flexion and hip external rotation. There was a significant main effect for degree of knee flexion in Gmax, HAM muscles activity. Gmax muscle activity was significantly greater in the $135^{\circ}$ knee flexion position than in the $90^{\circ}$ knee flexion position (p<.001). While HAM muscle activity was significantly less in $135^{\circ}$ knee flexion position than in the $90^{\circ}$ knee flexion position (p<.001). ES muscle activity was significantly less in the $135^{\circ}$ knee flexion position than in the $90^{\circ}$ knee flexion position (p=.002). The activity of both the Gmax and Gmed muscles was significantly greater with hip external rotation (p<.001 and p=.005, respectively). Conclusion: For patients performing the bridge exercise, positioning the knee in $135^{\circ}$ of flexion with hip external rotation is effective for improving Gmax and Gmed muscle activity while decreasing HAM, and ES muscle activity.

Comparison of Craniovertebral Angle and Muscle Properties after Smartphone Use in Healthy Individuals with and without Forward Head Posture

  • Purpose : Forward head posture (FHP) is one of the most common postural malalignment of the cranio-cervical region. Previous studies have reported that FHP might affect both temporomandibular joint (TMJ) and cervical muscles, but still remains unclear. The purpose of this study was to compare the changes of craniovertebral angle (CVA) and muscle properties after smartphone use in healthy individuals with and without FHP. Methods : Fifteen healthy individuals aged 18 to 22 years were included. CVA was evaluated using Dartfish motion analysis, and the subjects were divided into two groups according to their CVA: a FHP group (n = 7, CVA less than 48 °) and a control group (n = 8, CVA more than 48 °). MyotonPro was used to measure muscle properties of masseter, digastric and sternocleidomastoid muscles (SCM). Each subject underwent 15-minutes of smartphone task (web browsing or video watching) in relaxed sitting posture. CVA and muscles properties were assessed both before and after the smartphone task. Results : There were significant changes in post measurements of CVA between the groups. Masseter muscle showed significant differences in pre and post measurements of all muscle properties, and digastric muscle showed significance only in muscle tone. Amount of changes (post-pre), however, showed no significant difference in this study. Conclusion : 15-minutes of smartphone task did not affect CVA and muscle properties of masseter, digastric and SCM in both groups, however, there were significant changes in pre and post measurements of CVA and some muscle properties of masseter and digastric muscles. Therefore, CVA, masseter and digastric muscles might be significantly changed in a heavy duration of smartphone usage more than 15-minutes. Further studies are needed regarding duration of smartphone task, assessments in other various TMJ muscle groups, and participants with pathological FHP conditions.

Anatomic Description of the Infraorbital Soft Tissues by Three-dimensional Scanning System

  • Purpose: For minimally invasive procedures, three-dimensional (3D) anatomical knowledge of the structures of the face is essential. This study aimed to describe the thickness of the skin and subcutaneous tissue and depths of the facial muscles located in the infraorbital region using a 3D scanner to provide critical clinical anatomical guidelines for improving minimally invasive cosmetic procedures. Materials and Methods: The 3D scanning images of 38 Korean cadavers (22 males and 16 females; age range: 51~94 years at the time of death) were analyzed. Eight facial landmarks (P1~P8) were marked on the cadaveric faces. The images were scanned in three steps-undissected face, hemiface after skinning, and revealing the facial muscles. Student's t-test was used to identify significant differences. Result: The skin and subcutaneous tissue tended to become thicker from the upper to lower and medial to lateral aspects, and the muscles followed the same pattern as that of the most superficial located muscle and the deepest located muscles. No significant sex-related differences were found in the skin at any landmark. However, the muscles tended to be deeper in the female participants. Conclusion: The study data can serve as a basis for creating or enhancing clinical anatomy-based guidelines or improving procedures in the infraorbital region.

Immediate Effects of Roller Massage for Posterior Neck Muscles on the Muscle Strength and Range of Motion for Cranio-Cervical Flexion in Subjects With Forward Head Posture

  • Background: Forward head posture (FHP) causes various posture imbalances associated with the head and neck. Myofascial release is an effective treatment method used for relaxing muscles and reducing muscle hyperactivity, but no studies have been conducted on suboccipital and neck muscles related to FHP. Objects: The purpose of this study was to investigate the immediate effect of roller massages on the cranio-cervical flexion (CCF) range of motion (ROM) and CCF strength applied to suboccipital and neck muscles in subject with forward head posture. Methods: Twenty-four FHP subjects (male: 13, female: 11) were recruited for this study. All subjects were recruited with a craniovertebral angle (CVA) of 53 degrees or less and a head tilt angle (HTA) of 20.66 degrees or higher. CCF strength was measured using Pressure biofeedback unit (PBU) in the supine posture and CCF ROM was measured using smartphone-based inclinometer. Roller massage (RM) was applied to suboccipital and neck muscles for 2 minutes and CCF ROM and strength were remeasured. Results: These results of this study showed that CCF ROM was a significant difference in CCF ROM before and after RM (p < 0.05). CCF strength also showed a significant difference before and after RM (p < 0.05). Conclusion: RM method might be recommended to increase the immediate ROM and strength of CCF in subjects with forward head posture.

오버헤드 운동 중 탄성 바와 비탄성 바의 팔과 몸통 근육에 대한 근전도 비교 분석 (A Comparison Analysis of EMG on Arm and Trunk Muscle Between Elastic and Inelastic Bar During The Overhead Press Exercise )

  • Objective: This study was executed to compare and analyze shoulder muscle activation while using an inelastic bar and elastic bar during overhead press exercise. The stability and coordination of shoulder joints will be investigated by measuring and analyzing the EMG of the upper and lower arm muscles. Method: A total of 20 university male students were recruited by dividing into 2 groups; 10 elastic bar participants (age: 20.17 ± 0.41 yrs, height: 174.31 ± 3.34 cm, weight: 74.68 ± 5.65 kg) and 10 inelastic bar participants (age: 20.09 ± 0.23 yrs, height: 173.53 ± 4.11 cm, weight: 75.32 ± 3.31 kg) participated in this study. Results: The EMG analysis results of the four muscles measured in this study showed that there was no difference between the left and right muscles between the groups in Upper Trapezius muscle. In Deltoid, Infraspinatus, and Rectus Abdominis muscles, the elastic bar group was significantly higher than the inelastic bar group between groups, and there was no difference between left and right. Conclusion: Among the four muscles measured in this study, there was no difference between left and right in Deltoid, Infraspinatus, and Rectus Abdominis, but the elastic bar showed significantly higher muscle activity than the inelastic bar. Therefore, it was found that the elastic bar increases muscle activation during exercise than the inelastic bar, and in particular, it further increases muscle activation of the arms and torso, and exercise using the elastic bar can increase neuromuscular stabilization.

Restoration of the Broken Lumbopelvic-hip Neuromuscular Chain and Coordinated Synergistic Activation in Low Back Pain

  • Background: The presence of the lumbopelvic-hip neuromuscular chain is essential for dynamic spinal stabilization; its therapeutic effects on dynamic movements of the distal extremity segment and underpinning motor mechanism remain unknown and warrant further study on participants with low back pain (LBP). Objects: We aim to compare the effects of the broken chain exercise (BCE) and connected chain exercise (CCE) on electromyography (EMG) amplitude and onset time in participants with and without LBP. Methods: Randomized controlled clinical trial. A convenience sample of 40 nonathletic participants (mean age: 24.78 ± 1.70) with and without LBP participated in this study. All participants underwent CCE for 30 minutes, 30-minute daily. We measured EMG amplitude and onset times on bilateral erector spinae (ES), gluteus maximus (GM), hamstring (HAM), transverse abdominis (TrA), internal oblique (IO), and external oblique (EO) during the prone hip extension (PHE) test before and after the BCE and CCE. We used multivariate analysis of variance (MANOVA) to analyze the amplitude and onset time difference between exercises (BCE and CCE) and Pearson's correlations to determine any synergistic relationship among the HAM, GM, bilateral TrA/IO, and ES muscles. The statistical analyses were used at p < 0.05. Results: MANOVA showed that CCE was more decreased on EMG amplitude in HAM and bilateral ES, while increased GM and contralateral TrA/IO than BCE (p < 0.05). MANOVA EMG onset time data analyses revealed that the main effect of the conditions was significant for all HAM, GM, and bilateral ES muscles, whereas the main effect for the group was significant only for GM and contralateral ES in healthy and LBP groups. Pearson's correlation coefficient was computed to assess the relationship between BCE and CCE on dependent variables. In most of the muscles, there was a strong, positive correlation between the two variables, and there was a significant relationship (p < 0.001). Conclusion: CCE produced more effective and coordinated core stabilization and motor control mechanism in the lumbopelvic-hip muscles in participants with and without LBP during PHE than BCE.

Comparison of Muscle Thickness of Abdominal Muscles According to Various Types of Abdominal Crunch Exercise

  • Purpose : The purpose of this study was to compare the effect of additional isometric contraction of trunk, shoulder, and hip muscles during abdominal crunch exercise on abdominal muscle thickness and to identify the most effective intervention for core muscle activation. Methods : This study was conducted on 22 healthy male adults. Subjects performed three types of crunch exercises (abdominal crunches accompanied by internal and external isometric rotation of the hip, horizontal shoulder adduction and abduction, and rotation of the trunk). The thickness of the transverse abdominis (TrA), internal oblique (IO), and external oblique (EO) were evaluated using ultrasonography. The collected data used one-way repeated ANOVA statistics. Wilcoxon signed-rank test of nonparametric statistics was used for post-test analysis. Results : The IO thickness was significantly lower than general abdominal crunch when shoulder adduction was added (p<.05). The crunch with shoulder abduction, hip external rotation, and ipsilateral trunk rotation was significantly higher than the abdominal crunch (p<.05). The EO thickness was significantly greater in the crunch with hip external rotation than in the abdominal crunch (p<.05). Conclusion : The level of contraction in abdominal muscles appears to vary when isometric contractions of the trunk, shoulder, and hip muscles are added to the abdominal crunch exercise. Therefore, the use of isometric contractions of other joints to selectively induce contraction of the abdominal muscles may be considered.

Muscle Radiation Attenuation in the Erector Spinae and Multifidus Muscles as a Determinant of Survival in Patients with Gastric Cancer

  • Purpose: To determine the prognostic role of muscle area and muscle radiation attenuation in the erector spinae (ES) and multifidus (MF) muscles in patients undergoing gastrectomy. Methods: Patients with stage I-III gastric cancer undergoing gastrectomy were retrospectively enrolled in this study. Clinicopathologic characteristics were collected and analyzed. Both paraspinal muscle index of ES/MF muscles (PMIEM) and paraspinal muscle radiation attenuation in the same muscles (PMRAEM) were analyzed at the 3rd lumbar level using axial computed tomographic images. Cox regression analysis was applied to estimate overall survival (OS) and disease-free survival (DFS). Results: There was only a weak correlation between PMIEM and PMRAEM (r= 0.28). Multivariate Cox regression revealed that PMRAEM, but not PMIEM, was an important determinant of survival. PMRAEM along with age, tumor-node-metastasis (TNM) stage, perineural invasion, and serum albumin level were significant determinants of both OS and DFS that constituted Model 1. Harrell's concordance index and integrated area under receiver operating characteristic curve were greater for Model 1 than for Model 2 (consisting of the same covariates as Model 1 except PMRAEM) or Model 3 (consisting of only TNM stage). Conclusion: PMRAEM, but not PMIEM, was an important determinant of survival. Because there was only a weak correlation between PMIEM and PMRAEM in this study, it was presumed that they were mutually exclusive. Model 1 consisting of age, TNM stage, perineural invasion, serum albumin level, and PMRAEM was greater than nested models (i.e., Model 2 or Model 3) in predicting survival outcomes.

앉는 면의 각도가 정상성인의 심부 복근과 다리근육 두께에 미치는 영향 (The Effect of Seating Surface Angle on the Deep Abdominal Muscle and Lower Limb Muscle Thickness in Normal Adults)

  • Purpose : Although many studies have explored the effect of seating side angles on the spinal curve and surrounding muscles during seating, only a few studies have investigated the effect of different seating face angles on different lower limb and deep trunk muscles. Therefore, this study investigated the effects of seating surface angles (0 degrees, 10 degree anterior, and 10 degree rear) on lower extremity and deep trunk muscles in healthy adults. Methods : Thirty people were asked to sit once on each seating surface three times during the day, and their muscle thicknesses were measured by ultrasound while sitting. The method of sitting was the same when sitting on the three seating surfaces. Results : From the comparison results of the muscle thicknesses according to the seating surface angles, a significant difference existed in the muscle thicknesses of the vastus medialis, vastus medialis oblique, vastus intermedius, soleus and gastrocnemius (p<.05). However, no significant difference was found in the transversus abdominis, internal obliques, rectus femoris and vastus lateralis (p>.05). Conclusion : Our findings revealed that the lower back load decreases, the leg load increases, and the legs specific muscles are affected as the body tilts forward when sitting on the seating surface inclined forward. Therefore, it is possible to suggest a forwardly inclined seating surface that reduces lower back loads and utilizes the posture-maintaining muscles of the legs when sitting in a person with a poor sitting posture or lower back pain at ordinary times.

안면비대칭 환자의 전악임플란트 수복 (Full Mouth Implant Rehabilitation in Facial Asymmetric Patient)

  • 전악 임플란트 수복시 기능적인 부분 뿐 아니라, 심미적인 측면에서 고려해야 할 부분들이 많다. 무치악 혹은 부분 무치악 상태에서 전악 수복이 이루어지게 되면, 하안모의 큰 변화를 불러 일으킨다. 하안모의 변화뿐 아니라 안모 전체의 변화로 인하여 인상이 많이 달라 지게 된다. 이러한 안모의 변화들은 교합 고경의 회복과 치아들의 3차원적 위치, 전후방과 좌, 우 위치에 의해 안모들 싸고 있는 근육들의 변화로 인하여 일어 나는 것이다. 악골의 비대칭과 함께 안모 비대칭이 있는 환자에 있어서, 전악 임플란트 수복을 통하여, 안모 비대칭을 개선한 치료 증례를 살표 보도록 하겠다. 하악 과두의 흡수로 인하여 짧아지고 과 긴장된 근육들을 임시치아 과정에서 교합을 높여 가면서, 근육을 훈련시켜서, 좌, 우균형을 맞추어 준 증례가 되겠다. 골격적 부조화를 좌,우 근육의 균형을 통화여 보상하여, 하안모 뿐 아니라, 안모전체, 두경부 쪽의 균형을 이루는 것을 살펴 보도록 하겠다.
