볼링 선수 상해 경험과 유형에 따른 근력과 비대칭 비교 분석 (Comparison Analysis of Muscle Strength and Asymmetry according to Bowler Injury Experience and Type)

  • 본 연구는 볼링 선수의 상해 경험과 유형에 따라 상 하지 근력과 좌우 비대칭 차이를 살펴보고자 하였다. 본 연구 대상자는 상지 상해 경험 집단(upper body injury group, [UG], n=16)과 하지 상해 경험 집단(low body injury group, [LG], n=8), 상해 미경험 집단(non injury group, [NG], n=15)으로 선정하였다. 볼링 선수의 상 하지 근력은 Manual Muscle Tester 01165 (Lafayette Instrument Company, USA)로 등척성 최대 근력(Isometric strength)을 측정하였으며, 비대칭 지수(symmetry index, [SI])를 산출하였다. 그 결과는 다음과 같다. 하지 근력의 좌우 비대칭 지수는 엉덩관절의 신전과 외측회전 최대근력에서 상해 무경험 집단이 하지 상해 경험 집단에 비하여 통계적으로 크게 나타났다(p<.05). 위 결과를 통하여 하지 근력의 좌우 대칭이 하지 상해와 밀접한 관련이 있다고 판단되며, 상해 예방을 위해 하지 근력의 좌우 대칭 훈련이 필요할 것으로 판단된다.

운동심상이 만성 경수 손상 환자의 근활성도와 일상생활에 미치는 영향 (Effect of a Motor Imagery Program on Upper Extremity Strength and Activities of Daily Living of Chronic Cervical Spinal Cord Injury Patients)

  • Purpose: The purpose of this study is to determine the effect of motor imagery training on residual upper extremity strength and activities of daily living of chronic cervical spinal cord injury patients. Methods: Twelve ASIA A B patients, who had more than a 12-month duration of illness and C5 or 6 motor nerve injury level, were randomly divided into experimental group (n=6) and control group (n=6). Patients in the experimental group performed motor imagery training for five minutes prior to general muscle strengthening training, while those in the control group performed general muscle strengthening training only. The training was performed five times per week, 30 minutes per day, for a period of four weeks. General muscle strengthening training consisted of a progressive resistive exercise for residual upper extremity. Motor imagery training consisted of imagining this task performance. Before and after the training, EMG activity using BTS Pocket Electromyography and Spinal Cord Independent Measure III(SCIM III) were compared and analyzed. Results: The residual upper extremity muscle strengths showed improvement in both groups after training. Comparison of muscle strength improvement between the two groups showed a statistically significant improvement in the experimental group compared to the control group (p<0.05). SCIM III measurements showed significant improvement in the scores for Self-care and Transfer items in the experimental group. Conclusion: Motor imagery training was more effective than general muscle strengthening training in improving the residual upper extremity muscle strength and activities of daily living of patients with chronic cervical spinal cord injury.

달리기 동호인들의 상해예방을 위한 맞춤형 발 보장구의 평가연구 (The Evaluation of Custom Foot Orthotics for Injury Prevention of Joggers)

  • The purpose of this study was to examine the effect of foot orthotics on the overall comfort and muscle activity during running. The subjects were 10 members from the joggers' club which consisted of 2 women and 8 men. These individuals ran on the treadmill by 4.0m/s speed with and without the custom foot orthotics. The data concerning the overall comfort was collected by a questionairre that examined the overall comfort, heel cushioning, forefoot cushioning, medio-lateral control, arch height, heel cup fit, shoe heel width, forefoot width, and shoe length The MegaWin ver. 2.1(Mega Electronics lid, Ma. Finland) was used to gain electromyography signals of the muscle activity; Tibialis anterior, medial gastronemius, lateral gastronemius, vastus lateralis, vastus medialis, biceps femoris, and rectus femoris were measured. The results of the study were as follows. 1. During running the overall comfort was higher for the foot arthotic condition than the nonorthotic condition. Among the inquiries the overall comfort showed the biggest difference comparing the two conditions. and the shoe heel width showed the highest score for contort. 2 The muscle activity of the biceps femoris, and vastus lateralis in the stance period decreased. due to the foot orthotics. The muscle activity of the vastus medialis in the swing period also decreased and the muscle activity tibialis anterior in the stance and swing stance decreased as well During running, orthotics showed positive result in foot comfort. The foot comfort related to decreased stress, muscle activity, and foot arch strain. Overall comfort and the adequate decrease of muscle activity were associated with injury prevention and the best method to prevent injury semms to be the maintenance of foot comfort.

Identification of Flexion Withdrawal Reflex Using Linear Model in Spinal Cord Injury

  • The aim of this study was to identify the characteristics of the flexion withdrawal reflex modulated by the hip angle and hip movement in spinal cord injury (SCI). The influence of the hip position and passive movement were tested in 6 subjects with chronic SCI. Each subject placed in a supine position and lower leg was fixed with the knee at 5 -45 degree flexion and the ankle at 25-40 degree plantar flexion. A train of 10 stimulus pulses were applied at 200 Hz to the skin of the medial arch to trigger flexion reflexes. From results of the regression analysis, static properties of normalized muscle activation of flexor muscles have the linear relationship with respect to hip angle (P< 0.05). In order to verify the neural contribution of flexion reflex, we compared the static and dynamic gains of estimated muscle activations with measured EMG of ankle flexor muscle. Form this study, we postulate that the torque and muscle response of flexion withdrawal reflex have linear relationship with hip angle and angular velocity.

지연발생 근육통의 냉치료 효과에 대한 연구 (The effects of cryotherapy on delayed onset muscle soreness)

  • Delayed onset muscle soreness is a sensation of discomfort that occurs 24 h after exercise, and it is associated with the performance of unfamiliar and high force muscle work, such as eccentric contractions. The injury to the muscle has been well described but the mechanism underlying the injury is not fully understood. Although the pathophysiological processes underlying delayed onset muscle soreness are not completely understood, many researchers have investigated various treatments in a attempt to reduce the soreness. Physical therapy is the most importance techniques to reduce delayed onset muscle soreness. The purpose of this study is to investigate the effect of a cryotherapt on DOMS. Thirty subjects were randomly assigned to experimental group : control, cryotherapy, and placebo group. Elbow flexion range, mechanical pain threshold. and subjective pain were measured 30 min before DOMS was induced and 24, 48, 72 hours after DOMS was induced. The results of this study were as follows: 1. Elbow flexion range showed significant difference each time, especially at 48 and 72 hours 2. Mechanical pain thershold and subjectively pain showed no significant difference between group.

  • An electrical stimulator was designed to induce locomotion for paraplegic patients caused by central nervous system injury. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. The electrical stimulator restored partially gait function in paraplegic patients.

Comparing Changes in Knee Muscle Strength after Reconstruction of the Anterior and Posterior Cruciate Ligaments

  • Purpose: The purpose of this study was to identify changes in knee muscle strength after reconstruction of the anterior cruciate ligament (ACL) and the posterior cruciate ligament (PCL). Methods: Thirteen subjects (males) with anterior ligament injury and ten subjects (males) with posterior ligament injury voluntarily participated in this study. Both groups were evaluated at the pre-and post-reconstruction stages using an isokinetic dynamometer. Peak torque, total work, and the hamstrings to quadriceps (H/Q) peak torque ratio were calculated at angular velocities of 60°/sec and 180°/sec. Statistical analysis was conducted on SPSS 18.0 for Windows using t-tests to compare mean differences. Results: At an angular velocity of 60°/sec, both the ACL and PCL groups showed a significant increase in muscle strength in the flexors and extensors. Muscle strength in the extensors was significantly increased in the PCL group compared to the ACL group. At an angular velocity of 180°/sec, the ACL group showed a significant increase in muscle endurance in the flexors and extensors, and the PCL group showed a significant increase in muscle endurance in the flexors. At angular velocities of 60°/sec and 180°/sec, the H/Q peak torque ratio increased in the ACL group but decreased in the PCL group. Consequently, the H/Q peak torque ratio was significantly different for the two groups. Conclusion: The results suggest that the patients with ACL injury should focus on strengthening the knee extensors and that the patients with PCL injury need to strengthen the knee flexors.

휠체어 사이클 경사로 주행 시 척수손상 장애인의 상체 근전도 특성 분석 (Electromyographic features of upper body during wheelchair cycle ramps ascent for disabled with spinal cord injury)

  • 본 연구는 휠체어 사이클 경사로 주행 시 척수 손상으로 인한 하지마비 장애인의 상체의 근전도 특성을 평가하고자 하였다. 이를 위하여 척수손상 장애인 3명을 대상으로 $0^{\circ}$, $3^{\circ}$, $6^{\circ}$의 경사로에서 휠체어 사이클 주행을 하도록 하였으며, 이때 이두근, 삼두근, 전 삼각근, 상승모근, 광배근, 복직근의 근 활성 최대값과 수축 시간, 수축 개시 종료 시기를 측정 및 분석하였다. 본 연구 결과 휠체어 사이클의 경사로 주행 시 수축 시간과 근 수축 종료시간이 경사도가 증가함에 따라 유의하게 길어지고 지연됐으며, 광배근의 최대 근 활성도는 평지 주행보다 유의하게 증가하였다 (p<0.05). 이와 같은 결과는 휠체어 사이클의 경사로 주행 시 광배근의 과도한 사용을 의미한다.

근골격계질환 예방프로그램의 필요성과 자동차 부품 제조업 근로자들의 근력과 자세의 특성 연구 (Characteristics of Muscle Strength and Posture among Automobile Parts Manufacturing Workers and the Necessity of the Musculoskeletal Injury Prevention Program)

  • PURPOSE: This study examined the necessity of a musculoskeletal injury-prevention program for automobile parts manufacturing workers by assessing the body composition, muscle strength, and posture according to sex and age. METHODS: Two hundred eighty-two workers (men, 218; women, 64; mean age, 38.97 years; and mean work tenure, 10.29 years) participated in this study. They completed a questionnaire on their general characteristics, excluding personal identification codes. The participants were evaluated for body composition, muscle strength, and posture using InBody, a hand dynamometer, and the Image J program through photography, respectively. An independent t-test and one-way analysis of the variance were used for the comparisons according to sex and age, respectively. RESULTS: Significant sex-related differences in muscle strength and grip force were observed (p<.05). In addition, the left-side pelvic inclination was significantly higher in the female participants than in the male participants (p<.05). Significant age-related differences in the skeletal muscle mass and muscle strength were observed around 40 years (p<.05). Significant correlations were observed among the body composition (skeletal muscle and body fat masses), muscle strength, and posture (pelvic inclination; p<.01). CONCLUSION: Significant differences in pelvic inclination by sex and muscle strength by age were found, even in healthy workers. Additional posture and muscle strength measurements will be needed for workers who participate in the musculoskeletal injury prevention program because they are exposed to an incorrect posture for many hours or in many repetitive tasks.
