Multiple Attempts at Embryo Transfer do not Adversely Affect In-vitro Fertilization Pregnancy Rates: Related Mucus Contamination (반복 배아 이식이 임신율에 미치는 영향: 이식관의 점액 유무)

  • Objective : We investigate the effects of multiple attempts of embryo transfer because of retained embryos in the catheter and of contaminated mucus on the transferred catheter. Materials and Methods: We respectively analysed data between November 1998 and August 2002 from 305 patients of 369 cycles who underwent IVF-ET. Of these patients, 47 patients of 50 cycles (Group 2) were required multiple trial of embryo transfer. They were compared with an age-matched control groups (Group 1) with female factor infertility. Pearson's $?^2$ and Fisher's tests were used to compare proportions between discrete variables. Noncategorical data were compared using t-test. Statistical significance was set at p<0.05. Results: Embryos were significantly more likely to be retained when catheter was contaminated with mucus (Group 1: 22.4%; Group 2: 44.0%). The clinical pregnancy rates, however, for the contaminated mucus or not, were 46.8%, 43.5% respectively. There was no significant difference clinical pregnancy rate between those who had all their embryos transferred at the first attempt (45.4%) and those who required more than one attempt (48.0%). Conclusions: Contaminated mucus in the catheter is associated with failed embryo transferred at the first attempt. Embryo transfers, however, that are repeated attempts do not adversely affect pregnancy rates following IVF-ET.

  • The purpose of this study is to contribute to the development of the women's fashion field with the goal of creating creative and experimental fashion styles through the categorization of cases of women's fashion design expressed with Maximalism, a type of expression used in modern fashion, as well as analyzing and examining the formativeness of this style. Qualitative research was conducted through a literature review and design case analysis. The scope of this research was from 2011 S/S to 2016 F/W, and the regional scope included the women's collections from Paris, Milano, London and New York. The results acquired are as follows. Initially, exaggeration was expressed through excessively exaggerating the overall silhouette of clothing or exaggeratedly distorting certain parts of the overall silhouette. Secondly, decoration was expressed through the use of the same colors and materials and the addition of excessive details and trimmings to express an exaggerated style of clothing as well as to express a luxurious and handcrafted style of clothing. Thirdly, fusion was expressed through integrating multiple items from different cultures, deconstruction, which destroys the concept of gender and use of clothing, and fashion style that transcends time and space. Fourthly, non-structurality was expressed through differentiating multiple weight feelings applied to the left/right sides of clothing or expressing a structural designs that is asymmetrical to the upper/lower/left/right directions to create a silhouette that displays a creative avant-garde style. As examined above, it was found that the maximalism expressed in fashion after 2011 created new images and expressed maximized emotional orientations through the exaggerated use of multiple silhouettes and decorations or through the integration of colors, patterns, ages and images. Through this process, maximalism is expected to not only provide an infinite possibility to the modern fashion that pursues diversification and multiplication, but also serve as an example that represents diversified post-modernism fashion trends in the future.

A Study on Evaluation for Sampling and Analytical Methods of Diatomaceous Earth Dust (규조토 분진 측정 및 분석 방법의 비교에 관한 연구)

  • This study was performed in a diatomite factory located at Pohang City, Kyeongsangbuk-Do. The major objectives were to evaluate sampling and analytical methods of diatomaceous earth dust. Concentrations of total and respirable diatomaceous earth dust were measured. Size distribution of dust was analyzed by a personal cascade impactor and the particle size analyzer which is an application of multiple diffraction method. Also crystalline silica in respirable and total dust samples was analyzed quantitatively by X-ray diffraction and Fourie Transform Infrared Spectroscopy(FTIR). The results were as follows: The airborne total and respirable dust concentrations, particle size distribution, and cristalline silica(quartz) concentrations showed approximately a log-normal distribution. The means of totaldust concentrations at flour maufacturing, fire brick grinding and packaging processes exceeded the Korean and American Conference of Governmental Industrial Hygienists standards, $10mg/m^3$. The size distribution of diatomaceous earth dust was log-normal and identified as the rspirable particle mass and thoracic particle mass. The crystalline silica in respirable and total dust samples was identified to quartz and contained about 10 % in those samples. Finally, it is necessary to study the applicability of multiple diffraction for particle size distribution to compare the ACGIH's size selective sampling with other materials containing crystalline silica. Also, advanced quantitative study to X-ray diffraction and FTIR methods shoud be carried out 10 verify general and specific characteristics for respirable crystalline silica.

Effects of Anisotropic Fiber Packing on Stresses in Composites (이방성 섬유의 배열이 복합재료의 응력에 미치는 영향)

  • In order to investigate effects of anisotropic fiber packing on stresses in composites, a Volume Integral Equation Method is applied to calculate the elastostatic field in an unbounded isotropic elastic medium containing multiple orthotropic inclusions subject to remote loading, and a Mixed Volume and Boundary Integral Equation Method is introduced for the solution of elastostatic problems in unbounded isotropic materials containing multiple anisotropic inclusions as well as one void under uniform remote loading. A detailed analysis of stress fields at the interface between the isotropic matrix and the central orthotropic inclusion is carried out for square, hexagonal and random packing of orthotropic cylindrical inclusions, respectively. Also, an analysis of stress fields at the interface between the isotropic matrix and the central orthotropic inclusion is carried out, when it is assumed that a void is replaced with one inclusion adjacent to the central inclusion of square, hexagonal and random packing of orthotropic cylindrical inclusions, respectively, due to manufacturing and/or service induced defects. The effects of random orthotropic fiber packing on stresses at the interface between the isotropic matrix and the central orthotropic inclusion are compared with the influences of square and hexagonal orthotropic fiber packing on stresses. Through the analysis of plane elastostatic problems in unbounded isotropic matrix with multiple orthotropic inclusions and one void, it will be established that these new methods are very accurate and effective for investigating effects of general anisotropic fiber packing on stresses in composites.

Decreased HDL-Dependent Paraoxonase and Arylesterase Enzyme Activity May Indicate a Worse Prognosis in Multiple Myeloma

  • Background: Multiple myeloma (MM) is a haematological cancer characterized by clonal proliferation of plasma cells.The aim of this study was to investigate the activity of serum paraoxonase-1 (PON1) and arylesterase (ARE) in multiple myeloma with and without free light chain excretion(FLCe-MM and NFLCe-MM); as well as to investigate possible alterations in oxidative stress parameters. Materials and Methods: Total thiol (T.thl), oxidative stress index (OSI), total oxidant status (TOS) and total antioxidant status (TAS) were examined in addition to the PON1 and ARE enzyme activities in twenty one FLCe-MM and nineteen NFLCe-MM subjects. Routine parameters like lipid panel, serum total protein, albumin, creatinine, blood urea nitrogen (BUN), uric acid and hemoglobin levels were compared with the oxidative stress markers. Results: Serum total protein, BUN, creatinin, and uric acid levels were significantly higher (p=0.04, p=0.001, p=0.001 and p=0.0022, respectively), while hemoglobin and albumin levels were significantly lower in FLCe-MM patients (p=0.009 and p=0.04,respectively). PON1 and ARE activities were significantly lower in patients with FLCe-MM compared to those with NFLCe-MM (p=0.001 and p=0.008, respectively). Conclusions: Depending on our results of prognostic markers of MM such as age, hemoglobin, albumin, and creatinine we feel confident to presume FLCe-MM as a subgroup with a worse prognosis. A decrease in PON1 and ARE activities may contribute to the prognosis and may be used as a prognostic tool in MM.

Knowledge, Attitude and Practices of Women in Maldives Related to the Risk Factors, Prevention and Early Detection of Cervical Cancer

  • Background: A population-based cervical cancer screening program using visual inspection with acetic acid was launched in Maldives in 2014. Our study aimed to assess the knowledge, attitude and practices of women in relation to risk factors of cervical cancer, early detection of the disease and its prevention. Materials and Methods: The questionnaire based survey was conducted among 20 to 50 year old women, systematically sampled to represent three regions of Maldives. Trained investigators interviewed a total of 2,845 women at home. Results: The prevalence of the risk factors of cervical cancer like early age at marriage and childbirth, multiple marriages, multiple marriages of the husbands, and multiple pregnancies was high. More women knew about breast cancer than cervical cancer. Even among the small number of women who knew of cervical cancer, only 34.6% had the knowledge of at least one early symptom. Very few women knew that the cancer could be prevented by any test. Only 6.2% of the women reported having ever undergone a Pap smear. Many women had the misconception that cervical cancer was infectious. In Maldives the younger women have high literacy rate due to the policy of universal free education and those with higher levels of education had improved knowledge of cervical cancer and its risk factors. The prevalence of risk factors also reduced with improved literacy. Conclusions: Awareness about risk factors and prevention of cervical cancer is limited among Maldivian women in spite of having high exposure to some of the risk factors. A universal literacy program in the country has helped to improve the knowledge of cervical cancer prevention and to reduce the exposure to various risk factors in the younger population.

Clinicopathology Profile and Bone Involvement of Multiple Myeloma Patients in Dharmais National Cancer Hospital, Indonesia

  • Background: Even though rarely found in Indonesia, the incidence of multiple myeloma (MM) is increasing every year. Bone involvement of MM is the most often a clinical disorder which leads to worsening clinical conditions and low quality of life of patients. Purpose: To determine the clinicopathology profile of bone involvement of MM patients in Indonesia. Materials and Methods: The cross-sectional study of MM conducted at Dharmais National Cancer Hospital (DNCH) by collecting data from medical records of MM patients who came to DNCH in period 2008-2012. Results: There were 39 MM patients all with age above 60 years. There were 56.4% male and 43.6% female patients. Most were diagnosed at stage III (32.4%), and 41% had obesity. The comorbid conditions were anemia (82.9%), hypoalbuminemia (60%), increased creatinine level (38.5%), increased ${\beta}2$ microglobulin level (94.1%), increased LDH level (23.1%) and plasmocytes above 30% (65%), but only 4.2% patients presented with hypercalcemia. Meanwhile, bone involvement occurred in 76.9% of MM patients with 4 lesions on average and a maximum of 16 lesions. The locations of bone lesions were spine (70%), skull (70%), pelvis (33.3%), humerus (30%), and femur (30%). Conclusions: The incidence of MM in Indonesia is increasing annually with bone involvement in more than three-fourths, but interestingly without hypercalcemia.

Clinical and Biochemical Profile of Monoclonal Gammopathies in Caribbean Patients in a Resource-limited Setting

  • Background: Multiple myeloma is the most common malignant plasma cell dyscrasia and ranks second among primary haematological malignancies. This study describes the epidemiologic, clinical and pathologic profile of monoclonal gammopathies seen in the University Hospital of the West Indies (UHWI), a tertiary care referral centre. Materials and Method: A retrospective analysis of 85 cases diagnosed at UHWI over the 5-year period 2003-2007 was conducted. The cases were identified from the bone marrow records as well as the computerized database of the Medical Records Department. Clinical presentation, family and personal history and demographic data were retrieved. Haematological and biochemical results were also analyzed. Results: There were 85 patients diagnosed with monoclonal gammopathies. The M:F ratio was 1.2:1 and the mean age was $65.7{\pm}1.3$ years. Eighty percent of the patients had skeletal pain and 40% experienced weight loss. Of the patients experiencing bone pain 56.7% had multiple lytic lesions, 26.7% had pathological fractures and 26.7% had compression fractures. Seventy-four patients (87.1%) had a haemoglobin level <12.0 g/dL with 52.9% having values <8.0 g/dL. Renal impairment was evident at diagnosis in 36.5%. Hypercalcemia was seen in 26.5% and hyperuricemia in 45.9%. Of the 79 patients who had serum protein electrophoresis performed, 77.2% had at least one monoclonal band and of these 24.6% had a monoclonal protein also present on urine protein electrophoresis. Conclusions: The demographic profile in this group of patients is largely similar to other studies in predominantly Caucasian populations; however there was a notable increase in prevalence of severe disease at presentation, with the majority of patients presenting at the most advanced stage. It is probable that these differences reflect socioeconomic factors and not merely inherent ethnic variation in disease biology.

A Comparative Study on Variability of Oxidants Out of Air Pollution Materials in Seoul: Metropolitan vs. Suburban Area (서울지역의 대기오염 물질중 산화성 물질의 지역간 차이에 대한 연구)

  • A Continuous monitoring of Air Pollution in city of Seoul was carried out from January 1 to December 31 of 1979 at two selected sites, Kwanghwamun observatory and Kwanag observatory. The measured data were averaged on monthly basis. The maximun value of oxidant pollution was observed in July, and the minimum in February. It is the purpose of this study to determine the effect of hydrocarbon, nitrogenoxide, wind velocity and ambient temprature on the observed values of oxidant pollution for the above two months. The results of the study may be summarized as follows. 1) The oxidant concentration in February was higher than in July by about 2 times in both downtown area and the suburbia. The concentration in downtown area was $25.75\pm 4.75ppb$, and that in suburbia was $29.83\pm 5.16ppb$. As for the oxidant concentration in July, it was observed that the suburban area ($26.464\pm 7.59ppb$) had about 2.8 times higher value than the downtown area ($9.284\pm 1.55ppb$). 2) The peak oxidant concentration of suburban area during the daytime is occured from noon to 5:00 P.M.. These patterns are similar to the classical patterns, but the peak Oxidant Concentration of downtown area in February was occured at 9:00A.M. 3) The overall level of nitrogen oxide pollution was much higher in downtown area than in suburban area. Two peaks of nitrogen oxide concentration occured at 10 A.M. and 12 midnight in downtown area. This observation agrees with the report that the air pollution is higher in the area where the pollution sources are concentrated. 4) The multiple correlation analysis for the oxidant and the other variables measured in February in downtown area showed close correlation with nitrogen oxide and ambient temprature. The multiple correlation coefficient of oxidant with nitrogen oxide was 0.872, and that with nitrogen oxide and temperature simultaneously was 0.903. International Scoring System in Symptomatic Multiple Myeloma: Experience from a Tertiary Care Center

  • Background: Symptomatic multiple myeloma (MM) is an acquired B-cell malignant proliferation of antibody secreting plasma cells, characterized by end organ damage due to monoclonal immunoglobulin secretion. The aim of this study wa to determine the stage stratification according to an international scoring system in adult Pakistani MM patients at presentation. Materials and Methods: This single centre retrospective study extendedfrom January 2012 to December 2015. Data were retrieved from the departmental maintained records. Results: A total of 39 patients were diagnosed at our center with MM during the period of the study, 25 males and 14 females. Age ranged between 36 and 81 with a mean of $54.5{\pm}14.8$ and a median of 57 years. Common presenting complaints included fatigue (80.9%), backache (79.3%) and bone pain (66.2%). Overall, 9 patients were in ISS stage I (23%), 12 were in stage II (30.7%) and 18 were in stage III (46.1%). Out of the total, 29 (74.3%) had kappa immunoglobulin andthe remaining 10 (25.6%) had lambda type myelomas. IgG myeloma was commonest, seen in 26 (66.6%) followed by IgA in 11 (28.2%) with non secretory myeloma in one (2.5%) and light chain disease also in one patient (2.5%). Conclusions: MM in Pakistani patients is seen in a relatively young population with male predominance. Primarily patients are symptomatic and risk stratification revealed a predominance of advanced stage III disease in our setting.
