A Case Report on the Risk of Enterobacteriaceae Infection in the Oral and Maxillofacial Region

  • Osteomyelitis is an inflammatory condition of the bone caused by pathogenic bacteria. The causative pathogen is usually oral residing bacteria, but this is a report of patients with osteomyelitis infected with Enterobacteriaceae, which is not common. Enterobacteriaceae has been known to cause in-hospital infections for over last 30 years and is known to have multiple antibiotic resistances. Both cases in this study developed osteomyelitis after removal of the dentigerous cyst. Enterobacter aerogenes was cultured in one patient and Serratia marcescens in the other. After changing antibiotics through antibiotic susceptibility testing, clinical symptoms subsided and radiographic images confirmed that the callus formed and recovered at the same time.

Scalp metastasis of advanced gastric cancer

  • Head and neck cutaneous metastasis of advanced gastric cancer is uncommon, and scalp metastasis is particularly rare. We present the case of a 60-year-old man who was diagnosed with cutaneous metastasis on the scalp originating from advanced gastric cancer. The patient was referred to the plastic surgery department for a scalp mass near the hairline. He had been diagnosed with advanced gastric cancer and undergone total gastrectomy and Roux esophagojejunostomy 3 years previously. The differential diagnosis for a single flesh-colored nodule on the scalp included benign tumors such as epidermal cyst or lipoma; therefore, the patient underwent excision and biopsy. In the operative field, the mass was found to be located in the frontalis muscle. The biopsy result showed that the mass was a metastatic lesion of advanced gastric cancer. Whole-body computed tomography revealed a gastric tumor with blood vessel infiltration, peritoneal carcinomatosis, liver metastasis, and multiple disseminated subcutaneous metastases. Although scalp metastasis originating from an internal organ is extremely rare, plastic surgeons should always consider a metastatic lesion in the differential diagnosis if a patient with a scalp lesion has a history of malignant cancer.

Primary cutaneous mucinous carcinoma in a periorbital lesion: two case reports and literature review

  • Primary cutaneous mucinous carcinoma (PCMC) is a rare malignancy of the sweat glands that most commonly affects the periorbital area. It is characterized by slow growth over a prolonged period, and its morphology can be easily confused with a benign tumor, such as an epidermal cyst. Consequently, many patients experience recurrence after undergoing multiple resections. However, there are few reports concerning the surgical management of PCMC. We present two cases of PCMC originating in the periorbital area. The first case involved a 76-year-old man with a mass measuring 3.0×1.5 cm that had been increasing in size. The second case was a 61-year-old man with two masses, each measuring 1.0×1.0 cm, that were also growing. Both patients underwent wide excision with a 5-mm safety margin, which was determined based on the widest view of the cross-section of the mass on the magnetic resonance imaging. Subsequently, based on the intraoperative frozen biopsy results, both patients underwent additional excision with a 5-mm safety margin in only one direction. This report shows that, when determining the surgical margin of PCMC in periorbital area, employing imaging modalities and intraoperative frozen biopsies can be helpful for narrowing the surgical margin.

폐에 발생한 간엽 낭성 과오종 - 1례 보고 - (Mesenchymal Cystic Hamartoma of the Lung - 1 Case Report -)

  • 폐에 발생하는 간엽 낭성 과오종(mesenchymal cystic hamartoma: MCH)은 매우 드문 종양으로 1986년에 처음으로 발표되었고 아직까지 유병율은 밝혀지지 않았다. 방사선 사진과 조직 검사에서 특징적으로 양측 폐에 다발성의 결절과 크기가 다양한 낭포들이 보인다. 결절들은 미성숙 간엽 세포들의 증식으로 이루어지고 결절의 크기가 점차 커지면서 낭포를 형성하게 되는데 낭포의 내경은 정상적 또는 화성 호흡 상피 세포로 이루어지고, 그 벽의 중간층은 방추형의 간엽세포충으로 되어 있다. 주증상은 객혈과 재발되는 기흉, 그리고 혈흉이다. 비교적 양성종으로 알려져 있지만 악성 변화의 가능성이 있다. 폐기포 절제술 시 폐 전체 표면에 다양한 크기의 낭포성 병소와 결절들이 육안적으로 관찰되었고, 조직 검사상 낭포의 내경은 호흡 상피로 둘러져 있었고, 그 벽의 중간층은 원시 간엽 세포층로 이뤄져있었다. 육안적 소견과 광학현미경하 소견이 간엽 낭성 과오종에 적합한 소견이었다.. 저자들은 자주 재발되는 기흉과 객혈이 있었던 27세의 여자에서 간엽 낭성 과오종을 경험하였기에 보고하는 바이다.

급성 골수성 백혈병 소아환자에서 초음파 검사에서 낭미충증으로 오인된 표피 포도알균에 의한 파종 감염 병변이 발생한 증례 (Disseminated Septic Lesions Caused by Staphylococcus epidermidis Mimicking Cysticercosis Detected on Ultrasonography in a Pediatric Patient with Acute Myeloid Leukemia)

  • 표피 포도알균은 사람 피부에 있는 정상균이나, 체내 이물질을 가진 사람이나 면역 저하자에게는 심각한 감염을 일으킬 수 있다. 13세 남자가 발열, 근육통으로 입원하였고 두피, 팔과 다리에 통증이 있는 결절성 병변이 만져졌다. 혈액검사에서 범혈구 감소증과 모세포 80% 소견을 보였고 급성 골수성 백혈병으로 진단되었다. 전신 자기공명영상 검사에서 가장자리 조영 증강을 보이는 다발 낭성 병변이, 초음파 검사에서 에코성 낭성 병변과 그 내부의 에코성 결절이 팔과 다리의 근육 내부에서 관찰되어, 낭미충증이 강력히 의심되었다. 그러나 초음파 유도하 조직 검사에서 농양이 확인되었고, 조직 배양검사에서 표피 포도알균이 동정되었다. 저자들은 백혈병 환자에서 낭미충증으로 오인되었던 표피 포도알균에 의한 전신 다발 병변이 발생한 예를 경험하였기에 보고하는 바이다.

  • 동물원에서 사육 중이던 7세령 암컷 일본원숭이가 짝짓기 과정에서 피부에 외상을 입은 후 치료 도중 폐사하였다. 부검시 간에서 $1.3\times1.2\times1.0cm$ 크기를 비롯한 다양한 크기의 다발성 낭포가 관찰되었으며 낭포내에 점액성 액체가 저류되어 있었다. 현미경 소견상 낭포는 담관 상피세포로 덮혀 있었고 입방상피에서 원주상피까지 다양하였으며 대부분 단층이었으나 일부는 여러층으로 덮혀 있었다. 악성도나 다른 장기로의 전이 소견은 찾을 수 없었다. 본 증례는 일본 원숭이에서 발견된 첫 번째 담관 낭샘종 증례이다.

다발성 폐 낭종을 보인 쇼그렌 증후군의 폐 침범 1예 (A Case of Lung Involvement Showing Multiple Lung Cysts in Primary Sjögren's Syndrome)

  • 쇼그렌 증후군은 림프구 침윤과 관련된 만성적인 염증성 자가면역 질환으로 아직 정확한 병태생리학적 기전은 밝혀지지 않았다. 45세 여자 환자가 내원 2년 전 전신 쇠약 및 피로감으로 입원하여 혈청 검사에서 anti-Ro/La antibody 양성, 흉부 단순방사선 및 컴퓨터 촬영에서 양 폐야의 다낭성 병변이 관찰되어 비디오 흉강경을 이용한 폐 생검 시행 결과 세기관지 주위에 림프구 침윤 및 다양한 크기의 폐낭종들이 관찰되어 쇼그렌 증후군의 폐 침범 의심하에 추가 검사 시행하려 하였으나 추적 관찰 되지 않았다. 2년 후 폐렴으로 입원하였으며, 다시 시행한 흉부 컴퓨터 단층촬영에서 다발성 낭성 변화는 큰 차이를 보이지 않았다. 쇼그렌 증후군의 폐 침범은 다양한 형태로 나타나는데, 단순히 세기관지 주위에 림프구 침윤에 의한 다낭성 폐 질환에 대한 보고는 극히 드물다. 따라서 본 저자들은 일차성 쇼그렌 증후군 환자에서 비디오 흉강경을 이용한 폐 생검으로 진단된 세기관지 주위에 림프구 침윤을 동반한 다낭성 폐 질환 1예를 경험하였기에 보고하는 바이다.

다발성 악골의 치성 각화낭을 동반한조대술 후 적출술을 이용한 기저세포모반증후군의 치험례 (TREATMENT OF BASAL CELL NEVUS SYNDROME WITH ENUCLEATION FOLLOWING MARSUPIALIZATION : A CASE REPORT)

  • Basal cell nevus syndrome is a hereditary disease of an autosomal dominant trait with variable conditions such as basal cell carcinomas of the skin, deformity of rib, fusion of vertebrae, mental retardation, hypertelorism, and multiple odontogenic keratocysts. A 32 years old man with pus discharge from fistula on the vestibule of left upper 1st molar visited to Chosun University Dental Hospital. Radiographic evaluation revealed multiple maxillary and mandibular cysts that had multilocular radiolucency on left mandibular body area, thining of inferior border of left border of ramus and well defined unilocular radiolucency above right upper 1st and 2nd molar and from left upper 1st premolar to 2nd molar. In chest PA view, he had a forked rib in the left 4th rib and in skull PA view the calcification of falx cerebri was observed. There was not any skin lesion. After the preliminary evaluation, the patient was diagnosed with basal cell nevus syndrome and he underwent marsupialization for decreasing the size of cystic lesion and came to hospital for dressing 3days a week. As time goes by, the size of lesion decreased. So, one and half year after marsupialization, he underwent cyst enucleation and iliac bone graft for the mandibular lesion and buccal fat pad grafts for the maxillary lesions. After the surgery, the patient experienced normal healing without any complications and he is on long-term follow-up.

복부 피하조직으로의 폐흡충증 이소기생 치험례 (Paragonimiasis in the Abdominal Subcutaneous Tissue: A Case Report)

  • Purpose: Paragonimiasis is infectious disease occurred by Paragonimus Westermani, which invades into human body as a final host. Habitual eating the freshwater crab or crawfish unboiled is one of the reason of infection. Paragonimiasis raged in 1970s in Korea, Japan, China and other Asian countries but the incidence decreased rapidly. Once people eat infected second host, parasite penetrates the duodenal wall and migrates to the lung. During this migration period, the parasite can migrate to other organ, such as brain, spinal cord, liver and subcutaneous tissue, but the cases are rarely reported. The objective of our study is to present our experience of the ectopic migration of parasite to the subcutaneous tissue of the abdomen, which was easily treated with excision and Praziquantel medication. Methods: A 59-year-old woman who likes eating unboiled freshwater crab was diagnosed as Paragonimiasis 15 months ago. Her symptoms were fever and cough, and she was treated with Praziquantel medication. 3 months after discharge, she visited our hospital with left pleuritic chest pain, cough with fever, and palpable mass formation on left lower quadrant of the abdomen. Wedge resection of the left lung and Praziquantel medication was maintained for a week. Nevertheless, fever persisted after the treatment. The patient received total excision of the abdominal soft tissue mass, and the fever was relieved. Results: Pathologic findings of the mass showed multiple cyst and abscess formation with crystal structure which is suspicious parts of the parasite or calcified egg shells. Uncontrolled fever was relieved after the operation, and there was no evidence of recurrent Paragonimiasis and ectopic migration for 1 year follow up period. Conclusion: Ectopic migration of Paragonimus is rare, but multiple organ can be involved. Patient with Paragominiasis who was refractory in fever control after Praziquantel medication or surgical evaluation of the lung should be considered as ectopic migration of the Paragonimiasis.

부적절한 일차 처치와 연관된 이하선 종양 2례 (Tumors of the Parotidomasseteric Area Associated with Inadequate Primary Treatment: Report of 2 Cases)

  • Purpose: Parotid neoplasia are relatively frequent, representing approximately 3% of all tumors in the head and neck regions. But incomplete resection and misdiagnosis of parotid gland is followed by multiple tumor invasion, tumor recurrence, and other iatrogenic tumor formation. In patients undergoing parotidectomy for confirmed or suspected malignancy, the traditional or modified rhytidectomy incision may prove suboptimal because it does not easily lend itself to a continuous neck dissection. Similarly, patients with tumors of the anterior accessory lobe or patients with large anterior tumors may also require the modified Blair incision for adequate surgical exposure. This report serves to revisit the topic of accessory and parotid gland neoplasms to emphasize proper management, particularly the surgical aspects, so that consequences of recurrence are avoided. Methods: This is a retrospective review of our experience with two cases of parotid tumors; one accessory parotid gland neoplasm and one parotid gland neoplasm. We report the case of parotid tumor and epidermal cyst in a 54-year old male patient and the case of case of recurrent parotid tumor with local invasion in 30-year old male patient. Results: All were removed through a modified Blair incision. Pathologic report notified that One was found pleomorphic adenoma and epidermal cyst, and the other one pleomorphic adenoma with subcutenous invasion. The patients recovered well without any complication such as infection, hematoma, facial nerve palsy, and necrosis of skin flap. Patients were discharge POD#7. Patients were followed up to for 1 year and they have no sign of recurrence. Conclusions: A high index of suspicion, prudent diagnostic skills(including fine-needle aspiration biopsy, CT, US), and meticulous surgical approach are the keys to a successful management of these lesions. We experienced two cases of parotid neoplasia, in the treatment of tumor reccurence & iatrogenic tumor arising from the parotid gland and are presented with the review of literatures.
