Comparison of Intradermal Skin Test and Multiple Allergen Simultaneous Test Results in Canine Atopic Dermatitis

  • Intradermal skin test (IDST) is generally considered a useful tool in identifying causal allergens in canine atopic dermatitis. Currently, multiple allergen simultaneous test (MAST), an in vitro testing method for allergen-specific immunoglobulin E, is being used as an alternative method. However, there are no reports comparing the IDST and MAST results in the same dogs. This study compared the results of both tests to evaluate the agreement and correlation between them. The sensitivity, specificity, and accuracy of the MAST were 76.2%, 64%, and 66.7%, respectively. Moderate positive predicted value (PPV, 50-75%) or high sensitivity (80-100%) were identified for indoor allergens, such as cat epithelia, house dust, and house dust mites. In contrast, high negative predicted value (NPV, 93.3-100%) and specificity (60-100%) were observed for environmental allergens and fungi. Although the agreement between IDST and MAST for all allergens was fair (κ = 0.301), that for each allergen was poor (κ < 0.01), except for birch (κ = 0.158). Spearman's rank correlation analysis revealed a low correlation between the MAST and IDST results (ρ = 0.308, p = 0.001). As compared to the IDST results, the MAST results did not identify the causative allergens sufficiently. IDST may not be performed for environmental allergens and fungi with high NPV and specificity if the MAST result is negative, but it may have to be performed for indoor allergens with moderate PPV and high sensitivity when the MAST result is positive.

  • Among the inhalant and foods which caused allergy, which is increasing nowaday, this research has a purpose of studying allergy-causing materials (allergen) which most frequently cause allergy. In order to conduct this research, we selected 100 hundred patients (M : F = 65 : 35) from in Kwangju Christian Hospital, who were tested through allergy multiple antigen simultaneous test (MAST) immunoblot assay (RIDA$^{(R)}$ X-Screen; r-Biopharm Co., Darmstadt, Germany). According to the test, the total IgE positive rate (above Class 2) from the inhalant is 58.3%, and that from food is 35%, respectively. This research showed that the first most allergen was the tick (Derm. farinae/Derm. pteronyssinus), the second most was housedust, and the third was the hair of pets such as cats and dogs; 65%, 35%, 26.3% in inhalant panel, and 52.5%, 40%, 50% in food panel, respectively. Thease allergens newly added to inhalant and food panels were not contributable to the detection of specifical allergen. We needs further improvement to be used as primary screening or quantitative test for allergy.

다중 알레르기 항원 동시 검사 결과의 사상체질 간 비교 연구 (A Comparison study of the Multiple Allergen Simultaneous Test Results by Sasang Constitution)

  • Objectives The purpose of this study was to explore the differences in allergic reactions by type of Sasang constitution using multiple allergen simultaneous test(MAST). Methods This study was conducted using the big data of Korean medical health checkups. Subjects with a history of allergic disease, Sasang constitution type information, and MAST results were included. Subjects who had taken drugs before MAST that affected allergic reactions were excluded. We compared total immunoglobulin E(IgE) concentrations, and the number of positive antigens by Sasang constitution using analysis of variance. The presence or absence of a positive reaction and specific IgE concentration classes of 92 antigens in MAST were analyzed by Fisher's exact test or Chi-squared test. Results The distribution of Sasang constitutional types of Tae-Yang, Tae-Eum, So-Yang, and So-Eum among 147 subjects was 2.7%, 42.9%, 38.1%, and 16.3%, respectively. There was no statistically significant difference in the total IgE concentration, the number of positive antigens, and specific IgE concentration classes for each antigen. There was a statistically significant difference in MAST-positive reaction for alder and pine antigens by type of Sasang constitution, and Tae-Eum had the most. Conclusions Tae-Eum showed a relatively strong positive reaction to a large number of antigens compared to the other types, but no statistically significant difference was observed. It is necessary to continuously investigate allergic reactions to provide evidence for the treatment of allergic diseases by Sasang constitution.

Serum allergen-specific and total IgE concentrations in dogs with allergic disorders in South Korea

  • Canine atopic dermatitis (CAD) has a genetic basis and is primarily a skin disorder characterized by allergic features. It is caused by an excessive immune response, associated with immunoglobulin E(IgE), to allergens and shows clinical signs of itching and inflammation. The present study evaluated the allergen-specific and total IgE concentration in dogs with allergic disorders. Allergen-specific IgE analysis was performed using a Multiple Allergen Simultaneous Test (MAST) assay, and total IgE analysis was conducted using an ELISA in 82 dogs with CAD. The subjects were divided into two groups: dogs aged ≤3 years and dogs aged >3 years, enabling a comparison of total IgE concentrations between the two age groups. The results showed that the major affecting environmental allergen was Alternaria alternate, and the major affecting dietary allergen was raw chestnut. The results revealed that dogs aged ≤3 years had higher total IgE concentrations than dogs aged >3 years with significant difference. The results of this study can contribute to the diagnosis and treatment of CAD in South Korea.

  • 알레르기 환자의 증가로 인하여 알러겐을 찾는 것은 중요하다. MAST는 알러겐 특이 IgE항체를 측정하는 간단한 방법이다. 본 연구에서는 알레르기 환자에서 알러겐 특이 IgE 항체, 총 IgE, 호산구수와의 관련성을 평가하고자 하였다. 연구 결과 총 IgE 양성률은 흡입형 패널에서 96.97%, 식품형 패널에서 98.06%였다. 알러겐 특이 IgE 항체의 양성률은 흡입형 패널에서 집먼지 51.52%, 북아메리카 집먼지 진드기 45.46%, 고양이 31.99% 였으며, 식품형 패널에서 각각 55.34%, 42.72%, 34.96%였다. 혈청 총 IgE는 알레르기와 관련이 있었으나, 알레르기가 항상 호산구증가와 관련이 있는 것은 아니었다.

Validity of MAST-CLA for diagnosis of arthropod allergy using receiver operating characteristic (ROC) analysis

  • Many allergists are currently focusing on the development of new diagnostic tools, and are attempting to improve both the sensitivity and specificity. A multiple allergen simultaneous test-chemiluminescent assay (MAST-CLA) is one of the most popular diagnostic tools used in the Republic of Korea. However, there remains controversy among allergists with regard to the cut-off point for a positive result. The present study was conducted in order to determine the validity of MAST-CLA as compared with that of the skin prick test, with particular emphasis on arthropod allergens, on the basis of percentage agreement rates and k-values, and also to suggest the optimal positive cutoff points using receiver operating characteristic (ROC) curves. The study was conducted with 97 subjects (54 men, 43 women). Optimal individual cut-off points were calculated as follows; class II for Dermatophagoides farinae, class I for Dermatophagoides pteronyssinus, and trace for a cockroach mix. These findings suggest that attempting to apply optimal individual cut-off points will be a good way of improving diagnostic tests, particularly MAST-CLA.
