Objective: Intracranial arteriovenous malformation (AVM) associated with aneurysm has been infrequently encountered and the treatment for this malady is challenging. We report here on our clinical experience with AVMs associated with arterial aneurysms that were managed by multimodality treatments, including clipping of the aneurysm, microsurgery, Gamma-knife radiosurgery (GKS) and Guglielmi detachable coil (GDC) embolization. Methods: We reviewed the treatment plans, radiological findings and clinical courses of 21 patients who were treated with GKS for AVM associated with aneurysm. Results: Twenty-seven aneurysms in 21 patients with AVMs were enrolled in this study. Hemorrhage was the most frequent presenting symptom (17 patients: 80.9%). Bleeding was caused by an AVM nidus in 11 cases, aneurysm rupture in 5 and an undetermined origin in 1. Five patients were treated for associated aneurysm with clipping followed by GKS for the AVM and 11 patients were treated with GDC embolization combined with GKS for an AVM. Although 11 associated aneurysms remained untreated after GKS, none of them ruptured and 4 aneurysms regressed during the follow up period. Two aneurysms increased in size despite the disappearance of the AVM nidus after GKS and then these aneurysms were treated with GDC embolization. Conclusion: If combined treatment using microsurgery, GKS and endovascular treatment can be adequately used for these patients, a better prognosis can be obtained. In particular, GKS and GDC embolization are considered to have significant roles to minimize neurologic injury.
Background: This study was to evaluate the survival of patients with pleural and intraperitoneal malignant mesothelioma and to investigate the efficacy of chemotherapy (CT) as well as radiotherapy (RTH) and surgery compared to best supportive care (BSC). Materials and Methods: Forty patients with malignant mesothelioma (38 with pleural and 2 with intraperitoneal) were enrolled. Twenty seven patients underwent (CT) chemotherapy of which 2 also received (RTH) and surgery was only for biopsy in 15/40. Combination chemotherapy included cisplatin-gemcitabine, cisplatin-navelbine and cisplatin (or carboplatin) with premetrexed. Thirteen patients received only best supportive care. Results: A total of 12 (30%) patients were male, and 28 (70%) female. Median age was 54.0 years and the male/female ratio was 1/2.33 (P=0.210). Residential exposure played a major role in two regions, Helwan and Shoubra, in 20% and 15%, respectively. Overall mean survival time was $13.9{\pm}2.29$ months. That for patients who had received best supportive care was $7.57{\pm}1.85$ months, for chemotherapy was $16.5{\pm}3.20$ months, and multimodality treatment regimen $27{\pm}21.0$ months (P=0.028). Kaplan-Meier survival did not significantly vary for sex, residence and the pathological types epithelial, mixed and sarcomatous. The median survival for performance status and treatment modalities was significant (P=0.001 and 0.028). Best supportive care using opioids with a mean dose of 147.1 mg (range 0-1680) of morphine sulphate produced good subjective response and reasonable quality of life but did not affect survival. Conclusions: We conclude that CT prolongs survival compared to BSC in patients with malignant mesothelioma. Moreover, using escalating doses of opioids provides good pain relief and subjective responses.
Since the introduction of chemotherapy for the treatment of childhood leukemia more than 50 years ago, the results of childhood cancer have improved dramatically. The 5-year survival rate of disease, many of which were uniformly fatal in the prechemotherapy era, reached to more than 75%. This remarkable improvement in survival is a direct result of the incorporation of chemotherapeutics into treatment regimens that previously relied only on surgery or radiotherapy for the primary tumor. The multimodality approach, which integrates surgery and radiotherapy to control local disease with chemotherapy to eradicate systemic or metastatic disease, has become the standard approach to treating most childhood cancers. The overall improvement in outcomes in childhood solid tumors has been related to the development of multidisplinary cooperative studies that has permitted the development of well-designed tumor treatment protocols characterized by uniform staging criteria, sharing informations in pathologic classification, uniform methods for tumor markers, oncogenes, and other biologic and genetic factors. Important advances in the biologic study of cancer and its genetic basis led to a number of observations that impact directly on the management of childhood solid tumors. Identification of specific genes, oncogenes, tumor markers, and other biologic and pathologic factors plays an important role in both staging and clarifying the risk categorization of individual patients. Treatment of the patient is influenced by the recognition of specific risk factors. This knowledge has resulted in a change in the approach to care based not only on staging criteria, but also on risk-based management. This concept uses various risk factors of outcomes. Risk-based management allows for each patient to maximize survival, minimize long-term morbidity and improve the quality of life, especially for children's growth and development.
Purpose: We investigated the role of radiotherapy (RT) for pancreatobiliary neuroendocrine tumors (PB-NETs). Materials and Methods: We identified 9 patients with PB-NETs who received RT between January 2005 and March 2012. Of these 9 patients, 4 were diagnosed with NETs in the pancreas and 5 were diagnosed with NETs in the gallbladder. All patients received RT to the primary tumor or resection bed with a median total irradiation dose of 50.4 Gy, with or without chemotherapy. Results: The tumor response rate and tumor control rate in the RT field were 60% and 100 %, respectively. All 4 patients who underwent surgery had no evidence of disease in the RT field. Of the 5 patients who received RT to the primary gross tumor, 1 had complete response, 2 had partial response, and 2 had stable disease in the RT field. The median time to progression was 11 months. Of the 9 patients, four patients had no progression, and 5 patients had progression of disease (locoregional, 2; distant, 2; locoregional/distant, 1). Of the 4 patients without progression, 3 were treated with RT in adjuvant or neoadjuvant setting, and one received RT to primary tumor. One patient experienced radiation-induced duodenitis at 3 months after concurrent chemoradiation without treatment-related mortality. Conclusion: RT can yield local control for advanced PB-NETs. RT should be considered an essential part of multimodality treatment in management of advanced PB-NETs.
본 논문에서는 소동물 생체내 실험시 서로 다른 장비에서 획득된 영상의 융합 및 정합을 위한 방법을 제안한다. 마우스의 꼬리 정맥에 $[[^{18}F]FDG$를 주사하여 60분 섭취후 서로 다른 장비에서 동일한 위치의 영상을 획득하기 위하여 아크릴 재질의 소동물 가이드에 기준마크를 설정하고 microPET과 CT 영상을 획득하였다. MicroPET으로 획득된 리스트모드(list-mode) 데이터는 Fourier Rebinning(FRB) 방법을 사용하여 사이노그램(Sinogram)으로 변환 후 4 번의 반복횟수를 가지는 Ordered Subset Expectation Maximization(OSEM) 알고리즘으로 재구성하였다. MicroPET 영상획득후 PET/CT의 CT를 이용하여 CT영상을 획득하였다. MicroPET 영상에서 폐영역을 정확히 찾아내는 어려움이 있어. 해부학적 정보를 제공하는 CT 영상을 이용하여 폐 영역을 구분하였다. 영상 융합을 위한 불일치 부분을 해결하기 위하여 기준마크의 정보와 폐 영역의 정보를 이용하여 회전과 이동정보를 가지는 어파인 (affine) 변환 행렬 구하여 영상 정합에 사용하였다. 이 방법은 정량적 정확성과 영상 해석의 정확성을 개선할 것으로 기대된다.
미만형 악성 중피세포종은 4∼18개월의 평균생존율을 지닌 치명적인 질환으로 여겨져 왔다. 그러나 종양을 최대한 절제한 후 다양한 접근을 통한 치료방법으로 생존기간을 연장할 수 있다 최근 저자들은 미만형 악성 중피세포종을 가진 49세 남자환자를 치험하였기 에 보고하는 바이다. 환자 는 수개월 동안 혈성객담 및 우측흥통을 호소하였고, 단순흉부사진 및 컴퓨터 단층촬영에서 우측 전흥 강이 진한 음영으로 가리워 져 있었으며 많은 양의 혈성삼출, 미만성 늑막비후와 폐허탈, 파괴 등의 소견 을 보였다. 저자들은 늑막케절제술을 시행하였고, cisplatin과 mitomycin으로 보조적인 항암화학요법을 하였다. 그후 환자는 수 개월간 외래 추적관찰을 받았으며 술후 4개월 현재까지 국소재발의 증거는 없었다.
Synovial sarcoma is usually found near large joints and bursae of the extremities. Only 10% of all synovial sarcomas occur in the head and neck. The most common site affected in the head and neck is the hypopharynx, and very rare in the infratemporal fossa. Treatment of synovial sarcoma of the head and neck is difficult and must have multimodality which consists of operation, radiation and chemotherapy. Prognosis is associated with completeness of resection. We reported a case of synovial sarcoma arising from the infratemporal fossa, the first case in Korea, with review of literature.
다양한 의학 영상장비로부터 획득된 영상들간의 정합의 정확성은 방사선치료계획에서 매우 중요한 쟁점 중의 하나이다. 본 연구에서는 수제작된 뇌팬톰(brain phantom)을 이용한 영상정합의 정확성 평가방법을 연구하였다. 다중영상정합을 위해 CT-MR, CT-SPECT간의 Chamfer 정합(Chamfer matching)법을 적용하였다 영상정합의 정회성은 팬톰 내에 삽입된 표적(target)들의 중심정의 비교를 통하여 평가되었다. CT-MR, CT-SPECT간의 삼차원 제곱근평균제곱(root-mean-square) 이동편차는 각각 2.1$\pm$0.8 mm와 2.8$\pm$1.4 mm이었다. 회전편차는 세 직교좌표축에서 2$^{\circ}$이내였다. 이 오차들은 기존의 팬톰연구와 비교하여 합리적인 오차 허용범위 내에 들었다. 중첩한 CT-MR, CT-SPECT영상의 육안검증 또한 좋은 정합 결과를 보였다.
Liver tumors in children are rare, relatively complex, and encompass a broad spectrum of disease processes. This study reviews our experience of liver tumors during the last 10 years. Medical records of 36 cases of liver tumors in children, treated at Samsung Medical Centers, from October 1994 to December 2005, were reviewed in this study. We analyzed disease characters and survival rates as a whole and by specific disease. The median age was 3.6 years. Male and female ratio was 1:1. The most common symptom was the palpable mass in 15 cases. Others were abdominal distension in 9 cases, jaundice in 2, vomiting in 2, weight loss in 2, and pubic hair growth in 1. CT or US and liver biopsy were performed for diagnosis. There were 28 malignant tumors: malignant rhabdoid tumor (1 case), hepatocellular carcinoma (3 cases), hemangioendothelioma type II (3 cases), angiosarcoma (1 case), and hepatoblastoma (20 cases). Eight tumors were benign; hepatic adenoma (1 case), focal nodular hyperplasia (2 cases), hemangioendothelioma type I (2 cases), mesenchymal hamartoma (3 cases). In this study the clinical characteristics were not different from the other reports. Liver transplantation was performed in 3 cases-1 with hepatoblastoma and 2 with hepatocelleular carcinoma. Accurate and early diagnosis, and individualized multimodality therapeutic approaches might be important for better outcome.
In this paper, we propose pyramid appearance and global structure action descriptors on both RGB and depth motion history images and a model-free method for human action recognition. In proposed algorithm, we firstly construct motion history image for both RGB and depth channels, at the same time, depth information is employed to filter RGB information, after that, different action descriptors are extracted from depth and RGB MHIs to represent these actions, and then multimodality information collaborative representation and recognition model, in which multi-modality information are put into object function naturally, and information fusion and action recognition also be done together, is proposed to classify human actions. To demonstrate the superiority of the proposed method, we evaluate it on MSR Action3D and DHA datasets, the well-known dataset for human action recognition. Large scale experiment shows our descriptors are robust, stable and efficient, when comparing with the-state-of-the-art algorithms, the performances of our descriptors are better than that of them, further, the performance of combined descriptors is much better than just using sole descriptor. What is more, our proposed model outperforms the state-of-the-art methods on both MSR Action3D and DHA datasets.
