휴대용 디바이스를 이용한 완곡추적 안구운동이 만성 편측무시에 미치는 영향: 예비연구 (The Effect of Smooth Pursuit Eye Movement Using Portable Device to Chronic Hemispatial Neglect: A Pilot Study)

  • 목적 : 본 연구의 목적은 만성 편측무시 환자에게 휴대용 디바이스를 이용한 완곡추적 안구운동을 30분간 적용하였을 때 편측무시에 미치는 효과를 알아보는 것이다. 연구방법 : 만성적으로 편측무시를 보이는 한 명의 환자를 대상으로 8.4인치 화면의 휴대용 디바이스를 이용하여 완곡추적 안구운동을 실시하였다. 실험기간은 총 4주로 1주간의 기초선 측정과 2주간의 완곡추적 안구운동 중재, 1주간의 기초선 측정으로 이루어졌으며, 중재는 2주 동안 1일 1회, 주 5회, 회기당 30분씩 적용하였다. 편측무시는 선이분검사와 별지우기검사를 통해 매 회기 측정하였다. 완곡추적 안구운동은 시각적으로 영향을 줄 수 있는 주변의 환경을 줄이고 바른 자세로 앉아 정면에 있는 8.4인치 태블릿pc의 영상을 바라보는 것이다. 영상은 완곡추적 안구운동을 유발하기 위한 영상으로 24개의 노란색 직사각형이 끊임없이 화면 오른쪽에서 왼쪽으로 움직이는 영상이며, 영상을 보는 동안 고개의 움직임 없이 도형을 따라 안구운동을 하도록 하였다. 결과 : 2주 동안의 시운동자극 적용 결과 기초선A와 비교하여 편측무시가 감소되는 경향을 보였지만, 중재기 이후 기초선A'에서 편측무시 양상이 다시 증가하는 결과를 보였다. 결론 : 본 연구를 통해 휴대용 디바이스를 이용한 30분간의 완곡 추적 안구운동이 만성편측무시를 감소시키는데 효과가 있는 것으로 확인되었다.

측두하악장애와 교합상태와의 관계에 대한 연구 (Relationship between Temporomandibular Disorders and Occlusal States Dental Students)

  • In this study, 88 dental students were examined to evaluate the relationship between occlusal states and TM disorders for the epidemiologic study of TM disorders. The clinical evaluation were composed of mandibular movement, TMJ noise, occlusal states and muscle palpation. The following results were disclosed. 1. The frequencies of pain on mandibular movement were 3.4% on maximum opening, 1.13% on protrusion and no pain on laterotrusion. 2. The frequencies on TMJ sound were 21.6% in click, 1.13% in crepitus. 3. The frequency of tenderness on palpation was 12.5% on extra oral, intraoral and neck muscles, tenderness on palpation of TMJ capsule were reported 5 cases, and 4 of them were female. 4. The distribution of Angle's classification was found 79.5% in class I, 4.5% in clasII-div.1 and 15.9% in class III. There was no significant differences on TM disorders between Angles classifications. 5. There was no significant differences on TM disorders between the subjects of canine guided occlusion and group function occlusion, and also for the differences between the subjects of nonworking side interferences and no interferences on laterotrusion. 6. There was no significant differences on TM disorders between the subjects of anterior teeth trauma in C.C. and no anterior teeth trauma, but there were significant differences between the subjects of posterior protrusive contact and no posterior protrusive contact.

기능성 작업복의 인간공학적 평가 - 방염복을 중심으로 - (Ergonomic Evaluation of Functional Working-Clothes - Focused on Flame-Proof Clothing -)

  • This study was aimed to find out the problems of wearing flame-proof clothing and to analytically assess functional working-clothes, thus to propose a new design. We carried out a questionnaire with workers in the field and evaluated the facility for wearing, the sensation of wearing and clothing microclimate in the environmental chamber. We found out the problems were in the parts of neck, armpits, armhole, sleeve, crotch and suspenders. Thermal sensation, wet sensation and comfort sensations were evaluated to be 'hot', 'wet' and near to discomfort', respectively. Therefore, improvement of movement and air permeability is needed. If we can add gusset on armpit for air permeability, this gusset will enable to enlarge the surface in armpit area and finally give a movement efficiency. We suggested several new partly modified design for flame-proof clothing which can be applied to other protective clothing for improvement. This must be further considered in the ergonomic evaluation of new proposed functional working-clothes as well as in the suggestion of design.

Comparative Immediate Effects of Isometric Chin-tuck and Dynamic Neuromuscular Stabilization on Neck Flexor Muscle Thickness and Upright Sitting Height Posture

  • Background: Cervical dysfunction is a common pathomechanical marker in individuals with forward head posture (FHP). To overcome the limitations of the isometric chin-tuck (ICT) exercise, dynamic neuromuscular stabilization (DNS), which emphasizes an entire spinal chain exercise, has recently shown promising clinical results. Objects: Purpose of this study was to compare the immediate effects between ICT and DNS techniques. Methods: 43 young subjects (mean age, $24.0{\pm}5.0$ years) were recruited. Group of subjects with FHP were measured under baseline, ICT, and DNS conditions. Outcome measures included sitting height, longus colli (LC) and sternocleidomastoid (SCM) muscle thickness and LC/SCM thickness ratio. One-way repeated measures ANOVA was used to compare the continuous dependent variables among FHP, ICT, and DNS conditions at p<.016. Results: Both ICT and DNS exercise conditions yielded significantly increased LC muscle thickness, LC/SCM thickness ratio and sitting height than did FHP condition (p<.0001, respectively). Sitting height was significantly greater in DNS exercise than in the ICT exercise (p<.0001). Conclusion: The present results demonstrated that sitting height was greater in the DNS exercise than in the ICT exercise, as well as both corrective postural training exercises were effective on LC/SCM muscle balance ratio when compared with the baseline FHP condition. Therefore, it is considered that DNS exercise can be the recommended exercise for people with FHP.

  • Arytenoid motion has long been recognized as complex. Misunderstandings about the specifics of arytenoid motion remain prevalent. The resultant misunderstandings have led to erroneous or suboptimal clinical approaches to the treatment of vocal fold immobility. A thorough understanding of the anatomy of the arytenoid and cricoid cartilages, the cricoarytenoid joint, and related ligaments, muscles, and other structures is essential in order to fully understand laryngeal motion disorders. Arytenoid motion occurs in three directions. Movements involving a change anteriorly and posteriorly, as well as vertically, are due to the revolving or pitchlike motion of the arytenoid along the minor axis of the cricoid's elliptically shaped facet. The medial and lateral movements are due to the orientation of the arytenoid which in turn is determined by the forward, lateral, and inferior inclination of the cricoid-arytenoid facet. During adduction it is the outward angulation of the vocal process from the body of the arytenoid that allows the entire length of the vocal proceses to approximate one another and to have this meeting occur at the proper vertical height.

환추축관절 차단술 -증례 보고- (Atlanto-Axial Joint Block -Case reports-)

  • Until several years ago we didn't think seriously about cervical problems as a cause of headaches, but since the publication of articles by Bogduk et al they have gotten more attention. Cervical headaches are associated with movement abnormalities of the structures of the neck such as cervical nerve roots, discs, joints and soft tissues. Considering this, we thought that the atlanto-axial joint could be one of the causes of these headaches. Headaches originating from this joint can be recognized by the fact that the pain worsens with rotation of the head in the horizontal plane. Pain can also be referred to the frontal area or around the orbit. We did atlanto-axial joint blocks using a posterior approach on 10 patients who suffered from this type of headache. The results were promising with 9 out of 10 patients showing more than 50% improvement on the numeric rating scale. There were no serious complications observed. We concluded that the atlanto-axial joint block can be an effective procedure in treating this specific type of headache.

  • For an effective acupuncture treatment, the location of muscles around Sutaeumkyongkun and Suyangmyongkyongkun were researched and they were made contracted. The conclusion is as follows; 1. The contraction of muscles around Sutaeumkyongkun gives appearance of the postures ; free movement of thumb, abduction of extension of wrist, flexion and pronation of elbow, depression and abduction of girdle of superior limb, flexion, internal rotation and horizontal flexion of shoulder joint These postures all together consequently produces the action 'holding something in arms'. 2. The contraction of muscles around Suyangmyongkyongkun gives appearance of the postures; extension of metacarpophalangeal and interphalangeal joint of index finger extension and abduction of thumb, extension of wrist, extension and supination of elbow, adduction, elevation and upward rotation of girdle of superior limb, extension, abduction, adduction, internal rotation, external rotation, horizontal extension of shoulder joint, flexion and opposite rotation of neck. These postures all together consequently produces the action 'raising arms'.

  • Temporalis tendon transfer is a technique for dynamic facial reanimation. Since its inception, nearly 80 years ago, it has undergone a wealth of innovation to produce the modern operation. Temporalis tendon transfer is a relatively minimally invasive technique for the dynamic reanimation of the paralyzed face. This technique can produce significant and appropriate movement of the lateral oral commissure, more closely mimicking the normal side. The aim of this article is to review the technique of temporalis tendon transfer involving transferring of the coronoid process of the mandible with the insertion of the temporalis tendon via intra-oral and transcutaneous approach.

자동 지그 바이스용 조오-웨지의 클램핑 특성 연구 (A Study on Clamping Characteristics of Jaw-wedge for Automatic Jig Vise)

  • A jig vise is a device to clamp workpiece precisely, which is widely used for various machine tools and manufacturing purpose. A new elastic structured jaw-wedge of jig vise is developed, in this paper, so as to satisfy the clamping requirement and the suppression effect of upright movement of workpiece. The advanced design parameters of jaw-wedge are derived step by step considering the stress distribution and the displacement profiles of ANSYS analysis, and it could find the optimum model which shows the uniform displacement profiles and exhibits the non-concentrated stress distribution of jaw neck. As a result, it is ascertained that an jaw-wedge developed in this study is the simple elastic structure which is effective for automatic multiple clamping purpose without the danger of shear crack or bucking of jaw.

IIHS small overlap 성능개발을 위한 대차 시험 방법 연구 (A study on the sled test methods for IIHS small overlap performance development)

  • Small overlap crash caused fatal injury in real-world crash. IIHS(Insurance Institute for Highway Safety) proposed the small overlap test. The objective of this study is to analyze dummy injury criteria and dummy excursion on the sled reinforced body angle. Result of the comparisons of dummy injury criteria of a head, neck, and chest was best correlation between sled and vehicle test on base $angle+3^{\circ}$. However, lower extremity was not correlation because sled test could not copy of intrusion. There were a correlation between dummy movement and sled reinforced body angle. Sled reinforced body angle affects the lateral direction of excursion more than longitudinal excursion.
