A Study on the Clinical Characteristics in Oral Lichen Planus (구강편평태선 환자의 임상적 특징에 관한 연구)

  • Oral Lichen Planus(OLP) is a idiopathic chronic inflammatory disease with more difficult to clear and higher recurrent rate than cutaneous lesions. But, there has been no estabilished theories about the proper treatment for OLP. The purpose of this study is to examine the clinical feature, relationship with systemic disease and dental treatment of OLP patients of Korea and to gain helpful information about clinical characteristics and treatment of OLP. The subjects chosen for the study were 54 patients who had visited Department of Oral Diagnosis & Oral Medicine at Yonsei University Dental Hospital Dental Hospital and diagnosed as OLP. Previous clinical records has been reviewed and questionnaires, oral examination, laboratory examination were done and recorded. The following results were obtained : 1. Of the 54 patients, 21 were men and 33 were women with an average age of 47.8 years. 2. The most common intraoral site was bilateral buccal mucosa, followed by unilateral buccal mucosa, gingiva, vesibule, lip mucosa, glossal mucosa, palatal mucosa and mouth floor. 3. The mixed, erosive and reticular form of OLP was most frequent(83.3%) clinical form. 4. OLP patients with liver disease were 5, and drug medication patients were 7. But, we could not find its evidence of association with OLP. 5. Associated events on onset of symptom were stress, denture wearing, dental treatemtn, and common cold. 6. Associated symptoms were dry mouth, tingling, sore throat, and altered taste perception. 30.8% of patients had no specific associated symptoms. 7. Aggravating factors of symptom were peppery food, hot food, fatigue, toothpaste, salty food, sour food, tension, and conversation. Reducing factors were cold food, sleeping. 69.2% of patients had no specific reducing factors. 8. There were no significant differences between normal papulation and OLP patients in CBC, SGOT< SGPT, Serum iron, Total iron binding capacity. 66.7% of subjects were positive response to fungus study for Candida Albicans. The incidence of stress and dental treatment on onset of symptom appeared high in OLP patients. Especially, high incidence of positive response to fungus study for Candida albicans, prescription of anti-fungal agents and dental treatment considerations may be helpful to treatment of OLP.

COMPARISON OF ABSORBED DOSES RESULTING FROM VARIOUS INTRAORAL PERIAPICAL RADIOGRAPHY (전악 치근단 방사선사진 촬영시 촬영조건에 따른 흡수선량 변화에 대한 연구)

  • This study was designed to measure the absorbed dose to organs of special interest from full mouth with intraoral film(l4 films) and to compare the five periapical techniques. Thermoluminescent crystals(TLD-100 chip) were located in brain, orbit, bone marrow of mandibular ramus, bone marrow of mandibular body, bone marrow of 4th cervical spine, parotid gland, submandibular gland and thyroid gland. X -ray machine was operated at 70kVp and round collimating film holding device(XCP) and rectangular collimating film holding device(Precision Instrument) were used. The distance from the X-ray focus to the open end of the collimator was 8 inch, 12 inch and 16 inch. The results were as follows : 1. The absorbed dose was the highest in bone marrow of mandibular body(5.656mGy) and the lowest in brain (0.050mGy). 2. Generally, the lowest absorbed dose was measured from 16 inch cylinder, rectangular collimating film holding device with paralleling technique. But, in bone marrow of mandibular body and the floor of mouth, the highest absorbed dose was measured from 12 inch cylinder, rectangular collimating film holding device with paralleling techniques. 3. Comparing of five intraoral radiographic techniques, it was appeared statistically significant reduction of the absorbed doses measured with rectangular collimating film holding device compared to XCP film holding device (P<0.05). 4. No statistically significant reduction in the absorbed dose was found as cylinder length was changed(P>0.05).

  • Purpose: The objective of this study was to estimate the radiation absorbed doses in certain critical organs in the head and neck region with 16 imaging programs available on the Orthopos/sup (R)/ panoramic machine. Materials and Methods: A Rando phantom and LiF TLD chips were used for dosimetry. The absorbed doses were measured at the thyroid gland, the submandibular gland, the parotid gland, the mouth floor. the maxillary sinus, the brain, the mandibular body, the mandibular ramus. the 2nd cervical spine and the skin over TMJ area. Results : The overall absorbed doses with imaging programs available on the Orthopos/sup (R)/ panoramic machine were much less than that of standard program (program 1) except program 8, 11, and 16. Generally, the absorbed doses to the bone marrow of the mandibular ramus and the parotid gland were high, but the absorbed doses to the bone marrow in the mandibular body, brain, maxillary sinus, and, especially, the thyroid gland were very low. Conclusion : The modified imaging programs available on the Orthopos/sup (R)/ panoramic machine can be effectively used in aspect of radiation protection.

Successful Localization of Intraoral Foreign Body with C-arm Fluoroscopy

  • During surgical procedures, unexpected material, including surgical instruments and tissue segments, may get lost in the surgical field. Most of these should be immediately removed to prevent further complications, such as vital organ irritation, infection, and inflammatory pseudo-tumor formation. However, it is not always easy to define the exact location of the foreign body, especially if the item is very small and/or it is embedded in the soft tissue of the head and neck region. Intraoperative real-time radiological imaging with C-arm fluoroscopy can be useful to trace the three-dimensional location of small and embedded foreign bodies in the oral and maxillofacial area. We describe an unusual case of an embedded micro-screw in the intrinsic tongue muscle that had been dropped into the sublingual space during a lower alveolar bone graft procedure. The lost foreign body was accurately identified with C-arm fluoroscopy and safely removed without any further complications.

  • Gastrointestinal cyst found within the oral cavity has been reported as a rare occurrence with less than 40 cases. The tongue is the preferred site with almost 60% of cysts occurring within oral cavity. The cyst usually shows an asymptomatic swelling in the floor of the mouth, about 30% of affected individuals have symptoms related to difficulties with feeding, swallowing and respiration. The histopathologic features of the cyst vary with respect to the type of epithelium lining; gastric, squamous, intestinal and respiratory epithelium. The pathogenesis is not known, but it may be related to entrapment of undifferentiated endoderm within oral cavity during early fetal development. In this article, we report a case of a gastrointestinal cyst of the tongue in a 16-month-old girl.

Clinical study on mandibular invasion by oral cancer (구강암의 하악골 침윤에 관한 임상적 연구)

  • For the complete cure of oral cancer suspected to have invaded the mandible in clinical & radiological evaluation, the mandible resection in planned. The aim of this clinical study was to help in decision making in the method & the extent of the mandibular resection surgery. This study was conducted on 46 oral cancer patients, who received cancer surgery including mandibulectomy. And we evaluated the relationship between the pathologic results of resected mandible and the location, size and clinical newk node involvement, tumor cell differentiation. The results are that ; (1) Hiher incidence of bone invasion patterns were observed in tumor of mouth floor & gingiva compared to those of tongue & tonsil, and (2) No significant relationship was found between bone invasion of tumor and tumor size, neck node involvement tumor cell differentiation. The approximation between tumor and bone seems to be the most reliable factor among the other factors in decision making of mandibular resection.

  • PAP (Palatal Augmentation Prosthesis) may be given to the patients with dysphagia; especially, who cannot achieve tongue-palate contact. PAP fills hard palate area where the tongue cannot make contact and then the distance of tongue elevation is shortened. 1bat may be expected to improve swallowing and to prevent from aspiration. The purpose of this report is to show the effects of PAP in patients with dysphagia through the videofluoroscopic study. Oral-pharyngeal swallowing post PAP is analyzed in 2 cases; one is a person who had subarachnoid hemorrhage due to aneurysmal rupture, right hemiparesis, hydrocephalus and aphamia. And the other is a person who had squamous cell carcinoma on mouth floor and he had radical neck dissection and marginal mandibulectomy. In this report, the rate of aspiration, the transit time and length measurements of anatomical structure are examined in the each frame of videofluoroscopy. The results are as follows; 1) PAP decreased the aspiration in both cases. 2) In the cases of patients with PAP, the pharyngeal transit time was decreased.

Full mouth implant rehabilitation of a patient with ectodermal dysplasia after orthognathic surgery, sinus and ridge augmentation: a clinical report

  • An 18-year-old male presented severe hypodontia due to hypohidrotic ectodermal dysplasia was treated with Le Fort I maxillary osteotomy with simultaneous sinus floor augmentation using the mixture of cortical autogenous bone graft harvested from iliac crest and organic Bio-Oss to position the maxilla in a right occlusal plane with respect to the mandible, and to construct adequate bone volume at posterior maxilla allowing proper implant placement. Due to the poor bone quality at other sites, ridge augmentation with onlay graft was done to construct adequate bone volume allowing proper implant placement, using tissue harvested from the iliac bone. Seven implants were placed in the maxilla and 7 implants were inserted in the mandible and screw-retained metal ceramic FPDs were fabricated. The two year follow up data showed that dental implants should be considered as a good treatment modality for patients with ectodermal dysplasia.

  • The cervical flap, comprising skin, fascia, and platysma muscle, has significant application in the head and neck region after radical ablative surgery for cancer of the oral cavity. The flap may be used for reconstruction of the cheek, floor of the mouth, and lateral side of the tongue. This flap minimizes donor morbidity by use of cervical operation wound and flap size available is adequate for most oral defects and the procedure is relatively simple and time-saving. However the flap is not applicable in patients where there are large tissue defects and metastasis is suspected. We have used the cervical flap for its rapid, simple, and effective closure of oral defects after cancer ablation and found it is very useful for the reconstruction of relatively small oral defects.

  • The information concerning epithelial dysplasia of the oral cavity has been relatively little published. Of the histolgic grades, most of the cases were classified as moderate and severe category. Their ages ranged from 27 to 78 years(mean 57 years). The developmental incidence by sex was superior in male by ratio of 6 : 1. Epithelial lesions were located in the buccal mucosa, Floor of the mouth, the gingiva and the tongue. It is generally believed with a few exceptions that mild degree of epithelial dysplasia do not indicate any great danger for the patient. But severs dysplasia indicates that there is a very considerable risR of malignant transformation. Surgical excision is the effective method of controlling epithelial dysplasia. The authors investigated 7 cases of epithelial dysplasia histopathologically, 2 patients of them had malignant transformation. We discuss the clinical feature, treatment, prognosis, and histopatholgic analysis with literature reviews.
