광릉 산림의 플럭스 자료 처리와 품질 관리 (Processing and Quality Control of Flux Data at Gwangneung Forest)

  • 보다 자동화된 방법으로 신뢰성 있는 난류 플럭스의 자료를 생산하기 위해서 Hong and Kim(2002)의 난류 품질 관리 프로그램을 개선하고 개선된 프로그램을 광릉산림에 적용하여 복잡한 산림지역에서 난류 플럭스의 특성을 조사하였다. 개선된 프로그램을 이용하여 2005년 1월부터 5월까지 광릉 수목원에 위치한 주 타워의 두 고도(20m와 40m)에서 관측된 난류 자료에 대하여 품질 검사를 실시하였다. 개선전과 비교해 개선된 프로그램은 이상점(outlier)에 해당되는 자료들을 많이 제거하였다. 자료의 품질체계는 4등급(Good, Dubious, Missing, Bad)으로 분류하였으며 본 분석에서 사용된 기간의 자료 중 25%는 결측이었고(Missing 등급), 60%는 Good 등급으로 분류되었다. 고도 별로는 40m에서 관측된 자료가 20m에서 관측된 자료보다 Bad 등급의 자료수가 적었는데 이는 20m가 식생 꼭대기에 인접한 거칠기 아층에 해당하고 또한 풍속도 더 낮은데 기인한다. Bad 등급으로 분류된 자료의 주원인은 낮은 풍속으로 나타났다. 분석 기간 동안의 에너지 수지의 닫힘은 약 40%로 나타났고 이러한 에너지 불균형의 부분적인 이유로는 열 저장항들이 고려되지 않은 점, 토양열 플럭스 측정의 불확실성, 복잡한 지형 등에 의한 국지풍에 의한 이류 등이 복합적으로 작용했을 것으로 생각된다. 광릉에서 발생하는 상향 운동량 플럭스는 국지풍의 발달 시 높은 발생률을 보여 이 둘이 밀접히 관련되어 있음을 나타낸다. 야간에 낮은 음의 $CO_2$ 플럭스가 발생하는 경우에 대하여 평균 시간을 증가시킴에 따른 $CO_2$ flux의 변화를 조사한 결과 평균시간이 10분 이상 증가함에 따라 $CO_2$ flux의 절대값이 빠르게 증가하는 경향을 보였다. 이는 야간에 $CO_2$ 플럭스는 중규모 운동이나 비정상성(nonstationarity) 등의 영향을 많이 받고 있음을 시사한다. 그러므로 야간에 보다 정확한 난류 플럭스 값을 산출하기 위해서는 평균시간의 적절한 조절이 필요할 것으로 보인다.

슬관절 전외측인대의 해부학, 생역학, 수술법 및 임상적 결과 (Anterolateral Ligament of the Knee: Anatomy, Biomechanics, Techniques, and Clinical Outcome)

  • 전방십자인대 재건술은 슬관절 수술 중에서 흔하게 시행되는 수술 중 한가지이나 해부학 및 생역학에 대한 이해가 증가하여 수술 기법의 다양한 변화가 있었음에도 수술 실패는 7%-16% 정도로 보고되고 있어 회전 불안정성에 대한 관심은 지속적으로 강조되고 있다. 최근 들어 전외측인대가 회전 불안정성에 대한 2차적인 지지 구조물로 많은 관심을 받고 연구되고 있다. 하지만 아직까지 전외측인대의 해부학적 형태, 생역학 및 임상 결과에 있어서 논란이 있다. 전외측인대는 대퇴골 외상과 부위에서 경골 근위부의 Gerdy's 결절과 비골 골두 사이에서 슬관절 전외측으로 주행하는 구조물로 알려져 있으며 생역학적 연구에서 경골의 내회전에 따라 전외측인대의 긴장도가 증가하는 슬관절 회전안정성에 기여를 하는 구조물로 알려져 있다. 전외측인대 손상의 진단은 신체검사, 방사선적 검사, 자기공명영상 등의 다양한 검사를 이용하여 종합적으로 판단하며 그중 주로 pivot-shift 검사와 자기공명영상의 결과를 종합하여 진단하게 된다. 최근에는 장경인대의 Kaplan 섬유 등과 같이 전외측인대 복합체로 판단하기도 하며 그 구조물들의 손상여부를 각각 고려하기도 한다. 치료에 있어서 다양한 수술법이 소개되어 사용되고 있고, 연구마다 다른 수술의 적응증을 제시하고 있는 실정으로 최근의 임상적 연구에서 회전 불안정성에 있어 긍정적인 효과를 보고하였지만 앞으로 더 많은 연구가 진행되어야 전외측인대를 강화하는 외측 강화 술식의 유용성에 대한 정확한 평가가 될 수 있을 것으로 판단된다.

수목 도복의 3D 충격량 산출 기법 개발 (Development of 3D Impulse Calculation Technique for Falling Down of Trees)

  • 수목의 파손은 주변 재산과 생명에 치명적인 위험을 초래한다. 특히, 도복으로 인한 위험은 피해범위가 넓고, 충격의 강도가 높기 때문에 선제적으로 대응하는 것이 중요하다. 이에 도복 시 예상되는 잠재적 위험범위를 산정하거나 위험등급을 평가한 시도가 있었지만, 구체적인 물리량으로 위험을 정량화하지 못하였다. 또한, 수목과 피해대상의 형상을 입체적으로 반영하지 못했다는 아쉬움이 있다. 본 연구는 수목의 도복 시 발생할 수 있는 위험범위와 충격량을 정량적, 입체적으로 예측하기 위한 기법의 개발을 목적으로 하였다. 주요 결과는 다음과 같다. 첫째, 수목의 도복 시 발생하는 위험범위와 충격량 산출식을 정립하였다. 위험범위 산출 시 수목이 쓰러져 미끄러지는 가능성을 반영해 수고의 1.5배를 반경으로 하는 원의 내부 범위로 확대하여 계산하였다. 수목의 기울기에 따라 직립으로 생육하는 수목은 근원부를 중심으로 360° 범위, 기울어져 생육하는 수목은 기운 방향에서 180° 범위로 설정하였다. 충격량은 충돌이 발생하기 직전 수목의 나중운동량이 피해대상에 전달되는 현상을 이용해 산출하였다. 또한, 수목이 쓰러질 때 근원부를 기점으로 회전운동 하는 것을 반영해 각운동량을 산출하였으며, 이를 선운동량으로 전환하여 충격량을 계산하였다. 둘째, Rhino3D와 Grasshopper를 이용하여 위험범위, 충격량 산출 알고리즘을 개발하였다. 알고리즘은 3차원 모델 제작, 산출, 조회 기능으로 구성하였다. 3차원 모델은 Rhino3D를 이용하여 지형, 건물, 수목의 형상을 입체적 모델로 제작하였고, 이를 Grasshopper에 연결하여 공간정보를 구축하였다. 산출 기능에서는 산출식을 활용하여 알고리즘을 코딩하였다. 산출 시 수고, 기울기, 중량 등 수목의 생육 정보와 인접수목, 피해대상, 분석범위 등 주변 환경을 고려하였다. 조회 기능에서는 산출 결과를 종합하여 3차원 모델에 가시화하였다. 산출값에 따라 색상으로 구분하여 위험수목과 위험구역을 효율적으로 판단하였다. 본 연구는 수목의 도복 시 발생하는 잠재적 위험범위와 충격량을 정량적으로 산출하고, 이를 가시화하여 우선관리가 필요한 대상을 효율적으로 판단하는 방법을 제시하였다. 이는 도복 발생 시, 주변 건물과 관람객의 안전을 위한 대책 수립 및 재난 예방의 기초자료로 활용될 수 있을 것이다. 또한, 도심지와 공원, 문화재구역에서 명확한 기준 없이 진행되는 수목의 제거를 방지하는데 기여할 것이다.

Spine SBRT 치료시 Coaxial MLC VMAT plan의 유용성 평가 (Evaluating efficiency of Coaxial MLC VMAT plan for spine SBRT)

  • 목 적 : Spine SBRT 치료 시 보편적인 치료방법인 콜리메이터 각도 $30^{\circ}$$330^{\circ}$를 각각 사용한 2회전 치료계획 (이하 Universal MLC VMAT)과 MLC 운동 방향과 척수 또는 마미 (이하 OAR)의 장축을 일치시킨 콜리메이터 각도 $273^{\circ}$$350^{\circ}$를 사용한 2회전 치료계획 (이하 Coaxial MLC VMAT) 유용성을 비교, 평가하고자 한다. 대상 및 방법 : 본원에서 Varian TBX을 이용하여, Coaxial MLC VMAT 치료 계획으로 치료 받은 spine SBRT환자 10명을 대상으로 하였다. 전산화치료계획은 Eclipse (ver 10.0.42, Varian, USA), PRO3 (Progressive Resolution Optimizer 10.0.28), AAA (Anisotropic Analytic Algorithm Ver 10.0.28) 알고리즘을 이용하였다. 치료계획은 VMAT로 겐트리 회전반경이 각각 $360^{\circ}$인 두 개의 ARC, 10MV FFF (Flattening Filter Free)를 이용하여 수립하였고, 각 ARC는 콜리메이터 각도 $273^{\circ}$, $350^{\circ}$로 설정하였다. 기존 치료계획을 바탕으로 실험군인 Universal MLC VMAT 치료계획을 수립하였다. 콜리메이터 각도를 제외한 모든 조건은 동일하게 설정하였으며, 특히 최적화 (VMAT optimization) 과정에서 무작위하게 나타나는 선량차이를 최소화하기 위해 각각 2회의 최적화, 선량 계산 과정을 거쳤다. 계산 grid 는 0.2 cm, normalization은 타겟 $V_{100%}=90%$로 설정하였다. OAR의 선량 $V_{10Gy}$, $D_{0.03cc}$, Dmean, 타겟의 H.I (Homogeneity index) 그리고 각 치료 계획의 Total MU를 평가 지표로 설정하였고, Mapcheck2 (Sun Nuclear Co., USA) 와 Mapphan (Sun Nuclear Co., USA) 그리고 SNC patient (Sun Nuclear Co., USA Ver 6.1.2.18513) 를 이용하여 Coaxial MLC VMAT 계획의 임상 적용 가능 여부 확인을 위한 IMRT verification QA (gamma test)를 실시하였다. 결 과 : 두 치료계획을 비교한 결과 OAR의 $V_{10Gy}$차이는 최대 4.1%, 최소 0.4%, 평균 1.9%로, $D_{0.03cc}$ 는 최대 83.5 cGy, 최소 2.2 cGy, 평균 33.3 cGy로 Coaxial MLC VMAT plan 이 더 낮은 것으로 나타났다. Dmean 또한 최대 34.8 cGy, 최소 -13.0 cGy, 평균 9.6 cGy로 Coaxial MLC VMAT plan 이 낮은 것으로 나타났다. H.I. 는 최대 0.04, 최소 0.01로 Coaxial MLC VMAT plan 이 평균 0.02 낮은 것으로 나타났으며, Total MU의 평균값을 비교한 결과 Coaxial MLC VMAT plan 이 평균 74.1 MU 더 낮게 나타났다. Coaxial MLC VMAT plan에 대한 IMRT verification gamma test 결과는 1 mm / 2%, pass rate 90.0% 기준을 모두 통과하였다. 결 론 : Coaxial MLC VMAT 치료계획은 Universal MLC VMAT 치료계획에 비해 대부분의 평가지표에서 유리한 것으로 나타냈으며 특히 OAR의 선량 $V_{10Gy}$을 낮추는데 있어 탁월한 것으로 사료된다. 실험결과를 바탕으로 두 치료 계획을 비교해 볼 때, 같은 MU를 사용한다면 Coaxial MLC VMAT 치료계획이 Universal MLC VMAT 치료계획에 비해 효율적이라 사료된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The frame and leaflet of the polymer valve were made from polyurethane, To evaluate the hemodynamic performance of the polymer valve a comparative study of flow dynamics past a polymer valve and a St. Jude Medical prosthetic valve under physiological pulsatile flow conditions in vitro was made. Comparisons between the valves were made on the transvalvular pressure drop, regurgitation volume and maximum valve opening area. The polymer valve showed smaller regurgitation volume and transvalvular pressure drop compared to the mechanical valve at higher heart rate.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N.
