Osteoporosis is the most common form of metabolic bone disease and is an important cause of morbidity and mortality in the elderly and especially in postmenopausal women. The possible beneficial action of essential fatty acids and their metabolites both in the prevention and treatment of osteoporosis needs attention. Therefore in this study, fatty acid levels of the normal groups was compared with those of patients. And then star symbol plots were drawn based on the fatty acids values of patients normalized to the corresponding normal group values to transform into visual patterns discriminating from the normal pattern. Between April 1986 and September 1990, 34 patients with a single or dominant right ventricle underwent modified Fontan procedure for definite palliation in Seoul National University Children`s Hospital. Their age at operation ranged from 8 months to 14 years [Mean 5.5 years]. The ventricular chamber was solitary and of indeterminate trabecular pattern in 6 patients. 28 patients had posteriorly located rudimentary chamber, all of which were trabecular pouches having no communication with outlet septum. The patterns of atrioventricular connection were common inlet[9], double inlet [11], left atrioventricular valve atresia [12] and right atrioventricular valve atresia with L-loop [2]. Pulmonary outflow tracts were atretic in 7 patients and stenotic in 26 patients. Major associated anomalies included anomalous systemic venous drainage [15], dextrocardia [12] and total anomalous pulmonary venous connection[3]. Shunt operations were previously performed in 13 patients and pulmonary artery banding and atrial septectomy in 1 patients. Surgery included intraatrial baffling in 26 patients, bidirectional cavopulmonary shunt in 13 patients, atrioventricular valve obliteration in 3 patients and atrioventricular valve replacement in 3 patients. Central venous pressure measured postoperatively at intensive care unit ranged from 18cm H2O to 28cm H2O [mean 23.2cm H2O]. Hospital mortality was 35.3% [12/34], all died out of low output syndrome. Suspected causes of low output syndrome include ventricular dysfunction [8], hypoplastic or tortuous pulmonary artery [2] and elevated pulmonary vascular resistance [2]. 19 patients had 31 major complications including low output syndrome [18], arrhythmia [4], acute renal failure [3] and respiratory failure [3]. Mortality rate was significantly higher in the groups receiving intraatrial baffling and AV valve replacement respectively [p<0.05]. 20 patients were followed up postoperatively with the mean follow-up period 15.0$\pm$11.6 months. There were no late death and follow-up catheterization was performed in 10 patients. Mean right atrial pressure was 15.4$\pm$6.8mmHg and ventricular contraction was reasonable in all but one case. Thus, Fontan principle can be applied successfully to all the patients with complex cardiac anomaly of single ventricle variety and better results can be anticipated with judicious selection of patient and improvement of postoperative care.
We analyzed crown development in Aucuba japonica Thunb. var. japonica resulting from the responses of phytoelements to habitat light conditions over a long period of time. Over the years, the degree of extension unit (EU) dimorphism and the degree of anisophylly were higher under shaded conditions than in brighter conditions. An overall temporally increasing pattern in the degree of EU dimorphism was found while no clear-cut trend was found in the case of anisophylly. EU length and number of leaves per EU co-varied in a spatio-temporal context. The number of terminal buds and their sizes acted as the key initiators of morphological differences of phytoelements which were further amplified following bud break. Leaf area density was displayed mostly in the apex peripheral layer of the crown and the apex layer received most of the incident light. There was a tradeoff between annual leaf production and mean leaf size. Depending on the heterogeneity of irradiance level within a crown, correlative growth inhibition caused higher EU mortality at brighter sites. Due to high mortality, shorter EUs had a mere role in the construction of structural framework of the crown except for the formation of some gaps. There was a strong convergence of EU dimorphism, anisophylly, EU extension growth and variations in leaf size towards formation of functional crown to reduce potential self-shading. Depending on the irradiance level, Aucuba japonica Thunb. var. japonica showed two different modes of crown expansion. At the brighter sites, individual crown expansion was progressive while at the darker sites, individual crown expanded in a diminishing manner and maintained a stable size. A plant's "growth diminishing phase" appeared earlier at shaded sites than brighter sites.
On an international scale, the burden of cancer in absolute numbers continues to increase, mainly due to aging of population in many countries, the overall growth of the world population, changing lifestyle with increasing cancer-causing behavior, like cigarette smoking, changing dietary habits and sedentary life. Cancer is the second-leading cause of death and disability in the world, after only heart disease. Recently, increasing incidence and mortality of cancer have also become evident in the developing world. In Iraq and particularly in Basrah in the southern part of the country, the burden has definitely increased and deserves extensive research. The present paper is part of an extensive household survey carried out in Basrah in 2013. Among the objectives was to validate official cancer registration in the governorate. The cross-sectional survey had a retrospective component to inquire about the incidence of cancer and cancer-related deaths during the three years preceding the date of inquiry (2010-2012). A convenient sample of 6,999 households with 40,688 inhabitants using multistage cluster sampling was surveyed involving all urban and rural areas of Basrah. The official cancer registration activities in Basrah seemed to have attained a high level of registration coverage (70-80%) but the gap, represented by missed cases, is still high enough to criticize the system. Most of the missing cases were either not notified by treating facilities or they were diagnosed and treated outside Basrah. Using a set of parameters, the pattern of cancer was consistent based on data of the household survey and data of the cancer registry but a gap still existed in the coverage of incident cancer and mortality by cancer registration. Integrated serious steps are required to contain the risk of cancer and its burden on the patient through improving the registration process, improving early detection, diagnostic and management capabilities and encouraging scientific research to explore the hidden risk factors and possible causes of low registration coverage. Periodic household surveys seemed feasible and essential to support routine registration.
To determine the pathogenicity of Acanthamoeba spp. isolated in Korea and to develop a isoenzymatic maker, the mortality rate of infected mice, in vitro cytotoxicity against target cells and isoenzyme band pattern were observed. Five isolates of Acanthamoeba spp. (YM-2, YM-3, YM-4, YM-5 and YM-7) were used in this study as well as three reference Acanthamoeba spp. (A. culbertsoni, A. hatchetti, and A. royreba). According to the mortality rate of infected mice, Korean isolated could be categorized into three groups: high virulent (YM-4), low virulent (YM-2, YM-5, YM-7) and the nonpathogenic group (YM-4), In addition, the virulence of Acanthamoeba spp. was enhanced by brain passage in mice. In the cytotoxicity assay against chinese hamster ovary cells, especially, the cytotoxicity of brain-passaged amoebae was relatively higher than the long-term cultivated ones. The zymodeme patterns of glucose-6-phosphate dehydrogenase (G6PD), malate dehydrogenase (MDH), hexokinase (HK), glutamate oxaloacetate transaminase (GOT) and malic enzyme (ME)of Acanthamoeba spp. were different among each isolate, and also between long-term cultrued amoebae and brain passaged ones. In spites of the polymorphic zymodemes, a slow band of G6PD and K, and an intermediate band of MDH were only observed in pathogenic Acanthamoeba spp., which should be used as isoenzymatic makers.
Purpose: The purpose of this study was to determine the essential factors for prompt arrangement of angio-embolization in patients with pelvic ring fractures. Methods: A total of 62 patients with pelvic ring fractures who underwent angio-embolization in Dankook University Hospital from March 2013 to June 2018 were retrospectively reviewed. There were 38 men and 24 women with a mean age of 59.8 years. The types of pelvic ring fractures were categorized according to the Tile classification. Patient variables included sex, initial hemoglobin concentration, initial systolic blood pressure, transfused packed red blood cells within 24 hours, Injury Severity Score (ISS), mortality rate, length of hospital stay, and time to angio-embolization. Results: The most common pelvic fracture pattern was Tile type B (n=34, 54.8%). The mean ISS was $27.3{\pm}10.9$ with 50% having an $ISS{\geq}25$. The mean time to angio-embolization from arrival was $173.6{\pm}89minutes$. Type B ($180.1{\pm}72.3minutes$) and type C fractures ($174.7{\pm}91.3minutes$) required more time to angio-embolization than type A fractures ($156.6{\pm}123minutes$). True arterial bleeding was identified in types A (35.7%), B (64.7%), and C (71.4%). Conclusions: It is important to save time to reach the angio-embolization room in treating patients with pelvic bone fractures. Trauma surgeons need to consider prompt arrangement of angio-embolization when encountering Tile type B or C pelvic fractures due to the high risk of true arterial bleeding.
New formulation of diazinon as granule preparing with piperonyl butoxide(PBO) and triphenyl phosphate(TPP) known as the inhibitor of monooxygenase(mo) and esterase was tested with the mortality of rice leaf hopper. Also, the mixed formulation of diazinon with tricyclazole, carbofuran and EPN were investigated about the residual pattern of diazinon in submerged soil condition at $30{\pm}2^{\circ}C$. Although the mortality about rice leaf hopper was higher only 10% on new formulation of diazinon than commercial at recommended dose but was 30-60% at half dose. Half life of diazinon in the formulation with 0.1% of PBO+TPP was 4.53 days in sterilized soil and 2.33 days in non-sterilized soil, and it was delayed about 0.74 and 0.45 respectively in contrast with the commercial granule. Also, half life of the new formulation with 1% PBO and PBO+TPP was delayed 0.44(PBO) and 0.65day(PBO+TPP) in non-sterilized soil condition, respectively. Half life of mixed formulation with tricyclazole, carbofuran, EPN or EPN+Carbofuran was delayed 2.61, 1.04, 0.43 or 2.7 days, respectively. As a result, the persistence rate of new formulation and mixed formulation was increased by inhibition of two enzymes affecting to the dgradation of diazinon in submerged soil.
Background: This study was undertaken to evaluate the clinical pattern of the patients with penetrating torso injury. We retrospectively analyzed the clinical symptoms, mechanism of injury, injury type including injured organ, and ultimate outcome of treatment. Our purpose of the study was to establish guideline of management in penetrating torso injury. Methods: This study consists of an analysis of a consecutive series of 94 patients with penetrating injury of trunk treated at one general hospital during 7year period (from January 1995 to April 2003) who was admitted through in our emergency department. All data were collected from the medical records and entered in a database for analysis on the following: age, sex, mechanism of injury, vital sign at admission, clinical outcome including hospital mortality, length of hospital stay, length of intensive care unit stay, requirement of crystalloid fluid and blood product. Results: Among 94 patients, there were 68 men and 26 women, with ages ranging from 19 to 82 years (average 38.2 years). The most frequent mechanism of injury was violence by others including rob (n=54, 57.4%) followed by suicidal attempt (n=24, 25.5%) and accidental injury (n=16, 17.0%). No injury was inflicted from gun. In 37 patients, systolic blood pressure at admission was under 90mmHg. The time interval from injury to admission, and from admission to operation was 57.8minutes and 4hour 12minutes each. Laparotomy was required in 70 patients, thoracotomy in 5 patients, and 3 patients required thoracotomy and laparotomy. Among 94 patients, an average of 1.7 organs were injured. The small bowel and colon were the organs most commonly wounded followed by liver, mesentery, pleura. Of the 94 patients, 6 died for an overall mortality rate of 6.4%, and two of them were not related with hemorrhage. The average length of hospital stay was 18.1 days, and 40 patients required ICU care. Conclusion: Of the 94 patients who were admitted from penetrating torso injury, no patient was injured from firearm. Overall mortality rate was 6.4%. In our hospital, firearm injury was relative rare.
The purpose of this study was to control root-knot nematode, Meloidogyne incognita on pumpkin seedling with 10%, 20% coffee ground compost and 1% plant velvet bean powder (plain skin bean, leopard skin bean, hole plant, bean pod). Among the root-knot nematode insecticide treatments, the velvet bean plant powder was the highest mortality rate with 83.2% in greenhouse condition. In addition, the root-knot nematode second juvenile mortality significantly increased with the lapse of processing time for each concentration of leopard skin velvet bean extract in vivo. After 24 hours, the mortality rate of root-knot nematode showed the highest at 1.2% and 2.4% of leopard skin velvet bean extract. The growth promoting effect of seedlings pumpkin, 20% of coffee ground compost treatments inhibited the growth of pumpkin seedlings and 1% the tiger pattern velvet beans powder was the most to promote the growth of pumpkin seedlings. However, the results indicated that the organic materials of velvet beans and coffee ground compost are expected to be able to effectively control the root-knot nematode and further studies will be needed for the concentration and application methods.

Seasonal dynamics of kelp forest-forming algae, Ecklonia cava population formed following the large-scale disturbance by Typhoon 'Bolaven' in August 2012 were investigated in Jeju Island, Korea. Morphological characteristics, recruits density, mortality rate, total density and biomass were monitored bimonthly from June 2013 to June 2015. Total and longest blade lengths, and individual weight of E. cava showed distinct seasonal trends. Stipe length increased from winter to spring, but did not show increase or reduced from summer to autumn. This indicates that morphological characteristics of E. cava are mainly affected by the change of blades. The optimal temperature for E. cava growth was about $15-18^{\circ}C$ during winter to spring while the growths were inhibited at the water temperature above $20^{\circ}C$ during summer. E. cava exhibited very low recruitment during spring-summer. However, high recruitment was observed on April 2015 when canopy cover was very low due to low density. This indicates that recruitment of E. cava was controlled not by seasonal effects but by physical factors such as canopy and space. The mortality rate of juvenile plants was highest due to their unstable settlement. By June 2015, 34 months after the disturbances, E. cava was almost recovered to the pre-disturbance population size structure. These results suggest that recovery of kelp forest following the large-scale disturbance requires a considerable period of time (more than three years). This study should provide valuable ecological information on management, restoration and protection of kelp species.
