Monitoring of Mycophenolic Acid Trough Concentration in Kidney Transplant under Cyclosporine Is Beneficial in Reducing Acute Rejection within 1 Year

  • Background: This study was designed to analyze the clinical usefulness of mycophenolic acid trough concentration monitoring in kidney transplantation patients who were maintained with cyclosporine. Methods: The data of patients who underwent mycophenolic acid trough concentration monitoring after their first kidney transplant between November 2006 and August 2013 and were prescribed with cyclosporine, mycophenolate, and methylprednisolone were reviewed retrospectively. Cox analysis was used to analyze the risk factors for acute rejection within 1 year post-transplantation. Results: Among 90 patients, 41 (45.6%) achieved both the target levels of cyclosporine and mycophenolic acid, while three patients (3.3%) failed to achieve the target level of either cyclosporine or mycophenolic acid. Nine patients (10.0%) only achieved the mycophenolic acid target level and 37 patients (41.1%) only achieved the cyclosporine target level. While patients who achieved only the mycophenolic acid target concentration had no statistically increased risk compared to patients who achieved both target levels (hazard ratio [HR], 1.569; 95% confidence interval [CI], 0.316 to 7.778; P=0.581), patients who only achieved the cyclosporine target concentration showed an increased risk of rejection compared to the both achievement group (HR, 4.112; 95% CI, 1.583 to 10.683; P=0.004). Patients who had no achievement in the target levels showed significantly increased rejection risk compared to the patients who achieved both target levels (HR, 17.811; 95% CI, 3.072 to 103.28; P=0.001). Conclusions: Mycophenolic acid trough concentration monitoring combined with cyclosporine trough concentration monitoring is useful for avoiding acute cellular rejection if the first 1 year post-transplantation.

  • Purpose: This study aimed to analyse the effect of End-Tidal Carbon Dioxide (ETCO2) monitoring on patients who had been poisoned and presented in the emergency department with decreased consciousness. Methods: The data of patients over 18 years old presenting with poisoning from 2016 to 2020 was collected from the emergency department. We retrospectively analyzed their medical records, and defined patients with a Glasgow Coma Scale (GCS) score of 9 or less as having decreased consciousness. We divided the patients into two groups, one with ETCO2 monitoring and the other without, and the difference between the two groups was compared. Results: 168 patients participated in this study and 83 (49.4%) of them belonged to the ETCO2 monitoring group. In this group, the interval between arterial blood tests was statistically significantly longer and the rate of intubation was lower. In addition, in the monitoring group, the incidence of pneumonia and the rate of poor prognosis was not significantly higher. Conclusion: Although ETCO2 monitoring does not directly affect the prognosis of poisoned patients with decreased consciousness, it should be actively done as it can help to adequately treat patients while avoiding invasive techniques or unnecessary intubation.

Effect of Sampling Frequency for the Storm Runoff on BOD, T-P Loads Estimation of the Mixed Landuse Watershed (강우-유출 채수간격이 복합지목 유역의 BOD, T-P 부하량 산정에 미치는 영향)

  • In order to quantify nonpoint source pollution, it was proposed to sample at regular intervals of 1 hour for the first 24 hours of storm runoff process by National Institute of Environmental Research for the mixed landuse watershed. However, high frequency sampling requires intensive laboratory analysis and labor costs. In order to investigate the effect of longer sampling interval on the load estimation compared to the 1 hour sampling method, analysis was conducted using monitoring data from rural subwatershed, urban subwatershed, and outlet of the Pungyeongjeongcheon watershed. Statistical analysis revealed that mean of load estimation was not significantly different up to 4 hour sampling frequency. However, 3 hour sampling interval was found to be appropriate for the BOD and TP when it is judged that 10% or less of the difference in loading amount between the 1 hour and other sampling interval is reasonable. The results of this study can be used to conduct an effective monitoring system.

  • We have analyzed the performance of 58 kWp photovoltaic (PV) power systems installed in Jeddah, Saudi Arabia. Performance ratio (PR) of 3 PV systems with 3 desert-type PV modules using monitoring data for 1 year showed 85.5% on average. Annual degradation rate of 5 individual modules achieved 0.26%, the regression model using monitoring data for the specified interval of one year showed 0.22%. Root mean square error (RMSE) of 6 big data analysis models for power output prediction in May 2016 was analyzed 2.94% using a support vector regression model.

  • In this paper, an Internet-based monitoring system is studied which analyzes visual image of drifted snow and measures the amount of snowing. The system is composed with reference ruler where LED's are attached, a camera, and PC with a controller. The length of the ruler is 1.5m and the interval of the LED's is 1cm. The controller synchronizes the on-off control of each LED with the image frame of the camera. When the snow is accumulated along the ruler, the corresponding LED is blocked and the lowest position of the lighted LED is estimated with this captured image of the camera. Because of week intensity of LED light and light rays of outdoors, it is hard to discriminate which LED is blocked due to snow.   • The purpose of this study was to analysis water quality monitoring data and to estimate future trends using ARIMA model of time series analysis. Water quality data in Chin yang reservoir were used with monthly monitoring interval during past 7 years. The variations of water quality parameters with periodicity and trend could be estimated by multiplicative ARIMA models and the statistical tests showed a good agreement with the observed data. Therefore, the monthly values of water quality parameters could be forecasted using these models.

  • This paper presents an intelligent electronic device(IED) platform which is based on a new frequency estimation algorithm. The IED platform features not only the accurate estimation of frequency but also the fast network-based transmisstion of fault data and waveforms. The simulation results show that the proposed frequency estimation algorithm improves convergence speed, and has lower frequency estimation error compared with those of conventional algorithms. The network-based IED is designed to ensure transmission latency being less than 1.0% of sampling interval.

An Evaluation of Error Performance Estimation Schemes for DS1 Transmission Systems Carrying Live Traffic

  • DS1 transmission systems use framing bit errors, bipolar violations and code-detected errors to estimate the bit error rate when determining errored and severely errored seconds. Using the coefficient of variation under the memoryless binary symmetric channel assumption, a basic framework to evaluate these estimation schemes is proposed to provide a practical guideline in determining errored and severely errored seconds which are fundamental in monitoring the real-ime error performance of DS1 transmission systems carrying live traffic. To evaluate the performance of the cyclic redundancy check code (CRC), a computer simulation model is used. Several drawbacks of the superframe format in association with real time error performance monitoring are discussed. A few recommendations are suggested in measuring errored and severely errored seconds, and determining service limit alarms through the use of the superframe format. Furthermore, we propose a new robust scheme for determining service limit alarms which take into consideration the limitations of some estimation schemes for the time interval of one second.

  • 그리드 환경은 수많은 컴퓨팅 자원을 활용하는 시스템으로 자원에 대한 정보가 필요하다. 자원 정보 검색을 위해서는 모니터링이 전체 자원에서 일어나야 하므로 이에 따른 오버헤드가 예상된다. 본 논문에서는 모니터링의 오버헤드를 줄이면서도 효율적으로 정확하게 자원 정보를 수집 할 수 있는 모니터링 기술을 구현하였다. 여기서 제안된 모니터링 기술은 시스템 성능에 따라 자동적으로 모니터링 주기를 조절하게 된다. 이를 통해 모니터링에 의한 오버헤드가 줄었음을 측정 결과를 통해 보일 것이다.
