Comparison of EEG Changes Induced by Action Execution and Action Observation

  • Purpose: Recent electrophysiological studies have shown that the sensorymotor cortex is activated during both actual action excuted by themselves and observation of action performed by other persons. Observation of action based on mirror neuron system can be used as a cognitive intervention to promote motor learning. The purpose of this study was to investigate the brain activity changes during action observation and action execution using EEG. Methods: Thirty healthy volunteers participated and were requested to perform hand action and to observe the video of hand action performed by another person. The EEG activity was evaluated by a method which segregated the time-locked for each condition. To compare the differences between action observation and execution, the Mu suppression and the relative band power were analysed. Results: The results showed significant mu suppression during the action observation and execution, but the differences between the two conditions were not observed. The relative band power showed a significant difference during the action observation and execution, but there were no differences between the two conditions. Conclusion: These results indicate that action execution and observation involve overlapping neural networks in the sensorymotor cortical areas, proposing positive changes on neurophysiology. We are expected to provide information related to the intervention of cognitive rehabilitation.

암환자 700예의 진단 및 치료 CT 이용율과 CT 확대장치 (The Availability of Diagnostic and Treatment Planning Computer in 700 Cancer Patients and Magnification Devices for CT)

  • It has been evident since 1972 that computed tomography(CT) can play an important role in treatment and managment of the cancer patients as four steps; diagnosis, satging Treatment and follow-up. In this paper, we intended to investigate the availability of CT scan and treatment planning computer in 700 cancer patients who have undergone radiation therapy at the division of radiation therapy, Kangnam St. Mary's Hospital, Catholic Medical College between Mar. 1983 and Dec. 1985. The result were as follow; 1. Of 700 irradiated cancer patients, 342 patients ($48.9\%$) were performed CT scan prior to radiation therapy. 2. The distribution of lesions in 342 patients having CT scans was like this; CNS (83 of 104 patients, $79.8\%$), abdomen (44 of 76 patients, $57.9\%$), pelvis (100 of 188 patients, $53.2\%$) etc. in order. 3. The treatment planning computer were used in 280 cancer patients ($40\%$). 4. Of the 280 cancer patients using treatment planning computer, 167 patients ($59.6\%$) applied diagnostic CT scan and remaining 113 patients ($40.4\%$) were made body contour to be used for radiation therapy planning by the treatment planning computer. Authors also made some magnification devices used for small multiformat CT images to magnify into life size, consisting of overhead projector (3M) I.V. stand and mirror. These enabled us to make less errors in tracing the small-sized CT images during input of the anatomical data into the treatment planning computer.

동작관찰훈련이 뇌졸중 환자의 손 조작능력에 미치는 영향 (The Effect of Action Observational Physical Training on Manual Dexterity in Stroke Patients)

  • The aim of this study was to determine the effect of action observational physical training (AOPT) on manual dexterity and corticomotor facilitation in stroke patients. Ten hemiparetic patients participated in this study. Each subject was asked to participate the three conditions; base condition (Base), physical training (PT), AOPT. Participants were asked to observe the action in the video that a therapist moved the blocks during the AOPT conditions. Corticomotor facilitation was determined in three conditions by monitoring changes in the amplitude of motor-evoked potentials (MEPs) elicited in hand muscles by transcranial magnetic stimulation. MEP responses were measured from the first dorsal interosseous after participants attended to three conditions. For the manual dexterity, Box and Block test (BBT) was used. The results of present study were summarized as follows: MEPs amplitude significantly tended to be larger than PT and Base condition. The scores of BBT in the AOPT condition were also significantly larger than other conditions. In conclusion, this finding of present study indicates that physical training for observation of an action is beneficial for enhancing a dexterity of paretic arm in stroke patients.

The Comparison of 'Knowledge of Result' and 'Knowledge of Performance' in the Children with Cerebral Palsy

  • Purpose: The purpose of this study was to compare the effect of 'knowledge of result' and 'knowledge of performance', two types of extrinsic feedbacks, during the sit-to-stand movement in children with hemiplegic cerebral palsy. Methods: A total of ten children with hemiplegic cerebral palsy (ages 8 to 12 years) were recruited for the study. Subjects with hemiplegic cerebral palsy performed sit-to-stand movement in front of a mirror. Their performance was supervised and revised for normal movement by a pediatric physiotherapist. In the knowledge of the result, subjects performed sit-to-stand using a chair with an armrest in their mind with normal movement. In the knowledge of performance, subjects performed sit-to-stand under verbal instructions. Randomized cross over trials were used in this study. Main outcome measurements were as follows: mediolateral speed, anteroposterior speed, velocity moment, extent in mediolateral direction, extent in anteroposterior direction, and vertical distance of the center of pressure. Results: The mediolateral speed and extent of center of pressure was higher for 'knowledge of performance' in comparison with the other type of extrinsic feedbacks (p<0.05). The other parameters, including anteroposterior speed and extent, and vertical speed of the center of pressure, did not differ between the two types of extrinsic feedbacks (p>0.05). Conclusion: These findings suggested that training in sit-to-stand movement with 'knowledge of result' may result in better use of extrinsic feedback.

아급성기 뇌졸중 환자의 팔 기능을 위한 작업치료프로그램에 대한 체계적 고찰 (Systematic Review of Occupational Therapy Programs for Upper Extremity Functions in Subacute Stroke Patients)

  • Purpose : This study systematically reviewed occupational therapy programs for upper extremity functions in subacute patients implemented over the last decade at home and abroad to utilize the findings as the basis for intervention protocols. Methods : This study was conducted with domestic and foreign randomized controlled experimental studies published from January 2013 to May 2023. Acute or subacute, CVA or stroke, upper extremity function, and program or therapy were used as search keywords in the search databases Scopus, PubMed, Riss, and DBpia. A total of 3,723 documents were retrieved, and 1,007 duplicate papers were excluded. In addition, 2,640 papers that did not meet the inclusion and exclusion criteria were excluded by the researcher examining the titles and abstracts. Thereafter, the full texts were checked. Consequently, 67 documents were excluded, and nine documents were finally used for analysis. Results : Due to the evaluation of the quality of the documents of nine studies, five items were rated "low," and two items were rated "unclear" in many studies. Males predominated the genders, and infarction predominated the types. The average age was over 50 but under 60 years, and the number of days after onset was predominantly fewer than one month. Occupational therapy programs were classified into mental programs, mirror therapy, music programs, and virtual reality programs, and virtual reality programs were frequently used. The intervention periods were predominantly 15-30 sessions for less than 30 minutes, and the Fugl-Meyer assessment-upper extremity was predominantly applied as an evaluation tool. Conclusion : This study is significant because it attempted to present the direction of intervention protocols for upper extremity functions in subacute stroke patients in clinical settings. Studies should be conducted with an expanded scope for a literature review.

유비쿼터스 환경에서의 거울신경세포시스템에 근간한 동작관찰-신체훈련 (뇌졸중 환자의 상지기능과 일상생활활동에 미치는 영향) (The effect of action-observational physical training based on mirror neuron system on upper extremity function and activities of daily living in stroke patient)

  • 본 연구는 유비쿼터스 환경에서 동작관찰-신체훈련이 뇌졸중 환자의 상지기능과 일상생활활동에 어떠한 영향을 미치는 지 알아보고자 실시하였다. 연구 대상자는 19명이며 실험군 10명, 대조군 9명으로 나누었다. 실험군은 일반 작업치료와 동작관찰-신체훈련을 병행하였고, 대조군은 일반 작업치료와 동작관찰만을 시행하였다. 훈련 과정은 4주였으며, 주 3회, 회당 30분씩 훈련에 참여하였다. 대상자 선별 후 WMFT와 MBI 평가를 시행하였으며 전-후 평가를 실시하여 증진 정도를 보고자 하였다. 그 결과 대상자들의 상지기능과 일상생활활동은 두 그룹 모두중재 후 유의한 증진을 보였다(p<.05). 하지만 각 측정 시점간의 두 군의 차이는 유의한 차이를 보이지 않았다.

Motor imagery on upper extremity function for persons with stroke: a systematic review and meta-analysis

  • Objective: The purpose of this review was to investigate whether motor imagery training has an effect on the recovery of upper extremity function in individuals with hemiparetic stroke or not. Design: A systematic review and meta-analysis. Methods: PubMed and three other databases were searched up to December 18th, 2018 and randomized controlled trials (RCTs) evaluating motor imagery training on upper extremity function in persons with a diagnosis of hemiparetic stroke were included. This review selected the following information from each study: surname of the first author, published year, nation, population, intervention, therapeutic intensity of intervention, therapeutic comparison, outcome measures, additional therapy, summary of results, and descriptive statistics of outcome measures. Results: This review selected seventeen RCTs with 487 stroke survivors and the following intervention methods: six motor imagery training with additional therapeutic technology, two motor imagery training with additional modified constraint-induced therapy, four mirror therapy, and five motor imagery training. Ten RCTs were eligible for meta-analysis after systematic review. The motor imagery group were more effective than the control group based on the Fugl-Meyer assessment (3.43; 95% confidence interval [CI], 1.65 to 5.22; heterogeneity [$chi^2=8.03$, df=8, $I^2=0%$], test of overall effect Z=3.76; test for subgroup differences [$chi^2=2.56$, df=2, $I^2=21.8%$]) and the Action Research Arm Test (1.32; 95% CI, -8.12 to 10.76; heterogeneity [$Tau^2=70.74$, $chi^2=15.22$, df=3, $I^2=80%$], test of overall effect Z=3.76). Conclusions: The results of this review suggests that motor imagery shows positive effectiveness on improving upper extremity function in persons with hemiparetic stroke.

Comparison of Postural Control Ability according to the Various Video Contents during Action Observations

  • Purpose: This study examined the effects of the type of video contents used for action observations on the ability to control posture. Methods: The participants were 48 healthy adults. The two hands of the participants were crossed on both shoulders, and the other foot was placed in a straight line in front of the target to allow them to watch a video of the monitor. The videos were presented in random order with three video contents (natural, stable balance posture, and unstable balance posture) consisting of 30 seconds each. A 15-second resting time was given between each video. During action observation using various video content forms, the posture control ability was measured using a TekScan MetScan® system. Results: The results revealed statistically significant differences in the area of movement and the distance by COP and distance by the type of action-observation videos, and the distance by the anteroposterior and mediolateral sides (p<0.05). The stable balance posture and unstable balance posture video showed significant differences in the distance by the COP, anteroposterior, and mediolateral distance. (p<0.05) Conclusion: This study suggests that choosing the contents of the videos is important during action-observation training, and action-observation training can help improve postural control.

전자포탈영상장치의 제작과 방사선치료장치의 QA 적용에 관한 연구 (A Study on QA for Radiation Therapy Machine by Using Implemented Electronic Portal Imaging Device)

  • 고 에너지 방사선을 이용하여 종양을 치료하는 과정 중 발생되는 오차를 확인하여 보다 정교한 치료를 수행함으로써 방사선치료 효율을 향상시킬 수 있다. 본 논문에서는 지금까지 주로 사용되어진 필름을 이용한 아날로그 방식대신 실 시간적으로 영상을 얻을 수 있는 디지털 방식의 비디오 기반 전자 포탈 영상 장치를 개발하였다. 시스템은 $Gd_2O_2S$ 인광판, $45^{\circ}$반사경 및 후레임 그래버를 이용 비디오 기반 전자 포탈 영상 장치를 제작하였으며 이를 방사선치료기의 QA장치로 활용하고자 하였다. 이 장치를 이용 치료기 자체의 정확도를 검증하기 위해 방사선 조사면 검증을 수행하였다. 방사선 조사면이 콜리메이터 회전오차에 의해 약 $0.6^{\circ}$틀어짐을 전자 포탈 영상 장치를 통해 획득된 영상의 윤곽선을 검출한 후 알 수 있어서 치료 위치 설정 중 발생할 수 있는 오차 확인을 위한 방사선 치료장치의 Q.A도구로 사용할 수 있었다.
