한국한의학 관련학회지의 SCI등재를 위한 준비과정에 관한 고찰 (Investigation of SCI Journal Selection Process for the Journals of Korean Traditional Medicine)

  • The purpose of this study is to investigate and analyze the selection process of the Science Citation Index(SCI). SCI has been the benchmarking medium of scientific journals for over 40 years by reviewing thousands of journals published each year. The discipline of Traditional Oriental Medicine has been gaining global interest in recent years, yet the number of related journals indexed by SCI has been minimal. Korean Medicine has been producing number of quality journals for decades, but lack of journals in the English language and other problems has hindered consideration for registering with SCI. This paper will focus on the journal selection process at ISI and recommend the criteria needed for a journal to be considered for SCI selection.

단중혈(膻中穴) 침자극이 정상 성인의 심박변이도에 미치는 영향 (Study on the Effect of Acupuncture at Jeonjung($CV_{17}$) on the Heart Rate Variability in Healthy Adults)

  • Objectives : This study was accomplished to investigate the effects of acupuncture at Jeonjung($CV_{17}$) on the heart rate variability(HRV) in healthy adults. And based on that, we tried to find out how acupuncture at Jeonjung($CV_{17}$) effects on the activity and balance of the autonomic nervous system. Methods : We investigated on 74 healthy volunteers consisted of 24 subjects in acupuncture group, 24 subjects in sham acupuncture group and 24 subjects in control group. On the other hand, two volunteers were ruled out by standard of elimination. The study was established by randomized trial. Acupuncture was applied at Jeonjung($CV_{17}$) during 10 minutes in acupuncture group and minimal acupuncture was applied at the same area in sham acupuncture group. No treatment was executed in control group. We measured HRV 3 times : baseline measurement and after treatment in acupuncture group and sham acupuncture group (after rest in control group) twise. The SPSS 15.0 for Windows was used to analyze the data by paired t-test, Wilcoxon signed ranks test (in group) and One-Way ANOVA, Kruskal-Wallis test (between the groups). Results: 1. In acupuncture group, Mean HRT, PSI, Normalized HF decreased significantly. SDNN, Ln (TP), Ln (LF), Ln (VLF), Normalized LF and LF/HF ratio increased significantly. RMSSD, Ln(HF) were not affected. 2. In sham acupuncture group, all indicators were not affected. 3. In control group, mean HRT, Ln(TP) decreased significantly. SDNN, RMSSD, PSI, Ln(LF), Ln(HF), Ln(VLF), normalized LF, normalized HF and LF/HF ratio were not affected. Conclusions : We suggest that acupuncture at Jeonjung($CV_{17}$) increases the activity of autonomic nervous system and makes effects on the balance of the autonomic nervous system.

원발성 월경곤란증의 침치료 효과에 대한 임상시험 (Effect of Acupuncture Treatment on the Primary Dysmenorrhea;A Study of Single Blind, Sham Acupuncture, Randomized, Controlled Clinical Trial)

  • Objectives : Korean tradithional acupuncture Theory, Sa-am's 5 phases acupuncture method was originated in 1644-1742, in the middle of the Cho Sun dynasty. This study was to vertify the effect of Sa-am Acupuncture Treatment on Primary dysmenorrhea of Women. Methods : The subjects were 80 volunteers who was suffering for dysmenorrhea, employed using Measure of Menstrual Pain (MMP) questionnaire. Subjects were divided into two groups including Sa-am acupuncture treatment group(n=25), minimal acupuncture treatment group(n=22). They had agreed to take part in this experiment, with didn't take any anodyne drugs. In the acupuncture group, subjects were stimulated at G41, $SI_3$, B66, $SI_2$(Small intestine jung-guk), $SP_6$, $CV_6$ and one points were inserted additionally depending the symptoms among $ST_{36}$ or $LI_2$. In the control group, subjects were needled at 5 non-acupuncture points have any effect on Dysmenorrhea. A total of 8 acupuncture sessions were performed for each patient depending on the individual menstruation cycle The MMP score by using 7 questions and the Menstrual Symptom Severity List(MSSL-D) was measured before and after menstruation cycle. Collected data were analyzed as frequency, percentage, paired t-test, independent t-test using SPSS 10.0 WIN Program. Results : Acupuncture treatment produced a significant decreasing the symptom of Primary dysmenorrhea in two groups, but a statistically significant difference was not shown in the acupuncture group compare to control group. Conclusions : Based on the above results, it was verified that Acupuncture Treatment was effective in decreasing the symptom of Primary Dysmenorrhea.

Report of the 3rd Japan-Korea Workshop on Acupuncture and EBM;Protocol development for the acupuncture trial on the osteoarthritis of the knee

  • The 3rd Japan-Korea Workshop on Acupuncture and EBM was held at Kanazawa on June $16^{th}$. From Korea team, 4 papers were presented. Dr. Hahn introduced a new approach of data analysis on series of n-of-1 trials using the Bayesian statistics. It offered important information for the future n-of-1 trials. Dr. Park clearly demonstrated the significance of various sham devices proposed and stressed the importance of research questions when we choose the control intervention in RCT. Dr. Lee reported the results of survey in Korean Medical Doctors (KMD) for their point selection and techniques to the distal and local points. Dr. Kim presented the results of face to face survey on the KMD with 28 items for acupuncture treatment on the knee OA. Finally, a draft of protocol was introduced by Dr. Kim. The title was "multi-center, a randomized, single blinded, two arms, parallel-group study to compare the effectiveness and safety of 'individualized acupuncture' and 'standardized minimal acupuncture' in Korean and Japanese patients with knee osteoarthritis (Phase IV)". From Japan team, 7 speakers presented their comments and proposals on the protocol. Dr. Takahashi introduced several issues regarding n-of-1 trials and pointed out the importance of obtaining generalizability from n-of-1 trials. Dr. Shichidou pointed the importance of research design, selection of outcome measures and reduction of biases. Dr. Itoh presented the results of point selection for the knee OA based on the literature survey. Dr. Sumiya introduced several differences between KMD and Japanese acupuncturists based on the questionnaire used in KMD survey. Dr. Furuya demonstrated a result of press tack needle and its sham device on shoulder stiffness. Dr. Yamashita introduced the results of literature survey regarding adverse events occurred by acupuncture on knee OA. Dr.Tsukayama stressed the importance of responsibility of Institutional Review Board (IRB) for the conduction of clinical trials. After several issues were discussed, the need of continued meeting for final protocol development was agreed, then the workshop was closed.

침 시술 이후 발생한 경추 척수 손상 (Cervical Spinal Cord Injury Caused by Acupuncture)

  • 침술은 치료적 목적으로 신체의 특정부위에 바늘을 삽입하는 시술로 근골격계 질환의 증상 완화 목적으로 대중적으로 행해지는 술기이다. 침술 이후 심각한 합병증은 20만 예 중 1예 가량으로 발생률이 낮고 대부분의 경우 증상이 가볍고 일시적이므로 비교적 안전하다. 하지만 신경계 손상, 감염, 혈종, 폐기종 등 심각한 합병증이 발생할 수 있다. 저자들은 침술 이후 발생한 경추 척수 손상을 유발한 1예를 경험하였기에 이를 문헌 고찰과 함께 보고하는 바이다.

견비통에 대한 침치료 임상연구의 체계적 문헌 고찰 - Jadad Scale 및 개정된 STRICTA 권고안을 중심으로 - (A Systematic Review of Randomized Controlled Trials on Acupuncture for Shoulder Pain Based on Jadad Scale and revised STRICTA recommendations)

  • Objectives : This study aims to review randomized controlled trials on acupuncture for shoulder pain according to Jadad Scale and revised STRICTA. Methods : Seven electronic databases including PUBMED, SCOPUS, RISS were searched for randomized controlled trials (RCT) of acupuncture for shoulder pain. Results : 16 RCTs were included: 14 were published in English and 2 were published in Korean. According to Jadad scale, 15 RCTs had high quality. However, most of the studies could not meet the double blindness criteria. All RCTs meet 12.9 items on average in STRICTA. Conclusions : This systematic review shows four conclusions as follows. Traditional Chinese Medicine is often used in style of acupuncture. Spots of LI15, TE14, GB21 and LI11, stainless steel needle($0.3mm{\times}40mm$), 20 retention time, manual stimulation, $Deqi$, and 2 times a week treatment (total 12) are often used. Double-blinded clinical trials needs to be conducted. It is controversial to use minimal acupunctures on controlled groups of RCTs on acupuncture for shoulder pain. Description about practitioner's background needs clarifying.

Single-dose Toxicity of Guseonwangdo-go Glucose 5% Intravenous Injection in a Rat Model

  • Objectives: The purpose of this study was to examine the single-dose intravenous toxicity of Guseonwangdo-go glucose 5% pharmacopuncture (GWG5). Methods: Forty Sprague-Dawley rats were divided into four groups of five males and five females per group: an intravenous (IV) injection of 1.0 mL of normal saline solution per animal was administered to the control group; IV injections of 0.1, 0.5, and 1.0 mL of GWG5 per animal were administered to the experimental groups (G: 0.1, G: 0.5, and G: 1.0). Observation of clinical signs and body weight measurements were carried out for 14 days following the injections. At the end of the observation period, hematological, biochemical, and histopathological tests, as well as necropsy examinations, were performed on the injected parts. Results: No mortalities or adverse clinical signs were observed in any of the groups. The body weights of all groups continuously increased. In the hematological and the biochemical tests, females in G-0.1 had minimal changes, but those changes were not dose dependent. On necropsy examination, no abnormalities were observed. In the histopathological test, focal inflammatory cell infiltrations were observed in two female rats, one in the control group and one in G-1.0. Also, one female rat in the control group had an epidermis crust. These changes were concluded to have been caused by the insertion of the needle into a vein. Conclusion: The above findings suggest that the lethal dose of GWG5 administered via IV injection is more than 1.0 mL per animal in both male and female rats. Further studies are needed to establish more detailed evidence of its toxicity.

A 4-week Repeated dose Oral Toxicity Study of Mecasin in Sprague-Dawley Rats to Determine the Appropriate Doses for a 13-week, Repeated Toxicity Test

  • Objectives: In this study, we investigated the 4-week repeated-dose oral toxicity of gami-jakyak gamcho buja decoction (Mecasin) to develop safe treatments. Methods: In order to investigate the 4-week oral toxicity of Mecasin, we administered Mecasin orally to rats. Sprague-Dawley (SD) rats were divided into four groups of five male and five female animals per group: group 1 being the control group and groups 2, 3, and 4 being the experimental groups. Doses of Mecasin of 500, 1,000, and 2,000 mg/kg of body weight were administered to the experimental groups, and a dose of normal saline solution of 10 mL/kg was administered to the control group. We examined the survival rate, weight, clinical signs, and gross findings for four weeks. This study was conducted under the approval of the Institutional Animal Ethics Committee. Results: No deaths occurred in any of the four groups. No significant changes in weights or food consumption between the control group and the experimental groups were observed. Serum biochemistry revealed that some groups showed significant decrease in inorganic phosphorus (IP) (P < 0.05). During necropsy on the rats, one abnormal macroscopic feature, a slight loss of fur, was observed in the mid dosage (1,000 mg/kg) male group. No abnormalities were observed in any other rats. In histopathological findings, the tubular basophilia and cast of the kidney and extramedullary hematopoiesis of the spleen were found. However, those changes were minimal and had occurred naturally or sporadically. No other organ abnormalities were observed. Conclusion: During this 4-week, repeated, oral toxicity test of Mecasin in SD rats, no toxicity changes due to Mecasin were observed in any of the male or the female rats in the high dosage group. Thus, we suggest that the doses in a 13-week, repeated test should be 0, 500, 1,000, and 2,000 mg/kg respectively.

  • ■Objectives This case study is to report the effects of Korean medicine on parkinsonism patient's Gait Disturbance. ■Methods During 12 days of hospitalization, the patient was treated by acupuncture, pharmaco-acupuncture, moxibustion, herbal medicine, especially Cheongsimyeonjatang-gamibang. In order to assess the change of symptoms, we used a 3-Dimensional(3D) gait analysis system, Unified Parkinson's Disease Rating Scale(UPDRS), analysis of gait video and self-evaluation of discomfort. ■Results After treatment, The improvements of walking pattern were observed in both objective analysis results of gait analysis system and subjective video analysis. And the UPDRS score decreased, especially Part III score decreased more than minimal clinically important difference(MCID). In addition, There was improvement in self assessment of the patient. ■Conclusion This study suggests that Korean medical treatment might be effective in motor disorder of parkinsonism patient.
