Design of FIR filter using direct memory access for voice signal processing module in implantable middle ear hearing devices (이식형 인공중이용 음성신호 처리 모듈을 위한 직접 메모리 억세스 기반의 FIR 필터 설계)

  • An FIR filter for digital voice signal processing has been designed and implemented using a microcontroller in implantable middle ear hearing devices (IMEHDs). The designed digital voice signal processing filter which has fast and accurate filtering operation and controllable filter characteristics has been implemented using a hardware multiplier and a direct memory access (DMA) in the low power microcontroller, MSP430F169. It has been confirmed that each of the implemented 6-orders Remez FIR filters with 1 channel and 2 channels can be applied to the voice signal processing module of IMEHDs based on the evaluation results of the filtering performance experiment.

  • A 40-year-old man was admitted with chief complaints of sudden hearing loss, ear fullness and tinnitus of 12 days duration developed following SCUBA diving. An exploratory tympanotomy was done on 16th day of onset, and a small leakage of clear fluid was found on the anterior portion of the stapediovestibular joint, which was sealed off by small piece of fatty tissue from the ear lobe. Postoperatively, his PTA's was improved from 85㏈ to 15㏈, however high tone was remained unchanged above 4KHz. More exploration of the middle ear will be helpful to find the labyrinthine fistula causing sudden type of hearing loss.

  • A 12-year-old castrated male Shih Tzu presented with suspected hearing loss. The patient had no history of head trauma or exposure to ototoxic drugs. The results of neurologic and physical examinations were normal. An otoscopic examination showed that both the tympanic membranes and the external ear canals had a normal appearance. However, the results of brainstem auditory evoked response tests confirmed sensorineural deafness in the right ear and indicated conduction disturbances and brainstem abnormalities in the left ear. Magnetic resonance imaging was performed to confirm the causes of the conduction disturbances and brainstem abnormalities. Inflammatory changes in the left middle ear were highly suspected to be responsible for the findings in the left ear. The results of these examinations confirmed complete hearing loss in the right ear and indicated otitis media in the left ear, which could have been the cause of the conduction disturbances.

The Changes of External Ear Resonance after Surgery for Repair of the Postauricular Meato-Mastoid Cutaneous Fistula (외이도-유양동-귀뒤바퀴피부 누공환자에서 수술 전, 후의 외이도 공명의 변화)

  • Due to the structure with one end closed, the external ear resonance effect in which the high frequency is amplified can be generated, and the sound can be perceived well. The external ear resonance normally has a first peak and a second peak. On average, the first peak has a gain of 18.6 dB at 2620 Hz and the second peak has a gain of 18.2 dB at 4210 Hz. The resonance of the external auditory canal changes with the state of the tympanic membrane, the presence of the ventilation tube, and the structure (length, diameter, shape) of the external auditory canal. A patient with a postauricular meatomastoid cutaneous fistula was admitted to the hospital with a foreign body which is the molding of the hearing aid. After removal of the foreign body, the resonance of the external auditory canal was lost and the subjective sound cognitive ability decreased. In the case of postauricular meatomastoid cutaneous fistula, we confirmed the improvement of sound cognitive ability, the change of pure tone hearing threshold, and the change of the external ear resonance after reconstruction of the ear canal without middle ear reconstruction.

Effects of Nasal Packing on the Eustachian Tube Function (Nasal Packing이 이관기능에 미치는 영향)

  • It is clinically well-known observation that Nasal packing is associated with the eustachian tube dysfunction, and this study seldom appeared in the literature. In our country, Nasal Surgery requiring Nasal packing is still the most frequent operation in the field of otorhinolaryngology and it is our interest to study this effects of Nasal packing on the eustachian tube function. Using tympanometry, we experienced eustachian tube dysfunction in 40 patients (80 ears) with anterior packing due to Nasal Surgery and epistaxis and in 13 patients (26 ears) with anterior packing and posterior packing due to epistaxis. The results were as fllows; 1) Thirty of the 80 ears (37%) in the patients with anterior packing revealed reduction in middle ear pressure, of which 16 ears (20%) demonstrated negative middle ear pressure of greater than 100mm H20. 2) Fifteen of the 26 ears (50.7%) in the patients with posterior packing revealed reduction in middle ear pressure, of which 11 ears (42.3%) demonstrated negative middle ear pressure of greater than l00mm H20. 3) Posterior packing is associated with a greater incidence of eustachian tube dysfunction than anterior packing. 4) The eustachian tube dysfunction associated with Nasal packing is temporary and this function is returned to normal within 2 to 9 days after removal of the packing.

  • Recently the efficacy of all ear drops in perforated tympanic membranes has been questioned and the safety of using the drugs intratympanically has been challanged. Many drugs not normally ototoxic when given systemically become ototoxic when administered via the middle ear. According to Stupp and others (1973) among neomycin, polymyxin-G, gentamycin, erythr omycin, tetracycline and penicillin, only penicillin was found to be free of toxic effects. The ototoxic antibiotics probably reach the inner ear by permeating the round window membrane, into the perilymph and then through Reissner's membrane to the endolymph. The main pathological changes are degeneration of stria vascularis, degeneration of sensory epithelia and degeneration of ganglion cells. We report with reference, a case of profound sensorineural deafness following the application of Terramycin powder intratympanically, in a 63 year old woman.

Sudden Hearing Loss Treated by Chuna Manual Therepy; A Case Report (돌발성 난청 환자의 경추부 추나요법 증례보고)

  • The purpose of this study is to report a clinical progress of sudden hearing loss through korean medicine. Between 26th Mar. 2014 and 19th Apr. 2014, she was treated with Chuna manual therapy every day and observed by Pure Tone Audiometry (before and after) and visual analogue pain scale (VAS). A patient was diagnosed with sudden hearing loss on 23th Dec. 2013 in Severance Hospital, she had sudden hearing loss on left side. She had 19 times treatments. At the first of treatment, her audibility was 20 dB on right side, 74 dB on left side and she had tinitus and ear fullness (VAS 8 / VAS 9). At the middle of treatment (2nd Apr. 2014), symptoms of tinitus and ear fullness started improving. State was tinitus VAS 6, ear fullness VAS 6. At the end of treatment (21th Apr. 2014), state was tinitus VAS 4, ear fullness VAS 4, and recorded 35 dB on right side, 80 dB on left side in pure tone audiometry. Chuna manual therapy is estimated to be good for sudden hearing loss especially on tinitus and ear fullness. More cases are required to develop treatment of sudden hearing loss.

  • Objective This research is a study about constitutional diagnosis through the external appearances as a basic principle, and it is for finding shape differences of the ear, eye, nose, mouth according to the Sasang constitution Method We have collected 209 cases of patients of the Sasang Constitutional Department, including employees of the Kyung-Hee Medical Center and took pictures of the frontal view, lateral view, oblique view of face and measured heights, deapth, breadths of ear, eye, nose, mouth with 'The Measurement of R. Martin'. We analyzed shape differences of the face according to the Sasang constitution with certain results Results We got the morphologic characteristics of ear, eye, nose and mouth according to the Sasang constitution as Table 3. -Table 10. Conclusion : 1. The morphologic characteristics of Ear according to the Sasang constitution (1) Morphologic ear length, Physiognomic ear length, Ear lobule length is longer in Taeumin than Soeumin. (2) Physiognomic ear breadth is wider in Taeumin than Soeumin. (3) Physiognomic ear length, lobule length ratio is higher in Taeumin than Soyangin. 2. The morphologic characteristics of Eye according to the Sasang constitution. (1) Inner Palpebral fissure width, 5th Palpebral fissure length, Bizygomatic breadth-Outercanthal distance is the longest in Taeumin (2) Palpebral fissure inclination is widest in Soeumin. (3) Palpebral fissure length is longer in Taeumin than Soeumin. (4) Pupillary diameter ratio is the lowest in Taeumin (5) Palpebral fissure length, width ratio is higher in Soeumin than Taeumin. (6)zygomatic breadth, Bizygomatic breadth-Outercanthal distance ratio is the higher in Taeumin than Soeumin. 3. The morphologic characteristics of Nose according to the Sasang constitution. (4) Intercheilion breadth, total height ratio is lowest in Soyangin. (5) Total lip height, Upper philtrum height ratio is higher in Soyangin than Soeumin. (6) Lower lip height Lower quarter lip height ratio is higher in Soyangin than Taeumin. (8) Total lip area is wider in Taeumin than Soyangin.

  • The implantable middle ear hearing devices(IMEHDs) have been developed to overcome the conventional hearing aid's problem(ringing effect caused by the acoustic feedback, cosmetic problem, etc.). In the IMEHDs, the vibrating transducer is a key component because its vibration enables to hear for hearing impaired people. The vibrating transducer is implanted on ossicular chain by surgical operation. The coupling status between implanted transducer and ossicular chain has an effect on delivering vibrating force from transducer to stapes. Noninvasive method is required to investigate the output characteristics of IMEHDs after implementation. Recently, emitted sound pressure measuring method of tympanic membrane is proposed to investigate the output characteristics of IMEHDs. However, the relationship between displacement of stapes and sound pressure by tympanic membrane was not cleared. In this paper, displacement of stapes and sound pressure by tympanic membrane were measured using the differential floating mass transducer(DFMT) that implanted on the ossicular chain of the human temporal bone and physical ear model. Through the experiments results, the relationship between displacement of stapes and sound pressure by tympanic membrane was investigated.

An Adaptive Feedback Canceller for Fully Implantable Hearing Device Using Tympanic Membrane Installed Microphone (고막이식형 마이크로폰을 위한 이식형 인공중이 적응 피드백 제거기 구현)

  • Many implantable hearing aids are being developed as alternatives to conventional hearing aids which has inconveniences for use and social stigma that make hearing-impaired people avoid to wear it. Particularly, the fully-implantable middle ear hearing devices (F-IMEHD) are being actively studied for mixed or sensorineural hearing impaired people. In development of F-IMEHD, the most difficult problem is improving the performance of implantable microphone. Recently, Cho et al. have studied the tympanic membrane installed microphone which has better sensitivity and is easier to operate on patient than the microphone implanted under the skin. But, it may cause howling problem due to the feedback signal via oval window and ossicle chain from the transducer on round window in the middle ear cavity, therefore, a feedback canceller is necessary. In this paper, we designed NLMS (normalized least mean square) adaptive feedback canceller for F-IMEHD with tympanic membrane installed microphone and a transducer implemented at round window, and computer simulation was performed to verify its operation. The designed adaptive feedback canceller has a delay filter, a 64 point FIR fixed filter and a 8-tap adaptive FIR filter. Computer simulation of the feedback path is modeled by using the data obtained through human cadaver experiment.
