Digital Artery Perforator Flaps

  • In the hand, few vessels can be classified as 'perforators'. Even today, the debate continues on whether or not branches of the digital artery should be considered as perforators. However, 'perforator' and 'perforator flaps' have become extremely popular within the microsurgery literature, and as such, the terminology has found widespread adoption across the hand surgery field. The finger is the most important sensory organ for tactile stimulation. The glabrous skin can be distinguished between the fingertip and the proximal tissue, with the fingertip having a higher functional and, therefore, reconstructive priority. Thus, sacrifice of the proximal glabrous skin can be justified in the reconstruction of fingertip defects. In this paper, the author reviews two flaps, 'lateral digital artery perforator flap' and 'volar digital artery perforator flap', both of which uses short branches of the digital artery as a vascular pedicle and are useful in fingertip construction.

  • Microvascular anastomosis is too important to reduce the failure in operating room because of no choice in case of failure and prognosis of the failure would be more worse than any other success ones. Factors for microsurgery trainee influencing patency in microvascular anastomosis at the laboratory are numerous but three basic prerequisites are magnification, instruments and suture material and training. First Author trained microsurgical technique to the second author using femoral artery of the rat and patency of the anastomosis was evaluated by the milking test. Period which microsurgeon successes femoral arterial anastomosis in rat in both of 5 and 30 minutes was 7th week at the laboratory which has done 2 vessels a week under the supervise of the skilled first author.

  • Palmar soft tissue defects are best reconstructed using a replacement flap of proper size with adequate soft tissue stability for mechanical resistance as well as with protective sensation. Reconstructive approaches are dictated by injury mechanism, defect size and location, and the status of the wound bed and tendino-skeletal structure. While uninjured portions of the hand can be used as a source for local flaps, the use of free flaps allows for maximal access for selection of the most ideal replacement tissue for the defect to be restored as close to the initial state as possible. Here, we review the garden variety of free flaps used in reconstruction of palmar soft tissue defects.

  • Endodontic surgery is a procedure to treat apical periodontitis or abscess in cases that did not heal after nonsurgical treatment or retreatment. This might include situations with persistent intracanal infection after root canal treatment. Other reasons might be found in extraradicular infection, such as bacterial biofilm on the apical root surface or bacteria within the lesion. For many years, the treatment standard was the traditional approach with surgical burs and amalgam for root-end filling. Endodontic microsurgery is the most recent step in the evolution of endodontic surgery, applying not only ultrasonic tip and biocompatible filling materials but also incorporating high-power magnification and illumination. Although many studies have been published that advocate the use of modern technique, the traditional techniques are still widely used in the surgery community. The purpose of this study was to demonstrate the endodontic microsurgery procedure including the root-end preparation and filling with the use of a surgical operating microscope.

비복근 - 내측 비복 동맥 천공지 복합 피판을 이용한 연부조직 재건 (Soft Tissue Coverage Using a Combined Gastrocnemius-medial Sural Artery Perforator Flap)

  • Medial gastrocnemius flap has been known as a useful option for soft tissue reconstruction of the knee and upper 1/3 of lower extremity, but it has a limitation to cover the lateral defect of the knee joint. We performed the combined gastrocnemius-medial sural artery perforator flap for coverage of the anterolateral defects of the knee joint, which is compound flap using a medial gastrocnemius flap and a medial sural artery perforator flap. This flap is a useful method for reconstruction of anterolateral knee defects, providing a easy dissection without the microsurgery and intramuscular dissection of the perforators.

Management of Lymphedema

  • Lymphedema is a frequent complication after the treatment of various cancers, particularly breast cancer, gynecological cancers, melanomas, and other skin and urological cancers. Lymphedema patients have chronic swelling of the affected extremity, recurrent infections, limited mobility and decreased quality of life. Once lymphedema develops, it is usually progressive. Over time, lymphedema leads to fat deposition and subsequent fibrosis of the surrounding tissues. However, there is no cure for lymphedema. Recently, the development of microsurgery has led to introduction of new surgical techniques for lymphedema, such as vascularized lymph node transfer. We report here the latest trends in the surgical treatment of lymphedema, as well as diagnosis and conventional treatments of lymphedema.

후두질환에서 경구강 $CO_2$ 레이저 미세수술 (Transoral $CO_2$ Laser Microsurgery for Laryngeal Lesions)

  • Transoral $CO_2$ laser microsurgery has been widely used for various laryngeal diseases. Laser cordectomy for the management of early laryngeal carcinoma has advantages with regards to oncologic results, preservation of laryngeal functions, morbidity and cost in comparison to those of open surgery or radiation therapy. $CO_2$ laser is also applicable to benign vocal fold lesions such as nodules, polyps, cysts, reinke's edema, granulomas, papillomas, hemangiomas and glottis webs. $CO_2$ laser is suitable for microsurgery because it has small focus size and hemostatic effect. We should also recognize the risks of $CO_2$ laser for safe use.
