Usefulness of cyclic thermal therapy and red blood cell scintigraphy in patients with chemotherapy-induced peripheral neuropathy

  • Background: Pharmacological and non-pharmacological therapies have been used to treat patients with chemotherapy-induced peripheral neuropathy (CIPN). However, the effect of therapies in cancer patients has yet to be investigated comprehensively. We hypothesized that cyclic thermal therapy would improve blood flow and microcirculation and improve the symptoms driven by CIPN. Methods: The criteria of assessment were blood volume in region of interest (ROI) in the images, and European Organization for Research and Treatment of Cancer-Quality of Life Questionnaire-Chemotherapy-Induced Peripheral Neuropathy 20 questionnaire scores. The blood volume was quantified by using red blood cell (RBC) scintigraphy. All patients were treated 10 times during 10 days. The thermal stimulations, between 15° and 41°, were repeatedly delivered to the patient's hands. Results: The total score of the questionnaires, the score of questions related to the upper limbs, the score of questions closely related to the upper limbs, and the score excluding the upper limbs questions was decreased. The blood volume was decreased, and the variance of blood volume was decreased. During cooling stimulation, the blood volume was decreased, and its variance was decreased. During warming stimulation, the blood volume was decreased, and its variance was decreased. Conclusions: We suggest that cyclic thermal therapy is useful to alleviate CIPN symptoms by blood circulation improvement. RBC scintigraphy can provide the quantitative information on blood volume under certain conditions such as stress, as well as rest, in peripheral tissue.

헌혈이 헌혈자의 혈유변학적 지표에 미치는 영향 (Effect of Blood Donation on the Donor's Hemorheological Properties)

  • 순환계는 혈액의 흐름이 원활하게 되도록 그 해부학적 구조와 혈류의 물리학적 특성이 연결되어있다. 혈액의 흐름에 영향을 주는 혈액의 물리학적 특성을 혈유변학적 인자(Hemorheologic factors)라 하며, 혈액의 점도(blood viscosity) 및 적혈구 응집도(erythrocyte aggregation)와 같은 혈유변학적 특성은 헤마토크리트(hematocrit; Hct)와 밀접한 관계를 가진다. 헤마토크리트가 높을수록 혈액 점도가 증가하고 적혈구 응집도는 증가하며 순환을 저해하고 조직에 산소 전달 능력을 방해한다. 혈청 페리틴은 과도하게 있을 경우 산화유리기(oxygen free radial)를 통하여 혈관 내피세포(vascular endothelial cell)와 혈구 세포(blood cell)에 산화 손상을 유발하여 심혈관계 손상을 유발한다. 이러한 기전을 근거로 사혈 혹은 헌혈을 심혈관계 질환의 예방 및 치료에 응용하려는 시도가 오랫동안 있었다. 사혈은 의학에 있어 오랜 역사를 가지면서 특히 한방의 치료적 개념으로 최근까지도 사용되고 있으나 그 과학적 근거가 불충분하고 아직까지 논란의 여지도 있어 근거 중심의 의학을 근간으로 하는 의학에서는 도외시되어왔다. 하지만 혈액량이 증가하면 혈유변학적 인자들(hemorheological factors) 역시 악화되면서 대 순환에서 동맥경화의 발생 및 진행에 영향을 주고, 모세순환(microcirculation)을 악화시키는 것으로 알려져 있으며 심뇌혈관 사건에 영향을 주는 것으로 보고되고 있다. 정기적인 헌혈자(regular blood donors)에게 혈유변학적 인자들을 저명하게 호전시키고 과도한 철분의 함량을 줄여서 산화유리기에 의한 혈구 및 내피세포의 산화 손상을 줄임으로써, 순환계내에서 혈액순환을 촉진시킬 수 있다는 증거들이 보고되고 있다. 헌혈의 효과가 심혈관계 질환에 이익이 된다는 확고한 장기적 코호트 연구결과가 도출된다면 절대적으로 헌혈 혈액량이 부족한 현실과 심혈관계 질환의 예후를 호전시킬 수 있다는 측면에서 중요한 역할을 할 수 있다고 생각된다.

Red Blood Cell Velocity Field in Rat Mesenteric Arterioles Using Micro PIV Technique

  • As endothelial cells are subject to flow shear stress, it is important to determine the detailed velocity distribution in microvessels in the study of mechanical interactions between blood and endothelium. This paper describes a velocity field of the arteriole in the rat mesentery using an intravital microscope and high-speed digital video system obtained by a highly accurate PIV technique. Red blood cells (RBCs) velocity distributions with spatial resolutions of $0.8{\times}0.8{\mu}m$ were obtained even near the wall in the center plane of the arteriole. By making ensemble-averaged time-series of velocity distributions, velocity profiles over different cross-sections were calculated for comparison. The shear rate at the vascular wall also evaluated on the basis of the ensemble-averaged profiles. It was shown that the velocity profiles were blunt in the center region of the vessel cross-section while they were steep in the near wall region. The wall shear rates were significantly small, compared with those estimated from the Poiseuille profiles.

미세튜브 내부를 흐르는 혈액유동의 유변학적 특성에 대한 in-vitro 연구 (In-vitro Study on Hemorheological Behaviors of Blood Flow Through a Micro Tube)

  • In order to obtain velocity profile of blood flow with high spatial resolution, a micro PIV technique consisted of a fluorescent microscope, double-pulsed YAG laser, cooled CCD camera was applied to in-vitro blood flow experiment through a micro round tube of a diameter $100{\mu}m$. Velocity distributions of blood flow for rabbit were obtained. The viscosity profiles for shear rate were found at flowing condition. To provide hemorheological characteristics of blood flow, the viscosities for shear rate were evaluated. The viscosity of blood also steeply increase by decreasing shear rate resulting in Non-Newtonian flow, especially in low shear rate region caused by RBC rheological properties. The results show typical characteristics of Non-Newtonian characteristics from the results of velocity profile and viscosity for blood flow. From the inflection points, cell free layer and two-phase flow consisted with plasma and suspensions including RBCs can be separated.

어혈증(瘀血證)의 임상 질환 범위에 대한 고찰 (Study on Clinical Diseases of Blood Stasis Pattern)

  • Objectives : This article is a study on to which categories of modern diseases blood stasis patterns are assigned and the meaning of blood stasis interpreted with perspectives of Korean Medicine and modern medicine. Methods : We reviewed "Neijing", "Shanghanlun", "Yilingaicuo", "Xuezhenglun" and other books and modern clinical papers related with blood stasis. Results : 1. Blood stasis patterns are related with disorders of hemorrheology, hemodynamics, platelet function, microcirculation, microelements and endothelial damage. 2. From the types of syndrome differentiation, diverse diseases classified in type of qi deficiency with blood stasis and type of blood stasis due to qi stagnation are reported, which reflects qi and blood are closely connected. And many diseases are classified in type of kidney deficiency with blood stasis, which has something in common with chronic diseases can achieve effect from treatment considering blood stasis. 3. Diseases related with kidney involve menopausal disorder, mazoplasia, prostatitis, erectile dysfunction, chronic nephritis, renal calculus, osteoporosis and bursitis. Diseases related with heart involve coronary artery disease, arrhythmia and cerebral thrombosis. Diseases related with spleen involve gastritis, colonitis and digestive organ ulcer. Diseases related with liver involve hepatitis, hyperthyroidism and stroke. Diseases related with lung involve neurodermatitis, bronchitis and paranasal sinusitis. Conclusions : Blood stasis pattern which is one of the areas to draw medicine's attention has broad clinical application.

  • Cosmetic mask is one of the traditional cosmetic product and is used to keep skin healthy and beautiful through moisturizing effect, cleansing effect and acceleration effect on microcirculation. There are several types of cosmetic mask such as peel-off type, tissue-off type, wash-off type and cotton sheet type immersed in cosmetic water. It is important for cosmetic formulators to develop an adequate type of cosmetic mask that meets the needs of the target consumers because each type of cosmetic mask has benefits and problems in its usage and efficacy. Polyvinyl alcohol (PVA) is widely used as an active ingredient of peel-off type cosmetic mask and the physical property of the PVA film changes with the usage of other ingredients. We studied the change of physical properties of the film containing PVA by formulating peel-off type cosmetic mask with the variation of humectants. Methylguceth-20 and glycereth-26 reduce the interaction between skin surface and PVA film without serious weakening of tensile strength and they can alleviate irritation. Glycerin, urea and betaine help PVA film soften and they also have excellent efficacy as humectants.

  • To determine the factors which affect the flow of dentinal fluid through cat dentinal tubules in vivo, the flow of fluid was measured by observing the movement of the fat droplets of dilute milk in a glass capillary with a microscope connected to the monitor. After measuring the exposed area of dentin, hydraulic conductances of dentin were calculated. The mean pressure which stoped the outward flow of dentinal fluid was 9.5mmHg. The hydraulic conductance of dentin under the condition of pulp exposed was increased by 21 % from that under the condition of dentin exposed. Under the conditions of pulp cut and pulp removed, the hydraulic conductances of dentin were increased by 22 % and 31 % respectively from that under the condition of dentin exposed. These results show that the direction and rate of dentinal fluid flow in cat dentin is affected mainly by the hydrostatic pressure of interstitial fluid of pulp tissue in the state of low compliance. Both of the osmotic effect produced by the protein constituents of interstitial fluid across the odontoblast tell layer and the change of interstitial fluid pressure produced by the state of the microcirculation of the pulp also affect the direction and rate of dentinal fluid in some degree.

In-vitro study on the hemorheological characteristics of chicken blood in microcirculation

  • The flow characteristics of chicken blood in a micro-tube with a $100{\mu}m$ diameter are investigated using a micro-Particle Image Velocimetry (PIV) technique. Chicken blood with 40% hematocrit is supplied into the micro-tube using a syringe pump. For comparison, the same experiments are repeated for human blood with 40% hematocrit. Chicken blood flow has a cell-free layer near the tube wall, and this layer's thickness increases with the increased flow speed due to radial migration. As a hemorheological feature, the aggregation index of chicken blood is about 50% less than that of human blood. Therefore, the non-Newtonian fluid features of chicken blood are not very remarkable compared with those of human blood. As the flow rate increases, the blunt velocity profile in the central region of the micro-tube sharpens, and the parabolicshaped shear stress distribution becomes to have a linear profile. The viscosity of both blood samples in a low shear rate condition is overestimated, while the viscosity in a high shear rate range is underestimated due to radial migration and the presence of a cell-depleted layer.

Role of Kupffer Cells in Vasoregulatory Gene Expression During Endotoxemia

  • Although hepatic microcirculatory dysfunction occurs during endotoxemia, the mechanism responsible for this remains unclear. Since Kupffer cells provide signals that regulate hepatic response in inflammation, this study was designed to investigate the role of Kupffer cells in the imbalance in the expression of vasoactive mediators. Endotoxemia was induced by intraperitoneal E. coli endotoxin (LPS, 1 mg/kg body weight). Kupffer cells were inactivated with gadolinium chloride ($GdCl_3$, 7.5 mg/kg body weight, intravenously) 2 days prior to LPS exposure. Liver samples were taken 6 h following LPS exposure for RT-PCR analysis of mRNA for genes of interest: endothelin (ET-1), its receptors $ET_A$ and $ET_B$, inducible nitric oxide synthase (iNOS), heme oxygenase (HO-1), and tumor necrosis factor-$\alpha$ (TNF-$\alpha$). mRNA levels for iNOS and TNF-$\alpha$ were significantly increased 31.8-fold and 26.7-fold in LPS-treated animals, respectively. This increase was markedly attenuated by $GdCl_3$, HO-1 expression significantly increased in LPS-treated animals, with no significant difference between saline and $GdCl_3$ groups. ET-1 was increased by LPS. mRNA levels for $ET_A$ receptor showed no change, whereas $ET_B$ transcripts increased in LPS-treated animals. The increase in $ET_B$ transcripts was potentiated by $GdCl_3$. We conclude that activation of Kupffer cells plays an important role in the imbalanced hepatic vasoregulatory gene expression induced by endotoxin.

Structural damage to periodontal tissues at varying rate of anesthetic injection

  • Background: Incorrect administration of an anesthetic during local anesthesia is one of the most important causes of pain symptoms in patients scheduled for dental procedures. The current study assessed the severity of damage to periodontal tissue following different rates of anesthetic administration. Methods: The research was conducted on 50 outbred male rats with a body mass of 180-240 g. The anesthetic used was 1% articaine. Results: The results showed that administration of the anesthetic at a rapid pace caused structural damage to the periodontal tissue. Further, signs of impaired microcirculation were noted at all rates of administration. Biochemical studies demonstrated changes in the level of glucose and enzymes with the rapid introduction of the anesthetic, indicating severe systemic stress response of the body. Conclusions: Injection of local anesthetic at any rate of introduction induces vascular congestion in the microcirculatory bloodstream and exudative reactions. Rapid introduction of an anesthetic causes progression of structural changes in the gingival tissue.
