직교배열실험 방법 기반 해양플랜트 플로트오버 설치 공법용 수동형 DSF의 구조설계 민감도와 메타모델링 평가 (Evaluation on Structure Design Sensitivity and Meta-modeling of Passive Type DSF for Offshore Plant Float-over Installation Based on Orthogonal Array Experimental Method)

  • Structure design sensitivity was evaluated using the orthogonal array experimental method for passive-type deck support frame (DSF) developed for float-over installation of the offshore plant. Moreover, approximation characteristics were also reviewed based on various meta-models. The minimum weight design of the DSF is significantly important for securing both maneuvering performance and buoyancy of a ship equipped with the DSF and guaranteeing structural design safety. The performance strength of the passive type DSF was evaluated through structure analysis based on the finite element method. The thickness of main structure members was applied to design factors, and output responses were considered structure weight and strength performances. Quantitative effects on the output responses for each design factor were evaluated using the orthogonal array experimental method and analysis of variance. The optimum design case was also identified from the orthogonal array experiment results. Various meta-models, such as Chebyshev orthogonal polynomial, Kriging, response surface method, and radial basis function-based neural network, were generated from the orthogonal array experiment results. The results of the orthogonal array experiment were validated using the meta-modeling results. It was found that the radial basis function-based neural network among the meta-models could approximate the design space of the passive type DSF with the highest accuracy.

운동이 노인의 인지기능에 미치는 영향 : 체계적 문헌고찰 및 메타분석 (The effect of exercise on cognitive function in the elderly : A systematic review and meta-analysis)

  • 본 연구는 65세 이상의 정상 노인을 대상으로 운동요법이 인지기능에 미치는 영향을 체계적 문헌고찰 및 메타분석을 통해 확인하고자 수행되었다. 본 연구는 국내 주요 DB를 이용하여 논문검색을 하였으며 3.197편 중 선정기준에 적합한 최종 14편의 논문을 체계적 문헌고찰하였으며 이 중 효과크기를 산출할 수 있는 11편을 대상으로 메타분석을 실시하였다. 분석은 랜덤효과모형을 이용하였으며, 운동중재의 효과크기를 산출하였다. 본 연구에서 운동중재의 인지기능에 대한 효과크기는 Hedges' g=1.05 (95% CI: 0.61~1.50)로 큰 효과크기를 보였으며, 전체 이질성이 $I^2=82.3%$ (Q=56.61, p<.001)로 중재방법, 주당 중재횟수, 중재기간, 측정도구를 조절변수로 하여 조절효과를 분석하였다. 분석결과 중재방법, 주당 중재횟수, 중재기간에서는 유의한 차이가 없었으며, 측정도구에서 유의한 차이가 있는 것으로 나타났다. 본 연구는 우리나라 노인의 인지기능에 운동이 미치는 영향에 대한 국내연구결과들을 체계적으로 분석하고, 그 효과크기를 객관적으로 제시하여 향후 인지기능 향상을 위한 중재전략으로 운동요법을 설계할 때 기초자료를 제공하였다는 데에 의의가 있다.

뇌졸중 환자에게 적용한 로봇보행 재활훈련의 효과: 메타분석 (The Effects of Robot-Assisted Gait Training for the Patient With Post Stroke: A Meta-Analysis)

  • Robot-assisted rehabilitation therapy has been used to increase physical function in post-stroke patients. The aim of this meta-analysis was to identify whether robot-assisted gait training can improve patients' functional abilities. A comprehensive search was performed of PubMed, Cochrane Central Register of Controlled Trials (CENTRAL), Physiotherapy Evidence Database (PEDro), Academic Search Premier (ASP), ScienceDirect, Korean Studies Information Service System (KISS), Research Information Sharing Service (RISS), Korea National Library, and the Korean Medical Database up to April, 2014. Fifteen eligible studies researched the effects of robot-assisted gait training to a control group. All outcome measures were classified by International Classification of Functioning, Disability, and Health (ICF) domains (body function and structures, activity, and participation) and were pooled for calculating the effect size. The overall effect size of the robot-assisted gait training was .356 [95% confidence interval (CI): .186~.526]. When the effect was compared by the type of electromechanical robot, Gait Trainer (GT) (.471, 95% CI: .320~.621) showed more effective than Lokomat (.169, 95% CI: .063~.275). In addition, acute stroke patients showed more improvement than others. Although robot-assisted gait training may improve function, but there is no scientific evidence about the appropriate treatment time for one session or the appropriate duration of treatment. Additional researchers are needed to include more well-designed trials in order to resolve these uncertainties.

Effect of phosphatidylserine on cognitive function in the elderly: A systematic review and meta-analysis

  • Phosphatidylserine (PS) is an essential component of the cerebral cortex and is associated with cognitive function. In this systematic review, the effects of PS on cognitive function in the elderly population are examined. The literature search included PubMed, EMBASE, Cochrane, and Web of Science databases. Subsequently, nine studies, including five randomized controlled trials and four pre-post studies, were selected. There were 961 participants in the selected studies; PS dosage varied from 100 to 300 mg/d, and the experimental period ranged from 6 weeks to 6 months. Five out of the nine selected studies were assessed to have a 'low' risk of bias, whereas the other four studies were assessed to have 'some concerns' regarding the risk of bias. The results of the meta-analysis concluded that PS had a positive effect on the memory of older adults with cognitive decline. Thus, PS appears to improve age-associated cognitive decline, especially memory, with no adverse effects.

경피적 관상동맥 중재술을 받은 환자를 대상으로 심장호흡물리치료에 대한 효과 - 체계적 고찰과 메타분석 (Effect of Cardiopulmonary Physiotherapy on Patients With Percutaneous Coronary Intervention - Systematic Review and Meta-Analysis)

  • PURPOSE: This study examined the effects of cardiopulmonary physiotherapy on the cardiopulmonary function, metabolism, inflammatory markers, and quality of life in patients with coronary artery disease who underwent percutaneous coronary intervention (PCI). METHODS: Electronic bibliographic databases of a regional information sharing system (RISS) and PubMed were searched to identify studies with randomized and non-randomized controlled trials. As the final outcome, 320 publications were identified and 18 studies met the inclusion and exclusion criteria. All studies were assessed for the quality of study using Cochrane's risk of bias. RESULTS: Sixteen studies met the inclusion criteria, in which meta-analysis had been conducted to examine the effectiveness of cardiopulmonary physiotherapy on the cardiopulmonary function, metabolism, inflammatory markers, and quality of life in patients undergoing PCI. Meta-analysis based on a random effect model showed that the cardiopulmonary physiotherapy was beneficial in improving the cardiopulmonary function, metabolism, inflammatory markers, and quality of life. In particular, there was a significant effect on the peak oxygen uptake (effect size 5.30%; 95% confidence interval 3.62~6.97). Cardiopulmonary physiotherapy for a during period of 6 weeks or more was effective in significantly improving the cardiopulmonary function and metabolism function in a subgroup analysis, but cardiopulmonary physiotherapy for less than 6 weeks was not effective. CONCLUSION: Cardiopulmonary physiotherapy has positive effects on the cardiopulmonary function, metabolism, inflammatory markers, and quality of life in patients undergoing PCI.

심해공학수조 조파기 전달함수 근사 모델 개발 (Development of Meta Model of Transfer Function for Wavemaker of Deep Ocean Engineering Basin)

  • 본 연구는 심해공학수조의 파 생성 특성을 규명하고, 심해공학수조의 기하학적 특성을 반영한 조파기 전달함수의 근사모델을 개발하는 것을 목적으로 한다. 심해공학수조의 기하학적 특성을 반영한 효율적인 해석을 위해 2차원 주파수영역 경계요소법이 적용되었다. 개발된 수치해석법은 해석해와 비교하여 검증되었다. 다양한 주기와 이동식 바닥판의 위치 변화에 대한 조파기의 경계치 문제의 수치해석이 수행되었다. 수치결과를 통해 심해공학수조의 기하학적 특성이 조파기의 전달함수에 미치는 영향을 조사하였고, 이동식 바닥판의 위치를 변화시켜 파도 생성에 대한 깊이 효과를 확인하였다. 경계요소법의 다양한 결과를 효율적으로 활용하기 위해 박판 스플라인 보간 모델 기반의 조파기 근사모델을 개발하였다. 개발된 근사 모델의 타당성은 모델 테스트 결과 비교를 통해 확인하였다.

이중과제 훈련이 뇌졸중 환자에게 미치는 영향 : 체계적 고찰 및 메타분석 (Effects of the Dual-Task Training on Stroke Patients : A Systematic Review and Meta-analysis)

  • 목적 : 본 연구는 뇌졸중 환자를 대상으로 한 이중과제 훈련(Dual-task training)의 효과를 알아본 연구들을 분석하는 것을 목적으로 한다. 연구방법 : NDSL, RISS, PubMed, CoChrane, EMBASE를 통해 지난 10년 간 국내외 학술지에 게재된 논문들을 검색하였고, 최종적으로 10개의 논문을 선정하였다. 이를 전통적 단일계층 근거 모형에 따른 질적평가를 시행하고, Comprehensive Meta Analysis 3.0 프로그램을 통해 메타분석을 시행하였다. 결과 : 선정된 10편의 논문의 질적 수준은 모두 전통적 단일계층 근거 모형의 I과 II에 해당되어 높은 편에 속했다. 이중과제 훈련을 구성하고 있는 운동과제로는 7편의 문헌에서 보행 또는 균형 과제를 선택했으며, 3편의 연구에서 상지운동 관련 과제를 선정하였다. 메타분석을 실시한 결과 일상생활활동 수행능력이 0.65, 인지기능이 0.64로 보통(medium) 효과크기로, 하지운동 기능과 상지운동 기능은 각각 0.34와 0.22로 작은(small) 효과크기를 보였다. 일상생활활동 수행능력과 인지기능의 효과크기만 통계학적으로 유의미하였다(p<0.05). 결론 : 본 연구를 통해 이중과제 훈련이 뇌졸중 환자의 일상생활활동 수행능력 및 인지기능 회복에 유용한 중재기법이 될 수 있음을 확인하였다. 이는 국내 임상 환경에서 뇌졸중 환자에게 적절한 중재방법을 선정할 때 도움이 될 만한 자료로 활용될 수 있을 것이다.

Cardiac CT for Measurement of Right Ventricular Volume and Function in Comparison with Cardiac MRI: A Meta-Analysis

  • Objective: We performed a meta-analysis to evaluate the agreement of cardiac computed tomography (CT) with cardiac magnetic resonance imaging (CMRI) in the assessment of right ventricle (RV) volume and functional parameters. Materials and Methods: PubMed, EMBASE, and Cochrane library were systematically searched for studies that compared CT with CMRI as the reference standard for measurement of the following RV parameters: end-diastolic volume (EDV), end-systolic volume (ESV), stroke volume (SV), or ejection fraction (EF). Meta-analytic methods were utilized to determine the pooled weighted bias, limits of agreement (LOA), and correlation coefficient (r) between CT and CMRI. Heterogeneity was also assessed. Subgroup analyses were performed based on the probable factors affecting measurement of RV volume: CT contrast protocol, number of CT slices, CT reconstruction interval, CT volumetry, and segmentation methods. Results: A total of 766 patients from 20 studies were included. Pooled bias and LOA were 3.1 mL (-5.7 to 11.8 mL), 3.6 mL (-4.0 to 11.2 mL), -0.4 mL (5.7 to 5.0 mL), and -1.8% (-5.7 to 2.2%) for EDV, ESV, SV, and EF, respectively. Pooled correlation coefficients were very strong for the RV parameters (r = 0.87-0.93). Heterogeneity was observed in the studies (I2 > 50%, p < 0.1). In the subgroup analysis, an RV-dedicated contrast protocol, ≥ 64 CT slices, CT volumetry with the Simpson's method, and inclusion of the papillary muscle and trabeculation had a lower pooled bias and narrower LOA. Conclusion: Cardiac CT accurately measures RV volume and function, with an acceptable range of bias and LOA and strong correlation with CMRI findings. The RV-dedicated CT contrast protocol, ≥ 64 CT slices, and use of the same CT volumetry method as CMRI can improve agreement with CMRI.

가상공동체에서 지식탐색을 통한 지식공유에 관한 연구 (Understanding Knowledge Sharing in Virtual Communities through Knowledge Seeking Behavior)

  • This study investigated knowledge browsing behavior as the factor affecting the increase of knowledge sharing intention. To conduct this study in the specific context of knowledge seeking and sharing behavior of virtual community members, literature on knowledge seeking behavior, meta-knowledge, and knowledge sharing intention was reviewed. Structural Equation Modeling was conducted to analyze survey data to test the research model of this study. The result showed that knowledge browsing have positive effects on creating of virtual community members' subject knowledge and meta-knowledge, which, in turn, affected positively their knowledge sharing intention. One of the main contributions of this study is that knowledge seeking behavior influence one's knowledge sharing intention in a virtual community. Organization managers should consider knowledge seeking behavior as not only a self-interested, consuming activity, but also a productive one through its function of constructing subject knowledge and meta-knowledge.

A study on the duration of Ginkgo biloba extract effective in improving cognitive function in the elderly: A systematic review and meta-analysis

  • Evidence regarding the efficacy of Ginkgo biloba extract on cognitive function has been contentious. This study evaluated the effective period of G. biloba intake to improve cognition in the elderly. PubMed, EMBASE, Cochrane, Web of Science, and PsycArticles databases were searched for short-listing relevant studies. Twenty-five studies fulfilled the inclusion criteria. Cognitive efficacy was assessed based on the duration of intervention. G. biloba intake for 3-6 months statistically significantly affected cognitive function (SMD= -0.21; 95% CI -0.39, -0.03; p=0.02). However, the improvement in activities of daily living (ADLs) was not statistically significant. Thus, G. biloba intake for more than three months improves cognition in the elderly people with cognitive impairment and AD dementia without any safety risk. Intake for up to six months does not improve ADLs significantly in mild to moderate dementia patients.
