근로복지공단 보육시설의 급식 운영현황과 학부모대상 품질 만족도 (Parents' Opinions on Foodservices in Daycare Centers of Korea's Compensation and Welfare Service Institute)

  • 본 연구는 전국 근로복지공단 보육시설의 학부모를 대상으로 급식에 대한 이용실태, 인식 및 급식에 대한 요구도와 수행도를 조사함으로써 보육시설 급식의 질 향상과 합리적인 운영에 기초 자료를 제공하고자 하였다. 연구결과를 요약하면 다음과 같다. 조사대상 학부모의 연령대는 30대가 85.8%로 가장 많았으며, 어머니 직업은 직장인이 75.1%, 전업주부가 22.7%로 나타났고, 가정의 월 평균 소득은 350만원 이상이 23.7%, 200~250만원이 18.6%이었다. 재원 중인 유아의 연령은 만 2세가 20.4%, 만 3세가 29.4%, 만 4세가 23.2%이었고, 재원기간은 1년 미만이 37.5%이었다. 영양사가 배치된 곳이 73.9%이었으며, 배치되지 않은 곳이 26.1%이었고 영양사는 모두 상근직이었다. 원아의 식사 장소는 교실이 50.0%, 전용식당과 교실을 동시에 이용하는 곳이 36.4%이었다. 1일 급식인원은 평균 133.3명이었고, 1인당 급식 단가는 평균 1745.43원으로 조사되었다. 급식에서의 식사로 인해 변화된 식습관에 대해 조사한 결과 맛이 없거나 처음 보는 음식도 먹게 됨이 44.8%, 편식이 없어짐이 36.1%, 식사예절이 좋아짐이 29.9%이었다. 급식 필요성에 대한 인식도 점수는 평균 4.70점(필요하다)으로 보육시설에서의 급식 필요성을 높게 평가하였다. 급식이 필요한 이유에 대해서는 건강 향상을 위해서가 56.8%, 어린이집 종일제 프로그램 때문이 23.4%, 올바른 식습관 형성을 위해서가 18.2%로 나타났다. 보육시설의 급식 질 향상을 위해 가장 시급하게 고려되어야 할 사항에 대해 조사한 결과 안전한 식재료가 43.6%, 위생이 35.9%, 간식비 지원확대가 10.2%이었다. 보육시설 식사급식에의 요구사항에 대해 조사한 결과 유기농 식품 및 국산재료 사용이 60.9%, 제철재료사용이 9.2%, 조미료 사용 절제가 9.0%이었다. 간식급식에 대한 요구사항은 유기농 친환경 식재료 사용이 40.0%, 오전 간식으로 죽 제공이 35.1%, 오전 간식으로 제철과일 우유 제공이 13.4%이었으며 보육시설급식의 만족도를 높이기 위한 간식급식의 친환경 식재료의 사용 확대가 필요하겠다. 원아의 급식분량 지도에 대한 학부모의 의견을 조사한 결과 대부분의 학부모(66.4%)는 원아가 일정량을 다 먹도록 지도하길 원하는 것으로 나타났다. 식사급식에 만족하게 되는 판단 기준에 대해 조사한 결과 유기농 국산 식재료 사용이 62.3%, 제철 식품 이용이 54.3%, 위생관리가 52.5%, 편이 식품의 사용제한이 48.4%이었다. 급식 품질특성 32개 속성들을 축소된 영역으로 구분한 것에 관한 타당성을 검증하기 위하여 요인분석을 실시한 결과 식단, 급식효과, 시설, 위생, 식재료, 급식서비스 각 영역의 신뢰도 계수는 0.893~0.945로 문항의 내적일관성이 높은 것으로 분석되어 타당하게 구분된 것임을 알 수 있었다. 급식 품질특성에 대한 학부모의 중요도와 수행도 점수를 분석결과 속성과 영역 모두에서 중요도가 수행도보다 높게 평가되어 모든 품질에 대해 개선이 요구되는 것으로 나타났다. 중요도는 '적절한 1인 분량' 속성만을 제외한 31개 속성에서 4점 이상(요구한다)의 점수를 나타냈으며 위생영역과 식재료 영역에 속하는 속성들의 중요도 점수가 높았다. 현재의 급식 품질을 평가한 수행도 역시 '적절한 1인 분량 제공' 속성만을 제외한 모든 속성에서 4점 이상(그렇다)의 점수를 나타냈으며 시설영역에 속한 속성들의 수행도 점수가 상대적으로 낮았다. 중요도와 수행도 점수를 비교한 결과 메뉴영역의 '영양적 균형', '간식의 영양적 균형'(p<0.01), '간식 종류의 다양성' (p<0.05), '식사하는 장소의 분위기'(p<0.01), '식탁 및 의자의 위생'(p<0.001)과 위생영역의 '위생 및 소독시설 보유'(p<0.01), '음식의 위생', '식수의 위생', '조리실 내부의 청결', '급식종사자의 위생'과 식재료 영역의 '안전한 식재료의 사용', '신선한 식재료의 사용'이 유의한 차이를 나타낸 속성이었고 영역 중에는 위생영역과 식재료 영역(p<0.001)이 유의한 차이가 있었으며, 중요도 점수가 수행도 점수보다 높아 개선이 요구되는 속성과 영역이었다. 근로복지공단 보육시설 급식에 대한 전반적인 만족도를 조사한 결과 식사급식에 대한 전반적인 만족도 점수는 평균 4.33점(5점 만점), 간식급식에 대한 만족도는 평균 4.03점이었다. 또한 32개의 급식 서비스의 품질속성 중 '영양적 균형(p<0.05)', '간식의 종류의 다양성(p<0.001)', '급식 시 교사와의 의사소통(p<0.05)'이 전반적인 만족도에 유의한 영향을 미치는 것으로 나타났다. 따라서 전반적인 만족도에 영향을 미치는 신선하고 안전한 식재료의 사용과 다양한 제철음식 사용을 위한 급식 단가의 인상과 균형 잡힌 간식 및 급식 제공을 위한 급식 프로그램 개발이 요구된다. 식습관 교정 및 바른 위생습관을 위해 보육시설에서의 영양사에 의한 정기적인 영양교육을 실시하여야 한다. 영유아의 전문적인 급식 관리를 위해 영양사 배치기준 현실화가 필요하겠다. 또한 유기농 식품 및 국산재료 사용에 대한 학부모의 요구도가 높게 나타났고, 급식 식재료 질 관리가 요구된다. 중요도 점수와 수행도 점수 간 비교에서 유의한 차이를 나타낸 급식의 영양적 균형과 조화의 수행도 개선을 위해 식단 작성 시 영양소의 균형과 영유아 발달에 따른 기호도를 반영하여 개선시키도록 해야 하며 실질적이고 체계적인 영양관리가 요구되며 식탁 및 의자의 안전성과 위생의 개선을 위해 보육시설의 물리적 환경 개선이 요구된다. 전용식당이 없는 보육시설은 영유아기는 면역력이 약하여 세균에 감염될 가능성이 높고 보육시설에서 장시간 아동들이 같이 생활하기 때문에 급식이 각 반에서의 급식 실시 시 위생 관리상 문제가 제기될 수 있어 별도의 식당이 설치되어 운영하도록 해야 한다. 또한 위생, 청결, 안전관리에 문제가 발생될 수 있는 여지가 있다는 점에서 향후 전용식당 확보를 위한 개 보수 지원이 필요하다. 식단관리에서 식재료의 양 및 질의 관리, 영양가 분석, 맛, 비용관리와 조리원과의 정확한 의사소통을 위해 표준 레시피에 의한 식단관리체계를 확립하고, 근로복지공단 보육시설 특성에 맞는 표준 레시피 개발이 요구된다. 또한 근로복지공단 보육시설은 저소득 기혼여성근로자의 육아부담을 해소하여 지속적인 취업활동을 보장하고자 영세 중소기업체 밀집지역에 설치되므로 지원체계 구축이 요구된다. 또한 질 높은 보육서비스를 제공하고 보육시설의 현재 수준을 점검하며 개선하도록 하기 위한 보육시설 평가인증제도의 평가인증지표 중 조리실과 식자재 관리 및 급식과 잔반관련항목이 3~4 항목으로만 되어 있고 그 항목 또한 구체적이지 않다. 따라서 급식의 만족도에 영향을 미치는 급식품질 속성을 고려한 실질적인 평가 척도로의 개선이 필요하겠다.

홍콩 소비자의 식생활 라이프스타일에 따른 HMR 소비실태와 제품개발 요구도 (Home Meal Replacement Consumption Status and Product Development Needs according to Dietary Lifestyle of Hong Kong Consumers)

  • 본 연구의 목적은 홍콩 소비자를 대상으로 가정식사대용식(HMR, Home Meal Replacement) 제품의 소비실태와 HMR 제품 개발 요구도를 파악하고, 식생활 라이프스타일에 따른 HMR 시장세분화 전략을 제시하고자 하였다. 홍콩 전문기관에 등록된 패널 중 가정식사대용식을 이용한 경험이 있는 소비자 521명을 대상으로 온라인 설문조사를 통해 수집된 자료를 SPSS(ver. 23.0) 통계 프로그램을 이용하여 분석하였다. 홍콩 소비자의 식생활 라이프스타일에 따른 군집별 HMR 구입 특성은 HMR 유형별 구매횟수, 구매장소, 1회 구매 시 비용, 구매이유 등 모든 항목에서 집단 간 유의적인 차이를 나타냈다. 식생활 라이프스타일에 따라 '식생활 고관심 집단', '식생활 중관심 집단', '식생활 저관심 집단'으로 군집을 나누었으며, HMR 제품 개발 요구도는 '식생활 고관심 집단'은 저염 식품> 발열식 편의식> 저당 식품> 저칼로리 식품 순으로, '식생활 중관심 집단'은 저염 식품> 저당 식품> 저칼로리 식품> 영양적 완전식품 순으로, '식생활 저관심 집단'은 저당 식품> 저염 식품> 다양한 신메뉴> 휴대가 간편한 건조식품 순으로 집단 간 차이를 보여주었다. 군집별 '식생활 고관심 집단'은 남성의 비율이 높았고 연령대로는 20~29세의 젊은 층이며, 직업은 사무 관리직과 기혼의 비율이 높았다. HMR 구매 시 1회 비용은 15,000~20,000원 미만이 가장 높게 조사되었다. '식생활 중관심 집단'은 여성과 남성의 비율이 비슷하게 나타났으며, 연령대로는 30~39세, 기혼의 비율이 높게 나타났다. HMR 구매 시 1회 비용으로는 10,000원 미만과 10,000~15,000원대의 중 저가격대를 구매하는 것으로 나타났다. '식생활 저관심 집단'은 여성과 미혼 그리고 사무 관리직의 비율이 높았으며, HMR 구매 시 1회 비용은 10,000원 미만의 비율이 가장 높아 상대적으로 다른 집단보다 저가의 HMR 제품을 구입하는 것으로 나타났다. 홍콩시장 진출을 위한 세분시장별 HMR 제품 개발 및 마케팅 전략을 제시하면 다음과 같다. '식생활 고관심 집단'은 식생활 관심이 높은 집단으로 시장세분화 전략의 집중과 투자가 이루어져야 할 집단으로 생각된다. HMR 제품 유형의 대안으로는 현재 홍콩 소비자들이 관심을 보이는 건강 웰빙에 주목하여 영양적인 면을 고려한 프리미엄급의 HMR 제품, 건강을 코드로 한 제품 개발이 필요하며, 식품안전관리인증기준(HACCP)에 근거한 식재료 선택의 진정성과 안전성까지 고려해야 할 것이다. '식생활 중관심 집단'은 안전과 건강을 중요시하며 HMR 제품의 미각 요인에 관심을 가지는 집단으로 맛에 중점을 둔 다양한 HMR 신제품의 출시가 필요하리라 생각된다. '식생활 저관심 집단'은 간편성과 편의성을 추구하는 집단으로 간편하여 추가적인 조리가 필요 없는 완성된 음식으로 시간을 절약할 수 있으며, 가격의 합리화가 이루어져야 할 것이다. 앞으로 HMR 제품의 방향은 건강을 코드로 하여 집에서 만드는 것과 같은 맛과 정성까지 고루 갖추며, 편의성을 고려한 HMR 제품이 필요한 때이다. 이를 위해서는 유기농, 친환경 식재료를 사용하고 고객의 편리성을 극대화할 수 있도록 반조리 제품의 다양화가 필요할 것이다. 또한, 소량, 소포장, 12시간 이내에 소비자에게 배송되는 서비스, 바코드나 QR코드로 영양성분이나 조리법까지도 알 수 있는 서비스 등 간편과 웰빙을 추구하는 홍콩 소비자의 니즈를 반영한 HMR 제품의 출시에 기업과 정부의 지속적인 노력이 필요할 것으로 생각된다. 본 연구 결과는 HMR 제품 개발을 위한 마케팅 전략에 필요한 기초 자료로 활용될 수 있을 것이며, 수출용 HMR 제품 개발에 소비자의 니즈가 반영된 신제품의 방향을 제시하여 산업적 활용 전략을 구축하기 위한 시장세분화 전략에 기초 자료를 제공하고자 하였다.

폐암의 호흡동조방사선치료 시 변형영상정합을 이용한 4차원 선량평가 (4-Dimensional dose evaluation using deformable image registration in respiratory gated radiotherapy for lung cancer)

  • 목 적 : 폐암의 호흡동조방사선치료(Respiratory Gated Radiotherapy, RGRT)계획수립 후 표적 주변에 위치하고 있는 정상장기의 경우에는 움직임과 용적변화가 고려되지 않은 상태에서 선량평가가 이루어지는 경우가 많다. 본 연구에서는 적응형방사선치료(Adaptive Radiotherapy, ART)에서 많이 사용되는 변형영상정합(Deformable Image Registration, DIR)을 이용하여 호흡동조방사선치료 시 특정 위상에서의 정상장기의 움직임을 반영한 4차원-선량평가를 진행하였으며, 3차원 선량평가와의 차이를 연구하였다. 또한, 폐암의 치료계획평가 시 환자 호흡에 따른 정상장기의 움직임과 용적변화에 대한 분석 및 고려가 필요한 지 알아보고자 한다. 대상 및 방법 : 호흡동조방사선치료를 받은 폐암 환자 10명을 대상으로 하였다. Eclipse(Ver 13.6 Varian, USA)로 최고 위상 CT영상에 그려진 구조물을 모든 위상영상에 Propagation($Eclipse^{TM}$)이나 Segmentation Wizard($Eclipse^{TM}$)의 메뉴로 동일하게 설정하였으며, Center-to-Center 방식으로 구조물의 움직임 및 용적을 분석하였다. 또한, 4차원 선량평가를 위해 VELOCITY 프로그램(VELOCITY Ver 4.0, Varian, USA)을 이용하여 각 위상의 영상과 선량분포를 최고 위상 CT영상에 변형하였으며, 선량을 합산하여 정상장기의 4차원 선량평가를 실시하고, 3차원 선량평가와 비교분석을 하였다. 또한, 4차원 선량분포의 검증을 위해 $QUASAR^{TM}$ Phantom(Modus Medical Devices)과 $GAFCHROMIC^{TM}$ EBT3 Film(Ashland, USA)을 사용하여 4차원 감마분석을 시행하였다. 결 과 : 들숨과 날숨 구간의 움직임은 우측 폐가 축 방향 $0.989{\pm}0.34cm$로 가장 컸으며, 척수가 측 방향 -0.001 cm로 가장 작았다. 30~70 % 구간의 움직임은 식도가 축 방향 $0.52{\pm}0.21cm$로 가장 컸으며, 척수가 전후방향 $0.013{\pm}0.01cm$로 가장 작았다. 용적은 우측 폐가 33.5 %로 가장 큰 변화율을 보였다. 3차원 선량평가와 4차원 선량평가에서의 PTV 선량균질지수(Conformity Index, CI) 값과 처방선량지수(Homogeneity Index, HI) 값의 차이는 각각 최대 0.076, 0.021, 최소 0.011, 0.0으로 평가되었다. 정상장기의 경우 4차원 선량평가에서 0.0045~2.76 % 차이를 보였다. 모든 환자의 4차원 감마통과율은 평균 $98.1{\pm}0.42%$로 확인되었고, 모두 기준 95 %를 통과하였다. 결 론 : 모든 환자의 PTV 선량균질지수 값은 4차원 선량평가 시 더 유의한 값임을 확인할 수 있었으며, 처방 선량지수는 두 선량평가에서 차이를 보이지 않았다. 호흡에 의한 움직임이 고려된 4차원 선량분포에서 PTV 경계부분이 채워져 3차원 선량분포에서보다 선량이 더욱 균질한 것을 확인할 수 있었다. 정상장기의 4차원 선량평가에서 0.004~2.76 % 차이가 있었으며, 척수를 제외한 모든 정상장기에서 두 평가방법의 차이유의를 확인할 수 있었다. 정상장기의 3차원 선량평가 시 과소평가가 이루어 질 수 있다는 사실을 본 연구를 통해 알 수 있었으며, 호흡에 의한 정상장기의 선량변화가 예상되는 경우 변형영상정합을 이용한 4차원 선량평가를 고려할 수 있을 것이다. 변형영상정합을 이용한 4차원 선량평가는 환자의 호흡에 의한 정상장기의 움직임과 용적 변화를 반영하는 조금 더 현실적인 선량평가방법이 될 것이라고 사료된다.

제휴카드 할인프로그램이 외식업의 수익성에 미치는 영향 (A Study on Profitability of the Allianced Discount Program with Credit Cards and Loyalty Cards in Food & Beverage Industry)

  • 외식업계에서는 충성도제고를 위한 장기적 고객 관리 프로그램으로서 사용금액 대비 일정률의 포인트를 적립해주는 고객보상(로열티)프로그램을 시행해왔으며, 단기적인 매출 증대를 목적으로 신용카드사나 이동통신사와의 전략적 제휴를 통해 소비자에게 할인혜택을 제공하는 제휴할인카드를 경쟁적으로 도입하고 있다. 포인트 적립과 제휴카드할인 프로그램은 추구하는 목적은 서로 다르나 소비자에게는 동일한 구매시점에 둘 중에서 하나를 선택해야하는 경합적인 대안이 된다. 일반적으로 소비자는 손실에 대해 더 민감하게 반응하는 위험회피 성향을 보이며, 실용적 동기에 의한 취득 및 거래효용의 관점에서 할인이 강력한 구매 동기가 되므로, 분리된 이익으로서의 포인트 적립 보다 할인 혜택을 제공함으로써 손실의 감소로 인식되는 제휴카드할인이 더 선호될 것으로 예상할 수 있다. 기업의 입장에서도 제휴기업간의 비용분담을 통해 보다 적은 비용으로 큰 소비자 할인혜택을 제공할 수 있다는 비용효율의 장점 및 경쟁 대응 필요성 때문에 제휴카드할인을 도입하게 된다. 그러나 할인판촉은 실제적인 비용이 투입되므로 매출의 증가를 통해 비용 보다 큰 수익이 보장될 때만 의미를 가지는 촉진 수단이다. 외식업의 경우 방문객수가 증가해야 매출의 증대를 이룰 수 있으며 운영효율이 향상된다. 테이블당 고객수 증가는 공간효율을 높여주어 이익개선에 기여가 크다. 따라서 제휴카드할인이 실제적인 기업성과향상을 가져오려면 할인이 객수의 증가 특히 테이블당 객수의 증가에 기여를 해야 한다. 제휴카드할인이 객수증가 및 이익률의 향상에 도움이 되는지의 여부를 알아보기 위하여 패밀리 레스토랑업체의 실제 매출 데이터를 사용하여 검증해 본 결과 포인트 적립제도는 객수 증가와 유의적 관계를 보이지 않은 반면 제휴할인은 객수 증가에 실제적인 영향이 있음이 밝혀졌으며, 할인율이 증가하면 테이블당 고객수도 증가하며 약 25%의 할인이 제공되면 추가로 한 명의 고객이 유인되는 효과가 있었다. 또한 수익성의 측면에서는 1인당 지불금액 즉 객단가는 할인율의 증가에 따라 하락하는 경향을 보였지만 객수의 증가로 인한 추가이익이 추가비용을 상쇄하고 있어서 제휴할인카드의 도입이 비용 대비 효과의 측면에서 적극적으로 고려할 수 있는 촉진 수단임이 입증되었다. 본 연구는 기업의 실측 데이터를 활용하고 있다는 점에서 소비자 태도나 의도의 변화를 기준으로 기업의 의사 결정 정보를 제공하고자한 기존연구보다 현실적인 접근을 하고 있다. 즉, 기업의 실제 데이터로 제휴카드할인 프로그램의 효과를 측정하였고, 포인트 적립 및 차감제도를 직접 비교함으로써 기업에게 향후 마케팅 전략 수립에 관한 현실적인 시사점을 제공하였다는 점에서 연구의 의의를 찾을 수 있을 것이다.

  • 본 연구는 제주지역의 요양병원과 요양원 영양사의 직무중요도와 직무수행도 및 직무만족도를 조사 분석함으로써 장기요양 보호를 필요로 하는 노인을 대상으로 하는 시설의 영양사 업무의 문제점을 파악하고 노인급식 및 영양 관리의 질적 향상방안을 모색하는데 기여하고자 하였다. 조사대상 영양사의 성별은 여자가 94.7%로 대부분을 차지하였고 연령별로는 만40~49세 (36.8%)가 가장 많았으며 기혼 (68.4%)이 미혼 (31.6%)보다 높은 비율을 보였고 근무경력은 8년 이상 (34.2%)이 가장 많았고 현재 근무하는 요양 (병)원 근무기간은 1년 미만 (28.9%), 1~3년 미만 (21.1%)과 3~5년 미만 (21.1%), 5~8년 미만 (18.4%), 8년 이상 (10.5%) 순으로 나타났으며 학력은 4년제 대학 졸업이 65.8%를 차지하였고, 근무시설은 요양원이 84.2%, 근무지역은 제주시가 71.1%를 차지하였고 급여는 150~200만원 미만이 65.8%로 나타났다. 조사대상 요양 (병)원은 50~100 병상 미만인 시설 (78.9%)이 가장 많았고, 환자 수에 있어서는 50~100명 미만이 84.2%로 대부분을 차지하였으며, 1식 단가는 1,501~2,000원이 50.0%로 나타났고 영양사수에 있어서는 1명만 근무한다고 응답한 시설이 94.7%로 대부분을 차지하였으며, 조리종사원수는 3~5명 미만이 76.3%로 나타났다. 조사대상 영양사의 직무중요도는 5점 만점에 대해 평균 4.29점으로 나타났고 위생 및 안전관리 (4.77점), 조리작업 관리 (4.44점), 회계관리 (4.30점), 구매관리 (4.29점), 인사관리 (4.19점), 메뉴관리 (4.18점), 영양관리 (3.86점)의 순으로 나타났다. 각 영역별 세부 항목의 직무중요도에 있어서는 평균 점수 이상으로 중요도가 높게 나타난 업무는 구매관리 영역의 식품검수 (4.84점), 급식재료의 발주 및 수불 관리 (4.68점), 재고조사 및 저장식품 관리 (4.66점), 메뉴관리 영역의 일반식단 작성 (4.68점), 위생 및 안전관리 영역의 식재료 위생관리 (4.87점), 급식시설설비 위생관리 (4.84점), 조리원 위생관리 (4.76점), 조리장내 위생관리 (4.76점), 개인위생관리 (4.76점), 위생 및 안전관리일지 작성 (4.63점), 조리작업관리 영역의 조리지도 및 감독 (4.61점), 배식지도 및 감독 (4.50점), 작업일정계획 및 업무분담(4.42점), 인사관리 영역의 조회 및 업무회의를 통한 의사 소통 (4.61점), 회계관리 영역의 예산관리 (4.32점), 결산관리 (4.29점), 원가관리 (4.29점)였다. 반면, 조사대상의 직무중요도 점수가 낮게 나온 업무는 영양관리 영역의 직원 대상 영양교육 (3.61점), 인사관리 영역의 자원봉사자 모집배치와 급식서비스 교육관리 (3.63점), 메뉴관리 영역의 기호도 및 만족도 조사 (3.89점), 치료식 메뉴 개발 (4.00점)의 순으로 나타났다. 조사대상 영양사의 직무수행도는 5점 만점에 대해 평균 2.87점으로 나타나 직무중요도에 비해 매우 낮은 점수를 보였고, 업무 영역별로 살펴보면 조리작업관리 (4.42점), 위생 및 안전관리 (4.21점), 구매관리 (3.32점), 메뉴관리 (2.53점), 인사관리 (2.45점), 영양관리 (1.67점), 회계관리 (1.52점)의 순으로 나타났다. 영역별 세부 항목에 대한 직무 수행도를 분석한 결과 조리작업관리 영역의 조리지도 및 감독 (4.92점), 배식지도 및 감독 (4.79점), 위생 및 안전관리 영역의 위생 및 안전관리 일지작성 (4.71점), 식재료 위생관리 (4.66점), 구매관리 영역의 식품검수 (4.63점)의 순으로 수행도가 높게 나타났다. 한편 조사대상의 직무수행도가 낮게 나타난 업무는 영양관리 영역의 직원 대상 영양교육 (1.13), 치료식 대상 집단 영양교육 (1.24), 회계관리 영역의 결산관리 (1.32), 영양관리 영역의 영양 자료게시 및 노인영양교육 자료개발 (1.45)과 기호도 및 만족도 조사 (1.45) 순으로 나타났다. 조사대상 영양사의 직무중요도와 수행도를 영역별로 격자도 분석 (IPA)한 결과, 중요도와 수행도가 모두 높은 B사분면에는 위생 및 안전관리, 조리작업관리, 중요도와 수행도가 모두 낮은 C사분면에는 메뉴관리, 인사관리, 영양 관리가 속하였다. 구매관리와 회계관리 영역은 직무중요도는 평균 수준이었으나, 구매관리 수행도는 평균 이상으로 높았고 회계관리 수행도는 매우 낮은 것으로 나타났다. 영역내 세부 항목별로 격자도 분석 (IPA)한 결과 위생 및 안전관리 영역의 조리원 위생교육은 A사분면, 다른 항목들은 B사분면에 속하였고, C사분면에 속한 항목은 구매관리 영역의 시장조사하기, 급식통계작성 및 분석, 일반소모품의 청구 및 수급관리의 3항목, 메뉴관리 영역의 치료식식단 작성, 제공 식단의 영양분석, 치료식 메뉴개발, 일반식 메뉴개발, 표준레시피 관리, 기호도 및 만족도 조사의 6항목, 영양관리 영역의 모든 항목, 인사관리 영역의 리더십 관리, 급식업무 매뉴얼의 작성 및 비치, 조리종사원의 인사 관리, 자원봉사자 모집 배치와 급식서비스 교육관리의 4항목, 회계관리 영역의 결산관리, 원가관리의 2항목으로 나타났다. 구매관리 영역의 후원물품접수 및 관리 항목과 조리작업관리 영역의 잔식 및 쓰레기 감량관리 항목은 중요도가 낮고 수행도가 높은 D사분면에 속하였다. 조사대상 영양사의 직무만족도는 5점 만점에 대해 평균 3.37점으로 나타났고 동료에 대한 만족도가 3.72점으로 가장 높았으며, 직무자체 (3.63점), 의사소통 (3.43점), 근무환경 (3.40점), 상사의 감독 (3.34점), 평가와 보상 (3.15점), 전문성 신장 (2.95점)의 순으로 나타났다. 조사대상 영양사의 직무중요도와 직무만족도 (r = 0.395, p < 0.05), 직무수행도와 직무만족도 (r = 0.386, p < 0.05)는 유의적으로 높은 상관관계를 보였다. 이상의 연구 결과를 토대로 살펴볼 때, 조사대상자의 직무수행도 평균 점수가 직무중요도 점수에 비해 현저히 낮게 나타났고 위생 및 안전관리, 조리작업관리 등 급식 관련 업무에 비해 영양관리 영역의 중요도 및 수행도가 낮게 나타나 본 연구의 조사대상 시설이 만성 혹은 노인성 질환을 앓고 있는 고령자가 많다는 점을 고려하여 직무중요도와 수행도가 모두 낮은 영양관리 영역에 대한 인식 제고와 수행도 향상을 위한 방안이 모색되어야 할 것으로 사료된다. 또한, 조사대상의 직무만족도가 가장 낮게 나타난 전문성 신장과 관련하여 '섬'이라는 지역적 한계를 극복할 수 있도록 제주지역 내에서 다양한 전문 교육이 실시되어야 하며 영양사 스스로 전문가로서의 자질 향상을 위해 부단히 노력해야 할 것으로 사료된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. Because the point of maximun temperature of laser-irradiated gelatin exists below the surface due to surface cooling, evaporation at the boiling temperature is made explosively from below the surface. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm.   • 정보기술의 활용이 마케팅 전략의 중요한 부분으로 부각되는 시장 환경에서 본 논문에서는 패스트푸드 레스토랑에서 주문시 Self Service Technology(SST)를 사용 할 때 기술준비도와 소비자 준비도에 따른 사용동기, 태도, 사용의도의 차이를 검증하고자 하였다. Parasuraman이 TRI를 개발한 이후 SST와 TRI를 접목한 연구가 이루어져 왔지만 여전히 부족한 실정이다. 소비자 준비도 역시 SST사용에 대한 소비자의 동기와 태도 및 의도와 직접적인 영향을 미칠 수 있지만 이에 대한 연구는 이루어지지 않았다. 이에 본 연구에서는 패스트푸드 레스토랑에서 터치스크린 SST를 도입함에 있어 소비자의 기술준비도와 소비자 준비도가 Dabholkar & Bagozzi(1994)가 제안한 SST 핵심태도모델에 미치는 영향을 고찰하였다. 이때 모든 소비자와 상황적 요인에 따른 차이를 파악하고자 자아의식, 상호작용욕구, 기술에 대한 두려움 등의 소비자특성과 지각된 대기시간, 지각된 과밀 등의 상황적 요인에 따른 조절 효과를 검증하였다. AMOS 18.0프로그램을 사용하여 구조방정식 모델로 분석하였고 연구 결과 기술준비도 중 낙관성은 사용용이성과 재미 동기에 유의한 영향을 미치는 것으로 나타났다. 혁신성은 사용용이성과 성과에 유의한 영향을 미치는 것으로 나타났다. 역할의 명확성, 능력 및 자아 효능감으로 구성된 소비자준비도는 사용용이성, 성과와 재미 등 모든 SST사용동기 요소에 기술준비도보다 강하고 유의한 영향을 미치는 것으로 나타났다. 마지막으로 SST 핵심태도모델 내의 SST 사용 동기, 사용 태도 및 사용의도 간의 관계에서 소비자 특성과 상황적 요인의 조절효과를 검증한 결과 지각된 과밀을 제외한 모든 변수들이 조절효과를 지니는 것으로 나타났다. 이러한 연구 결과를 바탕으로 터치스크린 SST를 도입하려는 기업에 실무적 제언을 하였다.
