Observation Practice Using a Human Body Model in Medical Terminology Class (의학용어 수업에서 인체 모형을 이용한 관찰 실습)

  • Biomedical engineering is a discipline that diagnoses and treats human diseases using engineering techniques based on medical and biological understanding. Proper biomedical engineering education requires education on medical terminology, human anatomy, and human physiology, but students have a preconceived notion that these basic medical subjects are subjects to be memorized. In order to eliminate these students' preconceptions, various educational methods must be developed so that students can easily access basic medical subjects. In this paper, we present a method to increase learning effectiveness by introducing observation practice of a human anatomical model to the medical terminology subject. The half-body model of the human body is a form in which various organs are assembled and can be observed by disassembling them one by one. This observation exercise consisted of questions about the organs of the head, neck, chest, and abdomen, with students working in groups to find answers. After the practice, students evaluated that this practice motivated them to learn and made it easier to understand the lecture.

The scope and learning objective of Medical classics in the field of the education of Korean Medicine (한의학 교육 분야에서의 원전학(原典學)의 범위 및 학습 목적)

  • Objective : This study was conducted to examine the scope and learning objective of Medical classics in the field of the education of Korean Medicine. Method : This study was analyzed and figured out list of classes which was taken by department of medical classics in eleven College of Korean Medicine and one school of Korean Medicine. Results & Conclusions : 1. Now, 14 subjects out of 16 subjects which were taken by whole department of medical classics in Korea can be a proper area of education of medical classics. Now, Hwangjenaekyung and Nangyeong are the only aim of the lessons at the medical classics. Therefore, we present to modify the aims inclusively. 2. The subject of the class have to change as follows. 'Wonjeon' changes into 'Hwangjenaekyung', 'Medical Chinese character' changes into 'Korean medical chinese character', 'medical informatics' changes into 'korean medical informatics'. 3. As we consider the condition that 'Nangyeong' is educating in just four departments of medical classics, we have to discuss about the stature of Nangyeong and to extend education of Nangyeong. 4. In the department of medical classics, we can improve the level of understanding and reading skills by educating the class of 'Sanghanron' and 'Donguibogam'. 5. This study is actively involved in trying to include 'Korean Medical Informatics' and 'Korean medical terminology ' in the education field of the medical classics.

Initial clinical outcomes of proton beam radiotherapy for hepatocellular carcinoma

  • Purpose: This study aimed to evaluate the initial outcomes of proton beam therapy (PBT) for hepatocellular carcinoma (HCC) in terms of tumor response and safety. Materials and Methods: HCC patients who were not indicated for standard curative local modalities and who were treated with PBT at Samsung Medical Center from January 2016 to February 2017 were enrolled. Toxicity was scored using the Common Terminology Criteria for Adverse Events (CTCAE) version 4.0. Tumor response was evaluated using modified Response Evaluation Criteria in Solid Tumors (mRECIST). Results: A total of 101 HCC patients treated with PBT were included. Patients were treated with an equivalent dose of $62-92GyE_{10}$. Liver function status was not significantly affected after PBT. Greater than 80% of patients had Child-Pugh class A and albumin-bilirubin (ALBI) grade 1 up to 3-months after PBT. Of 78 patients followed for three months after PBT, infield complete and partial responses were achieved in 54 (69.2%) and 14 (17.9%) patients, respectively. Conclusion: PBT treatment of HCC patients showed a favorable infield complete response rate of 69.2% with acceptable acute toxicity. An additional follow-up study of these patients will be conducted.

Literature for YangsaengKigong (한국전통사상으로 살펴본 양생기공에 대한 고찰)

  • Although the demand on 'Ki' training because of the increased interest in health, there has not been an established theory and system in 'Ki' training from the perspective of oriental medicine. Therefore, many training associations are in operation without sound basis. The general use of the terminology 'Kigong' is the influence of China. As it was distributed to the general public through the translation and introduction of Chinese Kigong theory, the genuine Korean YangsaengKigong was not emphasized but it was included in or considered as the second-class to the Chinese Kigong. The purpose of the study was to establish and review YangsaengKigong based on our traditional ideologies. Korean Yangsaengkigong can be defined as the training method nurturing mind, energy, body based on the Sambeob training method such as Pause of thinking, Modulation of breath and Prohibition of sensibility with the goal of Yangsaengkigong, good health and long life and the maximum service for the benefit of humanity. Although this study can identify the differecnes between Korean Yangseangkigong and Chinese kigong through Korean traditional ideology, we could not develop this study to the necessity of ideological background of medical Kigong and concrete methodology of Yangsaengkigong and our traditional ideology because of scarce literature on Korea and Korean traditional ideologies. For the purspose of the study, we need cooperation between Studies of Korea, Studies of National History, and Oriental Medicine.

Treatment outcome of hepatic re-irradiation in patients with hepatocellular carcinoma

  • Purpose: We evaluated the efficacy and toxicity of repeated high dose 3-dimensional conformal radiation therapy (3D-CRT) for patients with unresectable hepatocellular carcinoma. Materials and Methods: Between 1998 and 2011, 45 patients received hepatic re-irradiation with high dose 3D-CRT in Samsung Medical Center. After excluding two ineligible patients, 43 patients were retrospectively reviewed. RT was delivered with palliative or salvage intent, and equivalent dose of 2 Gy fractions for ${\alpha}/{\beta}=10Gy$ ranged from $31.25Gy_{10}$ to $93.75Gy_{10}$ (median, $44Gy_{10}$). Tumor response and toxicity were evaluated based on the modified Response Evaluation Criteria in Solid Tumors criteria and the Common Terminology Criteria for Adverse Events (CTCAE) ver. 4.0. Results: The median follow-up duration was 11.2 months (range, 4.1 to 58.3 months). An objective tumor response rate was 62.8%. The tumor response rates were 81.0% and 45.5% in patients receiving ${\geq}45Gy_{10}$ and $<45Gy_{10}$, respectively (p = 0.016). The median overall survival (OS) of all patients was 11.2 months. The OS was significantly affected by the Child-Pugh class as 14.2 months vs. 6.1 months (Child-Pugh A vs. B, p < 0.001), and modified Union for International Cancer Control (UICC) T stage as 15.6 months vs. 8.3 months (T1-3 vs. T4, p = 0.004), respectively. Grade III toxicities were developed in two patients, both of whom received ${\geq}50Gy_{10}$. Conclusion: Hepatic re-irradiation may be an effective and tolerable treatment for patients who are not eligible for further local treatment modalities, especially in patients with Child-Pugh A and T1-3.

Analysis of the Effect of Self-Directed Learning Method in Medical Team Education (의학용어학습에서 자기주도학습준비도 촉진 수업방식의 효과 분석)

  • This study was designed to examine whether the self-directed learning method could improve self-directed learning readiness and the effects of academic achievement level. Self-directed learning readiness was investigated among 63 first-year Medical Terminology undergraduates in the C area. A repeat measurement variance analysis of the general linear model was conducted to evaluate the effects of improving self-directed learning readiness according to the general characteristics and level of academic achievement, while a regression analysis was performed to identify the factors affecting self-directed learning readiness. Self-directed learning readiness increased from 177.3 to 180.8 for those under 18 years of age, and 192.9 to 196.5 for those over 19 years of age (p<0.05). After the team activity, the overall self-directed learning readiness was improved, and both high- and low-achieving groups showed statistically significant improvements (p<0.05). The environment surrounding learners was confirmed to have a positive effect on improving self-directed learning when given the right degree of self-directed learning and appropriate feedback. The study results are expected to form basic foundation material for professors and class designers who want to draw self-directed learning skills from memorizing subjects.

Status of Herbal-drug-associated Adverse Drug Reactions Voluntarily Reported by EMR (1개 대학 한방병원에서 EMR을 통해 보고된 한약에 의한 약물유해반응의 현황)

  • Objectives : The aim of this study was to systematically investigate herbal-drug-associated adverse drug reactions (herbal ADRs) reports submitted by a single oriental hospital and to analyze the general characteristics, causative agents, clinical manifestations, severity and types of herbal medicines which caused herbal ADRs. Methods : This study proceeded with IRB approval. The data on herbal ADR were collected prospectively from January 2008 to February 2012 by EMR of Dongguk University Ilsan Oriental Hospital. The World Health Organization (WHO)-Uppsala Monitoring Center (UMC) criteria was used to determinate causality for each herbal ADR. WHO-Adverse Reaction Terminology (WHO-ART) System Organ Class (SOC) code and WHO severity category were also used in this study. Results : A total of twenty eight cases were reported. Twenty two cases were assessed to have over possible relations with herbal medication. The gender ratio of these cases were 64.6 percent female and 36.4 percent male, demonstrating no statistical significance. Patients aged over 60 were 59.1%. Gastro-intestinal system was reported to be the most frequently affected organ (38.8%), and followed by psychiatric system (22.4%), and integumentary system (22.4%). The most common clinical symptom was headache (12.2%), followed by diarrhea (10.2%), and pruritus (10.2%). The severity of most cases was assessed to be mild (89.8%). The percentage of moderate ones was 10.2%, and there were no severe cases. Conclusions : Progressive study and further analysis on herbal ADRs are warranted for safety in the clinical use of herbal medicines.

STATISTICAL STUDY ON HABITUAL METHODS Of TOILET TRAINING FOR INFANT AND TODDLER (한국 영.유아의 대소변 가리기 훈련습성에 관한 일 연구)

  • This study observed on habitual methods of toilet training for infants and toddlers in Korea thru 300 mothers at four medical institutions Ewha Woman′s University Hospital, Ewha Maternal and Child Health Center, Severance Hospital and Seoul National University Hospital) with a design to analyze beginning time, duration of period, methods employed, motives, equipments used, special terminology used for the subject training. The main purpose of this study was to generalize the proper methods of toilet training, and also to contribute a better psychological education for the mother and child. The results obtained from this study were as follows; 1. Majority of (67.3%) observed were in the age category between 30 and 40 years, and dominant numbers (64.7%) were housewives with high level of education and from middle class family background. 2. In the most cases (85%), toilet training was carried out by mothers(including wives who had a job) while more than half of mothers (53.7%) maintained their important motive for the training was "due to having high regard for cleanliness" 3. As for the time of beginning toilet training: finding indicated that starting period was decided (70%) at inconsiderate desertion of each mother. whereas, only minority group (30%) represents the cases where mother started the training when they consider tile child was physically and psychologically randy. Also greater number (77.7%) started bladder training prior to that of bowel. 4. It is noticeable that in course of training a large number of mothers (48.3%) applied strict training method when the child proper talenting, and the more rigid and strict in tile training. the more malformation of personality of the infant and toddler were seen after the training period (P<0.01). 5. Over the half of the total cases denoted (bowel 54.3%, bladder 67.7%) starting period before one year and in most cases (bowel 79.3%, bladder 729)the training was accomplished within 12 month, and therefore it was noted that earlier start(before 1 year) shortened the training period (with 12 month) . There was no significant difference between male and female infants in both starting period and duration of period in bladder training, however, in bowel training there was a tendency that female started earlier(7-12 months needed, 51.4%) than tile male (13-18 months needed, largest number 41.4%), and also in cases of female the period for needed for training were shorter than the cases of male. 6. Many a number (bowel 50.3%, bladder 97.7%) employed the method of continuous talenting at regular interval in accordance with that of child′s habit formed before training. Equipment used were various kinds, however, pieces of paper for male (45.5%) and piss pot or bedpost (42.3%) for female were common, on the other hand, "Eung-ga" for defecation (52.3%) and "Shii" for the urination (95.3%) were most standard expression that used during the training period.
