Legal Issues of Medical Institution Remodeling (의료기관 리모델링의 법적 제문제)

  • Medical institutions remodel existing hospital buildings rather than construct buildings to satisfy patient satisfaction and enhance competitiveness with other medical institutions. Medical institution remodeling is gradually increasing due to enhanced laws and systems to improve the quality of medical services and the level of patient safety. However, prior studies were discussed only within a limited range, including architectural elements, about medical institution remodeling. Therefore, this study aims to provide basic data to medical institutions planning future remodeling by examining the legal issues of medical institution remodeling in various ways. Medical institution remodeling extends the construction period due to restrictions on construction conditions, and causes damage such as noise and dust to patients. So it is necessary to prevent damage to patients in advance and to support appropriate remodeling of medical institutions. Therefore, it is necessary to prepare an Act on promotion of and support for remodeling of medical institutions. In addition, medical institutions should prepare a checklist for voluntary compliance with construction standards, preventive measures and post-improvement measures, and measures for infection and radiation exposure in hospitals.

  • Objectives : The aim of this study is to investigate the use experience of other medical institution of outpatients of Korean medical institution. Methods : Cross-sectional study using the 2017 Korean Medicine Utilization and Herbal Medicine Consumption Survey was conducted. Demographic variables and frequent diseases were analyzed according to the experience of using other medical institutions. The proportions of experience of using other medical institutions of patients with frequent diseases of Korean medical outpatient were analyzed. Results : Fifty-three percent of outpatient had experiencing using other medical institutions for the same symptoms. The frequent diseases of the two groups were similar. The proportion of single use of Korean medicine for injury of lumbar and pelvic, rhinitis, and menstrual disorders were relatively high. Conclusions : There was no notable difference in the frequent diseases according to use experience of other medical institution. Further studies on this topic are needed.

Clinical Study on Safety and Efficacy of Qinin® (Cantharidin Sodium) Injection Combined with Chemotherapy in Treating Patients with Gastric Cancer

  • Objectives: To assess the efficacy, side effects, and the impact on quality of life with $Qinin^{(R)}$ (Cantharidin sodium) injection combined with chemotherapy for gastric cancer patients. Method: A consecutive cohort of 70 patients were divided into two groups: experimental group with cantharidin sodium injection combined with chemotherapy, while the control group received chemotherapy alone. After more than two courses of treatment, efficacy, quality of life and side effects were evaluated. Results: The response rate of experimental group was not significantly different from that of the control group (P>0.05), but differences were significant in clinical benefit response and KPS score. In addition, gastrointestinal reactions and the incidence of leukopenia were lower than in the control group (P<0.05). Conclusions: $Qinin^{(R)}$ (Cantharidin sodium) injection combined with chemotherapy enhances clinical benefit response, improving quality of life of gastric cancer patients and reducing side effects of chemotherapy. Thus $Qinin^{(R)}$ (Cantharidin sodium) injection deserves to be further investigated in randomized control clinical trails.

A study of the impacts of medical institution information on daily medical expenses and medical expense reduction rate in convergence age (융복합시대에 의료기관 정보가 일당진료비와 삭감율에 미치는 영향에 관한 연구)

  • This research grasped the difference of between the daily medical expenses and the medical expenses reduction rate also the cause of change. In the centre, it was conducted in order to offer the necessary basic data to a recuperation pay management solution's establishment. This research surveyed across the country's urban region 50-general hospital's the infective enteritis disease for a year. Then it was conducted the average daily medical expenses and the average medical expenses reduction rate as a distinguishable survey that is a hospitalization and an outpatient the result is followed. The inpatient daily medical expenses correlated with the total number of medical institution personnel, and the outpatient daily medical expenses correlated with the number of beds.. Inpatient medical expense reduction rate was correlated with number of beds and the total number of medical institution personnel. The interaction between medical institution information and daily medical expenses was found to have a positive (+) effect on the total number of medical institution personnel, as well as the numbers of inpatients vs. outpatient. This research about the medical expense reduction rate and daily medical expense help medical institution's efficient, rational decision making and it will be an important basis of a priority recuperation pay's policy.

Information Searching Behavior of Health Care Consumers by Types of Medical Institutions (의료소비자들의 의료기관 종별 정보탐색 행태에 관한 연구)

  • The purpose of this study was to evaluate the information searching behavior of consumer by type of medical institution. A questionnaire survey was conducted of 1,507 persons who were selected through a multi-stage stratified area cluster sampling in nationwide level, excluding Jeju-Do. Personal survey was conducted through door-to door survey from 27 July to 10 August 1999. The main results of this research was as following; 1. The proportion of information searching of respondents ranged from 91.5-95.2%. Even though the proportion of user in university hospital was slightly high, there was not significant statistically by type of medical institution. In terms of information source, personal informer was most common information source in all type of medical institution. Public informers were more frequently used in university hospital visitors and professional informer in general and university hospital visitors. 2. Comparing to searching intensity, user informer and professional informer's influences were more powerful, but not statistically significant. In analysis of unit influence for information source, written informer or public informer was more powerful in clinic visitor, professional informer and written informer in university hospital visitor. 3. Information which consumer want to know mostly were about on special potential and career of physician. The clinic visitor wanted to know about institutional location and kindness of medical personnel. The university hospital visitor also wanted to know about facilities and convenience of process. Comparing to institution selection criteria of consumers at 1991, quality related criteria were recognized more importantly in outpatient and dental services. But in case of inpatient services, convenience factor was recognized more importantly. In conclusion, the effort for specific marketing plan by type of medical institution should be needed. And more concern on information searching behavior of consumer will be needed.

No Association of the rs17822931 Polymorphism in ABCC11 with Breast Cancer Risk in Koreans

  • ABCC11 is reported to be associated with breast cancer. However, whether ABCC11 polymorphisms relate to breast cancer risk remains unclear. This study aimed to evaluate any association of a single nucleotide polymorphism (SNP), rs17822931, in ABCC11 with breast cancer in Koreans. Genomic DNA samples of 170 women with breast cancer and 100 controls were assessed for SNP rs17822931 of ABCC11 by single-strand conformation polymorphism (SSCP) and DNA sequencing. A 27-bp deletion (${\Delta}27$) of ABCC11 was analyzed by PCR amplification. The genotype of SNP rs17822931 was confirmed to be AA in all samples from breast cancer patients and ${\Delta}27$ was found in none of the samples. Our finding indicated that the SNP rs17822931 in ABCC11 is not associated with breast cancer. However, this study does provide information on fundamental genetic aspects of ABCC11 with regard to breast cancer risk in Koreans.

A Survey Study on Use Condition of Korean Medical Institution and Demand of Korean Medical Treatment Project of Infertility in Infertile Couple (난임부부의 한방의료기관 이용 실태 및 한방난임치료 지원사업 수요에 관한 설문조사연구)

  • Objectives: The purpose of this study is to investigate use condition of Korean medical institution and demand of Korean Medical Treatment Project of Infertility in infertile couple. Methods: Questionnaires were distributed to all members of the incorporated association AGAYA(http://www.agaya.org/) via e-mail and to infertile women who visited to Korean medicine OB & GY outpatient clinic of university Korean medicine hospitals of the country and an infertility specialized Korean medicine hospital. One hundred twenty four of completed questionnaires were analyzed. Results: 1. Korean medical institution utilization rate of infertile couple was 89.6% and was similar with western medical institution utilization rate of 87.1%. 2. 96.8% of infertile couple answered that Korean Medical Treatment Project of Infertility is necessary. And 90.3% of infertile couple answered that herbal medicines should be supported first of all. 3. 90.3% of infertile couple answered that they will participate if Korean Medical Treatment Project of Infertility go ahead. Conclusions: It was confirmed that most of the infertile couple have been received Korean medical treatment for infertility, and think that Korean Medical Treatment Project of Infertility is necessary.

The Study on the Difference of Management Performance in Public Health Care Institution (공공의료기관의 경영성과 차이에 관한 분석)

  • This research analyzes on the difference of the management performance in public health care institution, especially between provincial medical center and national university hospital. The meaningful results of this study as follow. First of all, management performance was showed the loss in both of provincial medical center and national university hospital. but national university hospital is superior to provincial medical center in management performance. Secondly, It is noteworthy that social working expenses have influence on national university hospital. Finally, It shows that personnel expenses are the most important factor in the management performance in public health care institution. We hope that these results will be useful in the performance management of public health care institution.

  • This study describes a simple and sensitive semi-micro HPLC method with UV detection and direct deproteinization. The plasma protein was precipitated using perchloric acid (60%) and the supernatant was directly injected onto the semi-micro HPLC system. The separation was achieved on a C18 (25 mm ${\times}$ 2.0 mm I.D) analytical column with a mobile phase of sodium acetate buffer (pH 3.5, 50 mmol) - acetonitrile (60:40, V/V). (omitted)
