Is N-acetylcysteine Treatment Based on Ingestion Amount Valid in Acute Acetaminophen Overdose Patients? (급성 Acetaminophen 중독시 복용량에 의한 N-Acetylcysteine의 사용은 적절한가?)

  • Purpose: In many Korean hospitals, serum acetaminophen concentrations in cases of overdose cannot be measured initially because of inadequate laboratory facilities. Under these circumstances, physicians base the administration of the antidote, N-acetylcysteine, on ingestion amounts as determined by initial history taking. We therefore examined the correlated between ingested amounts and serum acetaminophen concentrations. Methods: Medical records were reviewed retrospectively for patients who presented to the ED with acetaminophen overdose between January 2002 and March 2006. Fifty-nine patients were recruited and sixteen patients were excluded. The forty-three remaining patients were placed into either the high-risk or low-risk group based on their ingested amount (140 mg/kg), and were separately categorized into the toxic or non-toxic group based on their serum acetaminophen concentrations, according to the Rurnack-Matthew nomogram. Results: Ten patients (83.3%) among twelve in the high-risk group were found to have non-toxic serum concentrations, and just one patient (3.2%) among thirty-one in the low-risk group fell into the toxic group based on their serum concentrations. The sensitivity and specificity of risk stratification of the ingested amount as a predictor of intoxication requiring antidote therapy were 66.7% and 75.0%, respectively. Conclusion: This study suggests that the therapeutic decision for acetaminophen overdose should not be based solely on ingested amount only, but requires assessment of acetaminophen concentration.

Breast Cancer in Lampang, a Province in Northern Thailand: Analysis of 1993-2012 Incidence Data and Future Trends

  • Background: The recent epidemiologic transition in Thailand, with decreasing incidence of infectious diseases along with increasing rates of chronic conditions, including cancer, is a serious problem for the country. Breast cancer has the highest incidence rates among females throughout Thailand. Lampang is a province in the upper part of Northern Thailand. A study was needed to identify the current burden, and the future trends of breast cancer in upper Northern Thai women. Materials and Methods: Here we used cancer incidence data from the Lampang Cancer Registry to characterize and analyze the local incidence of breast cancer. Joinpoint analysis, age period cohort model and Nordpred package were used to investigate the incidences of breast cancer in the province from 1993 to 2012 and to project future trends from 2013 to 2030. Results: Age-standardized incidence rates (world) of breast cancer in the upper parts of Northern Thailand increased from 16.7 to 26.3 cases per 100,000 female population which is equivalent to an annual percentage change of 2.0-2.8%, according to the method used. Linear drift effects played a role in shaping the increase of incidence. The three projection method suggested that incidence rates would continue to increase in the future with incidence for women aged 50 and above, increasing at a higher rate than for women below the age of 50. Conclusions: The current early detection measures increase detection rates of early disease. Preparation of a budget for treatment facilities and human resources, both in surgical and medical oncology, is essential.

The role of the Neonatal Nurse Specialist in 21st Century (21세기 신생아 전문 간호사의 역할과 전망)

  • The role of the neonatal nurse specialist has been well established over the past decade and now reform in 21st century. Neonatal nurse specialists responsibilities in caring for critically and long-term chronically ill infants and their families are very important. Neonatal nurse specialists have a two fold responsibility in caring for these infants. First, through acquiring advanced practice education in complex neonatal care and diagnostic skills, neonatal nurse specialists meet the physiologic needs of the infant. Second, neonatal nurse specialists provide a more holistic approach to their care through evaluating the family in treatment plans and involving the family in discharge planning for the infant. In some institutions, neonatal nurse specialists are directly involved in institutional and/or home follow-up care and case management also. It is the neonatal nurse specialists responsibility to function collaboratively with the multidisciplinary team in managing critically or chronically ill infants from admission to discharge. The role of the neonatal nurse specialist case manager can be described as one that focuses on individualized care of the infant, while providing continuity of care to both the infant and family. The neonatal nurse specialist's role will vary depending on the neonatal intensive care unit(NICU). Therefore, the multidisciplinary collaborative approach to long-term management of infants in the NICU is extremely important to provide successful transition to home or to long-term rehabilitative care facilities because care for the chronically ill infant is complex and multifaceted. I suggest the role of neonatal nurse specialist in 21st century are as follows. 1. Diagnostic/patient assessment 2. Management of patient health/illness 3. Administering/monitoring therapeutic interventions and regimens 4. Monitoring/ensuring quality of health care practices 5. Organization and work role 6. Helping role 7. Teaching/coaching role 8. Management of rapidly changing situations 9. Consulting role The advanced practice nursing model of care delivered by neonatal nurse specialist's in the NICU incorporates medical and nursing role functions and emphasizes holism, caring, and a health perspective for critically and chronically ill neonates and their families.

The Preference of Housing for the Elderly among the Middle-aged Households for Aging Society (고령화 사회에 대비한 중년층의 노후 주거선호 - 광주지역의 대학생 자녀를 둔 학부형을 대상으로 -)

  • This study was conducted as a survey to find out middle-aged persons's housing preferences for a future elderly housing and to grasp background characteristics of persons influencing the preferences and the effect of elderly's aging situations on the changes of their preferences. A sample of 300 middle-aged people was selected purposely from parents of university students living in Gwang-ju city. Questionnaires were administered to parents by students and analysed with SPSS Windows 10 program. Middle-aged persons's housing preferences were differed according to elderly's aging situations, in the case of healthy situation, they tended to prefer to stay present house without their children in irrespective to couple or single, whereas in the case of unhealthy situation, they tended to prefer to live with their children. and couples showed preferences for planned housing for the elderly and singles showed preferences for elderly housing facilities with care services. The most important characteristics of elderly housing among middle-aged people was a places where they can enjoy leisure and provide home-help services, located in the suburbs. Also middle-aged persons were likely to prefer to a green spaces such as garden, path for walking, and madang for the elderly housing. Needs for community care services (NCCS) were required above the average. Among the items of NCCS, a visiting medical examination was the most demanding, and a visiting nursing services, a emergency calling bell, and home repairs were sequently followed in the level of needs. The characteristics of the elderly housing which preferred to and needs for community care services were influenced by middle-aged persons's age, gender, subjective evaluation of economic abilities, tenure status, the cost of living, and the size of housing. This findings suggests that it must to be considered to develop various types of the elderly housing depending on socioeconomic status.

  • This study divided 409 local residents in G city of Gyeongsangbuk-do into urban area and rural area, and analyzed the satisfaction with using public health center. The subjects of this study were 284 people who had experience using public health centers among 409 people. In rural areas, satisfaction with general care, oriental care, dental care, physical therapy, examination, and vaccination was low. To improve this, it is necessary to expand and improve facilities so that smooth public health services can be provided to local residents. It is necessary to implement health care policies to resolve medical inequality between urban areas and rural areas.

  • This thesis is a study on the change and the process of the development of the hygiene administration during the period of modernization of our nation which promoted by the open door policy of our country chiefly under the pressure of the foreign countries. During that period Korea had reorganized the old political system radically and introduced the western medical system. Through the close study of literature concerning the system of the hygiene administration, the laws of health and hygiene, the environmental sanitation organizations and their practical activities the author obtained the conclusions as following 1. The government passed around 40 laws of health and hygiene from the time when the bureau of hygiene was founded in 1894 to the annex of Korea to Japan. Most of the laws were believed to be established by the enforecement of Japanese government. When the epidemics were prevailing, a decree for the prevention of the epidemics was proclaimed by the Japanese president of the headquarters of national police. It is shown well that the sovereignty of Korea had been deprived. 2. After the armex of Korea to Japan, the hygiene police and the general police belonging to the bureau of national police shared the tasks of health and hygiene. This was a temporary system which had existed just during the colonial period. As for the water-supply which is the most important facility among the various environmental hygiene facilities, the rate of supply was remarkably high in Japanese rather than in Korean. 3. During the period of American military administration, it is notable that the division of hygiene in the bureau of national police was replaced by the bureau of hygiene. This period is regarded as the turning point when hygiene administration was operated by means of education along with the introuction of American system of public health. 4. Ever since the foundation of the republic government, the laws of health and hygiene which had been established during the colonial period have been put into operation without rriuch change except several regulations concerning food.

The Causes of Death of the Institutionalized Population of Kkottongnae : Comparison between Severe Mental Illness Group(SMI) and Non-Severe Mental Illness Group(Non-SMI) (SMI군과 Non-SMI군의 사망원인 비교분석 : 일 장기요양기설 입소자를 대상으로)

  • Objectives : Schizophrenia and other psychiatric disorder are associated with an increased risk of premature death. For decades, there have been reports of shorter life expectancy among those with severe mental illness. The purpose of this study was to compare the risk of mortality among institutionalized population, treated for severe mental illness to control group who did not have severe mental illness. Methods : The medical records and the death certificates of 2,029 institutionalized population who had died from 1985 to 2003 in Kkottongnae were investigated. Results : The mean age of the death of severe mental illness(SMI) group(51.4${\pm}$15.3 years old) was lower than that of non-severe mental illness(non-SMI) group(65.0${\pm}$19.3 years old) and it was statistically significant(p<0.0001). The most causes of death among the SMI group were respiratory diseases(23.3%), infectious disease (13.0%) and digestive disease(12.3%). Also, we found that the death due to injuries of the SMI group(8.9%) were three times higher than that of non-SMI group(2.5%). The most causes of death among the non-SMI group were respiratory disease(26.3%), circulatory disease(26.2%) and neoplasm(10.8%). Conclusion : The SMI group demonstrated higher mortality rates compared with the rate in the non-SMI group. The finding suggests that careful intervention is needed not only for menal health but also physical health in long-term facilities.

Relationship between structural characteristics and hospital mortality rates on tertiary referral hospitals in Korea (우리나라 3차진료기관의 구조적인 특성과 병원사망률의 관계)

  • This study was to evaluate hospital characteristics as composition of manpower and facilities to the death rate of patient; and to earmark the factors affecting the overall hospital mortality rates. The data utilized were derived from survey material conducted by the Korean Hospital Association on 32 tertiary referral hospitals in Korea between 1986 and 1994. The findings are : 1. Those hospitals having the most capacity per bed had little difference to the mortality rates than the others. 2. Those hospitals having the most daily patients per specialist had significantly higher mortality rates than the others, but the number of daily patients per nurse had little effect on the mortality rates. 3. Those hospitals which had a relatively sufficient number of quality assurance activities revealed a lower mortality, and particularly in case where such effort was directed to the clinicians, the outcome was remarkable. We concluded that the major factor affecting the hospital mortality rates seems to be the number of specialists per number of beds, the degree of quality assurance assessment of the clinicians, the quality assurance activities of each hospital as a whole, and the number of daily patient per specialist. According to the findings of this study, the composition and quality of specialist and adequate quality assurance activities seemed to be the essential for the improvement of hospital care. Therefore, in this regard e proper implementation of policy and support is highly recommended. Due to lack of available research material, the personal characteristics of specialists haven't been considered in this study. However, this longitudinal observation of 32 tertiary referral hospitals over a nine year period has significant merit alone.

  • Analysis of influential factors on water consumption in households will help predicting the water demand of end-use in household and give an explanation to cause on the change of trend. In this research, the data are gathered by radio telemetry system which is combined electronic flow-meter and wireless communication system in 140 household in Korea. Using this data, we estimate for each residential type to determine liter per capita day. we used real data to predict bathtub and washbowl water-uses and compared the ordinary least square regression model and autoregressive regression error model. The results of this study can be applied in the planning stages of water and waste water facilities.
