An Assessment of the Usefulness of Time of Flight in Magnetic Resonance Angiography Covering the Aortic Arch

  • Carotid angiography covering the aortic arch includes contrast-enhanced magnetic resonance angiography (CEA), which is applied to a large region and usually employs contrast media. However, the use of contrast media can be dangerous in infants, pregnant women, and patients with chronic renal failure (CRF). Follow-up patients informed of a lesion may also want to avoid constant exposure to contrast media. We aimed to apply time-of-flight (TOF) angiography to a large region and compare its usefulness with that of CEA. Ten patients (mean age, 58 years; range, 45~75 years) who visited our hospital for magnetic resonance angiography (MRA) participated in this study. A 3.0 Tesla Achieva magnetic resonance imaging (MRI) system (Philips, Netherland) and the SENSE NeuroVascular 16-channel coil were employed for both methods. Both methods were applied simultaneously to the same patient. Three TOF stacks were connected to cover the aortic arch through the circle of Willis, and CEA was applied in the same manner. For the quantitative assessment, the acquired images were used to set the regions of interest (ROIs) in the common carotid artery (CCA) bifurcation, internal carotid artery, external carotid artery, middle cerebral artery, and vertebral artery, and to obtain the signal-to-noise ratio (SNR) and the contrast-to-noise ratio (CNR) for the soft tissues. Three radiologists and one radiological resident performed the qualitative assessment on a 5-point scale - 1 point, "very bad"; 2 points, "bad"; 3 points, "average"; 4 points, "good"; and 5 points, "very good" - with regard to 4 items: (1) sharpness, (2) distortion, (3) vein contamination, and (4) expression of peripheral vessels. For the quantitative assessment, we estimated the mean SNR and CNR in each of the 5 ROIs. In general, the mean SNR was higher in TOF angiography (166.1, 205.2, 154.39, 172.23, and 161.95) than in CEA(92.05, 95.43, 84.76, 73.69, and 88.3). Both methods had a similar mean CNR: 67.62, 106.71, 55.9, 73.74, and 63.46 for TOF angiography, and 67.82, 71.19, 60.52, 49.45, and 64.07 for CEA. In all ROIs, the mean SNR was statistically significant (p<0.05), whereas the mean CNR was insignificant (p>0.05). The mean values of TOF angiography and CEA for each item in the qualitative assessment were 4.2 and 4.28, respectively for item 1; 2.93 and 4.55, respectively, for item 2; 4.6 and 3.13, respectively, for item 3; and 2.88 and 4.65, respectively, for item 4. Therefore, TOF angiography had a higher mean for item 3, and CEA had a higher mean for items 2 and 4; there was no significant difference between the two methods for item 1. The results for item 1 were statistically insignificant (p>0.05), whereas the results for items 2~4 were statistically significant (p<0.05). Both methods have advantages and disadvantages and they complement each other. However, CEA is usually applied to a large region covering the aortic arch. Time-of-flight angiography may be useful for people such as infants, pregnant women, CRF patients, and followup patients for whom the use of contrast media can be dangerous or unnecessary, depending on the circumstance.

VMAT를 이용한 유방암 방사선치료 시 반대편 유방의 Bolus 사용에 따른 흡수선량 평가 (Evaluation of Absorbed Dose according to the Use of Bolus in Opposite Breast during Radiation Therapy of Breast Cancer using VMAT)

  • 방사선치료 기법의 발전으로 치료가 더욱 정밀해졌음에도 불구하고 치료 부위 외에도 방사선에 의해 피폭되는 것은 피할 수 없다. 이에 본 연구에서는 유방암의 방사선치료 시 치료 반대편 유방의 bolus 두께에 따른 흡수선량을 평가하고 선량 저감 효과에 대해 분석하고자 하였다. 실험 및 방법으로 Rando phantom을 대상으로 VMAT 치료방법을 이용하여 실험을 진행하였다. 치료 반대편 유방에 A, B, C, D, E의 5개 지점을 선정하여 bolus를 사용하지 않았을 때와 5, 10, 15, 20 mm의 bolus를 사용하였을 때의 선량을 유리선량계를 사용해 평가하였다. 그 결과, 치료지점과 가장 가까운 지점에서 높은 흡수선량이 측정되었으며 치료지점과 가장 먼 B 지점에서는 가장 낮게 측정되었다. 평균 흡수선량은 bolus를 사용하지 않았을 때 8.61 cGy 그리고 두께에 따라 8.10, 7.94, 8.06, 8.10 cGy로 나타났다. 연구 결과 선량 저감 효과를 확인할 수 있었으며, 본 연구를 바탕으로 적절한 bolus 두께를 설정하여 정상조직의 선량 저감화를 위해 노력해야 할 것이다.

Natural and synthetic pathogen associated molecular patterns modulate galectin expression in cow blood

  • Pathogen-associated Molecular Patterns (PAMPs) are highly conserved structural motifs that are recognized by Pathogen Recognition receptors (PRRs) to initiate immune responses. Infection by these pathogens and the immune response to PAMPS such as lipopolysaccharide (LPS), Peptidoglycan (PGN), bacterial oligodeoxynucleotides [CpG oligodeoxynucleotides 2006 (CpG ODN2006) and CpG oligodeoxynucleotides 2216 (CpG ODN2216)], and viral RNA Polyinosinic-Polycytidylic Acid (Poly I:C), are associated with infectious and metabolic diseases in animals impacting health and production. It is established that PAMPs mediate the production of cytokines by binding to PRRs such as Toll-like receptors (TLR) on immune cells. Galectins (Gal) are carbohydrate-binding proteins that when expressed play essential roles in the resolution of infectious and metabolic diseases. Thus it is important to determine if the expression of galectin gene (LGALS) and Gal secretion in blood are affected by exposure to LPS and PGN, PolyI:C and bacterial CpG ODNs. LPS increased transcription of LGALS4 and 12 (2.5 and 2.02 folds respectively) and decreased secretion of Gal 4 (p < 0.05). PGN increased transcription of LGALS-1, -2, -3, -4, -7, and -12 (3.0, 2.3, 2.0, 4.1, 3.3, and 2.4 folds respectively) and secretion of Gal-8 and Gal-9 (p < 0.05). Poly I:C tended to increase the transcription of LGALS1, LGALS4, and LGALS8 (1.78, 1.88, and 1.73 folds respectively). Secretion of Gal-1, -3, -8 and nine were significantly increased in treated samples compared to control (p < 0.05). CpG ODN2006 did not cause any significant fold changes in LGALS transcription (FC < 2) but increased secretion of Gal-1, and-3 (p < 0.05) in plasma compared to control. Gal-4 was however reduced in plasma (p < 0.05). CpG ODN2216 increased transcription of LGALS1 and LGALS3 (3.8 and 1.6 folds respectively), but reduced LGALS2, LGALS4, LGALS7, and LGALS12 (-1.9, -2.0, -2.0 and; -2.7 folds respectively). Secretion of Gal-2 and -3 in plasma was increased compared to control (p < 0.05). Gal-4 secretion was reduced in plasma (p < 0.05). The results demonstrate that PAMPs differentially modulate galectin transcription and translation of galectins in cow blood.

Correlation analysis of muscle amino acid deposition and gut microbiota profile of broilers reared at different ambient temperatures

  • Objective: Temperature could influence protein and amino acid deposition as well as gut microbiota profile and composition. However, the specific effects of ambient temperature on amino acids deposition and gut microbiota composition remain insufficiently understood. Methods: A total of 300 one-day-old Avian broilers were randomly divided into three groups and reared at high, medium, and low temperature (HT, MT, and LT), respectively. Breast muscle and fecal samples were collected for amino acid composition analysis and 16S rRNA gene sequence analysis. Results: Our data showed that compared to the MT group, there was a decrease of muscle leucine and tyrosine (p<0.05), as well as an increase of methionine in the HT group (p<0.05) and a decrease of serine in the LT group. Examination of microbiota shift revealed that at genus level, the relative abundance of Turicibacter and Parabacteroides was increased in the HT group (p<0.05) and that the relative abundances of Pandoraea, Achromobacter, Prevotella, Brevundimonas, and Stenotrophomonas in the LT group were higher than those in the MT group (p<0.05). In addition, there were substantial correlations between microbes and amino acids. In the HT group. Turicibacter was negatively correlated with aspartic acid and tyrosine, whereas Parabacteroides was positively correlated with methionine (p<0.05). In the LT group, there were multiple positive correlations between Achromobacter and arginine, isoleucine or tyrosine; between Prevotella and cysteine or phenylalanine; between Brevundimonas and cysteine; and between Stenotrophomonas and cysteine as well as a negative correlation between Stenotrophomonas and serine. Conclusion: Our findings demonstrated that amino acid content of breast muscle and intestinal microbiota profile was affected by different ambient temperatures. Under heat exposure, augmented abundance of Parabacteroides was correlated with elevated methionine. Low temperature treatment may affect muscle tyrosine content through the regulation of Achromobacter.

  • WSS 측면 검사는 척추의 질환을 진단하기 위해 매우 중요한 검사이다. 최근 대면적의 Long Length Detector가 보급되고 있어 피폭선량과 검사 시간을 효율적으로 줄일 수 있으며, 낙상 고위험군 환자 및 소아, 청소년기 환자의 검사에 매우 효율적으로 적용할 수 있다. 하지만 1회 조사로 영상을 얻기 때문에 기하학적인 척추의 해부학적 구조상 허리뼈 보다 목뼈 부근이 상대적으로 체적이 적어 목뼈 부위의 영상 품질은 진단 가치가 매우 하락한다는 단점이 있다. 이에 본 연구는 WSS 측면 검사에서 균일한 영상품질을 얻기 위해 3D 프린팅 기술과 구리 필라멘트를 이용하여 부가 필터를 제작하고 결과를 분석하고자 한다. 3D 프린팅 기술은 원하는 모양을 손쉽게 출력할 수 있는 장점이 있어 산업 분야 전반에 광범위하게 사용되고 있으며, 최근 구리 소재의 필라멘트가 개발되어 부가 필터로서의 가능성을 확인하였다. WSS 측면 검사에서 부가 필터 적용시 CNR과 SNR이 우수하게 측정되어 부가필터의 사용 가능성을 확인 하였다.

전국 다중이용시설의 실내공기 부유 곰팡이의 현황 및 특성 분석 (An Analysis of the Current Status and Characteristics of Airborne Fungi in Indoor Air in Multi-Use Facilities Nationwide)

  • Background: Airborne fungi are ubiquitous in the air and exposure to an airborne fungus can be a significant risk factor. The composition of fungi has been potentially important for human health, especially for respiratory diseases like asthma and atopic dermatitis. Therefore, we attempted to ascertain what kind of airborne fungi affect human health at a nationwide level. Objectives: This study was carried out to provide information on indoor fungi distribution at multi-use facilities throughout South Korea. Methods: We classified our data by region and public facility after collection, cultivation, and identification via the sequencing of the ITS (internal transcribed spacer) region. We investigated whether or not the proliferation of HaCaT cells was affected by the identified airborne fungi. Results: In our data, the most isolated airborne fungi by region were Penicillium spp (Seoul, Daegu), Periconia sp (Gyeonggi-do), Iprex sp (Gangwon-do), Phanerochaete sp (Busan), Bjerkandera sp (Gwangju), and Aspergillus sp (Jeju-do). In the public facilities, the most detected fungi were Cladosporium sp (public transport), Penicillium sp (apartment house, retail market, financial institution, karaoke room), Bjerokandera sp (underground parking lot, public toilet, medical institution), Periconia sp (retail store), and Fusarium sp (general restaurant). Next, we selected twenty airborne fungi to examine their cytotoxicity and proliferation of human skin cells. In this experiment, the proliferation of the cells was influenced by most of the identified fungi. In case of the cytotoxicity test, most genera except for Rhodotorula sp and Moesziomyces sp showed cytotoxicity in HaCaT cells. Conclusions: The distribution of mold in the indoor air in multi-use facilities in South Korea differs from region to region, and this is an indicator that should be considered in future health impact studies. In addition, as a result of culturing about 20 types of bacteria dominant in indoor air, it was found that most (90%) inhibit the growth of skin cells, which can be harmful to health. An in-depth study of the health effects of floating fungi is needed.

건강보험 청구 자료를 이용한 소방 및 경찰공무원의 암 종별 위험도 분석 (Risk Analysis of All Types of Cancer among Firefighters and Police Officers Using National Health Insurance Claim Data)

  • Objectives: Firefighters and police officers are exposed to various occupational hazards. No studies in Korea have investigated the occurrence of cancer by type of cancer for the two occupations. This study aims to investigate the risk of occurrences associated with types of cancers in firefighters and police officers. Methods: Utilizing National Health Insurance(NHI) Claims data from 2006-2015, the study included 8,871,468 general workers, 25,001 firefighters, and 102,274 police officers. Using general workers as a control group, we calculated the standardized incidence ratios(SIR) by types of cancer for firefighters and police officers. After calculating the SIR for all subjects, the SIR was calculated by stratifying according to gender. Results: SIR of colon cancer 1.38(95% CI, 1.11-1.69), cancer of the liver and intrahepatic bile ducts 1.27(95% CI, 1.04-1.54), and 1.88(95% CI, 1.28-2.65) bladder cancer were higher firefighters than general workers. SIR of Lip, oral cavity, and pharynx 1.26(95% CI, 1.07-1.47), Stomach 1.14(95% CI, 1.06-1.23), colon 1.33(95% CI, 1.21-1.46), liver and intrahepatic bile ducts 1.21(95% CI, 1.10-1.32), pancreas 1.24(95% CI, 1.02-1.49), other skin 1.60(95% CI, 1.26-2.00), bladder 1.27(95% CI, 1.04-1.54), other urinary tract 1.46(95% CI, 1.27-1.68), other parts of central nervous system 1.68(95% CI, 1.10-2.46) were higher police officers than general workers. Conclusions: Both firefighters and police officers are exposed to various cancer occurrence risks, necessitating the development of occupational medical protection measures to reduce risk exposure factors.

사례 중심의 손상코드 별 손상외인코드 빈도수 분석에 따른 손상코드 발생 원인에 관한 연구 (A Study on the Causes of Injury Codes by Case-Based Injury Code of External Causes Frequency Analysis)

  • 본 연구에서 강원도 강릉시에 위치한 A기관의 2020년, 2021년의 2년간 데이터를 중심으로 손상으로 입원한 환자를 손상 코드별로 분석하고자 하였다. 각 연도별 발생인원이 많은 손상코드를 분석하였으며, 이에 따른 손상외인코드를 분석하였다. 손상외인 코드의 빈도수를 분석하여 손상코드의 발생 원인을 분석하였다. 손상코드 S0650는 손상외인코드 W189, X5999의 빈도수가 가장 많았으며, 이는 상세불명의 장소 또는 화장실에서 넘어졌을 때 열린 두개 내 상처 없는 외상성 경막하 출혈의 원인으로 판단하였다. 손상코드 S72120는 손상외인코드 W010, W180의 빈도수가 가장 많았으며, 주거지에서 넘어질 때 발생하는 폐쇄성 대퇴골전자간 골절의 원인이 되는 것으로 판단하였다. 손상코드 S32090은 X5999의 빈도수가 많았으며, 상세불명의 장소에서 노출에 의한 사고로 요추 부위의 폐쇄성 골절의 원인 되는 것으로 분석 하였으며, 손상코드 S72.090에서 발생되는 손상외인 코드는 W010, W180의 빈도수가 많은 것을 확인하였다. 주로 주거지에서 미끄러지거나 헛디딤으로 인해 대퇴골 경부의 폐쇄성 골절의 원인이 되는 것을 확인 할 수 있었으며, 손상코드 S0220은 Y049의 손상외인코드의 빈도수가 높았으며, 주로 완력 또는 주먹에 의해 비골이 골절되는 것을 확인하였다. 이와 같이 손상코드 별 손상외인코드의 빈도수를 분석하여 손상코드의 발생 원인을 분석하였다.

인지기능 저하와 체내 중금속 10종 간 연관성 분석 (Association between Cognitive Decline and Ten Heavy Metals)

  • Background: Due to the rapid aging of the South Korean population, neurological diseases such as dementia are increasing. Many studies have reported that the incidence of dementia is associated with environmental factors along with age. Objectives: This study analyzed the association between cognitive function and ten heavy metals in the body: arsenic, aluminum, chromium, manganese, cobalt, nickel, iron, copper, zinc, and lead. Methods: From 2018 to 2019, a total of 120 participants who suffered from cognitive impairment were recruited for this study. Blood and urine samples were collected and analyzed for heavy metal concentrations using an inductively coupled plasma mass spectrometer. Demographic information was obtained through face-to-face questionnaires completed by a trained investigator. Cognitive function was evaluated with the Korean version of the Mini-Mental State Examination and the Korean version of the Boston Name Waiting Test. The associations between cognitive function scores and heavy metal concentrations were analyzed using multiple logistic regression analysis. Results: The average age of the 120 participants was 72.7 years, and 69.2% were female. The mean of the MMSE-K and K-BNT scores were 22.9 and 37.9, respectively. The geometric mean of aluminum (Al) was 8.42 ㎍/L. MMSE-K was associated with iron (Fe), but the significance was removed in the logistic regression based on 24 points. K-BNT was significantly associated with aluminum and the odds ratio for K-BNT above 38 decreased by 45% as the aluminum concentration increased. Conclusions: The association between aluminum and the K-BNT score indicated that aluminum is associated with language-related cognitive decline. Based on this result, further study will be conducted by considering co-exposure effects of heavy metals including aluminum.

이형성 협심증 진단 조영 검사의 방사선학적 관점 (Radiological Perspectives for Diagnosis of Vasospastic Angina with Coronary Angiography)

  • 관상동맥 경련 유발로 혈관 완전 폐색이 발생할 경우 악성 부정맥 출현으로 사망까지 이를 수 있으므로 관상동맥 경축 협심증 조기 검사는 반드시 필요한 검사이다. 검사 방법 중 관상동맥 조영술을 통한 약물 주입 검사가 일반적으로 시행되고 있다. 따라서 관상동맥 조영 검사 중 경축 협심증 검사의 시술 시간과 조영제 사용량, 방사선 피폭 영향과의 연관성 등을 방사선학적 관점으로 이형성 협심증 조영 검사의 이점을 평가하고자 한다. 2021년 09월부터 2023년 02월까지 관상동맥 조영술과 변이형 협심증 검사를 시행한 142명 환자의 후행적 데이터를 이용하였다. 관상동맥 조영술과 변이형 협심증 검사 비교 분석 결과 체질량 지수를 제외하고 조영제 사용량 67.47 ± 21.81 cc, 흡수선량 15.98 ± 13.8 uGy/m2, 공기 중 입사선량 236.73 ± 135.91 mGy, 촬영 시리즈 수 13 장, 검사 시간 1573.6 ± 428.77 s로 관상동맥 조영술만 시행했을 때의 조영제 사용량 49.1 ± 7.73 cc, 흡수선량 9.93 ± 7.81 uGy/m2, 공기 중 입사선량 140.6 ± 79.76 mGy, 촬영 시리즈 수 6 장, 검사 시간 544.48 ± 185.76 s보다 통계적으로 유의할 만큼 높은 차이를 나타냈다. (p<0.001) 결론적으로 관상동맥 조영술 외 추가적인 변이형 협심증 검사가 방사선학적 관점에서 더 부정적일 수 있어 과도한 변이형 협심증 검사는 지양하는 것이 좋을 것으로 생각된다. 결론적으로 관상동맥 조영술 외 추가적인 변이형 협심증 검사가 방사선학적 관점에서 더 부정적일 수 있어 과도한 변이형 협심증 검사는 지양하는 것이 좋을 것으로 생각된다. 그럼에도 불구하고 변이형 협심증 검사를 진행해야 할 경우 검사 시간이 길어 질수록 투시 시간과 조영제 사용량도 증가하게 되므로 가능한 빠른 검사 또는 단축 검사를 진행해야 환자의 방사선학적 관점의 부정적인 측면도 해소할 수 있을 것으로 사료된다.
