Effect of Additional 1 hour T-piece Trial on Weaning Outcome to the Patients at Minimum Pressure Support (최소압력보조 수준에서 추가적 1시간 T-piece 시도가 이탈에 미치는 영향)

  • Background: Extubation is recommended to be performed at minimum pressure support (PSmin) during the pressure support ventilation (PSV). In field, physicians sometimes perform additional 1 hr T-piece trial to the patient at PSmin to reduce re-intubation risk. Although it provides confirmation of patient's breathing reserve, weaning could be delayed due to increased airway resistance by endotracheal tube. Methods: To investigate the effect of additional 1 hr T-piece trial on weaning outcome, a prospective study was done in consecutive 44 patients who had received mechanical ventilation more than 3 days. Respiratory mechanics, hemodymic, and gas exchange measurements were done and the level of PSmin was calculated using the equation (PSmin=peak inspiratory flow rate $\times$ total ventilatory system resistance) at the 15cm $H_2O$ of pressure support. At PSmin, the patients were randomized into intervention (additional 1 hr T-piece trial) and control (extubation at PSmin). The measurements were repeated at PSmm, during weaning process (in cases of intervention), and after extubation. The weaning success was defined as spontaneous breathing more than 48hr after extubation. In intervention group, failure to continue weaning process was also considered as weaning failure. Results: Thirty-six patients with 42 times weaning trial were satisfied to the protocol. Mean PSmin level was 7.6 (${\pm}1.9$)cm $H_2O$. There were no differences in total ventilation times (TVT), APACHE III score, nutritional indices, and respiratory mechanics at PSmin between 2 groups. The weaning success rate and re-intubation rate were not different between intervention group (55% and 18% in each) and control group (70% and 20% in each) at first weaning trial. Work of breathing, pressure time product, and tidal volume were aggravated during 1 hr T-piece trial compared to those of PSmin in intervention group ($10.4{\pm}1.25$ and $1.66{\pm}1.08$ J/L in work of breathing) ($191{\pm}232$ and $287{\pm}217$cm $H_2O$ s/m in pressure time product) ($0.33{\pm}0.09$ and $0.29{\pm}0.09$ L in tidal volume) (P<0.05 in each). As in whole, TVT, and tidal volume at PSmin were significantly different between the patients with weaning success ($246{\pm}195$ hr, $0.43{\pm}0.11$ L) and the those with weaning failure ($407{\pm}248$ hr, $0.35{\pm}0.10$L) (P<0.05 in each). Conclusion : There were no advantage to weaning outcome by addition of 1 hr T-piece trial compared to prompt extubation to the patient at PS min.

  • Hospital information system has been widely used and increased recently for a variety of many aspects. And Order-Communicating system(OCS), as like as hospital information system, has been used in many medical care facilities, which is simple and easily accessible, useful system. Also then the use of OCS in emergency care center in YUMC has been introduced since 1996. 10, above 70% of availability is noticed at present and increasing in the use rate, is considered that it is very simple and accurate, time-saving, widely applicable system. So authors say that, after the use of OCS in emergency care center, interhospital exchange of the patient's information and also accomplishment of EMSS can be possible.

A Research on Actual Status of Dietary Compliance of the Subjects with type 2 Diabetes mellitus in Daejeon (대전지역 제 2형 당뇨병 환자의 식사요법 실천 실태 조사 연구)

  • The study investigated medical treatment features of patients with type 2 diabetes mellitus, their further complications, levels of diet therapy education and the status of dietary compliance. The subjects were patients who came to oriental medicine clinics in Daejon. The percentage of male was 37.5% and female was 62.7% in the subject group's sex distribution. The outbreak of the disease was most common in the age of 50's. Also 40% of the subjects had been suffering for more than 5 years. 35.7% of men and 53.7% of women had family history. All the subjects had further complications, among them hypertension was the most common. Kidney, hepatic, vascular heart diseases were followed. 85.3% of the subjects answered they already had educations about diet therapy. Those educations were given in the public health center, general and private hospitals. 32% of subjects were not practicing diet therapy. The difficulties with which patients can be faced when they take dietary education were exchange(?exchanging?) food and calculating calories. Most of the subjects tend to overlook the importance of dietary habits and show low understanding to diet therapy, which leads to low participation rate of diabetes patients. Based on these results, therefore, we can conclude that diet therapy education to the patients should be more organized and easily practicable for them. To develop educational methods which can draw patients' attention and also be more effective is the most important task.

Inhibition of a Neutral Form of Sphingomyelinase by Alkylthioureido-l,3-propandiols, KY353X Series

  • Alkylthioureido-1,3-propandiols (KY353X series) were synthesized and evaluated as inhibitors for neutral sphingomyelinase (N-SMase). To examine whether KY353X series inhibit N-SMase, we purified the N-SMase from bovine brain. The N-SMase was partially purified by sequential chromatographies of DEAE-Cellulose anionic exchange and phenyl-5PW hydrophobic HPLC. These seqeuntial procedures for N-SMase resulted in a 67-fold purification and excluded other isoforms of SMase. Based on in vitro assay, KY353X series inhibited N-SMase activity in time, concentration-dependent manners and completely inactivated N-SMase at 50 $\mu$M. In particular, KY3535 and KY3536 inhibited more effectively than the others. To further determine the .inhibitory pattern, a Dixon plot was constructed, to showing that the inhibition by KY3535 and KY3536 were competitive. The inhibition constant (Ki) of KY3535 and KY3536 was 1.7 $\mu$M and 2.5$\mu$M in 100 mM Tris-HCl buffer, pH 7.0, respectively.

A SD approach to the Efficiency Improvement of Electric Power Industry in Korea -Focused on the Nuclear Industry (국내 전력산업의 효율성 제고모형에 대한 SD 모형 연구 - 원자력산업을 중심으로)

  • In this study, we tried to build a model which can deal with the efficient and effective operation of electric power industry, especially focused on the nuclear industry. Here, SD(system Dynamics) approach is used to visualize the underlying phenomenon of the nuclear power industry. SD is a methodology for studying and managing complex feedback systems, such as one finds in business and other social systems, The span of SD applications has grown extensively and now encompasses work in corporate planning and policy design, public management and policy, biological and medical modeling, energy and the environment. Recently, according to the report from KEPCO(Korea Electric Power Corporation), they are considering delaying a new power plant construction. It may be based upon business fluctuation downsized from Korean economic crisis in 1997 and freezing of construction funds due to unstable foreign exchange rate. At this point, we need desperately a kind of strategic model that would contribute to cope with the current business situation, energy generation, Production, and resulting Pollution. Specifically, this model, using SD approach, starts with the detailed drawing of influence diagram, which describes those relevant key points on nuclear power generation systems in electric power industry of Korea. These include such (actors as the operation of nuclear industry and parameters related to the decision making for business policy. Based upon the above-mentioned influence diagram drawn, we developed SD simulation model to evaluate and analyze strategic management of KBPCO. Based on our analysis, we could demonstrate how simulation model can be applied to the real electric power generation in Korea.

  • LOINC (Logical Observation Identifiers Names and Codes) is a database and universal standard for identifying laboratory and clinical test results that is developed and maintained by Regenstrief Institute. Exchanging laboratory test results is one of the most important area in EHR system and the terminology for laboratory test results has to be standardized. In this paper, we present a pre-preprocessing tool that converts a local database in healthcare organizations to LMOF format LMOF format is required by RELMA and our work helps mapping laboratory test results to LOINC very efficiently Our proposed tool provided user friendly interface and 15% keyword reduction in RELMA search compared to no pre-processing RELMA search.

  • The technique of localization is the core technology for information exchange or environment monitering to measure the position of an object or person. Today, the techniques of localization have been studied extensively and it can be applied to logistics, medical, robotics, etc. But, a lot of money to apply technique of localization is needed. Hence in this paper, we proposed AoA localization system based on Zigbee at low cost. The System measured the RSSI value by rotating the directional antenna using a step motor and Zigbee module. When the measured RSSI is the largest, the receiver measures the angles from beacons which are located at the corners with the rotating angle of a stepping motor and the position of the receiver will be calculated by appling AoA localization method. The measured results show an error about 35~36 cm.

A Study of the Classification of Nursing Diagnoses (간호진단 분석 일 연구)

  • This study was conducted to analyze the nursing diagnoses applied for case studies of nursing students through their clinical practices, and to provide the educational basis of nursing diagnoses with its results. The data were collected for two years(1995 and 1996) from 70 case studies reported by the 2nd and 3rd year nursing junior college students. The students made 259 nursing diagnoses among which 230 diagnoses qualified NANDA classification and were taken for analysis. The results of the analysis were as follows : 1. The number of diagnoses indicating response patterns was 35(35.7%), whereas 98 diagnoses in NANDA table. Among the 35 diagnoses, the pattern of exchange was most frequent, then feeling, moving, knowing in rank. 2. The diagnoses were analyzed in the categories of response patterns. For Instance, 'Altered in Nutrition' was most frequent in exhange, then Risk for Infection', 'Ineffective Airway Clearance', in rank. 3. Among 230 diagnoses, 'Knowle Deficit' was most frequently mentioned, then 'Activity Intolerance' 'Anxiety', 'Pain', 'Altered in Nutrition', 'Risk for Infection', 'Ineffective airway clearance', in rank. 4. The types of word expression of each diagnoses were various. 'Activity Intolerance' was expressed in 6 types. 5. The relating factors applied to each diagnosis were analyzed. For Instance, the relating factor of 'Knowledge Deficit' were illness, and therapeutic process, lack of motivation, occurrance of complication, short experience, operation, and so on. From the above study, the researcher would like to recommend as follows : 1) The current diagnoses need to be verified its content validity, when they are applied to our culture. 2) The most effective educational method for applying nursing diagnoses should be explored. 3) Further study could be focused on not only 'relating factors' but also 'sign and symptoms'.

Application of extraction chromatographic techniques for separation and purification of emerging radiometals 44/47Sc and 64/67Cu

  • Considerably increasing interest in using the theranostic isotopes/ isotope pairs of radiometals like $^{44/47}Sc$ and $^{64/67}Cu$ for diagnosis and/or therapeutic applications in the nuclear medicine procedures necessitates its reliable production and supply. Separation and purification of no-carrier-added (NCA) isotopes from macro quantitates of the irradiated target matrix along with other impurities is a cardinal procedure amongst several other steps involved in its production. Multitudinous methods including but not limited to liquid-liquid (solvent) extraction, extraction chromatography (EXC), ion exchange, electrodeposition and sublimation are routinely applied either solitarily or in combination for the separation and purification of radioisotopes depending on their production routes, radioisotope of interest and impurities involved. However, application of EXC though has shown promises towards the numerous separation techniques have not received much attention as far as its application prospects in the field of nuclear medicine are concerned. Advances in the recent past for application of the EXC resins in separation and purification of the several medically important radioisotopes at ultra-high purity have shown promising behavior with respect to their operation simplicity, acidic and radiolytic stability, separation efficiencies and speedy procedures with the enhanced and excellent extraction abilities. In this mini review we will be talking about the recent developments in the application and the use of EXC techniques for the separation and purification of $^{44/47}Sc$ and $^{64/67}Cu$ for medical applications. Furthermore, we will also discuss the scientific and practical aspects of EXC in the view of separation of the NCA trace amount of radionuclides.
