Purpose : This study was carried out from September 2011 to September 2012 to indicate improvement and performance schemes and planning strategies by way of scenario based practical examination to inspire adaptable capacity to EMS field for Emergency Medical Technician. Methods : In order to improve the examination, a survey was conducted to EMT-Paramedics (Level 1), EMT-Basics (Level 2), and other expert groups. The researchers visited the National Registry of Emergency Medical Technician (NREMT) headquarter and the psychomotor examination site in Ohio in April, 2011. The 21 EMT professors took part in a workshop experiencing the US psychomotor examinations provided by two NREMT examination experts in October, 2011. Results : The results showed that the general plan of new National Practical Examination for EMTs should consist of integrated emergency care examination based on clinical performance and simple skill examination based on objective structured skill protocol Conclusion : The National Practice Examination consists of two sessions and the examinees select the test number randomly in each session. The future examination should include the critical criteria and this criteria should be the decisive factor for the pass or fail.
Background: This paper describes the relationship and effect of health examination on personal medical cost by identifying the difference of the cost for medical care in physician visit between the population without and with health examination. Methods: After classifying into three cohorts in which, independent variables were designed according to the Andersen's behavioral model, the association of personal medical cost for medical care and prescription drugs which is dependent variable was analyzed by t-test and Mann-Whitney test for description and gamma regression model for inference. Results: In personal average medical cost, the population with health examination paid significantly more than without health examination, 11.6% more in cohort 2008, 26.6% more in cohort 2009, and 48.0% more in combined cohort. The odds ratio on medical expenditure of outpatients with health examination was 1.067, 1.126, 1.398 significantly in cohort 2008, 2009, and combined cohort respectively, comparing to the group without health examination. In independent variables, that is female, the elderly, never married, non-working, non-metropolitan, the higher family income, the smaller family size, people with disability, the people with chronic disease, and people with health examination have significantly being paid more tendency showing positive association with medical cost. Conclusion: This result showed that medical expenditure in physician visit has been increased after taking a health examination. Therefore reasonable limitation of getting preventive medical service is suggested to avoid medical shopping around and reduce being repeated health examination by unifying control to find out easily the clinical results from various medical facilities.
Background : To shorten processing time for variety of medical affairs of the patient at the outpatient clinic of a big hospital is very important to qualify medical care of the patient. Therefore, patient's waiting time for medical examination is often utilized as a strong tool to evaluate patient satisfaction with a medical care provided. We performed this study to investigate factors delaying related with waiting time for medical examination. Methods : The data were collected from June 26 to July 30, 1999. A total 275 case of medical treatment and 5,634 patients who visited outpatient clinics of a tertiary hospital were subjected to evaluate the waiting time. The data were analyzed using frequency, t-test, ANOVA, $X^2$-test by SPSS Windows 7.5 program. Results : The mean patient's waiting time objectively evaluated ($30.9{\pm}33.9$ min) was longer than that subjectively by patient evaluated ($25.1{\pm}26.2$ min). Patient waiting time objectively evaluated was influenced by the starting time of medical examination, consultation hours, patients arriving time etc, as expected. The time discrepancy between two evaluations was influenced by several causative factors. Regarding the degree of patients accepted waiting time with the medical examination is 20 min. Conclusion : The results show that, besides the starting time of medical examination, consultation hours and patients arriving time, influence the patient's subjective evaluation of waiting time for medical examination and his satisfaction related with the service in the big hospital. In order to improve patient satisfaction related with waiting time for medical examination, it will be effective examination rather than to shorten the real processing time within the consultation room.
Objectives: This study aimed to evaluate the feasibility of the Korean medical examination protocol, which included 14 questionnaires and 20 medical examination devices. Methods: We conducted a pilot observational study of 90 subjects to measure the time required to fulfill each item of the Korean medical examination, to evaluate patient satisfaction, and to report improvements that can be made to the Korean medical examination protocol based on clinical coordinator and subject feedback. Results: Among the 90 subjects included in the study (59 women and 31 men; mean [standard deviation] age, 37.2 [12.3] years), over 80% intended to receive a Korean medical examination if hospitals provided it and would recommend a Korean medical examination to others. The average time spent on the overall Korean medical examination was approximately 88.0 (21.4) minutes. Three areas for improvements were reported: survey issues, including the number of items, understanding, and readability; error issues in device measurements; and environmental issues affecting the sequence of medical examinations and temperature. Conclusions: Most subjects were satisfied with the Korean medical examination. Future studies should be conducted with larger samples to collect data continuously.
This study was designed and undertaken to identify objectively the degree and relationship of anxiety, depression which are chief essential elements of emotional status in Stroke medical examination patients. The subjects in this study were 58 Stroke medical examination patients and 58 Non-Stroke medical examination patients, and for the assessment of anxiety, depression. We used State-Trait Anxiety Inventory (STAI), Zung's Self-Rating Depression Scale(SDS). The results of this study are as follows : 1. There were significant, differences in the 16 items of State anxiety scale among 20 items and the 14 items of Trait anxiety scale among 20 items between Stroke medical examination patients and the control group(p<0.05 respectively). 2. There were significant differences in the 14 items of SDS among 20 items between Stroke medical examination patients and the control group(p<0.05 respectively). 3. There were significant differences in the mean scores of STAI and SDS between Stroke medical examination patients and the control group(p<0.001 respectively). 4. There were no significant relationships between State anxiety & Trait anxiety, State anxiety & Depression, Trait anxiety & Depression in the Stroke medical examination patients.
Purpose: The purpose of this study was to apply alternative standard setting methods for the Korean Medical Licensing Examination (KMLE), a criterion-referenced written examination, and to compare them to the conventional cut score used on the KMLE. Methods: The process and results of criterion-referenced standard settings (i.e., the modified-Angoff and bookmark methods) were evaluated. The ratio of passing and failing examinees determined using these alternative standard setting methods was compared to the results of the conventional criteria. Additionally, the external, internal and procedural evaluation of these methods were reviewed. Results: The modified-Angoff method yielded the highest cut score, followed sequentially by the conventional method and the bookmark method. The classification agreement between the modified-Angoff and bookmark methods was 0.720 measured by Cohen's ${\kappa}$ coefficient. The intra-panelist classification consistency of modified-Angoff method was higher than bookmark method. However, the inter-panelist classification consistency was vice versa. The standard setting panelists' survey results showed that the procedures of both methods were satisfactory, but panelists had more confidence in the results of the modified-Angoff method. Conclusion: The modified-Angoff method showed results that were more similar to those of the conventional method. Both new methods showed very high concordance with the conventional method, as well as with each other. The modified-Angoff method was considered feasible for adoption on the KMLE. The standard setting panelists responded positively to the modified-Angoff method in terms of its practical applicability, despite certain advantages of the bookmark method.
Objectives : This paper analyzes the exam paper titled, 'Is Traditional Chinese Medicine Just an Art?' of the Yixuetongzong(醫学統宗) to provide basis for research on regional physicians' examination in the Ming period. Methods : Cross examination, interpretation and content analysis of the exam paper were carried out. Results : This document is the examination paper that He Jian(何柬) submitted past the age of 40 at a regional examination for physicians that took place at the Duxue Wenyuan which is presumed to have been a regional education institute for medicine in Nanjing. While he had experience in anatomy, having been an army doctor when he was young, and was an experienced doctor who was an expert in acupuncture, it seems his goal to become a medical official at the royal medical center failed. He accumulated knowledge and experience as a Confucian doctor for more than 25 years under his teacher Pan Xiquan. It is unclear whether he had been educated at the regional medical education institute. Conclusion : The regional physicians' examination during the Ming period promoted test takers to learn both medical and Confucian knowledge quite thoroughly.
The purpose of this study was to assess third-year medical students' competency for development or revision of the undergraduate curriculum and assessments. One hundred and twenty-seven third-year medical students at the Pusan National University were included in the study. After third- and fourth-year students took a common written examination, clinical performance examination (CPX), and objective structured clinical examination (OSCE) with common items as a summative assessment, the third-year students' competency was compared with 132 forth-year students' results. The correlation of the written examination and CPX/OSCE was analysed, and the summative results were compared with the grade point average (GPA) through the second year, CPX/ OSCE in the second year, and GPA in the clerkship. On the written examination, the third-year students' mean score was lower than the fourth-year students' by over 11 points, whereas the gap in the CPX/OSCE was 4 points and there was no difference in the OSCE. There was a moderate correlation between the written examination and the CPX/OSCE scores (R=0.371, p<0.01). The written examination was highly correlated with GPA through the second year, which mainly evaluated medical knowledge (R=0.771, p<0.01). A relatively high correlation was observed between CPX/OSCE scores and GPA in the clerkship (R=0.641, p<0.01). The summative CPX/ OSCE scores showed a moderate correlation with formative CPX/OSCE scores in the second year (R=0.464, p< 0.01). The third-year students' score was quite low on the written examination and slightly low on the CPX/OSCE compared to that of the fourth-year students. The written examination and CPX/OSCE cannot replace each other and should be combined with other methods of evaluation to measure competency. Early OSCE and workplacebased assessment should be useful in the early assessment of clinical skills competency.
Purpose: This study aimed at investigating the current situation of Medical Humanities and Sociology-related-curriculum in Korean medical schools, and suggesting the courses of study for the contents and methods of the Medical Humanities and Sociology examination, which can be included in the National Examination for Medical Practitioners. Methods: We analyzed Medical Humanities and Sociology-related courses which are offered in Korean medical schools, and a survey was conducted by medical school professors and students and medical journalists. In the survey, the Medical Humanities and Sociology-related courses were divided into 8 parts, and the participants were asked to evaluate the importance of duty, necessity of education, necessity of evaluation and the evaluation method of each part using a seven-point scale. Results: A total of 207 medical school professors and students and 9 medical journalists participated in the survey. The results were similar for the importance of duty and necessity of education of each part, but those for the necessity of evaluation were different. - As a result, there seems to be a gap between the importance of duty and the importance of education of each course. Medical journalists and students group answered differently on the necessity of evaluation of each course was also reserved. Conclusion: It is necessarily recommended to include Medical Humanities and Sociology-related courses such as medical ethics, self-improvement and doctors' social responsibilities in the National Examination for Medical Practitioners.
Introduction : Kim-Young-hun (김영훈), with the pen name of Cheong-Gang (청강), was born in the late 19th century. He led an active life as an eminent Korean traditional medical scholar until the mid-20th century. He opened a Korean traditional medical clinic in the heart of Seoul and kept records of his clinical experiences. Methods : Filing of clinical records: Cheong-Gang's records, at present owned by the College of Oriental Medicine, Kyunghee University (경희대학교 한의과대학), are classified into prescription charts and medical examination charts. In this study, only the medical examination charts were filed. Results : The total number of the medical examination charts from 1915 until 1974 is 393, and the titles are sorted according to date. This paper is the first filing research on the medical examination charts.
