Oriental Medical Treatment System Based on Mobile Phone (모바일폰 기반 한방 의료 치료 시스템)

  • At present, the effect of oriental treatment system is proved in the west and using the data of tongue and pulse of body, the doctor can decide the patient's body state without Xray and CT data of large machines. In this paper, the patient's medical data is transmitted to the doctor and the real time decision algorithm is developed and so the doctor can decide the medical treatments. Using the mobile phone, the pulse data and bio data can be sent to the doctor and therefore the patients, who can't care in real time, can be treated in real time in the impossible medical treatment areas. Therefore in this paper, the oriental medical treatment system algorithm and artificial intelligence electrical needle simulation are processed for real time and checked and treated, so anyone can decide patient's state using mobile phone.

The Connecting Paradigm between Skills and Free Imagination (기술과 자유로운 상상의 연결 패러다임)

  • The status of medical doctors is relatively high in society. However, in spite of this acknowledged status, physicians are not aware of the extent to which they have the ability to care for patients or how much effort they should make to meet people's expectations. Therefore, we should examine what society asks of doctors and how doctors need to be educated to meet the expectations of society. In this article, the author asserts that physicians need four skills. First, doctors should know how to speak and communicate. In the work of a doctor, language is the most important for tasks such as understanding texts, communication with patients, analyzing data, and starting new projects. Second, doctors should have intuition. In a doctor's medical judgment, intuition is very important and it can initiate from an educated guess. In other words, good intuition can be developed based on a good educated guess, which in turn can derive from one's explored knowledge, communication with one's inner dialogues, and good interpretation skill. Third, doctors should have creativity. Doctors should produce an image about patients from intuition, and those intuitions are based on creativity. Usually, students in medical school have creative ability; therefore, the instructor should facilitate their learning to connect this creativity to free imagination ability and medical skills. Fourth, doctors should be humane. Patients want to communicate with doctors about their disease and further about their lives. The reason why a humane doctor is important is that this humane approach itself could cure patients and reduce their pain. When a doctor's humane attitude is realized in the hospital, the patients and doctors could be pleased sincerely.

Medical Service Variation of Urinary Incontinence Surgery and Uterine Polypectomy Using a Multilevel Analysis (다수준 분석을 이용한 요실금수술과 자궁폴립제거술의 의료서비스 변이)

  • Background: This study investigates the influence factors of medical service variations using medical charge and the length of stay (LOS) for urinary incontinence surgery and uterine polypectomy. Methods: The National Health Insurance claims data and Medical Resource Report by the Health Insurance Review & Assessment Service in 2016 were used. Frequency analysis, one-way analysis of variance, and Bonferroni post-hoc tests were executed for each surgery. A multilevel analysis was executed to assess the factors to the medical charge and LOS for each surgery in patient, doctor, and hospital level. Results: Fifty-two point eight percent of urinary incontinence surgery and 87.1% of uterine polypectomy were distributed in general and tertiary hospitals. Among three levels, the patient level variation was 61.5% or 77.2% in medical charge and 93.9% or 96.3% in LOS, respectively. The doctor level variation was 29.6% or 22.6% in medical charge and 0.6% or 0.0% in LOS, respectively. The institution level variation was 8.9% or 0.2% in medical charge and 5.5% or 3.7% in LOS, respectively. Number of other disease and organizational type were main factors that affected the charge and LOS for urinary incontinence surgery and uterine polypectomy. Conclusion: Medical service variations of the urinary incontinence surgery and uterine polypectomy were the largest for the patient level, followed by doctor level for the medical charge, and the institution level for the LOS.

  • Tobacco has become the world's leading cause of deaths and diseases. And !be tobacco use and dependence itself is a kind of diseases, so-called "mental and be-havioural disorders due to use of tobacco" in "International Statistical Classification of Diseases and Related Health Problems(ICD-10)" and "Korean Standard Classification of Diseases". The tobacco use and dependence is a chronic disease that requires repeated clinical interventions and multiple attempts to quit. But effective treatments to the tobacco use and dependence are developed and exist that can significantly increase the rate of long-tenn smoking abstinence. So the physicians should warn smoking patients about the dangers of smoking to the health and the life, and the clinicians ought to provide one of more of the treatments which have been proven effective in helping smokers quit to smoke. It has been concluded that if a doctor failed to provide effective treatment for smokers, and the smokers subsequently died of the smokers-related conditions(tobaccosis) or became incapacitated by the tobaccosis the smokers were considered in the medical malpractice. Thus the smokers could sue the physician for medical malpractice, claiming that the doctor's legal responsibility of appropriate treatments including smoking-cessation which the physician deliberately or negligently breached.

Factors Affecting In-Patient Satisfaction of Oriental Hospital (한방병원입원환자의 환자만족도에 영향을 미치는 요인)

  • It is very important to estimate the patients satisfaction level with medical services, to classify the objectvies according to the patients characteristics and sub-satisfaction factors. The purpose of this study is to determine the factors affecting satisfaction in oriental hospital. The 549 patients' hospitalized in five oriental hospital in Taegu city and one oriental hospital in Kyungbuk province were selected for this study. The results summarized are as follows. l. The general characteristics of 549 objectvies were included gender, age, education, occupation, income level, length of stay, health status of hospitalized, and expectation for medical care. 2. Patients characteristics affecting patients total satisfaction, as for age(b=0.05), health status of patients(b=-0.052), and expectation for medical care(b=0.117) were significant, while gender, education, job, income level, and length of stay were not. As the factors according to patients satisfaction, accessibility(b=0.09l), doctor's kindness(b=0.357), staff kindness(b=0.137), nurse's skills(b=0.111), hospital facilities(b=0.211), and medical fee(b=-0.160) were significant. In total patients' satisfaction, Doctor's kindness was the most significant of prediction variables. In general the factors affecting In-patient satisfaction of oriental hospital was highly associated with doctor's kindness.

Convergence outpatient medical service patient experience research using data mining (데이터마이닝 기법을 이용한 융복합 외래 의료서비스 환자경험조사 연구)

  • The purpose of this study is to find out specific measures that can help the management strategy of patient-centered medical institutions by conducting research on patient experience surveys of convergence outpatient medical services using data mining techniques according to changes in patient-centered medical culture. Using the raw data of the 2018 Medical Service Experience Survey, 8,843 people over the age of 15 who had patient experience in outpatient medical services were analyzed. Decision tree analysis was performed. The determinants of satisfaction with outpatient medical services patient experience were the doctor's area and patient's rights protection area, and the determinants of intention to recommend outpatient medical services were the doctor's area and facilities comfort. Women evaluated the experience positively in overall satisfaction as compared to men, and those over the age of 60 positively evaluated the overall satisfaction and intention to recommend. It is significant that the outpatient experience decision-making model is presented, and that the doctor's area, patient's rights protection area, and facility comfort are important factors. Long-term research on the 'Medical Service Experience Survey' is needed, and research on the inpatient medical service experience is needed.

Analysis of Consumer's Recognition and Satisfaction for the Improvement of the Doctor-Designation System (선택 진료 제도 개선을 위한 소비자 인식도와 만족도)

  • The purpose of this study was to examine the recognition of citizens in Busan on the doctor-designation system, the awareness of medical consumers with experience of using this system and their satisfaction in an effort to seek ways of improving this system. The subjects in this study were the selected citizens in Busan who were at the age of 20 and up. As a result, it's found that the largest group of the respondents was female, in their 20s, received college or higher education, students and in the service industry, and that the most common monthly mean income was between two and 2.99 million won. 27.7 percent were aware of the doctor-designation system, and 23.7 percent became cognizant of the system through others who had used it. The rate of knowing the rules of the doctor-designation system (the right answer) stood at 66.3 percent. They got to know about the system through mass media(31.9%), and used it since it offered highly specialized treatment(57.5%). The respondents who had used it intended to reuse it(76.3%), and the reason was that they were provided with high-class medical services (35.2%). The respondents who had used this system got a mean of 2.96 in satisfaction level, which was not high in general. They mentioned more publicity efforts(91.2%), offering information in a conspicuous place (96.7%) and cutting doctor-designation treatment fee as a means of improving this system. As for how to ensure the operating efficiency of the system, sustained publicity seem to be necessary to raise awareness of the system among patients, and it's required to take measures to relieve patients of financial burden caused by medical bills.

Attitudes of Medical Professions towards the Cooperated Medical Treatments of Korean Oriental Medicine and Western Medicine (한양방협진에 대한 의료전문직의 태도)

  • A survey was practiced to the medical doctors of Je-Han Medical Center and Taegu Catholic Medical Center from November to December 2004. The collected data was analysed by the SPSS10+. The analysis of frequency, cross table, the difference of mean was practiced. The focus of the analysis was to find the difference of attitudes between Korean Oriental medical doctors and Western medical doctors. The results of the analysis showed that Korean Oriental medical doctors are more active to the cooperated medical treatments than Western medical doctors. They also have more experience. And both of Korean Oriental medical doctors and Western medical doctors agree to the necessity of the cooperated medical treatments. But the preferred combination pattern of Korean Oriental medicine and Western medicine was different between two medical groups. Korean Oriental medical doctors prefer the one to one combination pattern. But Western medical doctors prefer to Western medicine centered combination pattern. This study has limitations in sampling prcesss and sample size. But considering that the study of the cooperated medical treatments is few, this study has considerable medical sociological meaning.

Problems and Developing Directions of the Legal System Related to Laboratory medical testing (검체검사 관련 법제도의 문제점 및 발전방향)

  • When we are totalizing the lawsrelated to the medical, as the Laboratory medical testing is a kind of the medical act, it is the regulation that the medical technologist can analyze the specimen using in vitro diagnostic devices and diagnosticdrugs under the guidance of doctor or dentist from a corresponding medical institution and can report through verification and interpretation. However, in real medical fields, 'the guidance of doctor' is seriously in-sufficient or even the person who is not the medical technologist is executing. Furthermore the cases that produce inspection results with devices or reagents which are not validated nor approved have been frequently occurred. The result of Laboratory medical testing derived from this procedure can become the important information for the disease control of a country, and also can be decisive to the definite diagnosis and the prognostic monitoring about the patient disease. In spite of its significant medical act to be applicable to an unique proof with the related expert appraisal result in the medical mal-practice lawsuit, our reality in which the quality control is not properly working due by the costs and the labor shortage related to the Laboratory medical testing is quietly in bad condition. Even from now, the government should recognize the significance of the Laboratory medical testing and must achieve more strict administrative management as well as the law maintenance.
