Does Beta-blocker Therapy Improve the Survival of Patients with Metastatic Non-small Cell Lung Cancer?

  • Aim: To determine whether beta-blockers (BBs) improve the overall survival (OS) of patients with metastatic non-small cell lung cancer (NSCLC). Materials and Methods: The medical charts of 107 patients with metastatic NSCLC were retrospectively assessed. Thirty-five patients (BB group) using BBs during chemotherapy (CT) were compared with 72 controls [control=(C) group] who did not use BBs following the diagnosis of NSCLC. The histological tumor subtype, performance status (ECOG), age, gender, smoking status, comorbidities, other medications and chemotherapeutics that were received in any line of treatment were recorded. We compared the overall survival (OS) of the patients in the BB and C groups. Results: The mean age of the patients was 61 years (range 42-81 years) and all patients were administered CT. The BB group was more likely to have HT and IHD and was more likely to use RAS blockers (p<0.01 for all) compared with the C group, as expected. The mean follow-up time was 17.8 months (range 1-102 months) for the entire group. The most commonly prescribed BB agent was metoprolol (80% of cases). At the time of the analysis, 74 (69%) of all patients had died. In the univariate analysis the median overall survival (OS) was 19.25 (${\pm}2.87$) months (95%CI: 13.62-24.88) in the BB group and 13.20 (${\pm}2.37$) months (95%CI: 8.55-17.85) in the C group (p=0.017). However, the benefit of BBs on survival disappeared in the multivariate analysis. Conclusions: The use of BBs during CT may be associated with an improved OS for patients with metastatic NSCLC.

Phase II Study on Voriconazole for Treatment of Chinese Patients with Malignant Hematological Disorders and Invasive Aspergillosis

  • Objective: To investigate the efficacy and safety of voriconazole in treating Chinese patients with hematological malignancies and invasive aspergillosis. Methods: From March 2007 to April 2012, patients with diagnoses confirmed by CT, GM test and/or PCR assays, were recruited into this study. Aspergillosis of all patients were treated with voriconazole 6 mg/kg intravenous infusion (iv) every 12 h for 1 day, followed by 4 mg/kg IV every 12 h for 10-15 days; Then, switch to oral administration that was 200mg every 12h for 4-12 weeks. Efficacy and safety were evaluated according to Practice Guideline of Infectious Diseases Society of America. Results: The overall response rate of 38 patients after voriconazole treatment was 81.6%. The median time to pyretolysis was 4.5 days. Treatment related side effects were mild and found in only 15.8% of cases. No treatment related deaths occurred. Conclusions: Voriconazole can considered to be a safe and effective front-line therapy to treat patients with hematological malignancies and invasive aspergillosis. Alternatively it could be used as a remedial treatment when other antifungal therapies are ineffective.

Comparison of Expression Signature of Histone Deacetylases (HDACs) in Mesenchymal Stem Cells from Multiple Myeloma and Normal Donors

  • Background: Histone acetylation in chromatin structures plays a key role in regulation of gene transcription and is strictly controlled by histone acetyltransferase (HAT) and deacetylase (HDAC) activities. HDAC deregulation has been reported in several cancers. Materials and Methods: The expression of 10 HDACs (including HDAC class I and II) was studied by quantitative reverse transcription-PCR (qRT-PCR) in a cohort of mesenchymal stem cells (MM-MSCs) from 10 multiple myeloma patients with a median age 60y. The results were compared with those obtained for normal donors. Then, a coculture system was performed between MM-MSCs and u266 cell line, in the presence or absence of sodium butyrate (NaBT), to understand the effects of HDAC inhibitors (HDACi) in MM-MSCs on multiple myeloma cases. Also, the interleukin-6 (IL-6) and vascular endothelial growth factor (VEGFA) gene expression level and apoptotic effects were investigated in MM-MSCs patients and control group following NaBT treatment. Results: The results indicated that upregulated (HDACs) and downregulated (IL6 and VEGFA) genes were differentially expressed in the MM-MSCs derived from patients with multiple myeloma and ND-MSCs from normal donors. Comparison of the MM-MSCs and ND-MSCs also showed distinct HDACs expression patterns. For the first time to our knowledge, a significant increase of apoptosis was observed in coculture with MM-MSCs treated with NaBT. Conclusions: The obtained findings elucidate a complex set of actions in MSCs in response to HDAC inhibitors, which may be responsible for anticancer effects. Also, the data support the idea that MSCs are new therapeutic targets as a potential effective strategy for MM.

뇌경막하수종의 수술적 치료에 따른 임상 경과 (Progression of Subdural Effusion after Surgical Treatment)

  • 본 연구에서는 단순천공배액술을 시행하고 호전된 환자들과 이차 수술로 단순천공배액술을 실시하거나 경막하복강단락술을 실시한 환자들의 임상적 경과 분석을 통해 뇌경막하수종 치료에 도움이 되고자 한다. 2006년 7월부터 2012년 6월까지 본원에서 뇌경막하수종으로 입원하여 단순천공배액술을 시행받은 16례 전체 환자를 대상으로 분석하였다. 연구결과 아스피린을 복용하고 있는 환자는 모두 이차 수술을 시행하였다(p<0.001). 경막하복강단락술을 시행한 환자 4명 중 2명이 아스피린을 복용하여 통계적으로 유의하였다(p=0.014). 경막하복강단락술을 시행한 그룹과 시행하지 않은 그룹 간의 비교 결과, 4명의 환자는 일차수술 후 GCS의 변화는 없었으나 추적 전산화단층촬영 상 정중선 편위가 심하였고 경막하수종은 증가하거나 변화가 없었으며 두통, 구토, 발열, 호흡곤란 등의 뇌압상승 증상을 보여 단락술을 결정하게 되었다(p=0.006). 아스피린을 복용했던 환자들의 수술적 치료는 주의를 요하며 개두술 과정에서 뇌척수액의 급격한 소실과 뇌압의 지나친 변화를 예방하는 것이 중요하다고 사료된다.

Successful and safe treatment of hemangioma with oral propranolol in a single institution

  • Purpose: Dramatic improvement of hemangioma to propranolol has been recently reported; however, details on dose and duration of treatment, potential risks, and monitoring have not been determined. The objective of this study is to describe and analyze the use of propranolol as a first-line treatment or as a single therapy in management of complicated hemangioma. Methods: A retrospective chart review of eight patients diagnosed with hemangioma and treated with propranolol in Kangbuk Samsung Hospital from February 2010 to April 2011 was performed. Results: Eight patients with hemangioma with functional impairment, cosmetic disfigurement, or rapid growth were treated with propranolol. Five patients had solitary facial hemangioma. The mean age of symptoms at onset was 5 weeks. The median age for starting propranolol treatment was 5.5 months. Propranolol at 2 mg/kg/day was finally administered in divided doses with a gradual increase. Significant regression was observed in seven patients, and shrinkage in size, softening in consistency, and decrease in redness were evident within 4 weeks. Among them, six patients were still taking propranolol, and one patient had stopped after 12 months. Other one patient did not show significant improvement with satisfactory result after 3 months of propranolol use. Treatment with propranolol was well tolerated and had few side effects. No rebound growth was observed in any of the patients. Conclusion: We observed that use of propranolol was very effective in treatment of hemangioma without obvious adverse effects or relapse.

히스토그램 현 탐색방식에 의한 레이다 표적 분할 알고리즘 (Radar Target Segmentation via Histogram Chord Search Method)

  • 적응 분할 알고리즘은 국부 비 균질 영상에서 효율적인 표적 탐지를 위해 사용된다. 지금까지 영상 분할에 여러 가지 적응방법이 제안되었다. 하지만 이들 알고리즘이 레이다 표적 탐지에 바로 적용하기는 어려운데 이는, 레이다 자체 신호 특성 때문이다. 일반적으로 레이다 표적은 배경신호에 비해 상대적으로 적은 도수 함수를 갖고 있으며, 배경 분포는 외부환경에 따라 다양한 형태를 갖는다. 본 논문에서는 도수함수의 최대 값에서 우측 하향 직선인 히스토그램 현을 이용한 적응 표적분할 알고리즘을 제안한다. 제안한 방식은 임계값 탐색에 사용되는 적용 현이 외부환경조건에 영향을 적게 받음으로 인해 다양한 레이다 환경에 최적인 임계 값을 선택한다. 시뮬레이션 결과, 제안한 알고리즘이 기존 표적분할알고리즘인 전역 임계 값 방법 및 분포 중앙값 추정방식에 비해 탐지성능측면에서 우수함을 보였다.

Roles of Non-cholinergic Intrapancreatic Nerves, Serotonergic Nerves, on Pancreatic Exocrine Secretion in the Isolated Perfused Rat Pancreas

  • It has been rereported that axons which display 5-hydroxytryptamine (5-HT) immunoreactivity are abundant in the pancreas and the majority of serotonergic axons terminate within intrapancreatic ganglia, islet and acini. This histological result strongly suggests that intrapancreatic serotonergic nerves could affect to the pancreatic endocrine and exocrine secretion. Thus, this study was aimed to investigate whether intrapancreatic serotonergic nerves could affect pancreatic exocrine secretion and an action mechanism of the intrapancreatic serotonergic nerves. The rats were anesthetized with a single injection of urethane. The median line and the abdominal aorta was carefully dissected and cannulated with PE-50 tubing just above the celiac artery, and then tightly ligated just below the superior mesenteric artery. The pancreatic duct was also cannulated with Tygon microbore tubing. With the addition of serotonin, pancreatic volume flow and amylase output were significantly inhibited electrical field stimulation (EFS). On the other hand, pancreatic volume flow and amylase output were significantly elevated in EFS with the addition of spiperone. EFS application, however, pancreatic volume flow and amylase output had no significant change in cholecystokinin (CCK) alone when serotonin was applied under a 5.6 mM glucose background. Pancreatic volume flow and amylase output under 18 mM glucose background were significantly elevated in CCK plus serotonin than in CCK alone. These data suggest that intrapancreatic serotonergic nerves play an inhibitory role in pancreatic exocrine secretion and an important role in the insulin action or release.

  • 전기·전자제품 생산업자와 판매업자는 자신이 배포한 장비가 속하는 등급에 해당하는 폐전기·전자제품을 직접 수거하거나 한국전자제품자원순환공제조합에 가입하여 공제조합이 대리할 의무를 이행하도록 할 수 있다. 본 연구에서는 분담금의 요율체계를 검토하고, 파악한 문제점과 이에 대한 개선방안을 제시한다. 첫째, 수집비 및 운반비에 대해 회수유형별 작업을 고려하여 표준작업 및 시간을 설정해야 한다. 둘째, 리사이클링센터의 선별방식에 차이가 있으므로, 이들 방식의 차이와 처리하는 제품군 특성을 반영하여 할당인자에 대한 표준원가를 설정해야 한다. 셋째, 방문수거비 예측모형에서 최소값이 아닌 중앙값 또는 평균값으로 결정하여 충분한 규모의 예산을 확보해야 한다. 본 연구는 공제회원이 공제조합에 납부하는 분담금의 문제점을 살펴보고 이에 대한 개선방안을 제시하였다.

A Peripherally Inserted Central Catheter is a Safe and Reliable Alternative to Short-Term Central Venous Catheter for the Treatment of Trauma Patients

  • Purpose: To determine whether a peripherally inserted central catheter (PICC) meets the goals of a low infection rate and long-term use in trauma patients. Methods: From January 2016 to June 2018, the medical records of patients who underwent central venous catheterization at a level I trauma center were retrospectively reviewed. Data collected included age, sex, injury severity score, site of catheterization, place of catheterization (intensive care unit [ICU], emergency department, or general ward), type of catheter, length of hospital stay during catheterization, types of cultured bacteria, time to development of central line-associated bloodstream infection (CLABSI), and complications. Results: During the study period, 333 central vein catheters (CVC) were inserted with a total of 2,626 catheter-days and 97 PICCs were placed with a total of 2,227 catheter-days. The CLABSI rate was significantly lower in the PICC group when the analysis was limited to patients for whom the catheter was changed for the first time in the ICU after CVC insertion in the ER with similar indication and catheter insertion times (18.6 vs. 10.3/1,000 catheter-days, respectively, p<0.05). The median duration of catheter use was significantly longer in the PICC group than in the CVC group (16 vs. 6 days, respectively, p<0.05). Conclusions: The study results showed that the duration of catheter use was longer and the infection rate were lower in the PICC group than in the CVC group, suggesting that PICC is a safe and reliable alternative to conventional CVC.

Hematocrit Determination using a Volumetric Absorptive Microsampling Technique in Patients with Pancreatic Cancer

  • Background: Hematocrit is usually measured from venous blood collected by invasive venipuncture. This study was performed to determine hematocrit accurately and precisely using minimally invasive volumetric absorptive microsampling (VAMS) technique. Such technique is to be applied to determining hematocrit in various clinical settings for the care, including therapeutic drug monitoring, of neonatal or epileptic patients, or patients with high risk of infection or bleeding. Methods: The study was performed using 31 VAMS samples obtained from 21 pancreatic cancer patients. Hematocrit was determined using the values of potassium concentrations obtained from blood in VAMS tips (HctVAMS). HctVAMS was compared with hematocrit measured from blood collected by venipuncture (HctVP). The accuracy and precision of HctVAMS in comparison to HctVP were evaluated using Bland-Altman plot, Deming regression and mountain plot. Results: Bland-Altman plot displayed a random scattering pattern of the differences between HctVAMS and HctVP with the mean bias of -0.010 and the 95% limit of agreement ranging from -0.063 to 0.044. Deming regression for HctVAMS and HctVP line demonstrated very small proportional and constant biases of 1.04 and -0.003, respectively. Mountain plot exhibited a narrow and symmetrical distribution of the differences with their median of -0.011 and central 95% range from -0.049 to 0.033. Conclusion: Hematocrit was accurately and precisely determined using less invasive VAMS technique. Such technique appears to be applicable to determining hematocrit in situations that venipuncture is not favorable or possible.
