Hypofractionated intensity-modulated radiotherapy in patients with localized prostate cancer: a preliminary study

  • Purpose: The aim of this work was to assess the efficacy and tolerability of hypofractionated intensity-modulated radiotherapy (IMRT) in patients with localized prostate cancer. Materials and Methods: Thirty-nine patients who received radical hypofractionated IMRT were retrospectively reviewed. Based on a pelvic lymph node involvement risk of 15% as the cutoff value, we decided whether to deliver treatment prostate and seminal vesicle only radiotherapy (PORT) or whole pelvis radiotherapy (WPRT). Sixteen patients (41%) received PORT with prostate receiving 45 Gy in 4.5 Gy per fraction in 2 weeks and the other 23 patients (59%) received WPRT with the prostate receiving 72 Gy in 2.4 Gy per fraction in 6 weeks. The median equivalent dose in 2 Gy fractions to the prostate was 79.9 Gy based on the assumption that the ${\alpha}/{\beta}$ ratio is 1.5 Gy. Results: The median follow-up time was 38 months (range, 4 to 101 months). The 3-year biochemical failure-free survival rate was 88.2%. The 3-year clinical failure-free and overall survival rates were 94.5% and 96.3%, respectively. The rates of grade 2 acute genitourinary (GU) and gastrointestinal (GI) toxicities were 20.5% and 12.8%, respectively. None of the patients experienced grade ${\geq}3$ acute GU and GI toxicities. The grade 2-3 late GU and GI toxicities were found in 8.1% and 5.4% of patients, respectively. No fatal late toxicity was observed. Conclusion: Favorable biochemical control with low rates of toxicity was observed after hypofractionated IMRT, suggesting that our radiotherapy schedule can be an effective treatment option in the treatment of localized prostate cancer.

Upfront Stereotactic Radiosurgery for Pineal Parenchymal Tumors in Adults

  • Objective : Pineal parenchymal tumors (PPTs) in adults are rare, and knowledge regarding their optimal management and treatment outcome is limited. Herein, we present the clinical results of our series of PPTs other than pineoblastomas managed by stereotactic radiosurgery (SRS) at upfront setting. Methods : Between 1997 and 2014, nine consecutive adult patients with the diagnosis of PPTs, either pineocytoma or pineal parenchymal tumor of intermediate differentiation, were treated with SRS. There were 6 men and 3 women. The median age was 39 years (range, 31-53 years). All of the patients presented with symptoms of hydrocephalus. Endoscopic third ventriculostomy and biopsy was done for initial management. After histologic diagnosis, patients were treated with Gamma Knife with the mean dose of 13.3 Gy (n=3) or fractionated Cyberknife with 32 Gy (n=6). Results : After a mean follow-up of 78.6 months (range, 14-223 months), all patients were alive and all of their tumors were locally controlled except for one instance of cerebrospinal fluid seeding metastasis. On magnetic resonance images, tumor size decreased in all patients, resulting in complete response in 3 patients and partial response in 6. One patient had experienced temporary memory impairment after SRS, which improved spontaneously. Conclusion : SRS is effective and safe for PPTs in adults and can be considered as a useful alternative to surgical resection at upfront setting.

Combined chemotherapy and intra-arterial chemotherapy of retinoblastoma

  • Purpose: Retinoblastoma (RB) is the most common primary malignant intraocular tumor in children. Although systemic chemotherapy has been the primary treatment, intra-arterial chemotherapy (IAC) represents a new treatment option. Here, we performed alternate systemic chemotherapy and IAC and retrospectively reviewed the efficacy and safety of this approach. Methods: Patients diagnosed with intraocular RB between January 2000 and December 2011 at Severance Children's Hospital, Yonsei University, were reviewed. Before February 2010, the primary treatment for RB was chemotherapy (non-IAC/CTX). Since February 2010, the primary treatment for RB has been IAC (IAC/CTX). External beam radiotherapy or high-dose chemotherapy were used as "last resort" treatments just prior to enucleation at the time of progression or recurrence during primary treatment. Enucleation-free survival (EFS) and progression-free survival were assessed. Results: We examined 19 patients (median age, 11.9 months; range, 1.4 to 75.6 months) with a sum of 25 eyes, of which, 60.0% were at advanced Reese Ellsworth (RE) stages. The enucleation rate was 33.3% at early RE stages and 81.8% at advanced RE stages (P=0.028). At 36 months, EFS was significantly higher in the IAC/CTX group than in the non-IAC/CTX group (100% vs. 40.0%, P=0.016). All 5 patients treated with IAC achieved eye preservation, although most patients were at advanced RE stages (IV-V). Conclusion: Despite the limitation of a small sample size, our work shows that an alternative combined approach using IAC and CTX may be safe and effective for eye preservation in advanced RB.

Gemcitabine And Cisplatin Followed by Chemo-Radiation for Advanced Nasopharyngeal Carcinoma

  • Concurrent chemo-radiation (CRT) has been established as the standard of care for non-metastatic loco-regionally advanced nasopharyngeal carcinoma (NPC) but recently the addition of induction chemotherapy in the already established regimen has presented an attractive multidisciplinary approach. This retrospective study was carried out to evaluate the efficacy of induction chemotherapy (IC) followed by CRT for the management of loco-regionally advanced NPC. Between July 2005 and September 2010, 99 patients were treated with cisplatin based IC followed by CRT. Induction chemotherapy included a 2 drug combination; intravenous gemcitabine $1000mg/m^2$ on day 1 and 8 and cisplatin $75mg/m^2$ on day 1 only. Radiotherapy (RT) was given as a phase treatment to a total dose of 70 Gy in 35 fractions. Concurrent cisplatin ($75mg/m^2$) was administered to all patients on days 1, 22 and 43. All patients were evaluated for tumor response and adverse effects after IC and 6 weeks after the completion of the treatment protocol. Statistical analysis was performed using SPSS version 17 and Kaplan Meier estimates were applied to project survival. Median follow-up duration was 20 months. The 5-year overall survival (OS), loco regional control (LRC) and relapse free survival (RFS) rates were 71%, 73% and 50%respectively. Acute grade 4 toxicity related to induction chemotherapy and concurrent chemo-radiation was 4% and 2% respectively, with only 3 toxicity-related hospital admissions. We conclude that induction gemcitabine and cisplatin followed by chemo-radiation is a safe and effective regimen in management of nasopharyngeal carcinoma, meriting further investigation in randomized clinical trials.

Predictors of Outcome in Patients with Advanced Nonseminomatous Germ Cell Testicular Tumors

  • Background: Predictor factors determining complete response to treatment are still not clearly defined. We aimed to evaluate clinicopathological features, risk factors, treatment responses, and survival analysis of patient with advanced nonseminomatous GCTs (NSGCTs). Materials and Methods: Between November 1999 and September 2011, 140 patients with stage II and III NSGCTs were referred to our institutions and 125 patients with complete clinical data were included in this retrospective study. Four cycles of BEP regimen were applied as a first-line treatment. Salvage chemotherapy and/or high-dose chemotherapy (HDCT) with autologous stem cell transplantation were given in patients who progressed after BEP chemotherapy. Post-chemotherapy surgery was performed in selected patients with incomplete radiographic response and normal tumor markers. Results: The median age was 28 years. For the good, intermediate and poor risk groups, compete response rates (CRR) were, 84.6%, 67.9% and 59.4%, respectively. Extragonadal tumors, stage 3 disease, intermediate and poor risk factors, rete testis invasion were associated with worse outcomes. There were 32 patients (25.6%) with non-CR who were treated with salvage treatment. Thirty-one patients died from GCTs and 94% of them had stage III disease. Conclusions: Even though response rates are high, some patients with GCTs still need salvage treatment and cure cannot be achieved. Non-complete response to platinium-based first-line treatment is a negative prognostic factor. Our study confirmed the need for a prognostic and predictive model and more effective salvage approaches.

Successful and safe treatment of hemangioma with oral propranolol in a single institution

  • Purpose: Dramatic improvement of hemangioma to propranolol has been recently reported; however, details on dose and duration of treatment, potential risks, and monitoring have not been determined. The objective of this study is to describe and analyze the use of propranolol as a first-line treatment or as a single therapy in management of complicated hemangioma. Methods: A retrospective chart review of eight patients diagnosed with hemangioma and treated with propranolol in Kangbuk Samsung Hospital from February 2010 to April 2011 was performed. Results: Eight patients with hemangioma with functional impairment, cosmetic disfigurement, or rapid growth were treated with propranolol. Five patients had solitary facial hemangioma. The mean age of symptoms at onset was 5 weeks. The median age for starting propranolol treatment was 5.5 months. Propranolol at 2 mg/kg/day was finally administered in divided doses with a gradual increase. Significant regression was observed in seven patients, and shrinkage in size, softening in consistency, and decrease in redness were evident within 4 weeks. Among them, six patients were still taking propranolol, and one patient had stopped after 12 months. Other one patient did not show significant improvement with satisfactory result after 3 months of propranolol use. Treatment with propranolol was well tolerated and had few side effects. No rebound growth was observed in any of the patients. Conclusion: We observed that use of propranolol was very effective in treatment of hemangioma without obvious adverse effects or relapse.

진행된 비소세포 폐암에 대한 High-VPP 복합화학요법의 효과 (High VPP Combination Chemotherapy for Advanced Non-Small Cell Lung Cancer)

  • 연구배경 : 절제가 불가능한 비소세포 폐암 환자에 대한 복합화학요법의 유용성은 아직도 분명치 않다. 그러나, cisplatin을 주축으로하는 복합화학요법의 반응율은 항암제 단독 투여시의 반응율 보다, 또한 high-dose cisplatin군의 반응율이 low-dose cisplatin군의 반응율보다 높은 것으로 최근 보고되고 있다. 복합화학요법(high VPP), 복합화학요법과 방사선 치료의 병행요법이 진행된 비소세포 폐암에서 생존율을 증가시키는지 알아보기 위해 본 연구를 시작하였다. 방법 : 조직학적으로 비소세포 폐암으로 진단된 stage III이상의 환자 35명을 대상으로 하였다. 이들중, 19명은 VP-16과 high-dose cisplatin(100 $mg/m^2$)으로 구성되는 복합화학요법 그리고 복합화학요법과 방사선 치료를 시행받았으며, 나머지 16명은 치료를 받지 않았다. 두군간의 생존율과 반응율의 차이 및 치료에 다른 부작용을 알아보기 위해 환자의 병록 일지를 검토하였다. 결과: 1) 전체적인 객관적 반응율은 한명의 완전관해를 포함하여 47%(9/19) 였다. 2) 복합화학요법과 방사선 치료를 병행하여 받은 환자군에서의 반응율은 한명의 완전관해를 포함하여 60%(6/10) 였으며, 3개월, 6개월 및 127개월 생존율은 각각 100%, 70% 및 40% 였다. 3) 복합화학요법을 받은 환자군에서의 반응율은 완전관해 없이 33%(3/9) 였으며, 3개월, 6개월 및 12개월 생존율은 각각 78%, 67% 및 33% 였다. 4) 전체적으로 치료군에서 비치료군에 비해 통계적으로 유의하게 (p<0.05) 생존기간이 연장되었다(중앙생간 307일과 95일). 5) 여러 예후인자에 따른 분석에서 운동능력이 좋을수록, stage III에서 그리고 편평상피암에서 좋은 반응율을 보였다. 6) 부작용으로서는 오성과 구토(100%), 탈모증(90%), 빈혈(79%), 백혈구 감소증 (69%), 혈소판 감소증(2%), creatinine의 증가(16%) 그리고 신경독성(5%) 등이 있었다. 결론 : 이상의 결과로 진행된 비소세포 폐암 환자에 대한 hign VPP 복합화학요법은 비치료군에 비해 치료군에서 생존율을 높이는 비교적 좋은 효과가 있으므로 좋은 적응증을 가진 환자들을 선택하여 효과적인 항암치료 및 방사선 치료를 시행하여야 할 것으로 사료된다. 그러나, hign VPP 복합화학요법에 따른 효과 개선은 본 연구 결과에선 뚜렷하지 않으므로 보다 많은 증례에서 추구 연구가 필요하리라 생각된다.

두개인두종의 감마나이프 치료 후 장기 추적 결과 (Long-term Results of Gamma Knife Radiosurgery for Craniopharyngioma)

  • Objectives : The optimal treatment of craniopharyngioma is controversial. Despite recent advances in microsurgical management, complete surgical removal of craniopharyngioma remains very difficult. Radiation added to surgery is effective, but radiation therapy resulted in untoward side effect in young patient. Gamma knife radiosurgery offers the theoretical advantage of a reduced radiation dose to surrounding structures during the treatment of residual or recurrent craniopharyngioma compared with fractionated radiotheraphy. We described retrospective analysis of tumor size and clinical symptoms of patients after gamma knife radiosurgery in residual or recurrent craniopharyngioma were performed. Material and Methods : From September 1990 to January 2000, 18 patients of craniopharyngioma were treated by gamma knife radiosurgery. All patient had undergone surgery, but residual or recurrent tumor was found and all of them treated postoperative gamma knife radiosurgery. The mean age was 19(from 6 to 66) and male to female ratio was 10 to 8 and 8 patients were below 15 years old. In young age group(below age 15), the average volume of the tumor was $2904.8mm^3$ and mean maximal gamma knife dose was 34.9Gy. In old age group(older than 15), the average volume of the tumor was $2590.4mm^3$ and mean maximal gamma knife dose was 45.2Gy. The size of the tumor was average $2730.1mm^3$($88-12000mm^3$), mean average radiation dose was 40.7Gy and the mean prescription dose was 17.6 Gy(4-35Gy) delivered to a median prescription 50.7% isodose. Results : The follow up was from 1 year to 9 years(mean 59.1 months) after gamma knife radiosurgery. The tumor was controlled in 13(72.2%) patients. The tumor decreased in 9 patients and not changed in 4 patients. The tumor size increased in 4(22.2%) patients during follow up period. In two cases the tumor size increased because of its cystic portion was increased, but their solid portion of the tumor was not changed. In another two patients, the solid portion of the tumor was increased. So, one patient underwent reoperation and the other patient underwent operation and repeated gamma knife radiosurgery. The tumor recurred in one case(5.6%) that is a outside of irradiated site. The presenting symptoms were improved in 4 patients(improved visual acuity in 1, controlled increased intracranial presure sign in 3 patients). In one case, visual acuity decreased after gamma knife radiosurgery. The endocrine symptoms were not influenced by gamma knife radiosurgery. Conclusion : Craniopharyngioma can be treated successfully by gamma knife radiosurgery. Causes of the tumor regrowth are inadequate dose planning because of postoperatively poor margination of the tumor, close approximation of optic nerve and residual tumors outside the target lesion. Recurrence can develop 4 years after gamma knife radiosurgery. Volume is important, but the accurate targeting is more important to prevent tumor recurrence. If the tumor definition is not clear during planning gamma knife surgery, long-term image follow up is required.

식도 편평세포암의 방사선치료 결과 (Results of Radiation Therapy for Squamous Cell Carcinoma of the Esophagus)

  • 목적: 식도암에서 수술 후 방사선치료 및 방사선 단독 치료의 효과를 검증하고 예후 인자를 알아보고자 하였다. 대상 및 방법: 1989년부터 2006년까지 방사선치료를 받은 132명의 환자를 후향적으로 분석하였다. 35명은 제2병기 88명은 제3병기 9명은 제4병기 환자였다. 상부 식도암이 18명, 하부 식도암이 33명이었으며 중등부 식도암이 81명이었다. 60명에서는 방사선 단독 치료를 시행하였고 72명은 수술 후 방사선치료를 받았다. 8명에서는 40 Gy 이하를 조사하였으며 78명에서는 40내지 50 Gy를 조사하였다. 나머지 46명은 50내지 60 Gy를 조사 받았으며 수술 후 방사선치료를 받은 대부분의 환자는 45 Gy를 조사받았다. 결과: 전체 환자의 2년 및 5년 생존율은 24% 및 5%이었고 중앙 생존기간은 11개월이었다. 수술 후 방사선치료군의 2년 및 5년 생존율은 19% 및 8%, 방사선 단독치료군의 2년 및 5년 생존율은 18% 및 2%이었다. 제2병기, 제3 병기 및 제4병기 환자의 2년 및 5년 생존율은 각각 43% 및 15, 22% 및 2, 0% 및 0% 이었으며 이는 통계적으로 유의하였다. 상부, 중등부, 하부 식도암의 2년 생존율은 각각 19, 29, 22%이었으며 통계적으로 유의한 차이는 나타나지 않았다. 방사선 단독치료군에서 13명(22%)은 완전관해를, 40명(67%)은 부분관해를 보였고 7명(12%)은 반응을 보이지 않았다. 완전관해를 보인 환자군이 부분관해를 보인 환자군보다 더 나은 2년 생존율을 나타내었다(31% vs 17%). 결론: 식도암에서 방사선치료는 효과적이고 유용한 치료 방법이었으며 병기 및 방사선치료에의 반응 정도가 유의한 예후인자이었다. 그러나 식도암의 장기 생존율 및 예후가 만족스럽지 못하므로 더욱 효과적인 치료 방법의 개발이 향후 필요하리라 사료된다.

IIIB 병기 비소세포폐암에서 Paclitaxel과 Cisplatin을 이용한 선행항암화학요법과 동시 항암화학방사선치료 (Paclitaxel and Cisplatin with Induction Chemotherapy Followed by Concurrent Chemoradiotherapy for Stage IIIB Non-small Cell Lung Cancer)

  • 목 적: III 병기 비소세포폐암의 치료는 항암화학요법, 수술, 방사선치료가 포함된 병용치료가 표준방법으로 알려져 있다. 본 연구에서는 IIIB 병기 비소세포폐암에서 paclitaxel과 cisplatin을 이용한 선행항암 화학요법과 동시 항암화학방사선치료를 시행하여 그 효과에 대하여 알아보고자 하였다. 대상 및 방법: 2000년 7월부터 2005년 10월까지 IIIB 병기 비소세포폐암으로 선행항암화학요법과 동시 항암화학방사선치료를 받았던 39명을 대상으로 하였다. 선행항암화학요법은 3주 간격으로 paclitaxel ($175\;mg/m^2$)과 cisplatin ($75\;mg/m^2$)을 1일째와 21일째 정맥투여하였다. 동시 항암화학요법은 43일째, 50일째, 57일째, 71일째, 78일째, 85일째 paclitaxel ($60\;mg/m^2$)과 cisplatin ($25\;mg/m^2$)을 정맥투여하였다. 흉부방사선치료는 1회 1.8 Gy씩, 주 5회 분할조사 하였으며 총방사선량은 $54{\sim}59.4\;Gy$이었다(중앙값: 59.4 Gy). 결 과: 추적관찰기간은 $6{\sim}63$개월이었으며 중앙추적관찰기간은 21개월이었다. 선행항암화학요법 후 치료반응은 부분반응 41.0% (16명), 무반응 59.0% (23명)였다. 동시 항암화학방사선치료 후 치료반응은 완전관해가 10.3% (4명), 부분반응 41.0% (16명), 무반응 49.7% (19명)로 치료 반응률은 51.3%였다. 1년, 2년, 3년 생존율은 각각 66.7%, 40.6%, 27.4%였으며 중앙 생존기간은 20개월이었다. 1년, 2년, 3년 무진행 생존율은 각각 43.6%, 24.6%, 24.6%였으며 중앙 무진행 생존기간은 10.7개월이었다. 동시 항암화학방사선치료 후 부작용으로 3도 이상의 식도염은 46.3% (18명), 폐렴은 28.2% (11명)에서 발생하였다. 결론: IIIB 병기 비소세포폐암에서 paclitaxel과 cisplatin을 이용한 선행항암화학요법과 동시 항암화학방사선치료를 시행한 결과 비교적 효과적이었다. 그러나 식도염과 폐렴이 많아 부작용을 줄이기 위해 적절한 항암제의 선택 또는 방사선치료와의 병용치료의 변화가 필요할 것으로 판단되었다.
