A Surgical Option for Multilevel Anterior Lumbar Interbody Fusion with Ponte Osteotomy to Achieve Optimal Lumbar Lordosis and Sagittal Balance

  • Objective : To document lumbar lordosis (LL) of the spine and its change during surgeries with the different height but the same angle setting of the anterior cage. Additionally, we attempted to determine if sufficient LL is achieved at different cage heights and to quantify the change in LL during multi-level anterior lumbar interbody fusion (ALIF). Methods : The medical records and radiographs of 42 patients who underwent more than 2 level ALIFs between 2008 and 2009 were retrospectively reviewed. We evaluated 3 parameters seen on lateral whole spine radiographs : LL, pelvic incidence (PI), and sagittal vertical axis (SVA). The mean follow-up time was 28.1 months and the final follow-up radiographs of all patients were reviewed at least 2 years after surgery. Statistical analysis was performed using the paired t-tests. Results : Lumbar lordosis had changed up to 30 degrees immediately and 2 years after surgery (preoperative mean LL, SVA : 22.45 degrees, 112.31 mm; immediate postoperative mean LL, SVA : 54.45 degrees, 37.36 mm; final follow-up mean LL, SVA : 49.56 degrees, 26.95 mm). Our goal of LL is to obtain as much PI as possible, preoperative mean PI value was $55.38{\pm}3.35$. The pre-operative and two year post-surgery follow-up mean of the Japanese Orthopedic Association score were $9.2{\pm}0.6$ and $13.2{\pm}0.6$ (favorable outcome rate : 95%), respectively. In addition, we were able to obtain good clinical outcomes and sagittal balance with a subsidence rate of 22.7%. Conclusion : We were able to achieve sufficient LL, such that it was similar to the PI, utilizing multi-level ALIF with the use of a tall cage with the same angle setting of the cage. We have found out that achieving sufficient lumbar lordosis and sagittal balance require an anterior lumbar cage with high angle and height.

The Effect of Dietary Protein Levels on Nitrogen Metabolism in Young Korean Women (한국여성의 단백질 섭취수준이 질소대사에 미치는 영향)

  • This study was performed to investigate the effect of dietary levels on protein metabolism in eight healthy Korean adult females. The 20-day metabolic study consisted of 2 day adaptation period and three 6-day experimental. Three experimental diets were low protein (LP : protein 44g), high protein(HP : protein 85g) and high animal protein (HAP : protein-84g). The apparent absorption and balance on nitrogen were significantly higher in high protein than in low protein diet. Nitrogen, absorption rate was about 75% for low protein and about 85% for high protein intake. The mean values of nitrogen balance were -1.28% for low protein and 0.78% for high protein diet. All the subjects were in negative nitrogen balance at the low protein intake while they were in positive nitrogen balance at the high protein intake. The mean daily urinary nitrogen excretion increased with increased level of protein intake. Urea nitrogen was the largest part of the urinary nitrogen. The ratio of urea nitrogen to total urinary nitrogen increased significantly for 79 to 85% as protein intake was doubled.

The Effect of Aquatic Exercise Therapy on Balance in Patients with Chronic Stroke : Meta-Analysis of Studies in Korea (수중운동치료가 만성 뇌졸중 환자의 균형에 미치는 효과 : 국내연구의 메타분석)

  • Purpose : This meta-analysis was aimed at guiding future research in stroke treatment and to provide real-world data relating to the effects of aquatic exercise therapy on balance in patients with chronic stroke. Methods : We performed a meta-analysis comprising 22 studies involving aquatic exercise therapy performed between 2006 and 2017. A meta-analysis software program was used to calculate the mean effect size, effect size by intervention, and effect size by outcome. We also performed a meta-regression analysis and an analysis of publishing bias. Results : The mean effect size was 0.563. The effect size by outcome was observed to be the largest for the functional reach test, followed by the Berg balance scale, balance equipment, the Timed Up and Go test and one leg standing. Meta-regression analysis showed that effect size increased with an increase in the duration, number, length of exercise session. Conclusion : Aquatic exercise therapy appears to show a moderate effect on balance in patients with chronic stroke. A meta-analysis is warranted for further research to determine the effects of aquatic exercise on walking, muscle strength, and range of motion.

Analysis of Posture Sway during Quiet Standing in Elderly (정적직립자세에서 노인들의 자세동요 분석)

  • Background: The purpose of this study was to assessment of posture sway on static standing in the elderly. Methods: The participants aged 60 to 90 years, were divided into age such 60s, 70s and 80s and with a history of exercise, arthritis and hang on one's stick. Posture sway were analyzed using the computerized BPM system. Each test was repeated three times. All the data were expressed means and standard deviation by using SPSS 12.0 program. Results: The posture sway test according to sex showed that mean balance, sway number and frequency performance were significantly stable in men than in women. All direction frequency, sway area, sway path and max velocity were significantly unstable in 80s than 70s and 60s. The results of the posture sway test according to exercise group, arthritis and hang on one's stick group were showed that mean balance function was significantly stable in exercise group than arthritis and stick groups. The weight was showed significant correlation by mean balance, sway no, frequency, sway area, sway path and max velocity. Sway area and max velocity were not significant correlation by frequency of lateral and left, right anterior and posterior direction. Conclusion: In this study was showed that posture sway was effected from health condition of elderly.

Comparison of Effects of Obstacle Training in Aqua and Land on the Balance of Chronic Stroke Patients (수중과 지상에서 장애물 훈련이 만성 뇌졸중 환자의 균형에 미치는 효과 비교)

  • The purpose of this study was to comparison the effects of aqua-and-land based obstacle training on balance of chronic stroke patients. Subjects were randomly divided into an aqua group(n=15) and d land group(n=15). Both group received obstacle training for 40 minutes, 3 times a week during 12 weeks. Static balance was assessed by measuring the mean velocity of mediolateral, anteroposterior and sway area with the eyes open using Good Balance System. Dynamic balance was assessed by measuring Functional Reaching Test(FRT) and the Timed Up and Go test(TUG). Following the intervention, both groups showed significant changes static balance(the mean velocity of mediolateral, anteroposterior and sway area) and dynamic balance(FRT and TUG). There were significant difference in the mean velocity of mediolateral, anteroposterior, sway area, FRT and TUG between the two groups after the interventions. The results of this study suggest that the aqua group and land group were increase balance functions of chronic stroke patients. The aqua group was significantly higher than the land group for patients with chronic stroke patients. We hope that aquatic training can be useful for patients with chronic stroke patients to improve balance functions and the aqua training research for improve balance functions will be conducted continuously.

Neuromuscular Electrical Stimulation of Abdominal Muscles to Improve Standing Balance

  • Background: Neuromuscular electrical stimulation (NMES) is used for muscle strengthening. While voluntary muscle contraction follows Henneman et al.'s size principle, the NMES-induced muscle training disrespects the neurophysiology, which may lead to unwanted changes (i.e., declined balance ability). Objects: We examined how the balance was affected by abdominal muscle training with the NMES. Methods: Fifteen young adults (10 males and 5 females) aged between 21 and 30 received abdominal muscle strengthening with NMES for 23 minutes. Before and after the training, participants' balance was measured through one leg standing on a force plate with eyes open or closed. Outcome variables included mean distance (MDIST), root mean square distance (RDIST), total excursion (TOTEX), mean velocity (MVELO), and 95% confidence circle area (AREA) of center of pressure data. Two-way repeated measures analysis of variance was used to test if these outcome variables were associated with time (pre and post) and vision. Results: All outcome variables were not associated with time (p > 0.05). However, all outcome variables were associated with vision (p = 0.0001), and MVELO and TOTEX were 52.4% (45.5 mm/s versus 95.6 mm/s) and 52.4% (364.1 mm versus 764.5 mm) smaller, respectively, in eyes open than eyes closed (F = 55.8, p = 0.0005; F = 55.8, p = 0.0005). Furthermore, there was no interaction between time and vision (F = 0.024, p = 0.877). Conclusion: Despite the different neurophysiology of muscle contraction, abdominal muscle strengthening with NMES did not affect balance.

A Longitudinal Study on Seasonal Variations of Physical Activity and Body Composition of Rural Women (농촌여성들의 계절별 활동량과 체성분 차이에 관한 종단 연구)

  • This study was attempted to estimate seasonal variations of physical activity level, energy balance & body composition of 38 rural women, aged 31 to 67yrs in three seasons ; farming season(June), harvesting season(October), nonfarming season(February) Energy intake, energy expenditure and body composition were assessed using questionnaire, daily activity diary & bioelectrical impedance. The type of activities & the period of the spent on each activity were changed significantly with the seasons. Daily mean duration(min) of farming activity was significantly higher in June & October than in February(p<0.005) & daily mean energy expenditure for farming activity was significantly high in June & low inFebruary (p<0.005). Daily mean energy expenditure was 2892.9㎉ in June, 2487.4㎉ in October and 2130.9㎉ in February and changed significantly in three seasons(p<0.005). Daily mean energy intake was significantly higher in June(=1950.3㎉) & October (=1946.9㎉) than in February(=1423.3㎉)(p<0.005). According to RDA for koreans, the level of physical activity fell into exceptional activity category in June, heavy in October, moderate in February. Mean energy balance was negative in all seasons ; 0.682 in June, 0.812 in October and 0.698 in February. In three seasons mean body weight, mean obesity rate and mean body mass index(BMI) were not changed significantly. But there were significant seasonal variations in body fat(%) & lean body mass(LBM)(%). The mean percentage of body fat(%) was within normal range(24.44-24.79%) & the mean percentage of lean body mass(LBM)(%) representing long term physical activity was significantly higher in June(75.56%) & October(75.21%) & October (75.21%) than in February(72.75%)(p<0.05).

Analysis of Runoff Characteristics in the Geum River Basin using Watershed Management Model (유역관리모형을 이용한 금강유역 유출특성 해석)

  • To operate scientifical and integrated management of water resources, it needs to identify clearly the quantitative variation and moving pathway of water resources in a basin. Moreover, it needs to also estimate more precisely the amount of runoff generating from the precipitation. Thus, in this study, to carry out more reliable hydrologic analyses, the runoff characteristics according to detailed runoff components and water balance in a basin are analyzed. As a result of yearly water balance analyses, during the period of drought year, the loss is bigger than that of 6-year mean loss and the return flow of groundwater is the most dominant component of runoff. During the period of flood year, the loss is smaller about 4% than that of 6-year mean loss and the subsurface water is the most dominant component of runoff. The loss due to the interception and evapotranspiration for 6-year mean loss is about 53% of the total rainfall, the mean runoff ratio is about 27% and the baseflow is about 22%.

Comparison Between Stroke Patients and Normal Persons for Trunk Position Sense and It's Relation to Balance and Gait (정상인과 뇌졸중 환자의 체간 위치감각 비교 및 보행과 균형에 미치는 영향)

  • Background: Stroke patients have reduced trunk control compared to normal people. The ability to control the trunk of a stroke patient is important for gait and balance. However, there is still a lack of research methods for the characteristics of stroke control in stroke patients. Objects: The aim of this research was to determine whether trunk position sense has any relation with balance and gait. Methods: This study assessed trunk performance by measuring position sense. Trunk position sense was assessed using the David back concept to determine trunk repositioning error in 20 stroke patients and 20 healthy subjects. Four trunk movements (flexion, extension, lateral flexion, rotation) were tested for repositioning error and the measurement was carried out 6 times per move; these parameters were used to compare the mean values obtained. Subjects with stroke were also evaluated with clinical measures of balance and gait. Results: There were significant differences in trunk repositioning error between the stroke group and the control group in flexion, lateral flexion to the affected side, lateral flexion to the unaffected side, rotation to the affected side, and rotation to the unaffected side. Mean flexion error: post-stroke: 7.95 ± 6.76 degrees, control: 3.32 ± 2.27; mean lateral flexion error to the affected side: 6.13 ± 3.79, to the unaffected side: 5.32 ± 3.15, control: 3.57 ± 1.92; mean rotation error to the affected side: 8.25 ± 3.09, to the unaffected side: 9.24 ± 3.94, control: 5.41 ± 1.82. There was an only significant negative correlation between the repositioning error of lateral flexion and the Berg balance scale score to the affected side (-0.483) and to the unaffected side (-0.497). A strong correlation between balance and gait was found. Conclusion: The results of this study indicate that stroke patients exhibit greater trunk repositioning error than age-matched controls on all planes of movement except for extension. And lateral flexion has correlation with balance and gait.

Analysis of the Static Uprighting Balance in the Visual Input Characteristics (시각정보 유입 특성에 따른 정적 기립균형 분석)

  • Purpose : The current study examines changes of static uprighting balance in the visual input characteristics. Method : Total 50 person(male 16, female 34) were participated in this study. They were tested with 'hole in the card' for identification of dominant eye's side, then they were divided 3 groups(both visual input group, dominant visual input group, and non-dominant visual input group). 3 groups were measured with Romberg test on the force platform device to compare the static uprighting balance characteristics ; moving distance, mean velocity, and sway area of the CoM(center of mass), during 20 seconds. Results : The results by one-way repeated measure ANOVA were as follows. In moving distance and mean velocity of CoM, non-dominant visual input group was unstable than dominant visual group and both visual input group(p<0.05). But, in sway area of CoM, significant difference was not existed statistically. Conclusion : These result can be applied to design the static uprighting balance program using visual input mediation.
