의치상의 종류에 따른 상악 의치상의 유지력에 관한 임상적 연구 (A CLINICAL STUDY ON THE RETENTION OF MAXILLARY COMPLETE DENTURE WITH DIFFERENT DENTURE BASE MATERIALS)

  • For the successful treatment of complete denture, obtaining a good retention is essential. There are lots of factors affecting denture retention. Denture material, one of those factors affecting denture retention, was the subject of this study, and internal surface treatment also considered for the method of enhancing denture retention. Two resin(Lucitone $199^{(R)}$(heat cured resin) Vertex $CP^{(R)}$(self cured resin)) and two metal($Biosil^{(R)}$(Co-Cr alloy), $Vitallium^{(R)}$(Co-Cr alloy)) denture base materials were used for making test denture base. Newly developed device was used for measuring denture retention. After the retention was measured. We treated internal surface of test denture base with $50{\mu}m\;Al_2O_3$ powder, under 90psi, for 1 minute. Then the retention was measured again. The result was analyzed with K-S test, one-way ANOVA test and independent t-test to deter mine the significant differences as the 95% level of confidence. The results are as follows : In cases of without internal surface treatment, the retention was increased in order of $Vitallium^{(R)},\;Biosil^{(R)},\;Vertex CP^{(R)}$ and Lucitone $199^{(R)}$. Except between Vertex $CP^{(R)}$ and $Biosil^{(R)}$, retention of the other materials was significantly different (p<0.05). After the treatment of internal surface, the retention was increased in order of $Vitallium^{(R)},\;Biosil^{(R)},\;Lucno\;199^{(R)},\;Vertex\;CP^{(R)}$. Except between Lucitone $199^{(R)}$ and Vertex $CP^{(R)}$, $Vitallium^{(R)}$ and $Biosil^{(R)}$ the retention of remaining groups was significantly different each other (p<0.05). In the matter of each material, after the internal surface treatment the retention was increased with Vertex $CP^{(R)},\;Biosil^{(R)}\;and\;Vitallium^{(R)}$ and the value of differences were statistically significant. When we compare the retention of resin and metal denture base, the retention of both denture bases increased significantly with internal surface treatment, and resin denture base showed better retention. As the results show, selecting denture base material could be an important choice of complete denture treatment. To increase denture retention, internal surface treatment can be considered as a possible method.

  • Statement of problem : Fracture and dimensional change of an acrylic resin denture are a rather common occurrence. Purpose : The purpose of this study was to compare differences in dimensional changes and flexural strength of separate maxillary complete dentures after immediate deflasking by injection molding and conventional compression processing. Material and method: To evaluate dimensional stability, the maxillary dentures were fabricated by using different materials and methods. Lucitone 199(Dentsply Trubyte. york, pennsylvania, USA) and Vertex(Dentimex, zeist, Netherlands) were used as materials. Compression and injection packing methods were used as processing methods. The impression surface of the dentures was measured by 3D Scann-ing System(PERCEPTRON USA) and overlapped original impression surface of the master cast. To evaluate flexural strength, resin specimens were made according to the different materials, powder/liquid ratio and processing methods. Flexural strength of the complete resin specimens (64mm$\times$10mm$\times$3.3mm) were measured by INSTRON 4467. (INSTRON, England) The data was analyzed by ANOVA, t-test and Tukey test. (p<.05 level of significance) Result: The results were as follows 1. There was no significant differences between master model and denture base for each group in overall dimensional changes. 2. Palatal area was more stable than flange or alveolar area in dimensional stability. but. there was no significant differences among each area. 3. Materials and power/liquid ratio had an effect on flexural strength. (P<.05) Especially materials was most effective. (P<.05) 4. Lucitone 199(powder/liquid ratio followed by manufacturer's direction) showed higher flexural strength than Vertex. Conclusion : Dimensional stability or flexural strength are affected by materials rather than packing techniques.

기존 임시의치와 디지털 안궁이전을 이용하여 상하악 CAD-CAM 총의치 수복 증례 (Fabrication of CAD-CAM complete denture using existing provisional denture and digital facebow transfer)

  • 본 증례는 요양병원에 입원치료 중인 환자에서 치과에 내원하기 힘든 점과 의치 분실이 많은 점을 고려하여 내원 횟수를 감소시키고 재제작이 쉬운 CAD-CAM 총의치를 제작하였다. 환자가 적응한 기존 임시의치의 형태 및 수직고경, 악간관계를 최종의치 제작에 반영하기 위해 기존 임시의치를 스캔하고 프린팅한 개인트레이로 폐구인상을 채득하였다. UTS CAD를 이용하여 디지털 안궁이전하고 CAD 소프트웨어에서 치아배열 후 상하악 총의치를 단일 구조의 디스크로 의치상과 인공치를 한번에 절삭하여 제작하였다.

Considerations for the Survival of Complete Arch Implant-Supported Zirconia Restorations; Status of Antagonistic Arches and Stress Distribution on Frameworks: A Case Report

  • This report describes two cases of complete arch implant-supported restorations. The first patient had seven dental implants in each arch with monolithic zirconia frameworks. At four weeks' follow-up, the one-piece maxillary framework was fractured, which was re-designed and re-fabricated using laser-sintered cobalt-chrome alloy. The second patient had four implants in the mandible only. A mandibular monolithic zirconia framework and a maxillary conventional complete denture were fabricated and delivered. At five years' follow-up, the patient reported no significant discomfort. Careful consideration and monitoring of the status of antagonistic arches and stress distribution on zirconia frameworks were suggested for complete arch implant-supported fixed restorations.

Management of flabby ridges using liquid supported denture: a case report

  • Flabby ridges commonly occur in edentulous patients. Inadequate retention and stability of a complete denture are the often encountered problems in these patients. A liquid supported denture due to its flexible tissue surface allows better distribution of stress and hence provides an alternate treatment modality in such cases. This case report presents the use of a liquid supported denture in a patient with completely edentulous maxillary arch with flabby tissue in anterior region opposing a partially edentulous mandibular arch.

A Study on the Accuracy of the record base of the Complete Denture to the Master Cast according to Kinds of Resin and Polymerization Method

  • Statement of the problem. The record base in fabricating procedures of the complete denture, as a temporary form for reproducing denture base, is used to record upper and lower jaw relation and to align artificial teeth and try-in it in the mouth. The accuracy of jaw relation record is affected by the accuracy, stiffness and stability of the record base. So, the accuracy of record base is the most important requirements of jaw relation records. Purpose of study. The purpose of this study was to evaluate the gap that occurred over the palatal area of a maxillary record base fabricated with autopolymerizing resin and light-curing resin. Methods/material. The maxillary record bases were fabricated out of autopolymerizing resin that is used the most frequently in clinics and light-curing resin that attracts special attention for its several merits. The light-curing resin was made by two kinds of polymerization methods, which are one step curing method and multiple step curing method. All record bases were cut in certain positions of the master cast 1 hour and 1 day later after fabrication and the accuracy of the master cast was measured and analyzed with a microscope. Results. A pattern of gap formation between the record base and the maxillary cast was observed in all specimens. According to kinds of resins, autopolymerizing resin was significantly more accurate than light-curing resin. There was no statistical difference according to time lapse, and in all three groups, the maximum discrepancy occurred at the posterior border in the mid-palatal region. Conclusion. The autopolymerizing resin is better than light-curing resin, and multiple step curing method is more accurate than one step curing method when using light-curing resin.

총의치 사용에 대한 환자와 술자간 평가 비교 (Comparison between denture wearer's evaluation and clinician's rating for complete denture)

  • 목적: 총의치를 사용하고 있는 무치악 환자에서 의치 사용에 대한 환자의 평가와 술자간의 평가를 비교하고자 하였다. 대상 및 방법: 의치를 사용한지 최소 1년 이상 경과된 총의치 장착자 중 43명의 환자가 정기검진에 응하였다. 환자에 관한 여러 정보를 얻기 위해 다양한 문헌에서 얻은 자료를 토대로 설문지를 개발하고 술자의 객관적인 평가간 일치도를 분석하였다. 술자의 평가 사항으로는 유지, 안정, 교합과 같은 기능적 항목과 의치의 상태를 포함한다. Friedman test와 Cohen's Kappa값을 이용하여 통계학적인 분석을 시행하였다. 결과: 의치 장착자의 평가와 술자의 평가 간에는 약하거나 약간의 차이가 있다. 상악 의치보다는 하악의치에서 차이가 더 크게 나타났으며 심미와 저작은 중등도의 차이가 있었다. 결론: 의치에 대한 환자의 주관적, 술자의 객관적인 평가는 일치하지 않으나 큰 차이를 보이지 않는다.

  • 구강안면 근긴장이상(Orofacial dystonia)은 자신의 의지와 상관없이 불수의적으로 얼굴, 입술, 혀 및 턱 주변을 불규칙적이거나 반복적으로 움직이는 신경운동학적 장애로 틱장애로 불리기도 한다. 이러한 증상을 갖는 무치악 환자는 총의치 사용의 어려움과 더불어 언어 장애 및 연하곤란, 구강안면의 통증까지 기능적이고 심미적인 문제점들을 겪게 된다. 본 증례에서는 비교적 젊은 나이에 완전 무치악 상태를 경험하게 된 구강안면 근긴장이상 환자를 위해, 양측 구치부 지르코니아 교합면을 갖는 상악 총의치와 하악 임플란트 지지 고정성 보철물의 수복치료를 시행하고 지속적인 미소훈련을 시행하여 안면근육의 심미적인 개선을 도모하고자 하였다. 치료의 결과, 환자는 저작기능과 심미성이 향상된 것 뿐만 아니라, 심리적 안정감을 회복하여 정상적인 일상생활이 가능해진 것에 대해 매우 만족하였기에 이를 보고하고자 한다.

Accuracy and time efficiency of conventional and digital outlining of extensions of denture foundation on preliminary casts

  • PURPOSE. The purpose of this diagnostic study was to assess the accuracy and time efficiency of a digital method to draw the denture foundation extension outline on preliminary casts compared with the conventional technique. MATERIALS AND METHODS. A total of 28 preliminary edentulous casts with no anatomical landmarks were digitized using a laboratory scanner. The outlining of the entire basal seat of the denture was performed on preliminary casts and digitized. Casts with no extension outline were digitized and outlines were drawn using software (DWOS, Straumann). The accuracy of the extension outlined between both techniques was evaluated in the software (GOM Inspect; GOM GmbH) by file superimposition. Specificity and sensitivity tests were applied to measure accuracy. The paired t-test (95% CI) was used to compare the mean total area and the working time. RESULTS. The accuracy ranged from 0.57 to 0.92. The buccal and labial frenulum showed a lower value in the maxilla (0.57); while the area between the retromolar pad and buccal frenulum (0.64) showed a lower score in the mandible. The maxillary denture foundation and the working time for both arches were significantly longer for the digital method (P < .001). CONCLUSION. The denture foundation extension outline exhibited a sufficiently excellent accuracy for the digital method, except for the maxillary anterior region. However, the digital method required a longer working time.

후경화에 따른 3차원 프린팅 의치의 체적변화 (Evaluation of the dimensional change of 3D-printed complete denture after post-curing)

  • 목적: 이 연구목적은 3차원 프린팅의치의 후경화후로 인한 의치의 체적변화를 평가하고자하였다. 연구 재료 및 방법: 상·하악 의치를 Exocad DentalCaD 프로그램을 이용하여 디자인하였으며, STL 파일로 추출하였다. DLP 방식의 치과용 3차원 플린터를 이용하여 상·하악 의치 STL파일을 상·하악 각기 10개씩 출력하였다. 출력된 상·하악 의치는 세척하였고, 후경화를 시행하기전과 후에 각 각 모형스캐너를 이용하여 스캔파일을 만들었다. 후경화 전후의 스캔된 의치파일을 중첩하여 변화를 평가하였으며, 의치상의 계측점사이의 거리를 측정하였다. 통계는 SPSS를 이용하였으며, 유의수준은 5%였다. 결과: 상악의치는 후경화 후에 전반적으로 수축하였으며, 후구개부위에서 가장 뚜렷한 변화를 보였다. 상악의치 전후방 (A-D, A-E, A-F)의 길이와 양측제1대구치사이(B-C)의 길이 감소가 통계학적으로 유의성을 보였다. 하악의치는 후경화 후에 전후방의 길이변화보다는 폭경의 변화가 비교적 뚜렷하였는데, 의치후방(A-M, A-D, A-E, A-L, A-H, A-I, H-I)과 설측(J-K, L-M)에서 통계학적으로 유의성있는 길이변화가 관찰되었다. 결론: 이 실험의 결과내에서, 3차원 프린터로 출력되는 상·하악 의치는 후경화 후에 길이와 폭이 모두 변화하였다. 결론적으로, 의치의 변형을 최소화할 수 있는 후경화 방법과 재료의 개발이 필요할 것으로 사료된다.
