Short-term improvement of masticatory function after implant restoration

  • Purpose: Dental implants present several advantages over other tooth replacement options. However, there has been little research on masticatory function in relation to implant treatment. Therefore, the aim of the present study was to evaluate the improvement of masticatory function two weeks after implant restoration. Methods: Masticatory ability was evaluated with the subjective food intake ability (FIA) and objective mixing ability index (MAI) methods. Fifty-four subjects with first and second missing molars completed the study. The subjects were asked to complete a self-reported questionnaire about 30 different food items, and to chew wax samples 10 times both before and two weeks after implant restoration. A total of 108 waxes were analyzed with an image analysis program. Results: Dental implant restoration for lost molar teeth on one side increased the FIA score by 9.0% (P<0.0001). The MAI score also increased, by 14.3% after implant restoration (P<0.0001). Comparison between the good and poor mastication groups, which were subdivided based on the median MAI score before implant restoration, showed that the FIA score of the poor group was enhanced 1.1-fold while its MAI score was enhanced 2.0-fold two weeks after an implant surgery. Conclusions: Using the FIA and MAI assessment methods, this study showed that masticatory function was improved two weeks after implant restoration. In particular, the enhancement of masticatory function by implant restoration was greater in patients with relatively poor initial mastication than in those with good initial mastication.

  • The purpose of this study was to standardized and classify the coordination pattern among the chewing side and non-chewing side masseter and anterior muscles, in terms of EMG values on lateral excursion, clenching, and mastication in presence of the non-chewing side. In this study, 25 subjects were selected for experiment of lateral excursion, clenching and mastication and EMG value of the masseter and anterior temporal muscle on both sides were recored 2 times respectively. The bioelectric processor model EM2(Myo-tonic research, INC. U.S.A.) with the surface electrodes were used to record the EMG activity during all experimental procedures. The results were as follows : 1. During lateral excursion on intereference of non-chewing side, the EMG values of the temporal muscle were significantly more prominent than those of the temporal muscle on the non-chewing side. The EMG values of non-chewing side were significantly more prominent than those of chewing side on the both side masseter muscle and those of chewing side were significantly more prominent than those of non-chewing side on the both side temporal muscle. 2. During clenching on the occlusal interferance, the EMG values of non-chewing side masseter muscle were most prominent. 3. During mastication on the occlusal interferance, the EMG values of the chewing side temporal muscle were most prominent and those of non-chewing side temporal muscle were the lowest. 4. The EMG values of temporal muscle of non-chewing side on interferance were significantly more prominent than those of canine guidance during lateral excursion. 5. During clenching on the occlusal interferance, the EMG values of the masseter and the temporal muscles of the non-chewing side were significantly more prominent than canine guidance, but those of chewing side temporal muscle on canine guidance were significantly more prominent than those of interferance. 6. During mastication on canine guidance, the EMG Values of the temporal muscle on the chewing side, the masseter muscle on the chewing side and the temporal muscle on the non-chewing side were more prominent than those of interferance, but temporal muscle of non-chewing side was not different between canine guidance and occlusal interferance on non-chewing side.

노인들의 의치 사용이 MMSE-K 점수에 미치는 영향 (Influence of denture wearing on a mini-mental state examination(MMSE-K) in the elderly)

  • Objectives: The purpose of the study was to investigate the association between denture wearing and dementia using a Mini-Mental State Examination (MMSE-K) in the community dwelling elderly. Methods: The study subjects were 184 community dwelling elderly using Korean version of mini-mental state examination(MMSE-K). The variables included the general characteristics of the subjects, denture earing, number of mastication tooth, subjective mastication ability, and MMSE(dementia). Denture wearing was divided into two groups of the natural dentition including the fixed prosthesis and removable denture including the complete and/or partial denture. The data were analyzed by ${\chi}^2$ test, t-test, and binary logistic regression using SPSS version 22.0 program. Logistic regression was used to analyze the association of MMSE-K score and denture wearing, and 95% confidence interval was calculated. The subjective mastication ability was measured by Likert 5 points scale. Dementia was measured by MMSE-K. Results: There was a significant association between MMSE-K score and denture wearing. The odds ratio(OR) of denture waering was 6.01(95% CI: 2.824-12.784). After adjusting the age, gender, residence and education, OR was 5.53(95% CI: 2.364-12.935). Conclusions: This study showed a significant association between MMSE-K score and denture wearing in the Korean elderly.

노인요양시설 노인들의 건강상태, 일상생활능력, 수면상태와 우울의 관련성 (Relationship Among the Health State, Daily Living Activities (ADL, IADL), Sleep State, and Depression Among Old People at Elderly Care Facilities)

  • 본 연구는 노인요양시설 노인들의 우울과 건강상태, 일상생활수행능력, 수면상태의 관련성을 파악하고자 실시하였다. 연구대상자는 노인요양시설에서 거주하는 만 65세 이상의 노인을 대상으로 하였다. 노인요양시설 노인들의 우울 관련성은 시력상태, 치아상태, 수면의 질, 수면상태 등과 관련이 있었으며, 시력상태가 나쁜 군, 치아상태가 나쁜 군, 수면의 질이 나쁜 군, 수면장애가 있는 군에서 높게 나타났다. 우울과의 상관관계에서는 우울과 연령과는 음의 상관관계로 나타났고, 치아상태, 수면의 질 등이 양의 상관관계가 있는 것으로 나타났다. 노인요양시설 노인들의 우울에 영향을 미치는 요인으로 시력상태, 치아상태, 수면상태, 청력상태 등이 관련성이 높게 나타났다. 결론적으로, 노인요양시설 노인들의 우울과의 관련성은 개인이 인지하는 건강상태와 관련이 높으며, 건강관리와 간호중재도 개인별 건강상태에 세심한 관심과 시력상태, 치아상태가 낮은 노인관리가 중요하다고 하겠다.

Texture Characteristics of Horse Meat for the Elderly Based on the Enzyme Treatment

  • Horse meat is nutritionally adequate to the elderly, but it has a comparatively hard texture in contrast to most of the food. In practice, the meat intake in the elderly is generally bated because the relatively difficult texture of the meat can diminish mastication. Thus, strategies are being developed to produce meat products remanding detracted mastication exertion and possibly exalt ingestion and nutritional stand, in the elderly. Hence, the effects of enzymes on textural characteristics of horse meat were studied, because they have well-known favorable efficacy on the meat tenderness by causing important demotion of the myo-fibrillar protein and collagen. Four treatments namely, papain, bromelin, pepsin, and pancreatin, alongside one control were invoked to the horse meat. Their effects on the texture parameters were determined. All the above enzymatic treatments significantly reduced hardness and resilience (p<0.001). These results present opportunities to produce essential fatty acids fortified horse meat with soft texture and satisfied technological characteristics. The intake of the essential fatty acids intensified horse meat could aid the elderly to get their aimed essential fatty acid demands. Results also suggest that horse meat tenderized through enzymatic processing stand for auspicious options for the comprehension of texture-revised diets in the elderly population.

임플란트 선택동기 및 인지도 조사연구 -울산지역 성인 남, 여를 대상으로- (An Investigation of a Motive for Dental Implant Treatment in Clinical Cases)

  • This study was to find a patient's motive for receiving implant treatment by age and gender using a self-reporting questionnaire survey with adults(men & women) in Woolsan. The result was as follows: 1. As a result of questionnaire survey with 155 patients in total(86 men, 69 women), 24 of 86 men(27.9%) valued the natural mastication feature of dental implant most, that is as similar as natural teeth, while 18 of 69 women(26.1%) had the most priority over the longer life of implant than common dental prostheses. By age, those in their 20s and 30s had a preference to implant treatment because it does not need to pulling out teeth for prostheses, while those in their 40s and over had a priority to the natural mastication feature of implant that is similar as much as natural teeth. By dental treatment, all of respondents said that the natural mastication feature is the most important in getting dental implant. It was founded that whether one can taste food as it is or not is less important, regardless of age or gender. 2. The greatest obstacle to implant surgery was expensive medical fees, 88 of 155 respondents(56.8%), with a fear of surgery itself being the least obstacle. 3. 90.2% of the total respondents said that they will want to receive dental implant treatment if their economy allow.

정상 성인 여성에 있어 실험적 견치부 등척성편측교합에 의한 동통성 증상 발현 현상 (A Feature of Producing the Symptom Complex Related to Pain and Discomfort by the Experimental Isometric Unilateral Canine Biting in Adult Females within Normal Masticatory Function)

  • In order to evaluate the effect of unilateral tooth clenching, the author observed a feature of producing the symptom complex related to pain and discomfort by the isometric unilateral canine biting under force of 5kg(FS) in Korean 31 adult females within normal masticatory function who were divided into the bilateral, the right and the left group according to their habitual sides of mastication, and analyzed the observed data statistically. The obtained results were as follows: 1. The duration of the isometric unilateral canine biting maintained before the occurrence of symptom complex related to pain and discomfort 66.72 seconds at right side and 39.50 seconds at left side in a group of subjects with bilateral habitual mastication. And the difference of the duration between of unilateral biting side was almost significant (P<0.05). 2. The most frequent region of occurrence of symptom complex related to pain and discomfort by the isometric unilateral canine biting was the contralateral superficial masseter muscle in a group of subjects with bilateral habitual mastication.

Immediate Effects of Asymmetric Chewing on Temporomandibular Joint Kinematics

  • Purpose: This study investigated the immediate biomechanical effects of unilateral mastication for 10 minutes on the temporomandibular joint (TMJ) with 21 healthy adult participants. Methods: The gum group chewed gum on the right side for 10 minutes, and the control group rested for 10 minutes. Biomechanical data were obtained using a three-dimensional infrared camera before and after intervention. An independent t-test assessed the variation of kinematic data to identify differences between before and after intervention. Results: Among biomechanical variables, the gum group's length of the left forehead middle region and the temporomandibular joint angle decreased compared to the control group (p<0.05). Conclusion: Caution with unilateral masticatory activity is recommended, as unilateral mastication causes biomechanical changes due to excessive load on the soft tissues of the contralateral TMJ.

  • Many concept of occlusion have been introduced in the past, but there isn't any generally accepted unified theory since a single occlusion theory cannot represent the natural perfection, especially considering the difference in clinical experience, the dynamic occlusion, and individual diversity in mastication pattern. NAT(Naturgemasse Aufwachstechnik)/NFR((Natur Funktionsgerechte Rekonstruktion) occlusion is not just a theory on occlusion, but a clinical occlusion that can be practically applied to clinical practice by providing dynamic occlusion adjusted to the individual mastication pattern and biomechanics. This report aims to provide a guide to a clinically biomechanical occlusion through the teamwork of both the dentist and the lab technician by introducing the NAT occlusion through following: reviewing the concept of occlusion in terms of morphology, its correlation with NAT/NFR occlusion, reviewing the limitation of conventional occlusion theories, and introducing the clinical application of NAT/NFR.

측두하악장애 화낮의 교합음 지속시간에 관한 분석 (An Analysis of Duration of Occlusal Sounds in TMD Patients)

  • The author has measured the duration of occlusal sounds during centric occlusal tapping in 30 patients with TMD and 30 normal subjects as controls, and then correlated the duration of occlusal sounds with CMI, amount of centric slide, midline deviation, habitual side of mastication, and presence of occlusal interference. The results were as follows 1. The duration of occlusal sounds during centric occlusal tapping were 36.26 msec in TMD patients and 22.8 msec in normal subjects (p<0.01). 2. The correlation between duration of occlusal sounds and CMI was significant (p<0.01). 3. The correlations between duration of occlusal sounds with amount of centric slide, midline deviation, habitual side of mastication, and presence of occlusal interference were not significant (p<0.05).
