The Impact of Manual Therapy on Pain Catastrophizing in Chronic Pain Conditions: A Systematic Review and Meta-analysis

  • Objective: Manual therapy is a commonly utilized approach in managing chronic pain, but its specific impact on pain catastrophizing remains uncertain. The objective of this systematic review and meta-analysis was to examine the effects of manual therapy on pain catastrophizing in individuals with chronic pain. Design: A systematic review and meta-analysis Methods: A comprehensive search was conducted in electronic databases to identify relevant studies published from 2014 onwards. Studies that evaluated the impact of manual therapy on pain catastrophizing in individuals with chronic pain were incorporated. The risk of bias in the selected studies was evaluated using the Cochrane tool for risk of bias in qualitative analysis. For the quantitative analysis, RevMan 5.4 software was utilized, employing a random-effects model as the analysis model. The effect measure used in the analysis was the standardized mean difference (SMD). Results: In total, 26 studies were collected, and following the screening process, three of them were incorporated into the final analysis. The included studies involved a total of 153 patients with chronic pain. The interventions comprised various manual therapy techniques targeting different areas of the body. Pain catastrophizing and pain intensity were the primary outcomes of interest. The meta-analysis revealed a significant reduction in pain catastrophizing scores following manual therapy intervention compared to control conditions (SMD = -0.91, 95% CI: -1.25 to -0.58). However, heterogeneity between the studies was observed. Conclusions: Despite the limited quantity and heterogeneity of studies, it has been demonstrated that manual therapy intervention is effective in reducing pain catastrophizing in individuals with chronic pain.

  • Objectives : The present study introduces the clinical application of Hamstring Chuna Manual Therapy to a patient who underwent the pain after spinal fusion. Methods : A patient who had the pain after spinal fusion was hospitalized at Bucheon Jaseng hospital of Oriental Medicine for 35 days. During the hospital treatment, the pain was relieved by Hamstring Chuna Manual Therapy. The improvement of the patient was measured by VAS(Visual Analogue Scale) score, SLR(Straight-Leg Raising) test, and walking distance. Results : The values of the patient's VAS score, SLR test, and walking distance measured before and after Hamstring Chuna Manual Therapy presented that the pain was relieved after Hamstring Chuna Manual Therapy. Conclusions : Therefore, this case demonstrates that Hamstring Chuna Manual Therapy is effective treatment method for relieving pain in the spine.

  • Objectives: To evaluate the evidence supporting the effectiveness of Chuna manual therapy for rheumatoid arthritis. Methods: We conducted a search across 9 electronic databases to find all randomized controlled clinical trials (RCTs) that used Chuna manual therapy as a treatment for rheumatoid arthritis. The methodological quality of each RCT was assessed using the Cochrane risk of bias tool. Results: Our inclusion criteria were met by 5 RCTs. The meta-analysis showed positive results for the use of Chuna manual therapy combined with oriental usual care (UC) in terms of the efficacy rate, pain, and duration of morning stiffness when compared to western UC. Positive results were also obtained in terms of the efficacy rate, when Chuna manual therapy combined with medication was compared to medication treatments alone. Conclusions: Our systematic review found encouraging, but limited evidence of Chuna manual therapy for rheumatoid arthritis. However, to obtain stronger evidence without the drawbacks of trial design and the quality of studies, we recommend a comparative research to test the effectiveness of Chuna manual therapy.

  • Objectives This study aimed to evaluate the effectiveness of manual therapy on constipation in patients with Parkinson's disease. Methods We searched electronic databases (PubMed, EMBASE, Cochrane Library, CAJ, KISS, KISTI, KMBASE, NDSL, OASIS, and RISS) for randomized controlled trials for manual therapy on constipation in patients with Parkinson's disease. Results Three randomized controlled trials were selected from the online database. One study demonstrated that manual therapy had positive results compared to routine nursing care with western medication. However, two studies demonstrated that manual therapy had no positive results compared to routine nursing care with western medication or western medication when used alone. Conclusions Our systematic review did not verify the clinical effect of manual therapy on constipation in patients with Parkinson's disease. Thus, more high-quality studies are needed to verify the effectiveness of manual therapy for constipation in patients with Parkinson's disease.

  • PURPOSE; The main purpose of this study is to develop a new learning model of Orthopedic Manual Therapy. METHOD; From 1997 to 1999, this survey was made of 318 students who had attended in Orthopedic Manual Therapy subject. RESULT; Results show that regard satisfaction with lecture was as 'good' by 235 person(73.3%). And the most interest subject among the therapists was orthopedic manual therapy(44.63%), next NDT(25.62%). CONCLUSION; This study suggest a new learning model of orthopedic manual therapy used by Role-play model and Coner study model. So we can be lecture effectively to focus students.

만성 역학적 목 통증을 가진 환자에게 도수치료만 적용할 때와 도수치료와 안정화운동을 함께 적용할 때 목 통증과 신체기능에 미치는 효과 비교 (A Comparison of The Effects of Manual Therapy Plus Stabilization Exercise with Manual Therapy Alone in Patients with Chronic Mechanical Neck Pain)

  • PURPOSE: This study aimed to compare the effects of manual therapy with stabilization exercises to manual therapy alone, on neck pain and body functions in patients with chronic mechanical neck pain. METHODS: Twenty patients with chronic mechanical neck pain were recruited and randomly allocated into two groups. A control group(n = 10) was given the manual therapy alone and an experimental group(n = 10) was given the manual therapy with stabilization exercises. The intervention was carried out 3 days per week for 4 weeks. The cervical resting pain, the most painful motion pain, craniocervical flexor endurance, forward head posture and neck disability index were used to assess participants at baseline and after 4 weeks. RESULTS: A comparison of the parameters before and after the intervention showed that both groups experienced significant improvements in the resting pain, the most painful motion pain, craniocervical flexor endurance, and forward head posture except for the forward head posture in the control group. A comparison of the parameters between the groups did not show a significant difference. CONCLUSION: The results of this study suggest that the combined intervention of manual therapy with stabilization exercise does not seem to be more effective than manual therapy alone for improving neck pain, craniocervical flexor endurance, forward head posture, and the neck disability index in patients with chronic mechanical neck pain.

근건이완수기요법에 대한 Scoping Review 및 임상활용 방향 제언 (A Scoping Review of Musculotendinous Releasing Manual Therapy and Suggestions for Clinical Application)

  • Objectives: The purpose of this study was to review the studies of musculotendinous releasing manual therapy and suggestions for clinical application. Methods: Derive research questions using the scoping review methodology. Searching for related studies, the studies were selected, and the data were analyzed and reported. We searched domestic database for studies on musculotendinous releasing manual therapy reported till April 2023. We reviewed 82 studies excluding duplicate studies and 13 studies were selected. Results: Five studies were literature studies and eight studies were clinical studies. Among the five literature studies, two studies investigating and analyzing general manual therapy, one study investigating manual therapy applied to frozen shoulders, and one systematic literature review to investigate the economic effects of manual therapy, one study using tools. There was one study examining research trends in manual therapy. Eight clinical studies were conducted on patients with Achilles tendinitis, tennis elbow, meralgia paresthetica and obesity. Conclusions: Musculotendinous releasing manual therapy is thought to be applicable not only to musculoskeletal disease but also to changes in body shape due to connective tissue.

Effects of Home Exercise Program and Manual Therapy on Shoulder Function and Quality of Life in Patients with Adhesive Capsulitis

  • Purpose: This study was conducted to identify the effects of home exercise programs and manual therapy on shoulder function and quality of life in patients with adhesive capsulitis. Methods: A total of 31 patients were enrolled in the study. All subjects were randomly assigned to a home exercise group (n=15) or a manual therapy group (n=16). Both groups performed each intervention program three times a week for four weeks. After four weeks, both groups performed a home exercise program continuously until week 12. Shoulder function was evaluated using the Korean Shoulder Scoring (KSS) system, while quality of life was evaluated using the WHO Quality of Life-BREF (WHOQOL-BREF) before and 4, 8, and 12 weeks after the intervention. Data were analyzed using two-way repeated measures analysis of variance (ANOVA). Results: There were significant differences in KSS before intervention and 4, 8, and 12 weeks after intervention for both the home exercise group and manual therapy group (p<0.001). There was also a significant difference in KSS at 4, 8, and 12 weeks when compared to the home exercise group and manual therapy group (p<0.001). Moreover, there was a significant difference in WHOQOL-BREF before and 4, 8, and 12 weeks after intervention in the home exercise group and manual therapy group (p<0.001). Conclusion: Home exercise and manual therapy improved shoulder function, but manual therapy led to a greater improvement in shoulder function better than home exercise. Home exercise and manual therapy improved quality of life, but there was no significant difference between groups.

종아리 근경련 환자에 대한 도수치료 사례 연구 (Case Study; Manual Therapy for Patients with Calf Cramps)

  • Purpose: The first purpose of this study was to determine the effect of reciprocal inhibition that influence changes in pain when applied to patients with culf cramps. The second purpose of this study was to determine the effect of manual therapy on iliaosacral joint that influence changes number of occurrences when applied to patients with frequent nocturnal culf cramps. Methods: The first study using reciprocal inhibition techniques to 20 patients with calf cramps and saw the change in pain. The second study using manual therapy on iliaosacral joint to 2 patients with frequent nocturnal culf cramps and saw the change in number of occurrences during the week. Results: The pain and number of occurrences were significantly difference between pre-treatment and post treatment. Conclusion: Based on the results of this study, we found that reciprocal inhibition decreased pain and that manual therapy on iliaosacral joint decreased number of occurrences.

  • The purpose of this study was to investigate useful manual therapy techniques for peripheral facial nerve palsy and to propose guidelines to be applied for current manual therapy techniques. Several databases were searched to find manual therapies for facial palsy. These therapies included cervical, and temporomandibular joint chuna manual therapy, proprioceptive neuromuscular facilitation, neuromuscular re-education, facial exercise, and mime therapy. Both cervical, and temporomandibular joint chuna manual therapy release nerve compression, helping blood circulation and nerve conduction. Proprioceptive neuromuscular facilitation uses irradiation, bilateral activation, and eccentric facilitation to improve muscle power and symmetry. Neuromuscular re-education, as a retraining tool for facial movement patterns, enhances neuromuscular feedback. Facial exercise helps the patient continuously move and massage facial muscle themselves. Mime therapy aims to develop a conscious connection between the use of certain muscles and facial expressions. The use of facial chuna manual therapy for peripheral facial nerve palsy can stimulate the proprioceptive neuromuscular receptors in the face. Peripheral facial nerve palsy has 4 phases; progress phase, plateau phase, recovery phase, and sequelae phase. Each phase needs different treatments which include relaxation, assistance, resistance, origin-insertion extension, and nerve pathway expansion.
