Internal derangement of the temporomandibular joint can be defined an abnormal relationships of the meniscus relative to the mandibular condyle, articuar fossa and eminence. This may cause variable mandibular dysfunctions and pain. For diagnosis, arthrography, computed tomography and magnetic resonance imaging are used. In this study, the author reviewed 98 TMJs of 88 patients who were diagnosed as internal derangement througth inferior joint space arthrography at the department of Oral & Maxillofacial Radiology, Dental Hospita, Dankook university through 1986 to 1992. 98 TMJs consisting of 30 disc displcement with reduction, 48 disc displcement without reduction and 20 perforation were studied about clinical and radiological findings. The results were as follows: 1. Internal derangement was found most frequently in the 2nd 3rd decades and the average age of perforation was higher than that of disc displcement with higher than that of disc displcement with reduction. The sexual predilection was 2 times hiher in females. 2. The most frequent chief complaints were TMJ sound in disc displcement with reduction, pain and limitation of mouth opening in disc displcement without reduction and pain in perforation. The duration of the chief complaints was longer in disc displcement with reduction with than in preforation and disc displcement without reduction. 3. Reciprocal click was the most frequently TMJ sound in disc displcement with reduction. History of joint sound in disc displcement without reduction an crepitus in perforation was the most frequent one. 4. The average maximum opening was 45.4㎜ in disc displcement with reduction, 31.4㎜ in disc displcement without reduction and 33.8㎜ in perforation. 5. In the centric occlusion, posterior condylar position was the most frequent in disc displcement with reduction. posterior and concentric condylar position was frequent in disc displcement without reduction, concentric and anterior condylar position in perforation. At 1 inch opening, the same position to articular eminence was most frequently found in disc displcement with reduction, posterior position in disc disp1cement without reduction, posterior and nterior position in perforation was frequently found. 6. Bony changes, especially sclerosis and flattening, was most frequently found in perforation.

Objective: In this study, we aimed to examine the relationship between chin deviation and the positional and morphological features of the mandible and to determine the factors that contributed to chin deviation in individuals with a unilateral cleft lip and palate (UCLP). Methods: Cone-beam computed tomography (CBCT) images of 28 adults with UCLP were analyzed in this study. Segmented three-dimensional temporomandibular fossa and mandible images were reconstructed, and angular, linear, and volumetric parameters were measured. Results: For all 28 individuals, the chin was found to deviate to the cleft side by 1.59 mm. Moreover, among these 28 individuals, only 7 showed distinct (more than 4 mm) chin deviation, which was toward the cleft side. Compared to the non-cleft side, the mandibular body length, frontal ramal inclination, and vertical position of the condyle were lower and inclination of the temporomandibular fossa was steeper on the cleft side. Furthermore, the differences in inclination of the temporomandibular fossa, mandibular body length, ramus length, and condylar volume ratio (non-deviated/deviated) were positively correlated with chin deviation. Conclusions: UCLP individuals show mild chin deviation to the cleft side. Statistical differences were noted in the parameters that represented positional and morphological asymmetries of the mandible and temporomandibular fossa; however, these differences were too small to indicate clinical significance.

The prosthodontic treatment of Class II division 2 malocclusions is challenging. Ideally, these malocclusions should be identified at an early age and corrected with orthodontic treatment; otherwise, the individual develops a habitual position characterized by deep overbite and significant retruded position of mandibular condyle at the TMjoint fossa. This article describes a clinical protocol for the occlusal rehabilitation of patients with Class II div.2 malocclusions. Within this protocol, an occlusal splint was used to locate the most suitable maxillary-mandibular relationship for function and range of motion. The splint increased the vertical dimension and reduced pain on TMjoints. After transfer this relationship to an articulator for fabrication of provisional restorations, the CR position and centric prematurity contact between maxilla and mandible was used to determine the tentative vertical dimension of occlusion(VDO). The amount of elevation of VDO was decided on the articulated model. The provisional restorations were accurately transfered to a patient's mouth in clinical procedures using tattoo points. The final restoration was delivered after some trial periods with provisional restorations. The theory behind this protocol and its associated clinical procedures is presented along with a discussion.
Temporomandibular ankylosis is defined as a situation in which the condyle is fused to the fossa by bone or fibrous tissue. Conditons such as trauma, infection, or systemic disease may predispose to various types of ankylosis, bringing about different levels of limitation in mandibular movement. Most patients with temporomandibular ankylosis are associated with limitation of maximal mouth opening, deviation of the chin toward the affected side, impaired occlusion, chronic pain, compromised oral hygiene, severe facial asymmetry & impeded mandibular molar eruption occurring in childhood. Several techniques to release ankylosis have been described in the literature, showing variable and often unsatisfactory results. The most frequently used operations are gap arthroplasty, interpositional arthroplasty, and exicision and joint reconstruction with autogenous or alloplastic materials. We have managed the two patients of TMJ ankylosis. They had previously TMJ surgery and we treated with gap arthroplasty & active physial therapy. We have obtained favorable results and report these cases with literatures review.

Pigmented villonodular synovitis (PVNS) is a benign but locally aggressive and destructive disease originating in the synovial membranes. It is a proliferative disorder of unknown etiology. Involvement of the temporomandibular joint (TMJ) is very rare. Computed tomography clearly reveals areas of lytic bone erosion and sclerosis, and also clearly defines the extent of the tumor which is the focal areas of hyperdensity within the soft-tissue mass. Magnetic resonance images invariably show profound hypointensity on both T1- and T2-weighted sequences due to hemosiderin pigmentation. Additionally, high signal intensity on T2-weighted images may indicate cystic loculation of the joint fluid. This case study describes a rare case of PVNS of the TMJ with bone destruction of the mandibular condyle. Complete surgical excision of the lesion was performed through a preauricular approach with temporal extension. During the 10-year follow-up, two more operations were performed due to local recurrence and the fracture of the reconstruction plate. Total joint reconstruction with Biomet was finally performed, and the absence of disease was confirmed with a biopsy report showing fibrosis with hyalinization and mild inflammation of the excised soft tissue from the old lesion.

Objective: Facial-type-associated variations in diagnostic features have several implications in orthodontics. For example, in hyperdivergent craniofacial types, growth imbalances are compensated by displacement of the condyle. When diagnosis and treatment planning involves centric relation (CR), detailed knowledge of the condylar position is desirable. The present study aimed to measure condylar displacement (CD) between CR and maximum intercuspation in three facial types of an asymptomatic orthodontic population. Methods: The study was conducted in 108 patients classified into three groups of 36 individuals each (27 women and 9 men; mean age, 20.5 years), based on the following facial patterns: hyperdivergent, hypodivergent, and intermediate. To quantify CD along the horizontal and vertical axes, the condylar position was analyzed using mounted casts on a semi-adjustable articulator and a mandibular position indicator. The Student t-test was used to compare CD between the groups. Results: Vertical displacement was found to be significantly different between the hyperdivergent and hypodivergent groups (p < 0.0002) and between the hyperdivergent and intermediate groups (p < 0.0006). The differences in horizontal displacement were not significant between the groups. In each group, vertical CD was more evident than horizontal displacement was. Conclusions: All facial types, especially the hyperdivergent type, carried a significantly high risk of CD. Therefore, the possibility of CD should be carefully evaluated and considered in the assessment of all orthodontic cases in order to accurately assess jaw relationships and avoid possible misdiagnosis.

The purpose of this study is to investigate on the clinical and radiographic patterns of cystogenic ameloblastoma of the jaws. The author studied 64 cases of cystogenic ameloblastoma with regard to age, sex distribution, the site of the lesion and several radiographic features. The results were as follows: 1. The average age was found to be 23.0 years, with a range of 4 to 56 years. The incidence was highest in the second and third decades (72%) and total 64 cases consists of 36 males and 28 females. 2. Fifty-nine cases were found in mandible and 5 cases in maxilla. 3. The specific site distribution was found to be 57.8% ramus, coronoid process and condyle, 34.4% premolar-molar region, and 7.8% were located in mandibular symphysis bilaterally. 4. From the total 64 cases, 28 (44%) were associated with an impacted tooth, especially mandibular second and third molar, 36(52%) failed to show any association with tooth impaction. It was found that the average age for impaction-associated tumors was 19.8 years whereas lesions without impaction occurred at an average age of 25.6 years, which was statistically significant. 5. Fifty-three(82%) cases showed unilocular radiolucencies, eleven (17%) cases showed multilocular radiolucencies. It was found that the average age for unilocular lesions was 22.3 years whereas lesions showing multilocularity occurred at an average age of 26.4 years, which was not statistically significant. 6. Of the 21 patients who had been followed for more than 2 years, only 4 patients treated by enucleation or curettage recurred (19% recurrence).
PURPOSE. The purpose of this study was to analyze the effects of the splinted implant prosthesis in a reconstructed mandible using three-dimensional finite element analysis. MATERIALS AND METHODS. Three-dimensional finite element models were generated from a patient's computed tomography data. The patient had undergone partial resection of the mandible that covered the area from the left canine to the right condyle. The mandible was reconstructed using a fibula bone graft and dental implants. The left mandibular premolars and molars remained intact. Three types of models were created. The implant-supported prosthesis was splinted and segmented into two or three pieces. Each of these models was further subcategorized into two situations to compare the stress distribution around normal teeth and implants. Oblique loading of 300 N was applied on both sides of the mandible unilaterally. The maximum von Mises stress and displacement of the models were analyzed. RESULTS. The stress distribution of the natural mandible was more uniform than that of the reconstructed fibula. When the loading was applied to the implant prosthesis of reconstructed fibula, stress was concentrated at the cortical bone around the neck of the implants. The three-piece prosthesis model showed less uniform stress distribution compared to the others. Displacement of the components was positively correlated with the distance from areas of muscle attachment. The three-piece prosthesis model showed the greatest displacement. CONCLUSION. The splinted implant prosthesis showed a more favorable stress distribution and less displacement than the separated models in the reconstructed mandible.
The facial asymmetries include maxillary, mandibular, and chin asymmetries, although the most common deformity is primarily in the mandible. Common causes of this type of asymmetry can include asymmetric growth of the condyle or the mandible. In these patients, the location of the Me would be deviated to the shorter side because of the asymmetric growth of the mandible, and, commonly, the maxillary occlusal plane would be tilted toward the deviated side because the maxilla likely grows asymmetrically according to the pattern of asymmetric mandibular growth. Three-dimensional CT images are ideal for evaluating the size and location of anatomic structures, and such reconstructed images allow the use of software that can show anatomic structures from numerous angles, allowing actual measurements of distances and angles without problems of magnification, distortion, or superimposition caused by 2-dimensional imaging. In the present study using 3D-CT imaging, the 8 parameters, including measurements of the upper midline deviation, maxillary canting in the canine and first molar regions, width of the upper arch, width of the mandible at the Go, vertical length of the ramus, inclination of the ramus, and deviation of the Me were easily measured. The dentition should be orthodontically decompensated and dental midline should ensure incisor midlines positioned in the midline of each jaw before surgical correction. Surgical correction could be considered such as canting or yawing correction in the frontal or horizontal aspect, respectively.
For the determination of instantaneous centre of rotation in the mandibular movement, the habitual opening path was recorded on the sagittal tracing plate by a Gnathorecorder attached on the lower anterior teeth which was designed for this study. The opening path was demonstrated mathematically and the instantaneous centre of rotation in the opening path were calculated and plotted by computer programs, and then a computer graphic movement of the mandible was obtained according to the opening path. The hinge path of posterior border movement was graphied by the Gnathorecorder as well and demonstated mathematically. This hinge path was compared with the same obtained by Visitrainer to prove a reliability of the Gnathorecorder. The results obtained from this study were as follows. 1. The habitual opening path and hinge path obtained from the Gnathorecorder movement were expressed mathematically as follows: $y=0.11x^2-0.66x+3.09$ (equation of opening path), $y=0.04x^2+0.21x+3.71$ (equation of hinge path), and a kinematic hinge axis was not shown in the equation of kings path. 2. The early habitual opening movement of the mandible demonstrated almost a straight opening path or an arc motion with the larger radius and it's centre of rotation moved parallelly in the more rear and lower position than a kinematic hinge axis. 3. Instantaneous centre of rotation occured along with the translation of the condyle, and mandible was rotated around an axis in the close to the maximum opening and then moved toward the forward and upward position. 4. There were some similarities between the articular eminence and the movement pattern of a special point which was the kinematic hinge axis.
