Treatment of condylar fracture is still controversial, but the indication of surgical mangement is increased due to improved fixation methods and surgical techniques. In this article, we report our clinical and radiological results with the use of lag screw osteosynthesis for fixation of fractured mandibular condyle. 12 case of condyle fracture treated by open reduction and fixation with lag screw were monitored for an average of 10 months (range from 6 months to 19 months) postoperatively. The obtained results are as follows : 1. Postoperative maximum mouth opening is achieved over 40mm except one patient. 2. Although slight crepitus and deviation of mandible during mouth opening were found, Temporomandibular joint problem is satisfactory in all but one patient. 3. Distal migration of screw and resorption of fractured fragment were found in each 2 patients, but good functional recovery was achieved in all. According to the above results, we think that lag screw osteosynthesis is relatively indicated in level III or IV condylar fracture.
Of all facial fractures in children, condylar fractures have the greatest propensity to produce a growth disturbance. This risk appears to be greatest when the injury is during the first 3 years of during adolescence. Yet, the ability of a child to undergo compensatory growth that decrease the effects of the injury is also the greatest. Fracture dislocation of the condyle in the preadolescent often results in excellent remodeling and function. Because of this factor and the higher risRs of avascular necrosis and ankylosis open reduction of a condylar fracture in a child is not widely recommended. This retrospective study analyzed mandibular condyle fractures in the children who admitted in dept. of Oral and Maxillofacial Surgery, Pusan paik hospital from 1984 to 1993 clinically and radiologically.

For the study of the temporomandibular joint in rheumatoid arthritis 30 patients were selected who were diagnosed as rheumatoid arthritis through the clinical, radiographic examination and laboratory findings. Temporomandibular joint involvement was evaluated through the clinical, radiographic examination. The results were as follows; 1. TMJ was involved in 15 patients of 30 patients with rheumatoid arthritis. (50% involvement). 2. Duration of rheumatoid arthritis was more longer in patients with TMJ involvement than in patients without TMJ involvement. 3. Osseous changes in TMJ were in order of frequency erosion, flattening, osteophyte, sclerosis, deformity, and most common involved site was mandibular condyle. 4. Most common positional change of condyle was forward position in centric occlusion, and restricted movement of condyle in 1inch mouth opening. 5. TMJ involvement of rheumatoid arthritis was almost bilateral. 6. Main symptoms of TMJ were pain, stiffness, tenderness, limitation of mouth opening, crepitation 7. There was not the case of ankylosis. 8. There was statistically insignificant correlation between mandibular deviation and TMJ involvement, but some cases showed severe deviation on mouth opening.

Condylar hyperplasia is a self-limiting condition characterized by a slowly progressing, enlargement of the mandible that results in facial asymmetry and a crossbite malocclusion. The facial asymmetry, open bite or crossbite, and radiographic evidence of an enlarged condyle confirm the diagnosis of condylar hyperplasia. The etiology of the condition is unknown. This condition usually first becomes apparent during the second decade of life, when one condyle continues to grow while the other is no longer active. Radiographically, the condyle may appear enlarged or the neck of the condyle maybe elongated or both may occur, Sometimes, however, no radiographically demonstrable condylar abnormality will be noted. Surgical correction with subcondylar osteotomy is the treatment of choice. We have observed two cases of condylar hyperplasia occurred in the left mandibular condyle of 24-year-old and 35-year-old women. We obtained that two cases were shown the followed results; 1. Clinically, both cases was unilaterally developed on C/C area, with temporomandibular disorders and pain, facial asymetry and malocclusion. 2. Radiographically, hyperplastic mass confined to the condyle. 3. Histopathologically, these cases shown increased hypertrophic region in parts, and lamellated bone with irregular trabeculae.

Distraction osteogenesis is a commonly used technique for mandibular lengthening, but changes in the temporomandibular joint(TMJ) have not been well documented. The TMJ is one of the most complex joint in the body and is composed of a fibrous surface layer, a proliferative zone, hypertrophic cartilage, and bone. The shape and role of the TMJ change and modify during a person's life-time. Possible complications that can arise after mandibular distraction include failure of the formation, failure of callus, infection, disturbance of TMJ and of occlusion. However, there are only a few reports on changes in the TMJ as a result of distraction osteogenesis. Hence, the goal of this study was to evaluate the change of the TMJ after experimental distraction of mandibular ramus in rabbit. We studied histological changes of mandibular condyle, articular disk and retrodiscal tissue, and also examined the collagen I gene expression and MMP-1 gene expression. The results were as follows. 1. In the histological staining, experimental condylar surface showed more thick fibrous articular layer and proliferative layer, compared with the control condyle and experimental articular disc showed thick and dense collagen fibers compared with the control disc. 2. In the collagen I and MMP-1 gene RT-PCR analysis, experimental discs showed increased collagen I expression compared with the control disc, while MMP-1 gene expression was decreased compared with the control disc. The retrodiscal tissue was almost equal expressions of the collagen I and MMP-1 genes compared with the control retrodiscal tissue. These findings suggest that histological and biomolecular changes occur in condyles and discs after unilateral mandibular distraction osteogenesis.
Purpose: This study analyzed the effects of digastric muscle injury on mandibular growth in young rats. Methods: Fourteen 4-week-old Wistar rats were divided into 3 groups; experimental group A (n=6) with unilateral (right) shortening of the posterior digastric muscle, experimental group B (n=6) with bilateral shortening of the posterior digastric muscles, and control group C (n=2) who underwent a sham operation. Eight weeks after the operation all animals were sacrificed and the outcomes were compared using body weight evaluation, mensurations on lateral radiograph of hemimandibles and histological evaluation. Results: There was no significant difference between groups A and B in body weight gain. Comparison of the mean values of hemimandible distance on radiograph was performed. The difference in group mean value of mandible-related distances was analyzed using the Wilcoxon test (rank sum test) and a comparison of the homonymous distances of group A and B was performed using the Mann-Whitney test. There were differences between sides in mandibular length in group B and mandibular ramus height and transverse width of the condyle in group A (all, p<0.05). There were differences in condylar height and mandibular length on the left side between groups A and B (p<0.05). Histologic examination of temporomandibular joint showed similar findings in all specimens. Conclusions: Injury of the posterior belly of the digastric muscle during the rat growth period induced shortening of mandibular ramus height and transverse width of the condyle. It can be inferred that trauma to the posterior belly of the digastric muscle affects horizontal and vertical growth of the mandible.
PURPOSE. When performing an occlusal procedure, it is recommended that the patient should be sitting straight with the head in a natural position. An inappropriate mandibular position caused by an incorrect occlusal record registration or occlusal adjustment can result in damaged teeth and cause functional disorders in muscles and temporomandibular joints. The purpose of this study was to clarify the influence of horizontal cephalic rotation on mandibular position by investigating the three-dimensional positions of condylar and incisal points. MATERIALS AND METHODS. A three-dimensional jaw movement measurement device with six degrees of freedom (the WinJaw System) was used to measure condylar and incisal points. The subjects were asked to sit straight with the head in a natural position. The subjects were then instructed to rotate their head horizontally $0^{\circ}$, $10^{\circ}$, $20^{\circ}$, $30^{\circ}$, $40^{\circ}$, $50^{\circ}$and $60^{\circ}$ in the right or left direction. RESULTS. The results indicated that horizontal cephalic rotation made the condyle on the rotating side shift forward, downward, and toward the inside, and the condyle on the counter rotating side shift backward, upward, and toward the outside. Significant differences in deviations were found for angles of rotation higher than $20^{\circ}$. The incisal point shifted in the forward and counterrotating directions, and significant differences were found for angles of rotation higher than $20^{\circ}$. CONCLUSION. The mandibular position was altered by horizontal cephalic rotations of more than $20^{\circ}$. It is essential to consider the possibility of deviation of the mandibular position during occlusal procedures.

The purpose of this study was to evaluate the detectability of various radiographic techniques for mandibular condylar lesions. Erosive lesion, osteophyte and flattening were formed on the artificial mandibular condyle, and panoramic, transcranial, transorbital radiography, lateral and frontal tomography were taken. The results were as follows; 1. The detectability for erosive lesions was superior in the order of frontal tomography(96%), lateral tomography(78%), transorbital(59%), transcranial(16%) and panoramic(48%) radiography. 2. The location of erosive lesion that showed the highest detectability was the medial third in panoramic, the lateral third in transcranial, the central portion of anteroposterior direction in transorbital, the central portion of mediolateral direction and the posterior third in lateral tomography. Frontal tomography disclosed all erosive lesions except one anterolateral lesion. 3. The detectability of osteophyte was 100% in lateral tomography, 78% in transcranial and 56% in panoramic radiography. 4. For flattening, lateral tomography showed the flattened condyle, but both panoramic and transcranial views showed only decreased bone density without the change of condylar shape.

The aim of this article was to examine the usefulness of costochondral graft in the temporomandibular joint reconstruction. We performed costochondral grafting in the mandibular reconstruction including condyle replacement in 3 patients, which 2 of ankylosed cases and 1 destructed case. In one case, unexpected fracture of costochondral junction has developed at post-operative 2 months, but, normal mouth opening and facial appearance have been acheived by continuing exercise. These patients showed maximum mouth opening of 35-40 mm and no restriction of lateral side movement. They have been improved esthetics and function. We proposed that the costochondral graft is one of the useful method for functional reconstruction of defected mandibular condyle.
