Study for Mechanical and Physicochemcial Properties of Silicone Gel Filled Mammary Implants (실리콘겔 인공유방의 기계적 및 물리화학적 특성에 대한 연구)

  • The purpose of this study is to develop the guideline of the physicochemical and mechanical properties evaluation for silicone gel filled breast implants. First of all, the use and development status for silicone gel filled breast implants were investigated, and then, standard and criteria about performance evaluation established by the international organizations such as ASTM, FDA guidance and ISO were examined. To evaluate the mechanical properties, data research and testing for breaking strength, elongation, tensile set, joint intensity, silicone gel cohesion, weight loss from heating, static rupture resistance, impact resistance test, fatigue test, and gel bleed were performed. On the other hand, to evaluate the physicochemical properties, volatile matter, extent of cross linking, heavy metals, and extractable were analyzed. In this study, results for general function, mechanical properties and physicochemical properties were examined and reviewed for the accordance with international standard, and objective and standardized guideline was provided.

Trends of Breast Cancer and its Management in the Last Twenty Years in Aden and Adjacent Governorates, Yemen

  • Background: Breast cancer is the most common cancer of women and the principal cause of death in middle aged women. The objective of this study was to describe the trend of breast cancer and its management in Aden and adjacent south-eastern governorates of Yemen during the last 20 years. Patients and Methods: This is a retrospective analysis of previous studies on patients with breast cancer in Aden and adjacent south-eastern governorates, Yemen (January 1989 through December 2007). The studied variables were: sex, age, time and type of presentation, disease stage, pathological types and the performed surgical treatment. The sources of information were the treatment registry of Aden health office, archives of Al-Gamhouria teaching hospital; major referral and other public and private hospitals in Aden and Aden Cancer Registry. Results: The total number of patients was 476, 99% being females. The age range was 19-88 years. The most affected age was 30-50 years (60.5%), 95% presenting after one month of having breast symptoms. Forty-five percent presented with signs of advanced local disease, while 59.2% had palpable axillary lymph nodes on presentation. Early breast cancer (stages I-II) occurred in 47%, and late breast cancer (stages III-IV) in 51.5%. Invasive ductal carcinoma was the commonest pathology (89.3%). The main surgical treatment was mastectomy (modified radical mastectomy (50%). Conclusion: Breast cancer is predominantly a disease of young with late presentation and advanced disease. Improving health awareness and earlier diagnosis of the disease by health education, encouraging breast self-examination, and providing the mammography equipment and mammary clinics in hospitals are recommended. Establishment of oncology and radiotherapy centers in Aden is a necessity.

Semicircular Horizontal Approach in Breast Reduction: Clinical Experience in 38 Cases

  • Background Various techniques are used for performing breast reduction. Wise-pattern and vertical scar techniques are the most commonly employed approaches. However, a vertical scar in the mid-lower breast is prominent and aesthetically less pleasant. In contrast, a semicircular horizontal approach does not leave a vertical scar in the mid breast and transverse scars can be hidden in the inframammary fold. In this paper, we describe the experiences and results of semicircular horizontal breast reductions performed by a single surgeon. Methods Between September 1996 and October 2013, our senior author used this technique in 38 cases in the US and at our institution. We used a superiorly based semicircular incision, where the upper skin paddle was pulled down to the inframammary fold with the nipple-areola complex pulled through the keyhole. Results The average total reduction per breast was 584 g, ranging from 286 to 794 g. The inferior longitudinal pedicle was used in all the cases. The average reduction of the distance from the sternal notch to the nipple was 13 cm (range, 11-15 cm). The mean decrease in the bra cup size was 1.7 cup sizes (range, a decrease of 1 to 3). We obtained very satisfactory results with a less noticeable scar, no complication such as necrosis of the nipple or the skin flap, wound infection, aseptic necrosis of the breast tissue, or wound dehiscence. One patient had a small hematoma that resolved spontaneously. Conclusions This technique is straightforward and easy to learn, and offers a safe, effective, and predictable way for treating mammary hypertrophy.

Histone deacetylases inhibitor and RAD51 recombinase increase transcription activator-like effector nucleases-mediated homologous recombination on the bovine β-casein gene locus

  • Objective: The efficiency of the knock-in process is very important to successful gene editing in domestic animals. Recently, it was reported that transient loosening of the nucleosomal folding of transcriptionally inactive chromatin might have the potential to enhance homologous recombination efficiency. The objective of this study was to determine whether histone deacetylases (HDAC) inhibitor and RAD51 recombinase (RAD51) expression were associated with increased knock-in efficiency on the β-casein (bCSN2) gene locus in mammary alveolar-large T antigen (MAC-T) cells using the transcription activator-like effector nucleases (TALEN) system. Methods: MAC-T cells were treated with HDAC inhibitors, valproic acid, trichostatin A, or sodium butyrate for 24 h, then transfected with a knock-in vector, RAD51 expression vector and TALEN to target the bCSN2 gene. After 3 days of transfection, the knock-in efficiency was confirmed by polymerase chain reaction and DNA sequencing of the target site. Results: The level of HDAC 2 protein in MAC-T cells was decreased by treatment with HDAC inhibitors. The knock-in efficiency in MAC-T cells treated with HDAC inhibitors was higher than in cells not treated with inhibitors. However, the length of the homologous arm of the knock-in vector made no difference in the knock-in efficiency. Furthermore, DNA sequencing confirmed that the precision of the knock-in was more efficient in MAC-T cells treated with sodium butyrate. Conclusion: These results indicate that chromatin modification by HDAC inhibition and RAD51 expression enhanced the homologous recombination efficiency on the bCSN2 gene locus in MAC-T cells.

Stimulation of the milk protein production in MAC-T cells by purified bee venom (정제봉독의 MAC-T 세포에서 유단백 합성 촉진효과)

  • Purified bee venom was collected from colonies of honeybees (Apis mellifera L.) using a bee venom collector under sterile conditions and then purified under strict laboratory conditions. Purified bee venom contained $63.9{\pm}5.4%$ melittin, $10.9{\pm}1.6%$ phospholipase A2, and $2.3{\pm}0.3%$ apamin. Purified bee venom has various anti-bacterial, anti-inflammatory and immunostimulating effects. In this study, we evaluated purified bee venom which are mammary gland cells, MAC-T cells are used to increase the synthesis of milk protein. Purified bee venom promoted the proliferation of MAC-T cells at concentrations below $1{\mu}g/mL$, but cytotoxicity at $10{\mu}g/mL$ and above. As a result of the increase in the synthesis of ${\beta}-casein$, a milk protein after treatment with MAC-T cells at a concentration of the bee venom without cytotoxicity, the ${\beta}-casein$ content in the cell culture was increased when treated at a concentration of 1 ng/mL or more. In addition, it was confirmed that purified bee venom significantly increased the expression of bovine ${\beta}-casein$ (bCSNB) mRNA, a ${\beta}-casein$ synthesis gene, at a concentration of 1 ng/mL or more. These results suggest that purified bee venom can be used to increase the production of livestock by ultimately increasing the expression of milk protein.

The Dose Distribution of Arc therapy for High Energy Electron (고에너지 전자선 진자조사에 의한 선량분포)

  • The treatment of tumors along curved surfaces with stationary electron beams using cone collimation may lead to non-uniform dose distributions due to a varying air gap between the cone surface and patient. For large tumors, more than one port may have to be used in irradiation of the chest wall, often leading to regions of high or low dose at the junction of the adjacent ports. Electron-beam arc therapy may elimination many of these fixed port problems. When treating breast tumors with electrons, the energy of the internal mammary port is usually higher than that of the chest wall port. Bolus is used to increase the skin dose or limit the range of the electrons. We invertiaged the effect of various arc beam parameters in the isodose distributions, and combined into a single arc port for adjacent fixed ports of different electron beam eneries. The higher fixed port energy would be used as the arc beam energy while the beam penetration in the lower energy region would be controlled by a proper thickness of bolus. We obtained the results of following: 1. It is more uniform dose distribution of electron to use rotation than stationary irradiation. 2. Increasing isocenter depth on arc irradiation, increased depth of maximum dose, reduction in surface dose and an increasing penetration of the linear portion of the curve. 3. The deeper penetration of the depth dose curve and higher X-ray background for the smaller field sized. 4. If the isocenter depth increase, the field effect is small. 5. The decreasing arc beam penetration with decreasing isocenter depth and the isocenter depth effect appears at a greater depth as the energy increases. 6. The addition of bolus produces a shift in the penetration that is the same for all depths leaving the shape of the curves unchanged. 7. Lead strips 5 mm thick were placed at both ends of the arc to produce a rapid dose drop-off.

A Study on Electron Beam Dosimetry for Chest Wall Irradiation (흉곽(胸廓)의 전자선(電子線) 조사시(照射時) 선량분포(線量分布)에 관(關)한 연구(硏究))

  • To obtain 7 MeV electron beam which is suitable for treatment of the chest wall after radical of modified radical mastectomy, the authors reduced the energy of electron beam by means by Lucite plate inserted in the beam. To determine the proper thickness of the Lucite plate necessary to reduce the energy of 9 MeV electron beam to 6 MeV, dosimetry was made by using a parallel plate ionization chamber in polystyrene phantom. Separation between two adjacent fields, 7 MeV for chest wall and 12 MeV for internal mammary region, was studied by means of film dosimetry in both polytyrene phantom and Humanoid phantom. The results were as follows. 1. The average energy of 9 MeV electron beam transmitted through the Lucite plate was reduced. Reduction was proportional to the thickness of the Lucite plate in the rate of 1.7 MeV/cm. 2. The proper thickness of the Lucite plate necessary to obtain 6 MeV electron beam from 9 MeV was 1.2 cm. 3. 7 MeV electron beam, 80% dose at 2cm depth, is adequate for treatment of the chest wall. 4. Proper separation between two adjacent electron fields, 7 MeV and 12 MeV, was 5mm on both flat surface and sloping surface to produce uniform dose distribution.

  • Mixture experiments are commonly encountered in many fields including food, chemical and pharmaceutical industries. In mixture experiments, measured response depends on the proportions of the components present in the mixture and not on the amount of the mixture. Statistical analysis of the data from mixture experiments has mainly focused on a continuous response variable. In the example of quantal response data in mixture experiments, however, the tumor incidence data have been analyzed in Chen et al. (1996) to study the effects of 3 dietary components on the expression of mammary gland tumor. In this paper, we compared the logistic regression models with linear predictors such as second degree Scheffe polynomial model, Becker model and Akay model in terms of classification accuracy.

Polymorphism Identification, RH Mapping and Association of ${\alpha}$-Lactalbumin Gene with Milk Performance Traits in Chinese Holstein

  • Lactose synthase catalyses the formation of lactose which is the major osmole of bovine milk and regulates the milk volume. Alpha-lactalbumin (${\alpha}$-LA) is involved in the synthesis of lactose synthase in the mammary gland. Therefore ${\alpha}$-LA is regarded as a plausible candidate gene for the milk yield trait. To determine whether ${\alpha}$-LA is associated with milk performance traits, 1,028 Chinese Holstein cows were used to detect polymorphisms in the ${\alpha}$-LA by means of single-strand conformation polymorphism (SSCP). Two nucleotide transitions were identified in the 5'flanking region and intron 3 of ${\alpha}$-LA. Associations of such polymorphisms with five milk performance traits were analyzed using a general linear model procedure. No significant associations were observed between these polymorphisms and the five milk performance traits (p>0.05). RH mapping placed ${\alpha}$-LA on BTA5q21, linked most closely to markers U63110, CC537786 and L10347 (LOD>8.3), which is far distant from the region of the quantitative trait locus (QTL) on bovine chromosome 5 for variation in the milk yield trait. In summary, based on our findings, we eliminated these SNPs from having an effect on milk performance traits.

Simultaneous Visualization of the Sublingual Thyroid and Lactating Breasts with Tc-99m Pertechnetate Scintigraphy (설하갑상선과 수유중 유방이 Tc-99m Pertechnetate 스캔에 동시에 관찰된 증례)

  • Purpose: The thyroglossal duct runs from the base of the tongue to the thyroid. Rarely the thyroid completely fails to migrate and results in ectopic thyroid tissue, which can be demonstrated scintigraphically. A 31-year old female patient was referred for thyroid scintigraphy due to protruding mass at the base of the tongue. She was mildly hypothyroid. Te-99m pertechnetate thyroid scan was performed to rule out ectopic thyroid gland. There showed a focal area of intense tracer uptake in sublingual area, suggesting the sublingual thyroid. In addition there noted diffusely increased tracer uptake in both breasts. The patient delivered a baby 6 months prior to the scan and was on breast-feeding. Free Tc-99m pertechnetate physiologically secrets into the salivary glands, the stomach, the gastrointestinal tract, the genitourinary tract and the mammary glands and sodium-iodide symporter plays a role in the accumulation of free Tc-99m pertechnetate. We report simultaneous visualization of lactating breasts and ectopic thyroid gland in the base of the tongue.
