In these days, the orthodontic surgery including lingual orthodontics has attracted a person' attention due to its functional and esthetic appreciation. The delivery of the optimal orthodontic treatment is greatly influenced by clinician' ability to predict and control the tooth movement by applying force system to dentition. The skeletal anchorage system with the miniscrew has been used recently in the lingual orthodontics to assist the anchorage control. Precise understanding of the force system produced from the various orthodontic appliances is necessary. However, the qualitative and quantitative effect of the miniscrew has not been identified well. In this paper, three dimensional finite element analysis is introduced on the lingual orthodontics to investigate the effect of anterior retraction force on the miniscrew and transpalatal arch wire. The purpose of this study is to determine the location of the miniscrew and the point of force application of the anchorage system in the lingual orthodontics. The analysis results indicate the efficient position of the miniscrew and the transpalatal arch wire in the lingual orthodontics.

Orthodontics is a branch of dentistry that is concerned with the study and treatment of malocclusion, which may result from tooth irregularities, disproportionate jaw relationships, or both. Orthodontic devices consist of brackets, archwire connected to each bracket, and bends and hooks for auxiliary functions. Basically, orthodontics involves the interaction of brackets and archwire. It should be noted that uncontrolled tipping can occur due to unwanted movement of the teeth. The bending of an archwire can control the angle of an archwire and the rotation of a tooth. In this study, we predict the relationship between the bending angle of an archwire and the rotation of a tooth using the Kriging interpolation method. Also, we calculate the angle of an archwire that occurs at the minimum value of tooth rotation.
To predict the pubertal growth peak in stature and study the skeletal maturity degree using hand-wrist radiograph, the author used the 70 malocclusions (male 24, female 46). After longitudinal measurement of stature and skeletal maturity indicators of hand-wrist radiographs were taken during 4 years, the rsults were as follows. 1) The pubertal growth peak in stature occurred mainly at SMI 6-7 (56.5%) in female, SMI 5-6 (37.5%), 6-7 (37.5%) in male (Table 5). 2) It was suggested that the pubertal growth peak in stature was already passed, if SMI 8 occurred. 3) Ages of SMI in female were about 2 years earlier than those of SMI in male, and the sexual difference was gradually decreased in puberty. 4) Duration of SMI was longest at SMI 6-7 in both sex and the mean was 8.5 months. After this stage, the velocity of skeletal maturity in female was decreased than in male. 5) The correlation coefficient between each SMI and pubertal growth peak was very high (Table 8).
To study the sexual maturity at puberty and interrelation between pubertal growth peak and sexual maturity, 68 malocclusions (female 44, male 24) were longitudinally studied for 4 years and cross-sectional samples of 210 malocclusions were studied together. The pubertal growth peak was determined by stature increment and sexual maturity was studied using developmental stages of pubic hair and breast, menarche in female and those of pubic hair in male. The following results were obtained. 1. Mean age at menarche was 12.8 years and it was 11.8 months later than PHV. 2. At pubertal growth peak, developmental stage 2 (54.2%) and 3 (29.2%) of pubic hair were most in male, and stage 1 (72.7%) and 2 (20.5%) were most in female. 3. At pubertal growth peak, the developmental stage 2 (50%) of breast was most and the stage 3 (43.2%) was next. 4. In interrelation between skeletal growth and sexual maturity, PHV had most high correlation with menarche and stage 3 of breast, and menarche had most high correlation with stage 3 of breast. 5. Appearance of remarkable sexual maturity suggested that the PHV was passed already.
This study was designed to compare the normal head posture group with the extended head posture group in order to investigate the relatinship between head posture and craniofacial morphology. The subjects were devided into two groups; one included 80 children with normal head posture and occlusion, and the other 60 malocclusion patients with extended head posture. Their lateral cephalograms were traced and analysed based on 38 selected items. The following conclusion were reached. 1. The craniocervical angulations in normal group; OPT to SN, CVT to SN, OPT to FH and CVT to FH angles were $101.7^{\circ},\;104.8^{\circ},\;91.7^{\circ}\;and\;100.7^{\circ}$, respectively. 2. Compared with normal group, experimental group showed increase in mandibular plane angle, decrease in facial plane angle, airway space and posterior facial height but, there were insignificant differences in anterior facial height and tongue level between two groups. 3. Of the craniocervical angulations, OPT to FH angle was most highly correlated to the variables of the craniofacial morphology. 4. The effect of craniocervical angulation on craniofacial morphology in experimental group was different from that in comparison of normal group and experimental group.

Cerebral palsy is one of the most common disabilities of childhood, which affects movement and co-ordination. Individuals with cerebral palsy are susceptible to traumatic dental injuries. High occurrence of Class II malocclusion with prominent maxillary incisors appears to increase risk of trauma. However, due to fear of behavior management and lack of cooperation, clinicians have avoided orthodontic treatment in cerebral palsy patients. This case report demonstrates that modified rapid maxillary expansion can be used as a simple and effective method to correct ectopic eruption of maxillary incisor in cerebral palsy patient.
Purpose: The authors report unusual one case of a patient presenting with maxillary sinus mucocele who had underwent Lefort I procedure 7 years ago. Methods: Case report and literature review Results: A 25 year old man came to us with fullness, pain and nasal obstruction on his left cheek area. He had a history of multiple operations due to cleft lip and palate since birth. Two jaw surgery was performed for correcting class III malocclusion 7 years ago. Computed tomography showed haziness, and fluid filled cystic mass on left maxillary sinus. Nasoendoscopy revealed the bulging of inferior turbinate and mucosa coincided in medial wall of maxillary sinus. Antrostomy with Caldwell-Luc approach was performed. Mucin contaning brownish exudate was leaked out. Severe inflammation of maxillary inner wall and exposure of 2 screws fixed previously were noticed. The curettage and marsupialization were accomplished. The symptoms of patient were improved after that procedure. Conclusion: Maxillary sinus mucocele is related with Lefort I procedure and it may occur even long after that procedure.

최근 의료 분야 소프트웨어가 발전함에 따라 3차원 가상 모델을 이용한 의료 소프트웨어의 개발도 꾸준히 증가하는 추세이다. 치열 교정 분야는 이러한 3차원 가상 모델을 적용할 수 있는 분야 중 하나이다. 치열교정 분야에서는 3차원 가상 모델을 이용하여 부정 교합 환자의 치료 초기부터 끝까지 여러 단계의 치아이동 과정을 제공한다. 치아 이동의 각 단계별로 Stereolithographic Model이 만들어지는데, 이는 투명하고 얇은 Overlay 장치를 제작하는 기초가 된다. 투명한 재질의 Overlay 장치는 심미적이고 사용이 간편하여 많은 환자들이 선호하고 있다. Overlay 장치를 제작하는 시스템을 구축한 회사로는 Invisalign System이 가장 대표적이다. Invisalign System은 미국에 본사를 둔 회사로 한국에 지사를 두고 적극적인 마케팅을 통해 3D 치열 교정 분야의 국내시장을 거의 독점하고 있다. 독점적이다 보니 비용이 다른 교정 치료보다 2배 가까이 들고 있지만, 현재 한국에서는 개발이 적극적으로 이루어지지 않는 실정이다. 이에 본 논문에서는 환자 치아의 정확하고 과학적인 진단자료를 얻어 높은 치료비용에 대한 개선과 치료 전 과정에 대한 시뮬레이션 및 Stereolithographic Model을 미리 환자에게 제공하여 만족도를 높일 수 있는 3D치열 교정 시스템을 제안한다.
Background: Anterior open bite is a challenging malocclusion to correct orthodontic treatment. Anterior open bite associated with over-erupted posterior teeth and long lower facial height should be treated by reduction of posterior dimension for esthetic results. Although the possibility of orthodontic treatment of an anterior open bite has increased with the introduction of skeletal anchorage, there are still cases requiring surgery for various reasons. Case presentation: This case report covers an anterior open bite of a 25-year-old man successfully treated with the posterior maxillary segmental osteotomy (PMSO) and miniplates. After the pre-surgical orthodontic treatment, the PMSO between canines and first premolars was performed under local anesthesia and miniplates were placed on the zygomatic buttress. As a result of 28 months of treatment, an impaction amount of 3.5 mm was obtained in the maxillary posterior teeth, and the facial esthetics improved at rest and smile. Conclusion: The impaction of the posterior dentoalveolar segment using the PMSO can be a good treatment option in patients with anterior open bite showing long lower facial height.

Objective: This study aimed to investigate the factors associated with masticatory performance, as measured with a chewing gum containing spherical resinous microparticles, and to evaluate the method by examining the relationship with self-reported masticatory status. Methods: The participants in this study comprised 903 industrial workers (mean age, $42.2{\pm}11.6years$). A questionnaire was administered to assess self-reported masticatory status. The masticatory performance score was calculated by counting the number of particles in the chewing gum. Clinical oral examinations were administered. Multiple linear regression analysis was conducted on the masticatory performance scores to examine the related factors. Analysis of covariance was conducted to investigate the association between the masticatory performance score and the self-reported masticatory status. Results: Significant predictors of the masticatory performance score were sex (p<0.001), age (p<0.001), decayed teeth (p=0.009), total-functional tooth units (p<0.001), periodontitis (p=0.003), and malocclusion (p=0.011). The relationship between the masticatory performance score and the self-reported masticatory status was attenuated after controlling for confounding factors. Conclusion: The masticatory performance increased with age and decreased as the oral health status worsened. Using this chewing gum method partly, but not comprehensively, reflects masticatory performance. Therefore, various masticatory performance-related indexes should be employed to measure masticatory performance accurately.
