안면골 섬유 이형성증이 악성 섬유성 조직구증으로 악성화된 환자의 치험례 (Malignant Transformation of Fibrous Dysplasia: A Case Report of Malignant Fibrous Histiocytoma of Facial Bone)

  • Purpose: Malignant degeneration of fibrous dysplasia is an uncommon recognized complication of this disease. Especially, degeneration of fibrous dysplasia to malignant fibrous histiocytoma(MFH) in facial bone is rare and the publications had been limited. The purpose of this report is to share our experience. Methods: A 46-year-old patient with facial fibrous dysplasia visited our clinic for recent facial tingling and swelling. Malignant degeneration of fibrous dysplasia was suspected. Results: Total excision of the mass and adjacent facial bone was performed. Defect was immediately reconstructed with bone graft and bone cement. At a month follow up, metastasis was detected at ipsilateral parotid gland. Superficial parotidectomy and neck dissection was performed. The patient is currently taking chemotherapy. Conclusion: Because of the uncommon presentation of this entity, clinical course of treatment was dependent on other histological types of malignant degeneration. We report this case to share our experience.

MRI Features for Prediction Malignant Intra-Mammary Lymph Nodes: Correlations with Mammography and Ultrasound

  • Purpose: To assess clinically significant imaging findings of malignant intramammary lymph nodes (IMLNs) in breast cancer patients and to evaluate their diagnostic performance in predicting malignant IMLN. Materials and Methods: A total of 110 cases with IMLN of BI-RADS category 3 or more, not typical benign IMLN, in MR of breast cancer patients between January 2016 and January 2021 were retrospectively reviewed. After excluding 33 cases, 77 cases were finally included. Among them, 58 and 19 were confirmed as benign and malignant, respectively. Qualitative and quantitative MR imaging features of the IMLN were retrospectively analyzed. Sizes and final assessment categories of IMLN on MRI, mammography, and ultrasound were reviewed. Diagnostic performances of imaging features on MRI, mammography, and ultrasound were then evaluated. Results: For qualitative MR features, shape, margin, and preserved central hilum were significantly different between benign and malignant groups (P < 0.05). For quantitative MR features, long diameter over 6 mm, short diameter over 4 mm, and cortical thickening over 3 mm showed high sensitivities in predicting malignant IMLNs (89.5%, 94.7%, and 100%, respectively). Size exceeding 1 cm showed high specificity and accuracy in predicting malignant IMLN on MR, mammography, and ultrasound (91.4% and 80.5%; 96.6% and 79.25; 98.3% and 80.5%, respectively). Conclusion: Various MR imaging features and size can be helpful for predicting malignant IMLN in breast cancer patients.

발과 발목에서 발생한 종양의 치료 및 예후 (Treatment and Prognosis for Tumors of the Foot and Ankle)

  • Purpose: Tumors of the foot and ankle account for approximately 3%~5% of all musculoskeletal tumors, and accurate diagnosis is often delayed due to their rare prevalence. Therefore, the authors aimed to analyze the incidence, treatment methods, and prognostic factors of foot and ankle tumors treated at the authors' hospital. Materials and Methods: A retrospective single-center study examined 342 patients treated for foot and ankle tumors at the authors' hospital from January 2011 to February 2022. Data were collected from the electronic medical records (EMR) and picture archiving and communication systems (PACS). The information analyzed included gender, age, follow-up period, diagnosis, tumor occurrence and recurrence, treatment, and clinical outcomes. Results: Most cases (93.3%) were benign, but 6.7% were malignant. The main treatment for malignant tumors was surgical resection (91.3%). Approximately 53.1% of benign tumors and 91.3% of malignant tumors were treated with surgery, and two of the malignant tumors that did not undergo surgery had metastatic cancer. After surgery, 8.2% of benign lesions and 19.0% of malignant lesions recurred, and 9.5% of the patients with malignant tumors died after surgery. Conclusion: Most foot and ankle tumors are benign tumors, and the prognosis is not poor if treated properly, but most malignant tumors often require amputation. In some cases, however, amputation can be avoided with a combination of chemotherapy and radiation therapy.

  • Malignant mixed tumor of the parotid gland account for a very small number of the head and neck malignancies. Authors had experienced the recurrent malignant mixed tumor on 70 years old female patient. The first surgical removal was done before the 5years ago and at that time, histological diagnosis was the malignant mixed tumor. We had done the second operation, and histopathological diagnosis was malignant mixed tumor.

  • Malignant hyperthermia has been reported by many authors since Denborough [1960] first described concerning anesthetic death in a family. Malignant hyperthermia is characterized by a hypermetabolic state [tachycardia, tachypnea, hypercarbia, hypoxia, cyanosis, hypotension, high fever and muscle rigidity] and is related to a hereditary defect of skeletal muscle. In susceptible individuals, it is triggered by potent inhalational anesthetics, depolarizing muscle relaxant [Succinylcholine], amide type local anesthetics [prototype lidocaine] and occasionally by stress due to emotional and environmental factors. Unrecognized and untreated malignant hyperthermia is associated with a very high mortality rate. Recently authors have experienced malignant hyperthermia in 5 year old male child who was diagnosed to have patent ductus arteriosus and interatrial septal defect associated with congenital physical deformities such as short stature, hypotrophic muscles and genu valgus deformity of lower extremity, indirect inguinal hernia and Ramphant caries.

  • Although malignant transformation of meningiomas has been reported, it is extremely rare in meningiomas of ventricular system. Less than 2% of all meningiomas show malignant transformation from benign meningioma. We report a case of meningioma with early recurrence and malignant transformation and investigated possible underlying factors using immunohistochemistry for PCNA and p53 protein expression.

보험의학적 악성도평가 (Differentiation between malignant and benign in terms of insurance claims)

  • In case of neoplasm claims, it is important to make a decision of differentiating malignant and benign. In Korean insurance market, there are many insurance products that cover cancer. In the insurance claims adjustment, differentiation between malignant and benign is according to histologic findings. However there are many neoplasms of bad clinical course in spite of benign histopathologic classification. In this article; astrocytoma, thymoma, gastrointestinal stromal tumor, colonic intramucosal carcinoma, gastric high grade adenoma/dysplasia, carcinoid tumor, MALT lymphoma, revision of Korean Classification of Disease-6th edition, and bladder tumors are reviewed in terms of differentiation between malignant and benign in the insurance claims. It may be helpful for claims staff to review important neoplasms in terms of differentiation between malignant and benign.

  • The paper presents the incidence rates of malignant tumors of the central nervous system assessed by the component analysis. The data on primary registered cases of malignant tumors of the central nervous system in the country were used as the material of the study for the period from 2004 to 2011. A general trend of increase in the number of patients with malignant tumors of the central nervous system in Kazakhstan was determined and the potential of their increase was evaluated, which can be due to changes in the morbidity risk and age specifics, as well as the increase in population.

  • Mammary cancer is the most common malignant neoplasm in the bitch. It constitute 40 percent of all tumors in female dogs, which is three times higher than incidence of mammary tumors in humans[2]. Cytological differentiation between benign and malignant canine mammary tumors is difficult, however, an irregular chromatin pattern was reported to be a significant criterion for malignancy[1]. It can be estimated that approximately 30 percent of the surgically removed mammary tumors are malignant[4]. Malignant mammary tumors often have some degree of infiltrative/destructive growth into adjacent tissues and/or invasion of vessels. Malignant mammary tumors often metastasize into local lymph nodes and lungs, and less frequently into other organs[3]. Based on histological and cytological criteria, this case was diagnosed as tubulopapillary carcinoma of the mammary gland. (omitted)

난소의 성숙기형종에서 발생한 편평세포암종 - 1예 보고 (Case of Squamous Cell Carcinoma Arising in Mature Cystic Teratoma of the Ovary - A case report -)

  • Mature cystic teratoma is a common ovarian lesion, approximately 10-20% of ovarian tumor. However, it can undergo a malignant transformation, but incidence of malignant transformation is very low. Squamous cell carcinoma is the most common type of malignant transformation in the mature cystic teratoma. Preoperative diagnosis of malignant transformation in the mature cystic teratoma is very difficult, because malignant transformed tissues are very small in the mature cystic teratoma components. And so most of the carcinoma arising in mature cystic teratoma were diagnosed after histologic examination. We experienced a case of squamous cell carcinoma arising in the mature cystic teratoma of the ovary and report a case with a brief review of the literatures.
