  • 최근 인공지능을 활용한 경혈의 위치를 추론하는 연구가 활발하게 진행되고 있다. 본 논문은 자세 추정(Pose Estimation)을 통해 경혈 검출을 위한 두 가지 접근법인 Top down 방식과 Bottom up 방식의 추론 결과를 비교함으로써 어떤 방식이 경혈의 위치를 추론하기에 더 적합한 지 알아보고자 한다.

  • Acupuncture and acupressure apply needles or pressure to anatomical points for therapeutic benefit. The over 350 mapped acupuncture points in the human body can each treat various conditions, but anatomical variations make precisely locating these acupoints difficult. We propose a computer vision technique using the real-time hand and face tracking capabilities of the MediaPipe framework to identify acupoint locations. Our model detects anatomical facial and hand landmarks, and then maps these to corresponding acupoint regions. In summary, our proposed model facilitates precise acupoint localization for self-treatment and enhances practitioners' abilities to deliver targeted acupuncture and acupressure therapies.

Classification of Muscles into Meridian Sinew: A Literature Review (근육의 경근 배속에 대한 국내 연구 고찰)

  • Objectives Although many studies explored the topic of meridian sinew in various perspectives and the term "meridian sinew" is widely used, the theory of meridian sinew is not applied for precise diagnosis and in-depth treatment in clinical practice. The aim of the study is to provide basic data classifying muscles into meridian sinew for future studies that investigate meridian sinew based on an anatomical basis. Methods Studies were identified with searches of six major Korean databases: OASIS, KoreaMed, KMBASE, KISS, NDSL and KoreanTK. Published primary studies classifying muscles into meridian sinew were included. Results A total of 20 studies met the inclusion criteria and were included in the analysis. Twelve studies conducted the classification of muscles into meridian sinew based on meridian/ acupoints distribution and six based on meridian sinew distribution, and two based on both. Muscles with fidelity level of 50 or more were 54 (85.7%) and muscles with 100 fidelity level were 7 (11.3%): occipitalis, adductor digiti minimi, frontalis, biceps femoris, rectus femoris, vatus lateralis and extensor digitorum longus. Conclusions Classification results of muscles into meridian sinew varied according to the classification criteria and interpretation of meridian sinew and acupoints distribution. To develop muscle sinew as a more useful theory in diagnosis and treatment, efforts should be made to reduce the gap between study results and build consensus on the anatomical entity of meridian sinew.

Telephone Survey for Developing Clinical Trial Protocol on Individualized Acupuncture Treatment for Knee Osteoarthritis (퇴행성 슬관절염의 맞춤형 침구 임상시험 프로토콜 개발을 위한 전화조사)

  • Objectives : This study was accomplished in order to find out how Korean medical doctors take acupuncture prescription for knee osteoarthritis in clinical practice Methods : The survey questions were developed by the consensus from 4 professors and 10 residents who major in acupuncture & moxibustion for developing clinical trial protocol on individualized acupuncture treatment for knee osteoarthritis. 206 persons having more than 10 years experience were randomly selected from a list of Korean medical doctors. Telephone interviews with them were conducted by the well-trained interviewers between May 21 and May 27 in 2005. Results : 139 of 206 Korean medical doctors made an acupuncture prescription prior to treatment of knee osteoarthritis. 103 of 139 Korean medical doctors used both local and remote points, and 26 of them used remote points only, and 10 of them used local points only. In case of doctors who use remote points, the five element (45 of 129) or Saam (28 of 129) acupoints were used dominantly, and its was based on the pattern identification of the Jangbu organs related with the pain (56 of 129) or of the pain location (35 of 129). Conclusion : This study showed that Koran medical doctors prefer to use remote points such as five element or Saam acupoints based on the pattern identification of the Jangbu organs related with the pain or the pain location. These result may be useful for developing protocols for clinical trial on acupuncture on knee osteoarthritis.

The Review on the Prostate Disease-related Studies with Acupuncture Therapy in PubMed (PubMed 검색(檢索)을 이용한 전립선(前立腺) 질환(疾患)의 침치료(鍼治療)관련 연구(硏究)에 대한 고찰(考察))

  • Objective : This study was to review on the prostate disease-related studies with Acupuncture therapy in renowned medical internet site of PubMed, and to make master plan of the study, especially, on Bee Venom Acupuncture(BVA) of Prostate disease and then to devise the idealistic therapeutic ways of it. Method : We made the internet search with the key words of bee venom(bee venom therapy, apitoxin, apitherapy, bee sting, bee sting therapy), acupuncture, prostate, prostatitis, prostrate cancer in Pubmed, from June 1st to July 1st,2004. Results : 1.25 papers were found in 19 publised jounals. of which two named'Urology'and Prostate' had three papars, two called 'JUrology' and 'Cancer Immunol Immunother' had two papers, and the others had a paper respectively. 2. In the classification by papers' types, Review papers were 8 and Original were 17 where there were 5 clinical trials, 11 experimental studies and 1 epidemiologic paper. Of 5 clinical trials, 2 belonged to Randomized Control Study, and of 11 experimental studies, 4 belonged to in vitro and 7 belonged to in vivo with in viro studies, and 1 epidemeologic belonged to meta-analysis. 3. In the classification by prostate diseases, 4 were about prostatitis, 3 were about prostate related symptoms, 16 were about prostate cancer, and two were about the others. 4. In the classification by applied treatment methods, 5 were related with Acupuncture, 10 were related with BVA(Bee Venom, Bee), and 10 were related with the others. Of 5 related with Acupuncture, 3 used general acupuncture, 1 used electrical acupuncture, and 1 used general acupuncture and electrical acupuncture at the same time. 5. In 2 RCTs of Clinical trials, Control group was set up to the group using different compatible treatment method or using meridians not related with treating prostate disease. Single or double blind methods couldn't be found. 6. In the clinical trials, IPSS, NIH, CPSI or subjective global assessment were used as the Index of Evaluation. 7. The Leg Greater Yang Bladder Meridian(B), The Leg Lesser Um Kidney Meridian(K) and Conception Vessel Meridian(CV) were used as major meridians, and B10(Taejo, Dazhu), B23(Shinsu, Shenshu), B28(Panggwangsu, Pangguangshu), B35(Hoeyang, Huiyang), B39(Wiyang, Weiyang), B40(Wijung, Weizhong), B54(chilbyon, Zhibian), K1(Yongchon, Yongquan), K10(Umgok, Yingu), CV3(Chungguk, Zhongji), CV4(Kwanwon, Guanyuan),S6(Hyopko, Jiache) were used as acupoints. Electrical acupuncture(EA) was considered to be more important and CV3(Chungguk, Zhongji), CV4(Kwanwon, Guanyuan) were mainly selected as EA applied acupoints. 8. It is mostly said that Acupuncture appeared to be a safe, effective, and durable treatment alternative in improving symptoms of patients with prostate diseases, refractory to conventional medicine. A larger controlled study was required to confirm these encouraging initial results. Conclusion : Papers about BVA of Prostate cancer or Prostatitis were not found, and low permeability of Prostate is concerned, BVA with the anti-inflammatory and anti-cancer effect can be adopted as a new alternatives of Prostate disease treatment, so it is thought that Study of how to make access to prostate, animal experiment including in vivo and in vitro and more clinical trials with using acupoints on related meridian should be followed.

A Myological Study of Hand Great Yin Lung Meridian Muscle System and Comparison with Deep Front Line in Anatomical Train (수태음폐경근의 근육학적 고찰 및 심부상지전방선과의 비교)

  • Objectives : This study was aimed to widen range of comprehesion about meridian muscle system through myological study of meridian muscle system and comparison with deep front arm line in anatomical train Methods : We have studied the similarity and difference between Hand Great Yin Lung Meridian Muscle System and Deep Front Line in Anatomical Train through Principles of Meridians & Acupoints, publications about myology, Anatomical trains. Results : I. Like another advanced studies, muscular system of hand great yin showed similarity to deep front line in anatomical train. II. It is considered that muscular system of hand great yin contains Musculus abductor pollicis brevis, Musculus extensor hallucis longus, Musculus brachioradialis, Musculus biceps brachii, Musculus subclavius, Musculus pectoralis major. III. Comparing muscular system of hand great yin to deep front arm line in anatomical train it showed similarity to part of muscles and pathological symptoms. But it showed difference to part of muscles and pathological symptoms. Conclusions : Hand Great Yin Lung meridian muscle system showed similarity and difference to deep front arm line in anatomical train. Further studies would be needed.

The effects of Sa-am Acupuncture Simpojeongkyeok Treatment on Hwa-byung : Randomized, patient-assessor blind, placebo-controlled acupuncture, pilot clinical trial (화병에 대한 사암침 심포정격의 치료 효과 : 환자-평가자 눈가림, 무작위배정 대조군 임상시험)

  • Objectives : The purpose of this research is to examine the effects of Sa-am acupuncture Simpojeongkyeok treatment on Hwa-byung. Methods : The study was performed through a patient-assessor blind, randomized, placebo-controlled trial in which the volunteers, data collector and analyst were unaware of individuals which were receiving the treatment. Fifty volunteers were divided into a trial and a control group, each of which consisted of 25 subjects. The trial group was treated with Sa-am acupuncture Simpojeongkyeok, while needle insertion at non-acupoints was given to the control group. The Likert scale for major symptoms of Hwa-byung was measured as the 1st evaluative instrument, and then a comparative analysis was conducted by comparing the results with those measured by of STAXI-K, STAI-K and BDI-K. Results : In the Likert scale comparison of major symptoms, total scores in both groups were decreased after treatment. However, a more significant decrease was found in those of trial group compared to those of the control group from a statistical perspective. The results measured by BDI scores also decreased significantly in the trial group after treatment. In comparison of STAXI and STAI scores, there were no significant differences found between the groups. Conclusions : The results suggest that Sa-am Acupuncture Simpojeongkyeok treatment is effective in the treatment of Hwa-byung.

Korean Herbal Medicine on Liver Function : A Systematic Review in Korean Literature (한약 복용이 간기능에 미치는 영향 : 국내 문헌에 대한 체계적 고찰)

  • Background : The safety of Korean herbal Medicine (KHM: prescribed herbal medicine by doctors of traditional Korean medicine) is an important issue in Korea. Although both fields. western medicine and traditional Korean medicine. have been studied on the safety of herbs and KHM, their results were not concordance with each other. Objectives : This study aims to review the influence of KHM on liver function in Korean literature systematically. Additionally, we tried to estimate the change of data of liver function test (LFT) and the incidence of liver injury (LI) after the use of KHM. Methods : Systematic literature searches were performed on 4 major databases of Korea from their inception to May 2008. Screening and selection of the studies and the extraction of data were performed independently by two authors. There were no restrictions on the types of publication including grey literatures. Results : Forty studies were included. Only sixteen studies were performed prospectively and fifteen studies collected data from outpatients. Only 8 studies reported the occurrence of LI after the use of KHM. Nineteen cases of LI showed no or mild symptoms and the elevation of LFT was not high. All of LI patients used conventional western drug and KHM concomitantly. and causality of LI was not assessed properly. The incidence of LI related to the use of KHM was estimated as 0.59%-0.76% from all data of these studies. The conflicting results were shown on the change of alanine aminotransferase (ALT) and total bilirubin (TB) after the use of KHM. Conclusions : KHM might be a minor cause of LI in Korea. However the results are not strongly supported as enough to make the safety issue clear because of the limitations of original studies. More rigorous studies are required for answering the safety issue of KHM with the cooperative investigation of both fields of Korean traditional medicine and western medicine.

Analysis on Ga-Rak market workers' disease and treatment of Traditional Korean Medicine(TKM) (pilot study) (가락시장 근로자의 병증과 한의학적 치료에 대한 예비 분석)

  • Objective : To broaden our understanding on occupational disease of market workers and to evaluate the effect of TKM treatment focusing on acupuncture and herb medicine and to promote base studies and clinical trials on occupational disease. Materials and Methods : Analysis was done on 33 Ga-Rak market workers' chart which had been selected through investigation of 1508 outpatient's chart of Kyung Won University Hospital from Jun. 1st, 2002 to May. 31th, 2003. Results : 1. Out of 33 patients, Men had more occupational diseases than women had and Most people were in their forties. 2. Major cause of the disease include repetitve bending, heavy weight lifting and overwork. 3. Past History of patients mostly include frequent lumbar sprain, periarthritis of shoulder, lumbar HNP 4. The patients with occupational disease were diagnosed as lumbar sprain, periarthritis of shoulder, lumbar HNP, degenerative spondylosis, spinal stenosis and their chief complaints were low back pain, omalgia, back pain with radicular pain. 5. The duration of treatment was mostly within a week. 6. Acupuncture, Bee Venom Acupuncture, moxibustion, Herb-medicine, extract, taping therapy, physical therapy were used as treatment methods. 7. Applied herb medicine were composed of 8 kinds of prescriptions and extract were made up of 6 prescriptions 8. Applied acupoints belonged mainly to 14 meridians 9. The treatment of herb medicine combined with acupuncture proved effective in treating the ocupational disease. Conclusion : The occupational disease of patients working in Ga-Rak market were closely related with overwork, especially with repetitive bending and heavy weight lifting and TKM treatment focusing on acupuncture combined with herb medicine was effective in treating occupational disease.

Review on Case Reports of Korean Medical Treatments for Sudden Sensory Neural Hearing Loss (돌발성 난청의 한방치료 치험례에 대한 고찰)

  • This study aimed to review case reports of Korean medical treatments for sudden sensory neural hearing loss published in Korea from 1980 to 2016. We searched sudden sensory neural hearing loss through 6 major Korean web article search engines and search period was January 1980 to September 2016. Two researchers included studies on sudden hearing loss, clinical studies on korean medical treatments, and excluded in vivo studies, in vitro studies, non-original studies, published abstracts only, and studies not published in Korean or English. 19 articles were included in this study from 63 articles. Only one case report used Korean medical treatment alone. The most tools for treatment were acupunture, herbal medicine, pharmacopunture, moxibustion, cupping treatment and laser therapy. Most acupoints used in the treatment is SI19(聽宮). When patients got treated sooner, recovery rate was better. There was no direct relationship between recovery rate and degree of hearing loss. This study suggests that more research about sudden sensory neural hearing loss is needed in the future.
