편평족에 대한 관절가동술과 자가신장이 요부의 신장성, 요통과 하지 체중부하 차이에 대한 비교 연구 (The Comparison Study of Ankle Joint Mobilization and Elongation on the Difference of Weight-bearing Load, Low Back Pain and Flexibility in Flat-foots Subjects)

  • Background: The purpose of this study was to investigate the relationship between the spine and the flat-foot, the stability and the alignment of the posture of the neck to prevent the alignment of the ankle joint operation and the lower back flexibility of the lumbar region according to the type of treatment using active stretching of the triceps, back pain, and to see how they affect weight bearing differences. Methods: The subjects of this study were 24 chronic low back pain patients. They were randomly divided into experimental group and control group. In the experimental group, ankle joint mobilization and active scraping of triceps were performed three times a week for a total of 6 weeks. The control group was performed in the same way without articulation. The range of flexion and extension motion of the lumbar spine and pain degree and difference of weight-bearing were measured before and after the experiment. Results: The model of ankle joint mobilization and calf muscle elongation of flat foot significantly improved the range of flexion and extension motion of the vertebrae (p<.05) and the VAS and distribution of weight-bearing were decreased in both of two groups (p<.05). In other words, the exercise and mobilization help to recover of the balance of the whole musculoskeletal, the vertebrae. Conclusions: The active exercise of the triceps muscle of the lower leg in this study It affects the flexibility of the lumbar spine, the pain and the difference in the weight support of the lower extremities, when we performed ankle joint mobilization for exercise and cramping, pain and the difference in weight support between the two lower limbs.

Bone-Preserving Decompression Procedures Have a Minor Effect on the Flexibility of the Lumbar Spine

  • Objective : To mitigate the risk of iatrogenic instability, new posterior decompression techniques able to preserve musculoskeletal structures have been introduced but never extensively investigated from a biomechanical point of view. This study was aimed to investigate the impact on spinal flexibility caused by a unilateral laminotomy for bilateral decompression, in comparison to the intact condition and a laminectomy with preservation of a bony bridge at the vertebral arch. Secondary aims were to investigate the biomechanical effects of two-level decompression and the quantification of the restoration of stability after posterior fixation. Methods : A universal spine tester was used to measure the flexibility of six L2-L5 human spine specimens in intact conditions and after decompression and fixation surgeries. An incremental damage protocol was applied : 1) unilateral laminotomy for bilateral decompression at L3-L4; 2) on three specimens, the unilateral laminotomy was extended to L4-L5; 3) laminectomy with preservation of a bony bridge at the vertebral arch (at L3-L4 in the first three specimens and at L4-L5 in the rest); and 4) pedicle screw fixation at the involved levels. Results : Unilateral laminotomy for bilateral decompression had a minor influence on the lumbar flexibility. In flexion-extension, the median range of motion increased by 8%. The bone-preserving laminectomy did not cause major changes in spinal flexibility. Two-level decompression approximately induced a twofold destabilization compared to the single-level treatment, with greater effect on the lower level. Posterior fixation reduced the flexibility to values lower than in the intact conditions in all cases. Conclusion : In vitro testing of human lumbar specimens revealed that unilateral laminotomy for bilateral decompression and bone-preserving laminectomy induced a minor destabilization at the operated level. In absence of other pathological factors (e.g., clinical instability, spondylolisthesis), both techniques appear to be safe from a biomechanical point of view.

노인 퇴행성디스크 환자의 안정화운동이 척추불안정과 피로도에 미치는 영향 (The Effects of Segmental Instability and Muscle Fatigue after Applying Sabilization Exercise Program In Degenerated Disc Disease Patients of Aged)

  • The purpose of this study was designed to find out the effectiveness of vertebral segment instability, muscle fatigue response on lumbar spine after apply lumbosacral stabilization exercise program to 4 patients with chronic low back pain and for 12 weeks. In this study, the lumbar spine motion with blind by MedX test machine and the difference of instability to lumbar vertebra segments in flexion, extension test of standing position and spinal load test(Matthiass Test) by Spinal Mouse. The stabilization exercise program was applied 2 times a week for 12 weeks in hospital and 2 times a day for 20 minutes at home. The results of the present study were as follows: 1. Instability test of lumbar vertebra segment is 2 type differential angle test between vertebrae segment and loading test of spine(matthiass) by Spinal Mouse. It appeared to improve stability of segments in sagittal plane after applying program. So lumbar spine curve increased lordosis toward anterior and was improved of the lumbar spine flexibility in flexion and extension. Specially, in matthiass test, (-) value was increased between lumbar vertebra segment when was the load on spine. And so applying stability improved after program. 2. Fatigue response test(FRT) results, in male, was raised muscle fatigue rate during increase weight, on the other hand female appeared lower than male. As a results, lumbosacral stabilization exercise was aided to improvement of lumbar spine vertebra segments stabilization. Spine instability patients will have a risk when in lifting a load or working with slight flexion posture during the daily of living life and it is probably to increase recurrence rate. Thus, not only lumbar extension muscle strength but also stability of vertebra segments in lumbar spine may be very important.

관절 가동술과 안정화 운동이 급성 요통 환자의 유연성과 통증에 미치는 영향 (The effect of stabilization exercise and movement with mobilization on flexibility and pain of patients with acute low back pain)

  • Purpose : The purpose of this study was to investigate effects of movement with mobilization(MWM) and stabilization exercise on pain and range of motion of patients with acute low back pain. Methods : The subjects were consisted of 24 patients who had non specific acute low back pain. All subjects randomly assigned to the MWM group and the stabilization exercise group. The MWM group received sustained natural apophyseal glides(SNAGs) with modality treatment and stabilization exercise group received stabilization exercise with modality treatment. The remodified schober test(RST) was used to measure forward flexion and lateral flexion range of motion of lumbar segment. Visual Analogue Scale(VAS) was used to measure subjective pain level of the patients. The Oswestry Low Back Pain Disability Scale was used to measure functional disability level of the patients. Results : The lumbar flexion range of motion of MWM group was significantly increased compared with stabilization exercise group(p<.01). The range of motion of lumbar segment of MWM group was significantly decreased compared with stabilization exercise group(p<.01). The left lateral flexion range of motion of lumbar of MWM group was significantly decreased compared with stabilization exercise group(p<.05). The right lateral flexion range of motion of lumbar of MWM group was significantly decreased compared with stabilization exercise group(p<.05). The VAS of both MWM group and stabilization exercise group was significantly decreased(p<.001). The Oswestry Low Back Pain Disability Scale of stabilization exercise group was significantly increased compared with mobilization group(p<.001). Conclusion : In the result of this study, mobilization with movement and stabilization exercise had significant difference on pain and flexibility of patients with acute low back pain.

테이핑 적용방법에 따른 버스 운전기사의 요통과 근 유연성에 미치는 효과 (Effects of Taping Therapy by Methods on Back Pain and Muscle Flexibility of Bus Drivers's)

  • 본 연구는 테이핑 적용방법에 따른 버스 운전기사의 요통과 근 유연성의 변화에 대하여 알아보기 위하여 실시하였다. 대상자는 창원, 마산, 부산의 버스 운전기사 중에 요통이 있는 척추기립근 테이핑 실험군과 요추부 테이핑 실험군, 대조군의 3그룹으로 각각 15명으로 나누어 테이핑 적용 전 후의 통증과 유연성을 측정 분석한 결과 다음과 같은 결론을 얻었다. 치료전 후 척추기립근 테이핑군과 요추부 테이핑군은 통증지수가 감소하였다. 치료전 후 척추 기립근 테이핑군과 요추부 테이핑군은 유연성이 증가하였다. 치료 후 대조군에 비해 실험군은 통증지수의 감소와 유연성의 증가를 보였다. 이상의 결론을 종합해 보면 테이핑 적용이 요통이 있는 환자의 통증과 유연성에 긍정적인 영향을 미치는 것을 알 수 있었고, 테이핑의 모양에 따라 요추 테이핑이 척추기립근 테이핑보다 긍정적인 영향이 있었다. 따라서 테이핑요법은 버스 운전자에게 값비싼 장비 없이도 쉽게 지속적으로 적용 가능한 치료 방법으로 사용할 수 있으리라 생각된다.

요부안정화운동이 요부불안정성을 가진 산업체 만성요통환자의 통증관련 기능에 미치는 효과 (The Effects of Lumbar Stabilization Exercise on Pain-related Functions of Industrial Workers with Chronic Low Back Pain and Lumbar Instability)

  • 본 연구의 목적은 요부안정화운동이 요부 불안정성을 가진 산업체 만성요통환자의 통증관련 기능에 미치는 효과를 알아보고자 하였다. 산업체 만성요통환자 20명을 대상으로 대조군과 실험군으로 10명씩 할당하여 신체기능은 배근력, 유연성, 균형능력을 측정하였고, 통증은 visual analog scale(VAS)를 이용하였다. 연구결과 두군 모두 운동 후 배근력과 균형능력은 유의하게 향상되었고, 통증은 유의하게 감소되었다. 군간 비교에서는 실험군이 대조군에 비해 균형능력이 유의하게 향상되었고, 통증은 유의하게 감소되었다. 결론적으로 요부안정화운동은 요부 불안정성을 가진 산업체 만성요통환자의 균형능력을 향상시키고 통증 감소에 긍정적인 영향을 미친다.

만성요통 여성노인에 대한 요부강화 프로그램의 효과 (Effects of a Strengthening Program for Lower Back in Older Women with Chronic Low Back Pain)

  • Purpose: The purpose of this study was to identify effects of a strengthening program for the lower back in older women with chronic low back pain. Methods: The research design was a nonequivalent control group pretest-posttest experiment. The experimental group consisted of 16 older women and the control group, 14, all of whom had experienced low back pain for at least 3 months. The strengthening program for the lower back included lumbar stabilization exercises and education on pain management in daily living. For an 8 week period, exercises were done 3 days a week and on one day education was also given. Results: Pain and disability scores decreased significantly in the experimental group compared to the control group. Flexibility, life satisfaction and lumbar muscle strength scores increased significantly in the experimental group compared to the control group. Conclusion: Low back pain and disability can be relieved, and flexibility, muscle strength, and life satisfaction increased through a program to strengthen the lower back. It is suggested that a program to strengthen the lower back would be an effective nursing intervention for older women with low back pain.

쭈그려 앉기의 평가척도에 대한 연구 (Study for Squat Scoring Scale)

  • The purposes of this study are to develop 'Squat Scoring Scale' and identify relationship between squatting score and flexibility of the lower extremity muscles such as quadriceps femoris and soleus in terms of relationship between squatting and functions of lower extremity. Subjects have normal gait patterns only with or without musculoskeletal problems of lower extremities, lumbar and pelvis and gender. There are sixty subjects who participated in this study. The scoring are checked after squatting posture according to 'Squat Scoring Scale', and measured ROM of the ankle dorsiflexion and knee flexion for flexibility of the soleus and quadriceps femoris, respectively. Fourteen of sixty subjects have 10 score, seventeen subjects have 9 score, seven subjects have 8, 7 and 6 scores, three subjects have 5 score, and five subjects have 4 score. The gap between 10 to 9, 9 to 8 and 8 to 7 score in all values of angles has the statistical significance for t-test(p<.05). This information indicates that flexibility of quadriceps femoris and soleus acts as important factors in squatting.

Acute effect of self-myofascial release using a foam roller on the plantar fascia on hamstring and lumbar spine superficial back line flexibility

  • Objective: The purpose of this study was to investigate the immediate effect of applying self-myofascial release (SMR) to the plantar fascia using a foam roller on hamstring and lumbar spine superficial back line (SBL). Design: Randomized controlled trial. Methods: Thirty-one healthy adults agreed to the method and purpose of the study. Selection and exclusion criteria were screened, and baseline measurements for the Toe Touch test and passive straight leg raise (PSLR) test were obtained. The participants were then randomly assigned to the SMR group or the sham group. After group assignment, the SMR group rolled the surface of the foot from the heel to the metatarsal head using a foam roller for 5 minutes. The sham group received passive mobilization of the ankle joint in the supine position. Afterwards, the Toe Touch test and the passive straight leg-raise test were re-assessed. Results: In the SMR group, the Toe Touch test results showed significant improvement (p<0.05). Left and right PSLR test results showed a significant increase (p<0.05). In the sham group, there was no significant difference between pre and post-test results. The SMR group showed a significant difference in the PSLR test and Toe Touch test compared to the sham group (p<0.05). Conclusions: The results of this study showed that SMR on the plantar fascia was immediately effective for improving the flexibility of the SBL of the lumbar spine and hamstring.
