Purpose: The purpose of this study was to explore the effects of the lower limb muscle strength exercise program during hemodialysis on the leg strength and falls efficacy (fall-related self-efficacy) of hemodialysis patients. Methods: The study was designed to be a quasi-experimental, nonequivalent control group pre and post test design. A total of 42 patients participated in the study. We applied lower limb muscle strength exercise program to the experimental group in every hemodialysis three times a week for 8 weeks. Data were analyzed using ${\chi}^2$ test, fisher's exact test, t-test and repeated measures ANOVA. Results: The mean age was 58 years old (t=-1.54, p=.132), the mean hemodialysis period was 67 months (t=1.949, p=.058) and there was no significant difference of dependent variable (t=1.17, p=.251)(t=-.89, p=.381) between the two groups before the experiment. After the experiment, leg strength was significantly improved in the experimental group compared to that of the control group (F=6.63, p=.004). However, falls efficacy was not significantly different between the experimental group and the control group (F=2.33, p=.104). Conclusion: The study results indicated that the lower limb muscle strength exercise program during hemodialysis may improve leg strength and potentially to prevent falls for hemodialysis patients. Further studies are warranted in which larger number of participants and longer duration of intervention are recommended.
PURPOSE: The purpose of this study was to determine the effect of corrective exercise on hip joint range of motion, lower limb alignment, radiographs of hip and knee joints, and quadriceps muscle activity in a patient with knee joint valgus deformity. METHODS: A single-subject A-B-A experimental design was used to determine the effects of therapeutic exercise. The single-subject was a 27-year-old male, who presented with knee joint valgus deformity. Corrective exercise program was performed for 40 min/day twice a week for 12-week. Range of motion (hip flexion, extension, abduction, adduction, internal rotation, and external rotation), lower limb alignment test (Q-angle, rear foot alignment, and leg length), standing anterior-posterior radiographs (neck shaft angle and knee joint space), and quadriceps muscle activity of both lower limbs were measured before (A 1), after 6 weeks (B 1) and after 12-weeks (B 2) of corrective exercise and after 6 weeks of exercise completion (A 2). RESULTS: Hip range of motion increased in all directions of both sides at B 2 and A 2 compared to at A 1. Q-angle of both side and leg length discrepancy decreased at B 2 and A 2 compared to at A 1. Neck shaft angle and knee joint space of both sides improve at B 2 and A 2 compared to at A 1. Quadriceps of both side muscle activities improved at B 2 and A 2 compared to at A 1. CONCLUSION: We demonstrated that corrective exercise increases range of motion, and improves lower limb alignment and muscle activity in a patient with knee joint valgus deformity.

The physical restoration technology for lower limb amputees is being advanced as the biomechatronics is being applied to the area of rehabilitation. As the advanced prosthetics for lower limb amputees are introduced, a suitable prescription of biomechanical rehabilitation training becomes important to utilize the advanced full features of the devices. Since lower limb amputation significantly affects biomechanical balance of mosculoskeletal system for gait, an appropriate and optimal biomechanical training and exercise should be provided to rebalance the system before wearing the prostheses. Particularly, biomechanical muscular training for hip movements in the both affected and sound lower limbs is important to achieve a normal-like ambulation. However, there is no study to understand the effect of hip muscle strength on the gait performance of lower limb amputees. To understand the hip muscle strength characteristics for normal and amputated subjects, the isokinetic exercises for various ratios of concentric contraction to eccentric contraction were performed for hip flexion-extension and adduction-abduction. As a results. biomechanical isokinetic training protocols and performance measurement methodologies for lower limb amputees were developed in this study. Using the protocols and measurement methods, it has been understood that the appropriate and optimal biomechanical prescription for the rehabilitation process for lower limb amputees is important for restoring their gait ability
Background: This study investigated effective posture for gluteus medius rehabilitation training and effects of isometric muscle activity by electrophysiology through EMG while performing dynamic isotonic behavior of weight placed differently on upper limbs. Method: 16 healthy male subjects 20 to 29 years of age volunteered for the study. Lateral stabilizer right gluteus medius activity was assessed using EMG while the right lower extremity maintains single limb support, and the left upper extremity elevation movement maintains 5 seconds without load, 1RM to 1 repetition, 5RM to 5 times, 10RM to 10 times, 5RM and 10RM maintain 5sec. Results: Comparison of the mean value of EMG data showed a statistically more significant difference in upper extremity elevation movement on opposite upper extremity added weight than one that was not added on a single limb weight bearing posture(p>.05). Weight supported side gluteus medius activity for 1RM, 5RM, 10RM weight difference and movement repetition did not differ(p>.05). Comparison in maximum value showed statistically significant differences in not adding weight on upper limb elevation exercise and 1RM, 5RM, 10RM repeated behavior. Elevation behavior and repetition appeared over 70% of MVIC. Conclusion: Unilateral weight bearing stance added weight in the opposite upper limb elevation movement was an indirect exercise to effectively stimulate gluteus medius activity. Applying various added weight will have effective exercise on the early stages of rehabilitation because activity gluteus medius did not differ through added weight.
Purpose : The purpose of this study was to research the effects on resistance CKC & OCK exercise method on lower limb muscle activity, in chronic stroke patients. Method : In this study, 18 patients with stroke caused by hemorrhage or infarction were participated. resistance exercise method was conducted in tow different group : one is a close kinetic chain exercise(CKC) group and the other is an open kinetic chain exercise(OKC) group. Under the researcher's guidance, exercise for CKC and OKC group carried out 3 times a week for 30 minutes during 4weeks. Result : There was significantly different for the vastus lateralis, tibialis anterior and gastrocnemius muscle activity within the intervention period both group. The vastus lateralis, tibialis anterior and gastrocnemius muscle activity was significantly increased within the CKC group. The vastus leteralis and tibialis anterior muscle activity was significantly increased within the OKC group. Conclusion : It was confirmed in this study that the CKC exercise was more effective than OKC exercise in improving lower limb muscle activity of chronic stroke patients. This study seggested that CKC may be suitable for individuals with a chronic stroke. furthermore study should be made a lot of researches regarding in other method and varying conditions for many hemiplegic patients.
Purpose: To examine the effects of a fall prevention exercise program on the community-dwelling elderly. Methods: The nonequivalent control group pretest-posttest design involved 16. subjects in the experimental group and 17 in the control group. The experimental group received the fall prevention exercise program for 50 minutes, three times each week for 12 weeks. Results: After program participation, the experimental group of subjects showed significantly higher lower limb strength higher endurance, and higher balance than the control group of subjects. The danger of being injured in a fall was also significantly lower in the experimental group. However, there were no significant differences in body constituent factors, agility, and flexibility between the two groups after the intervention. Conclusion: The 12 week fall prevention exercise program was effective in increasing lower limb muscular strength, endurance, balance, and body strength, and in decreasing the danger status of fall injuries. These results suggest that this fall prevention exercise program could be utilized as an effective nursing intervention modality in elderly persons.
PURPOSE: This study was examined the effects of aerobic exercise using a step box and lower extremity strengthening exercise with an elastic band on the balance ability and lower extremity muscular strength in elderly subjects. METHODS: Forty-one healthy adults were randomly divided into experimental group 1 (Exp 1, n=14), experimental group 2 (Exp 2, n=14), and a control group (n=13). The Exp 1 conducted lower limb strengthening exercise using an elastic band and the Exp 2 performed aerobic exercise with a step box 50 minutes per day, twice per week, for eight weeks. RESULTS: There was significant increase in the Exp 1 and Exp 2 in comparison of the change of lower limb muscle strength according to measurement time (p<.05). But there was no significant difference according to intervention methods. In one-leg standing test change among the three groups after the intervention, the Exp 1 and Exp 2 saw significant improvement compared to the control group (p<.05) but there was no significant difference according to intervention methods. The change of the functional reaching test results, the Exp 1 and Exp 2 saw significant improvement according to the time of measurement. In the change of the Timed Up and Go test, there was significant improvement in the Exp 1 and Exp 2 compared to the control group (p<.01), but there was no significant difference according to intervention methods. CONCLUSION: Exercise to lower extremity strengthening program accompanied with aerobic exercise is considered more effective in dynamical balance and ability to walk.
Purpose: The purpose of this study was to determine whether walking exercise improved physical function in elderly people using meta-analysis. Methods: Medical and nursing literature databases were searched to identify the studies on the effectiveness of walking exercise on physical function. In the databases, there were 16 articles reporting 21 interventions. Overall effect sizes for three outcome variables, elders' physical function in lower limb muscle endurance, whole body endurance and upper body flexibility, were calculated. Effects of study characteristics on outcome variables were analyzed. Results: The meta-analysis showed that walking exercise generally had positive effects on CST (chair stand test), 6MW (6 min walking), and SRT (standing or sitting reach test) with overall weighted effect sizes of 1.06, 0.41 and 0.29 respectively. This study also showed that the chronic disease status of the elders, intervention methods, and type of residence had different effects on CST, 6MW and SRT. Conclusion: The results indicate that walking exercise improves physical function in elders. Walking exercise which can be done at any time and any location is indeed a very effective exercise for elderly people.
