하지절단자의 보행 복원을 위한 단단부 잔존근육의 생체역학적 등속성 운동 효과에 대한 연구 (The effect of biomechanical isokinetic excercise of residual muscles in the stump on restoring gait of transfemoral and transtibial amputees)

  • The physical restoration technology for lower limb amputees is being advanced as the biomechatronics is being applied to the area of rehabilitation. As the advanced prosthetics for lower limb amputees are introduced, a suitable prescription of biomechanical rehabilitation training becomes important to utilize the advanced full features of the devices. Since lower limb amputation significantly affects biomechanical balance of mosculoskeletal system for gait, an appropriate and optimal biomechanical training and exercise should be provided to rebalance the system before wearing the prostheses. Particularly, biomechanical muscular training for hip movements in the both affected and sound lower limbs is important to achieve a normal-like ambulation. However, there is no study to understand the effect of hip muscle strength on the gait performance of lower limb amputees. To understand the hip muscle strength characteristics for normal and amputated subjects, the isokinetic exercises for various ratios of concentric contraction to eccentric contraction were performed for hip flexion-extension and adduction-abduction. As a results. biomechanical isokinetic training protocols and performance measurement methodologies for lower limb amputees were developed in this study. Using the protocols and measurement methods, it has been understood that the appropriate and optimal biomechanical prescription for the rehabilitation process for lower limb amputees is important for restoring their gait ability

Relationships Between the Transfemoral Socket Interface Pressure and Myoelectric Signal of Residual Limb During Gait

  • The biomechanical interaction between the stump and the prosthetic socket is critically important to achieve close-to-normal ambulation. Many investigators suggested that the pressure changes during gait of transfemoral amputees are closely related to the prosthetic alignment, the socket shape, the stump size, and the residual muscle activity. The effects of the prosthetic alignment, the socket shape, and the stump size on the interface pressure were investigated previously. However, there is no report how the residual muscle activities in the transfemoral stump affect the socket interface pressure characteristics during gait. Since designs of socket fur lower limb amputees need to consider the socket interface pressure characteristics, the interface pressure patterns by the residual muscle activities during gait should be investigated. In this study, myoelectric signals (MES) and socket interface pressure in residual limb of transfemoral amputees were measured during the stance and swing phases of gait. For the purpose, specially designed quadrilateral sockets that MES electrodes could be instrumented were fabricated. A total of two transfemoral amputees were participated in the experiments. The measured temporal MES amplitude and interface pressure in knee flexor (biceps femoris) and extensor (rectus femoris) had significant correlations (P < 0.05). Based on the test results, It was suggested that the residual muscle activity of transfemoral amputees stump is an important factor affecting socket pressure changes during walk.

대퇴 절단 장애인을 위한 4절 링크 구조의 전자 제어식 무릎형 의족 (Development of an Electronically Controlled Knee-Type Prosthetic Leg with a 4-Bar Linkage Structure for Lower Limb Amputee)

  • Lower limb amputees are increasing due to various reasons. It is difficult for lower limb amputees to walk without an assistive device such as a prosthetic leg. In this paper, an electronically controlled knee-type prosthetic leg with a 4-bar linkage structure for lower limb amputees was developed. The knee-type prosthetic leg has a 4-bar linkage structure and assists walking by using an integrated drive module. The torque is 90 Nm, the rotation speed is up to 120 deg, and it weight 1.9 kg, so it is lighter than a commercial prosthetic leg, so it can be used for a long time because there is less fatigue when walking. An integrated control board was developed by applying various sensors and microprocessor. The motor drive and encoder are built into the integrated drive module. The integrated control board and integrated drive module communicate using CAN. When a lower limb amputee wears a knee-type prosthetic leg and walks, it shows a shape similar to the swing phase graph of a normal people, and it is possible to walk naturally while walking.

대퇴절단자와 정상인 걸음걸이의 운동학적 요인과 발목관절 강성 비교 (Comparisons of Kinematic Factors and Stiffnesses of the Lower-limb Joints between Transfemoral Amputees and Normal Adults)

  • The purpose of this study was to compare kinematic variables and stiffnesses of ankle joints between normal person and transfemoral amputee gait in order to develop or fit prosthetic leg. Twenty subjects (ten normal persons and ten transfemoral amputees) participated in this experiment, and walked three trials at a self-selected pace. The gait motions were captured with Vicon system and variables were calculated with Visual-3D. The velocity, stride length, stride width, cycle time, double limb support time and right swing time of gaits were statistically significant. Because coefficients of variability of normal persons on velocity, double limb support time and swing time were greater than transfemoral amputees, normal persons controlled these gait variables effectively. The stiffnesses of ankle joints were not statistically significant, but patterns of stiffnesses of ankle joints during three rockers were absolutely different. The negative correlations between stiffnesses of ankle joints and cycle time and swing time were presented. These differences suggest that developing and fitting prosthetic leg were demanded. Further studies should develop fitting program and simulator of prosthetic leg.

외측웨지인솔이 대퇴절단자의 무릎내전모멘트에 미치는 영향 (Biomechanical Effect on Knee Adduction Moment by Lateral Wedge Insole in Transfemoral Amputee)

  • The prevalence of knee osteoarthritis was higher people with lower limb amputation. This was identified that transfemoral amputees have a greater external knee adduction moment than ablebodied subjects by biomechanical studies. Therefore, they need rehabilitative intervention for prevention and reduction of knee osteoarthritis. The purpose of this study was to determine the effect of lateral wedge insole used in the treatment of knee osteoarthritis. This study was participated in fourteen unilateral transfemoral amputees and we were analyzed the difference gait variables between without lateral wedge insole and with $5^{\circ}$ and $10^{\circ}$ lateral wedge insole during gait. Our results showed that step length ratio was more symmetrical and, hip adduction and ankle inversion angle were more close to normal value, and knee adduction moment was decreased as the wedge angle increases. We proposed that these data would be utilized conservative treatment of knee osteoarthritis in lower limb amputees.

하지 절단자를 위한 다축 회전이 가능한 인공발의 개발 (Development of Multi-rotational Prosthetic Foot for Lower Limb Amputee)

  • 두 발을 번갈아 옮기며 몸을 앞으로 움직이는 단순한 동작의 반복으로 보이는 보행이란 과정은 실제 인체 내의 대부분의 근육 및 인대, 뼈가 연관되어 이루어지는 복잡한 행동이다. 신체 일부가 소실된 하퇴 절단자의 경우 생체조직이 온전히 보전되어 있는 일반인과는 달리 남은 생체 부위와 의지의 공학적인 성능에만 의존해 보행을 해야 한다는 어려운 상황에 직면하게 된다. 따라서 하퇴절단자를 위한 하퇴의지는 우선 환자가 기본적인 보행이 가능케 함과 동시에 최대한 복잡한 지면 상태에 적응할 수 있도록 설계가 되어야 한다. 원활한 보행을 위해서는 배/저굴 방향의 발목의 회전이 매우 중요하고 고르지 못한 지면 상태를 극복하기 위해서는 내/외전 방향의 족부의 회전 기능이 요구된다. 최근 생체역학적인 연구를 접목한 다양한 하퇴의지가 개발되고 있으나, 이러한 고가의 고성능 하퇴의지의 경우 주로 활동성이 높은 하지절단 환자들에 초점이 맞춰져 있다. 본 연구에서는 하퇴의지 착용자의 대다수인 활동성이 낮은 K2 레벨의 환자들을 위한 하퇴의지를 개발했다. 기본적인 보행 능력 향상을 위해서 배/저굴 방향으로의 회전이 가능한 유압식 발목을 개발했고, 이를 통해서 배/저굴 방향으로 각각 $2.5^{\circ}$$4^{\circ}$의 회전이 가능함을 확인했다. 또한 수동 조절이 가능한 유압 노즐을 탑재해 환자 개개인에 적합한 발목 회전 저항력을 설정할 수 있도록 설계했다. 내/외전 방향으로의 족부의 회전이 가능하도록 이중고무 방식의 중간재를 삽입해 각각 $3.6^{\circ}$$4.1^{\circ}$의 회전이 가능하도록 설계되어 측경사나 작은 장애물들을 극복할 수 있도록 제작했다. 본 연구를 통해 개발된 하퇴의지는 K2 레벨의 하퇴절단자들이 일상생활 중에 겪게 되는 다양한 지면 환경을 극복하는데 도움을 줄 것이다.

적응적으로 특징과 채널을 선택하는 sEMG 신호기반 보행단계 인식기법 (sEMG Signal based Gait Phase Recognition Method for Selecting Features and Channels Adaptively)

  • 본 논문에서는 다수의 특징 값 중에서 적합한 특징 및 채널을 선택하는 sEMG 신호기반 보행단계 인식기법을 제안한다. 제안하는 방법은 sEMG 신호 기반 분류기를 이용하여 하지 절단 환자의 동력의족을 제어하며, 적응적으로 특징 및 채널들을 선택하여 임베디드 시스템의 신호처리과정에서 발생하는 오버헤드를 감소시킨다. 또한 피험자의 보행 습관에 따라 근육 발달도가 다르다는 특성을 이용하여 피험자의 보행단계에 따라 사용 빈도가 높은 근육과 특징 추출 알고리즘을 선택함으로서 정확도를 향상시킨다. 실험 결과 피험자마다 인식율이 높은 근육이 다르다는 것을 발견하였다. 또한 모든 특징들과 채널들을 이용하는 기존 방법의 경우 50%의 평균정확도를 보인 반면에 제안한 방법은 91%의 평균정확도를 보였다. 따라서 소수의 발달된 근육과 이에 맞는 특징을 사용한 sEMG기반 보행단계인식 방법이 하지절단환자의 동력의족을 제어하는 데 적용될 수 있음을 확인하였다.

집중재활프로그램이 하지 절단 환자의 신체활동 및 의지 착용감에 미치는 영향 -후향적 연구- (Effect of an Intensive Rehabilitation Program on Physical Activity and Wearing Satisfaction in Traumatic Lower Limb Amputees -A Retrospective Study-)

  • Purpose: This study aimed to investigate the effect of an intensively rehabilitation program on the gait, balance, functional performance, and wearing satisfaction of patients with traumatic lower extremity amputations caused by industrial accidents. Methods: In this study, the anonymized electronic medical records of individuals who participated in the intensive rehabilitation program (among those who were admitted to the hospital belonging to the Labor Welfare Corporation) due to an industrial accident from August 2018 to September 2019 were collected. As a result, the records of 12 subjects meeting the screening criteria were analyzed. Results: According to the time of application to the intensive rehabilitation program, the 10 Meter Walk Test (10MWT) (p < 0.01), Berg Balance Scale (BBS) (p < 0.01), Timed Up and Go Test (TUG) (p = 0.01), the L Test of Functional Mobility (L-test) (p < 0.01), Prosthetic Limb Users Survey of Mobility (Plus-M) (p < 0.01), and Houghton Score Question (HSQ) (p < 0.01) values significantly improved with time. Conclusion: This study confirmed the positive effect of an intensive rehabilitation program on the gait, balance, functional performance, and wearing satisfaction of patients with traumatic lower extremity amputations caused by industrial accidents. However, this study was limited by the absence of a control group, and, thus, it highlights the need for more extensive research with a large sample.
