PURPOSE: The purpose of this study was to propose clinical criteria to differentiate patients who are able to perform the step-through-step gait pattern in chronic stroke patients. METHODS: Sixty patients with chronic stroke patients participated this study. To differentiate patients who could perform the step-through-step gait pattern, age, gender, and causes of stroke were noted, a Chedoke-McMaster (CM) damage list, Fugl-Meyer (FM) assessment scales and the Berg Balance Scale (BBS) were determined. A 10 meter gait test and Timed Up and Go (TUG) test were conducted to determine the differences in gait speed and dynamic balance between patients walking with or without canes in the step-through-step gait pattern group. RESULTS: There was no significant statistical difference in age, gender, and stroke type between all subjects. There were significant differences in the CM scale for postural and lower extremities, and FM scale for lower extremities and BBS. The dynamic balance ability and gait speed showed significant differences between the subjects in the step-through-step gait pattern with or without a cane during gait. CONCLUSION: CM and FM scales for the lower extremities and postural control, as well as BBS scales, can be used as criteria to differentiate patients who are able to perform the step-through-step gait pattern. These results can also be used to provide beneficial information to patients that are walking with canes.
This study aimed to evaluate the effect of a weekly circuit-group exercise program on functional performance, flexibility of the trunk and lower extremities, and balance in individuals with chronic stroke who are living in a community. Thirteen community-living individuals (eleven males and two females) with chronic stroke participated in this study. The group exercise program was set up as a weekly circuit application with four stations aiming to strengthen the muscles, increase endurance, improve flexibility, and enhance functional capacity. The average duration of a session was an hour, and the sessions were conducted once a week for six months for a total of 24 sessions. Assessments for functional performance, flexibility of the trunk and lower extremities and balance were conducted four times: before treatment, after one month of treatment, after four months of treatment, and after six months of treatment. There were significant improvements in functional performance and flexibility of the trunk and lower extremities in the given time frame. No significant improvement was observed in balance ability. The findings suggest that a weekly circuit-group exercise program has some benefits in terms of managing the physical symptoms of individuals with mobility problems after stroke. Therefore, this program can be adapted and employed as a community-based rehabilitation program for such patients. Further studies with various community-based treatments will be conducted to validate these findings.

Prolotherapy, the technology for strengthening lax ligaments, has found increased acceptance in recent years. Prolotherapy involves the injection of the irritant solutions into the weakened or stretched ligaments which are a source of chronic pain. This induces an inflammatory response which mimics the normal repair sequence. Various musculoskeletal disorders of lower extremities such as degenerative arthrtitis, ligament and tendon injuries can be treated with prolotherapy. Prolotherapy is a very good, powerful nonsurgical conservative treatment in various musculoskeletal disorders. Ultrasound exam is very useful to find and confirm the exact location of disorders and check the progress in prolotherapy practices.
Cutting movements frequently occur in sports and influence much Lower Extremity injuries. The purpose of this study was to compare joint motion of lower extremities to cutting angles and running velocities. Seven male subjects performed cutting movements to three angles($0^{\circ}$, $30^{\circ}$, $60^{\circ}$). Subjects were instructed to run five meters at a speed of 2.5m/s and 4.5m/s before contacting their right foot on the force plate and then change direction to the left. The Peak hip, knee and ankle joint kinematics were influenced according to the running velocities and cutting angles. In conclusion, Fast running velocity and cutting angle will may influence on the lower extremity joint instability on real game situation.

Background: Peripheral arterial disease is frequently accompanied with systemic arteriosclerosis and more than half of the cause of deaths is due to the development of coronary arterial disease. Moreover, it is known that the most frequent cause of death after a bypass surgery of chronic arterial obstruction is heart related complications. Especially in patients with atherosclerotic arterial obstruction in the lower extremities who had no history of heart disease or had no presenting symptoms of ischemic heart disease showed a high rate of postoperative mortality and for this reason we suggest preoperative evaluation in these patients to evaluate whether or not coronary arterial disease is accompanied. Material and Method: Since Feb. 2001 to Oct. 2004, we analyzed 52 patients who were operated on for atherosclerotic arterial obstruction in the lower extremities, with the exception of patients with a past history of heart disease or symptoms of ischemic heart disease. They underwent on the same day a coronary and femoral angiography for evaluation of accompanying coronary arterial disease. Of among these patients, we compared those who received bypass surgery of the arteries of the peripheral extremities alone to those who underwent combined coronary artery bypass surgery. Result: 63% of the reported cases of atherosclerotic arterial obstruction in the lower extremities were accompanied with coronary arterial disease. Old age, hypertension, diabetes mellitus, smoking, and hypercholesterolemia are known risk factors for arteriosclerosis and of these, only old age and hypertension had statistically significance in patients with severe atherosclerotic arterial obstruction in the lower extremities accompanied with coronary arterial disease. Diabetes, smoking, and hypercholesterolemia showed no statistical significance in this group. With the increase in severity of the range and the degree of atherosclerotic arterial obstruction, coronary arterial disease is frequently accompanied and its severity also increased. Patients who received both peripheral artery and coronary artery bypass surgery showed no difference in the period of hospitalization and ICU stay period compared with patients who received bypass surgery of the arteries of the lower extremities alone. Conclusion: Patients with atherosclerotic arterial obstruction in the lower extremities without symptoms of ischem to evaluate coronary arterial disease for active treatment, especially in the patients with old age, hypertension and high AVD scores.
Purpose: The purpose of this study was to identify the effects of Ankle Pump Exercise on lower limb edema and pain of operating room nurses. Methods: This research was arranged on the basis of the nonequivalent control group pre-post test design. The participants were 41 nurses working in the operating room in 'S' hospital in Seoul and 'H' hospital in Guri City, Korea. Data were collected from August 20 to September 30, 2012. Edema of the lower extremities was measured in terms of the girths of ankles and calves. Pain of the lower extremities was measured in terms of VAS (Visual Analogue Scale) for pain. The data were analyzed by frequency, percentage mean, and standard deviation, using the chi2(p) and t-test, repeated measures ANOVA, and a paired t-test. The SPSS Win 20.0 program was used for analysis. Results: Hypothesis 1 - "There are significant differences in lower extremity edema between the experimental group and the control group." - was partially supported. Hypothesis 2 - "There are significant differences in lower extremity pain between the experimental group and the control group." - was supported. Conclusion: The results of this study demonstrate positive practical effects of the Ankle Pump Exercise in reducing the circumference of both ankles, reducing the circumferential edema of both calves, and alleviating the pain of nurses in the operating room. Based on these findings, application of the Ankle Pump Exercise can be considered as an effective method to prevent nurses' health problems in the lower extremities that can be induced by their work during surgical operations.

With an increasing aging population and improved standards of living, more attention has been paid to health. Although walking exercise is known as an aerobic exercise, it imposes repeated and continuous impacts on the joints of the lower extremities. Therefore, when overweight gives a burden to the lower extremity or there is a joint disease, exercise limit occurs. The articular cartilage, weakened with age, also makes it difficult for the elderly to perform walking exercises. Accordingly, this study conducted a comparative analysis between regular walking using only the lower extremity and Nordic walking, which has been known as stable. For analysis, electromyography (EMG) was performed and the ground reaction force of the upper and lower extremities were measured in the same exercise. Integrated EMG (iEMG) revealed that the upper extremity muscles were more active in Nordic walking than in regular walking, where lower extremity muscles were relatively more inactive. In addition, when EMG measurements were performed at each measurement point during walking exercise, the pattern was different. Nevertheless, the result was the same as in iEMG. The load that occurs in each exercise was measured using the ground reaction force system. As a result, Nordic walking had a lower load than regular walking. Therefore, it was found that Nordic walking minimized the load on the lower extremities owing to the effect of whole-body exercise and was a safer and more efficient exercise method.

The purposes of this study were to determine the effects of rehabilitation programs on functional performance of the lower extremities and whether additional therapeutic exercise with routine rehabilitative therapy improves functional performance more than just routine rehabilitative therapy by it self for inpatients who have suffered a stroke with below moderate severity within 3 to 6 months after the onset of the stroke. Fifty-eight subjects were divided into two groups. Group I was given routine rehabilitative therapy and group II was given additional therapeutic exercise along with the routine rehabilitative therapy. Each group received 6 weeks of rehabilitation. The timed get-up and go test (TUG), the Fugl-Meyer score (FMS), functional independence measure (FIM), functional reach (FR), gait velocity (GV), and the strength of knee extensor and flexor were selected to measure effect of rehabilitation programs. The main results were measured and analysed at baseline, 3 weeks, and 6 weeks after the start of the rehabilitation programs. The results revealed that all of functional performance of the lower extremities in 3 weeks after the start of the rehabilitation programs were significantly improved compared with before the rehabilitation programs in both groups. In 6 weeks, TUG, FIM, FR, GV, and the strength of knee extensor in group I, TUG, FMS, FIM, FR, GV, and the strength of knee extensor and flexor in group II were significantly improved compared with the results after 3 weeks. At 3 weeks after rehabilitation programs, group II made significantly greater gains in TUG, FR, GV, and the strength of the knee flexor compared to the group I. At 6 weeks, group II made significantly greater gains in TUG, FR, GV, and the strength of the knee extensor and flexor compared to the group I. In conclusion, rehabilitation programs for stroke patients within 3 to 6 months after stroke onset significantly contributed to improve functional performance of the lower extremities. It is desirable for improvement of functional performance of the lower extremities to perform additional exercise with routine rehabilitative therapy.
Purpose: To identify age, gender, medication, seasons and place of fall, and areas of the fractures from the fall among the hospitalized elderly patients in order to provide the basic data for future fall prevention program for the elderly. Methods: This study was conducted for 106 elderly patients admitted into a university hospital by fractures from the fall during the period from January 1, 1999 to December 31, 1999. Data on the age, gender, medication, season and place of the fall, areas of the fracture were collected based on their medical records. Result: The age range of the subjects were from 60 to 96 years old. The subjects were aged between 60-69 years old 49(46.2%), between 70-79 years old 31(29.2%), between 80-89 years old 24(22.6%), and over 90 years old 2(1.9%). Male patients comprised was 34(28.3%), while female patients comprised 76(71.7%). The fall occurred in Winter most frequently 34(32%). The place of the fall included room 81(76.4%), streets 13(12.3%), bathroom 6(5.7%), stair 4(3.8%), and mountain 2(1.9%). Twenty-two subjects (20.8%) had medication regularly, while 84 subjects (79.2%) had no medication. The areas of the fracture from the fall included upper extremities 20(18.9%) and lower extremities 86(81.1%). Radius fracture (7.5%) was the area where the fracture occurred most frequently in upper extremities and femur fracture (52.8%) was the area where the fracture occurred most frequently in lower extremities. A significant difference was found in the fracture area by age, season and place of the fall (p<.05). No significant difference was found in the fracture area by gender and medication. In all age groups, seasons and places of the fall, occurrence of fracture in lower extremity was significantly higher than that in upper extremity.

PURPOSE: The purpose of this study was to compare the lower extremities muscle activation between squatting exercise with gym ball and wall for improving muscle strengthening in lower extremities. METHODS: Participants were 21 university students (males 10, females 11) who didn't have any problem with orthopedic surgery. Participants performed squatting exercise with gym ball and wall. Squatting exercise with gym ball were performed using by gym ball behind back, and the gym ball were fixed in back and wall. We asked participants to push back the gym ball slightly to prevent fall of ball. Wall squatting exercise, we ask participants to contact their back in wall slightly in order to prevent trunk flexion during performed squatting exercise. Each squatting exercise had performed until knee joint were flexed at 60 degree, and maintained five seconds. We collected data from E.M.G of Biceps femoris, Gastrocnemius, Vastus medialis and lateralis, Tibialis anterior of lower extremity in isometric phase of knee joint angle 60 degree of each squatting exercise. We analysed data using by ANOVA and independent t-test of SPSS PC ver.20.0 in order to compare the muscle activation between squatting exercise with gym ball and wall. RESULT: All of lower extremities muscle activation showed more higher value in squatting exercise with gym ball than squatting exercise with wall, especially there was significantly difference of muscle activation in vastus medialis, tibialis anterior between squatting exercise with gymball and with wall. CONCLUSION: On comprehensively considering the results of the present study, we suggested that squatting exercise with gym ball was more effective method improving lower extremity muscle strengthening.
