  • It is important to understand the characteristics of amputee gait to develop more advanced prostheses. The aim of this study was quantitatively to analyze the stair climbing task for the above-knee amputee with a prosthesis and to predict muscle forces and joint moments at musculoskeletal joints by dynamic analysis. The three-dimensional musculoskeletal model of lower extremities was constructed by gait analysis and transformation software for one above-knee amputee and ten healthy people. The measured ground reaction forces and kinematical data of each joint by gait analysis were used as input data during inverse dynamic analysis. Lastly, dynamic analysis of above-knee amputee during stair climbing were performed using musculoskeletal models. The results showed that summed muscle farces of hip extensor of amputated leg were greater than those of sound leg but the opposite results were revealed at hip abductor and knee flexor of amputated leg. We could also find that the higher moments at hip and knee joint of sound leg were needed to overcome the flexion moment caused by body weight and amputated leg. In conclusion, dynamic analysis using musculoskeletal models may be a useful mean to predict muscle forces and joint moments for specific motion tasks related to rehacilitation therapy..

  • Understanding the characteristics of amputee gait is key in developing more advanced prostheses. The aim of this study was to quantitatively analyze a stair-climbing task for transfemoral amputees with a prosthesis and to predict the muscle forces and joint moments at musculoskeletal joints using a dynamic analysis. A three-dimensional musculoskeletal model of the lower extremities was constructed from a gait analysis using transformation software for two transfemoral amputees and ten healthy people. The measured ground reaction forces and kinematical data of each joint from the gait analysis were used as input data for an inverse dynamic analysis. Dynamic analyses of an transfemoral amputee climbing stairs were performed using musculoskeletal models. The results showed that the summed muscle forces of the hip extensor of an amputated leg were greater than those of a sound leg. The opposite was true at the hip abductor and knee flexor of an amputated leg. We also found that higher moments at the hip and knee joints of the sound leg were required to overcome the flexion moment caused by the body weight and amputated leg. Dynamic analyses using musculoskeletal models may be a useful means to predict muscle forces and joint moments for specific motion tasks related to rehabilitation therapy.

  • This study analyzes the characteristics of two different kinds of squat exercise through physical experiments and a computer simulation, i.e. one with a free weight and the other with a Smith machine are studied. This study also proposes a new design for the Smith machine, which has both the advantages of each type based on the results of the analysis. The muscle force and level of stimulation of the lower extremities during squatting were calculated by running an inverse dynamics analysis program on a musculoskeletal model together with the measured motion data. The calculated results were verified by comparing with the measured EMG data. The analysis showed that squatting using free weight is more effective than squatting using the Smith machine. Meanwhile, in order to design an improved Smith machine, which is the final goal of this study, the trajectory of the barbell of the subjects during free weight squatting was measured on the sagittal plane. The measurement showed that the average slope of the trajectory of the barbell is tilted backward by $10.7^{\circ}$. Based on this measurement, this study proposes a tilted design for an improved Smith machine.

  • PURPOSE: This study was to investigate the effectiveness of TENS on balance in stroke patients by analyzing some components such as foot pressure, limit of stability and velocity sway after providing somatosensroy input using TENS. METHODS: Twenty five subjects participated and were randomly divided into two groups, TENS group (n=13) and control group (n=12) by the computer program. Interventions were given to subjects 5 days a week for four weeks. TENS group were treated with TENS for 60 minutes in addition to the conventional therapy which included 30-minute exercise and rehabilitation ergometer training for 15 minutes. Control group performed only conventional therapy. TENS was applied on the skin of soleus, tibialis anterior, tensor fascia latae and vastus medialis in affected side. Foot pressure, limit of stability and velocity sway for balance test were measured using Biorescue. RESULTS: TENS group was significantly increased limit of stability and foot pressure in affected side more than control group. And in eye closed condition, TENS group was significantly decreased velocity sway more than control group. CONCLUSION: The application of TENS is effective to improve the somatosensory input of affected side and to increase the motor function and balance ability.

  • The purpose of this study was to compare and biomechanical evaluate the effects of three varying functional insoles on the kinematics of the lower extremities and foot pressure distribution during gait. For this 12 subjects participated in this study and each worn the 3 functional insoles during gait which kinematics, kinetics, electromyography and foot pressures were recorded. The function on the first insole was to absorb shock and increase the dynamic stability, the second was a gel type to absorb shock, and the third was to massage the center regions of the foot sole. the results were as follows; the first insole reduced the joints range of motion and reduced muscular fatigue, the second insole reduce the maximum, total and average foot pressures. Finally, the third insole produced larger values for the contact times and contact area.

  • This is a case report of successful surgical correction of coarctation of the aorta associated with the patent ductus arteriosus and the persistent left superior vena cava. The patient was a 15 year old girl and congenital heart anomaly was suspected at the sixth month after birth. Afterward there has been no embarrassing symptoms until the day of operation except slight dyspnea on exertion, The diagnosis of coarctation of the aorta and the patent ductus arteriesus detected by physical signs and X-ray studies including aortography. In the first place, coarctation of the aorta was corrected with following procedure: partial resections of the aortic wall just above and below the coarctating line of the aorta and removal of diaphragmatic structure of coarctation performed by. cross clamping aorta above and below coarctation, and then the defect of the aortic wall was closed by lateral aortorrhapic suture with atraumatic needle 3-0 silk continuously [Fig. 6]. In the second place, the patent ductus arteriosus was closed with double ligation method. The persistent left superior vena cava left as it has been, because there has been no evidence of hemodynamic abnormal shunt. After operation, excellent result was obtained; blood pressure in the upper and lower extremities was normalized and subjective complains of hypertension in the upper extremity was disappeared.

  • Eruptive vellus hair cyst is a rare lesion of the vellus hair follicles as a result of developmental abnormality. This cyst is usually found in children, adolescents, and young adults most commonly involving the chest, upper and lower extremities, and abdomen. Herein, we introduce a 22-year-old male patient with a mass of nasal root, growing since childhood. The mass has grown over the years, causing the protruding of the nasal root contours, leading to decline in the patient's aesthetic and mental quality of life. In response, we performed complete resection of the mass, which pathologically was a vellus hair cyst. The patient is now satisfied with his appearance and there was no local recurrence during follow up.

  • Russian rates for melanoma incidence and mortality are relatively low as compared to some other white populations but the tumor is of increasing importance. In this paper, data are based on a retrospective descriptive analysis of melanoma epidemiology and clinicopathological characteristics in Krasnoyarsk Territory belonging to the Siberian Federal District of the Russian Federation. The age-adjusted incidence and mortality rates for the period 1996-2009 were determined with subsequent retrospective analysis of clinicopathological data of 103 primary melanoma cases. Our results showed that incidence and mortality rates in the region under consideration match the Russian national trends and correspond to epidemiological data of the countries of Eastern Europe. Stratification of melanoma cases by age, sex, clinicopathological state and localization revealed a prevalence of lesions on the trunk and lower extremities. Most melanomas diagnosed were of superficial spreading type and the third Clark's level of tumor invasion and stage II according to AJCC. In spite of comparatively low rates of incidence and mortality the trend to increase of melanoma cases in the region under consideration obviously calls for more attention and further investigation.
