A Study on Health Service Utilization for the Low Income Elderly in Korea (전국 저소득층노인의 보건의료이용과 영향요인 분석)

  • The purpose of this study was to analyze the relation amongst health status, health care costs. health service utilization among the low income elderly who were 60 years of age or older, earning a half of the average Korean family income. The cross-sectional descriptive survey research we conducted used families randomly sampled nationwide. The data were collected from July 12 to August 7, 1999 and the total sample was 1.259 household members (421 households). These were the major findings. 1. As for the health status. 72.4% of respondents fell ill in the last 1 month; 54% of respondents had chronic disease. 2. As for the health care cost. the cost of hospitalization and the medical treatment were 1.069,000 won and 226.000won. respectively. 59.3% of respondents experienced a burden from the monthly health care expenses. 3. As for the health service utilization for the last 3months. 28.5% of respondents didn't utilize the health service. In addition, 22.2% of respondents gave up a medical treatment because of economic situation (88.8%). 4. The statistically significant determinants of health service utilization are old age, female, living with a spouse, unemployed state, medicare, and more days sick. 5. It is shown conclusively that equity and efficacy of the health care policy are to be considered for lower income older adult.

Needs for the Planning of Group Homes for the Elderly with Dementia of Lower Income Classes (저소득층의 치매노인 그룹홈 계획에 대한 요구)

  • This study aimed to examine the needs for the planning of group homes for the elderly with dementia in lower income class, who have more economic burden on the caring the demented aged. The survey was conducted by questionnaires collected from 300 respondents from December 2000 to March 2001. Methods of analysis were frequency distribution, mean and chi-square test. The results of this study were as follows: 1) The respondents perceived that the responsibility to take care of the elderly with dementia belonged to their family, not to society or the government, and the sons and daughters had to share the burden placed on care-givers. 2) The awareness of group homes for dementia was relatively low. 3) The respondents' needs for the group homes with dementia could be summarized as follows; small scale plan with homelike atmosphere, a total of 6-8 residents, structural type of detached house or three-storied town house, 2-3 persons per individual room, and management system by non-profit organization or the government. There were no big differences between the respondents of this study and upper and middle class studies conducted by other research, except for some details. Referring to results of this study, it is asserted that group homes for the elderly with dementia would be embraced throughout the country if poetical assistance combined with economic support were provided.

Life Satisfaction, Activities of Daily Living, Depression and Health Behavior of Low Income Elderly Living at Home (저소득 재가노인의 생활만족도, 일상생활활동, 우울 및 건강행위에 관한 연구)

  • Purpose: This study aimed to identify the relationships among life satisfaction, activities of daily living, depression and health behavior in low income elderly living at home. Methods: Study participants were 455 elderly who were receiving home visit services from the Daegu Regional Office of Patriots and Veteran Affairs. Data were collected through personal interviews using questionnaires from July to August, 2010. Results: Average scores of ADL and IADL were 7.82 out of 21 and 12.67 out of 33 respectively, which indicate relatively independent to everyday life. Mean scores of depression, health behavior and life satisfaction were 8.61 out of 15, 88.14 out of 132 and 48.57 out of 60. There were significant relationships among the variables of life satisfaction, activities of daily living (ADL, IADL), depression and health behavior. A significant factor influencing life satisfaction was health behavior ($\beta$=.134, p=.020). Conclusion: The findings of this study would be a useful information for constructing an intervention program to care for elderly.

  • This study aimed to diagnose the housing conditions of the low income single or couple elderly households. Specifically, this study divided the elderly households into total 8 housing situations according to the three factors of residential area, household composition and income level, and compare the housing satisfaction level by household situation. This study was conducted in the form of the 1:1 interview with structured questionnaire and the field study, targeting the low income single or couple elderly households over 65 years old in urban and rural areas. The 80 collected data was analyzed using the SPSS statistics program. First, the households living in the urban areas and those in the rural areas had different housing conditions. Secondly, there was a clear difference in housing conditions between the single elderly households and couple elderly households. Third, the housing conditions were different depending on the gap in income level. This research is expected to be used as a basic resource to understand their various and differentiated demands and plan the housing environment to properly meet the demand.

Qualitative study on home delivery meal services for the elderly (배달 도시락 수혜노인의 '도시락 밥'에 대한 질적 연구)

  • As the importance of elderly's dietary life have increased, low-income elderly's meal should be taken care by social policies. Nevertheless, there is a lack of systematic management in home-delivery meal service. This study aims to investigate the elderly's experience on home-delivered meal service, especially identifying the symbolic meaning of home-delivered meal service on elderly's dietary life. Using an in-depth interview, qualitative data was collected from five elderly participants who received home-delivered meals. Data was analyzed based on Colaizzi's 6 steps method, deriving 148 significant statements, 18 formulated meanings, and 5 themes: , , , , and . Elderly participants regarded the home delivered meal as not only a decent meal but also felt appreciated. The home-delivered meals were found to be their only meal all day; thus, they considered these meals as a means of survival. Home-delivered meals are considered as a lack of consideration for the elderly's physical condition, including their tastes, portion size, and cooking method. Also, these elderly seemed to lose their appetite due to aging and it caused the enjoyment of eating. The elderly have kept silence never expressing any opinions regarding the home delivered meals because they were offered for free. The results suggested that the home-delivered meal service should be developed with the multilateral evaluation methods to reflect the elderly's needs to improve the quality of the home-delivered meal service.

Study on Relationship between Eye Health and Household Income of the Elderly (노인의 가구 소득과 눈 건강과의 관계)

  • Purpose: In this study, eye-health inequity was investigated by analyzing the relationship between household incomes and eye-health of senior citizens. Further, this study suggested the preliminary data for establishment of public eye-health policy in order to improve low income senior citizens' life quality. Methods: The data from the 2009 Survey of Korea National Health and Nutrition Examination were analyzed in this study. The objectives of the KNHNE survey were over 65 year old group (1,668 people). Main factors of eye-health (visual acuity, cataract, pterygium, intraocular pressure, retinophathy, age-related macular degeneration, diabetic retinophathy, myopia, hyperopia, astigmatism, and anisometropia prevalence) were analyzed with t-test and chi square test. Results: Low income group revealed that refractive error rate and intraocular pressure were low, however, naked eye visual acuity and corrected visual acuity were high at 0.1 to less than 0.5. On the other hands, in the high income group, there was high prevalence of hyperopia. Cataract mainly occurred at low income group besides group which maximum corrected visual acuity was below 0.8 also highly showed cataract. Moreover, the prevalence of cataract showed that it related with smoking, drinking, occupation, and education level. Conclusions: Results revealed that there was inequity of eye-health which related with socioeconomic status of the elderly. Especially, the prevalence of cataract was correlated with life quality. Consequently, establishment of public eye-health policy seems to be required for eye-health inequity of low income senior citizens.

Unmet Healthcare Needs Status and Trend of South Korea in 2021 (2021년 미충족의료율과 추이)

  • Unmet healthcare is an important indicator for measuring accessibility of healthcare services. We analyzed the following four data from a nationally representative sample of South Korean population: Korea Health and Nutrition Examination Survey (KNHANES, 2007-2021), Community Health Survey (CHS, 2008-2021), Korea Health Panel Survey (KHP, 2011-2019), and Korean Welfare Panel Study (KOWEPS, 2006-2021). The proportion of individuals reporting unmet healthcare needs were 6.0% (KNHANES), 5.1% (CHS), and 13.1% (KHP). Annual percentage change (APC) which identifies trend for the follow-up period was -9.4%, -9.4%, and -5.3%, respectively. The proportion of individuals reporting unmet healthcare needs due to cost were 1.2% (KNAHANES), 0.5% (CHS), 2.7% (KHP), and 0.4% (KOWEPS). The APC was -10.4%, -16.1%, -11.5%, and -19.1%, respectively. Compared to the previous year, the rate of unmet healthcare needs decreased slightly, but the rate of unmet health care needs due to cost tended to increase. Overall, higher rates of unmet healthcare needs were reported in the low-income and the elderly population. Although it was confirmed through the APC that the rate of unmet healthcare experience has decreased over the past decade, it can be seen that there is still a disparity by income level and age. These results suggest the need for an appropriate health benefit coverage policy for the low-income and the elderly.

Effects of a Drug Misuse and Abuse Prevention Program on Knowledge, Attitude, and Preventive Behaviors Related to Drug Misuse and Abuse, and Depression in Low-income Elderly Women (약물 오$\cdot$남용 예방프로그램이 저소득 여성노인의 약물 오$\cdot$남용 예방에 대한 지식, 태도, 예방행위 및 우울에 미치는 효과)

  • Purpose: This study was conducted to determine the effects of a drug misuse and abuse prevention program on knowledge, attitude, and preventive behaviorsrelated to drug misuse and abuse, and depression in low-income elderly women in the urban area. Method: The design of this study was a nonequivalent control group pretest-posttest design. The subjects consisted of 26 in the experimental group and 23 in the control group. The program was performed for about 1 hour, once a week for 5 weeks. Data was analyzed by the SAS(ver.8.02) computer program, and it included descriptive statistics, Fisher's exact test, Mann-Whitney U test, Wilcoxon signed ranks test, and ANCOVA. Result: There were statistically significant differences in knowledge, attitude, and preventive behaviors related to drug misuse and abuse, and depression between the experimental group and the control group. Conclusion: This study showed that this prevention program of drug misuse and abuse is appropriate for low-income elderly women, Therefore this program is recommended as a nursing intervention strategy for the elderly.

A Study on Promoting the Linkage between Hospital Care and Community Health Services for the Low-income Elderly with Chronic Disease (입원·외래서비스 이용경험이 지역사회 보건복지서비스 이용(욕구)에 미치는 영향 : 저소득 만성질환보유 노인을 대상으로)

  • The increased needs for medical cost containment and the quality of life improvement for the low-income elderly with chronic disease require the complementary relationship between hospital care and community health services. This study investigated the current status of the linkage between Korean hospital care and community services using Seoul Welfare Panel Study(SWPS) data. Logistic regression analysis was used to examine the effects of experiences of hospitalization and outpatient health services on use and needs of community health services. The analysis results indicated that the elderly with chronic disease who have not experienced outpatient health services were unlikely to receive health and social services in the community. Even those who have experienced hospitalization were not provided complementary health services in the community despite the increasing demand.

The proposal of urban regeneration methods for deteriorated downtown residential area, considering the development condition of urban blocks - A case study of Kwangju City (쇠퇴한 구도심 주거지의 개발여건별 재활성화 방법 제안 - 광주광역시 구도심을 사례로)

  • The purpose of this study was to suggest urban regeneration methods for deteriorated downtown residential area, considering the development condition of urban blocks. Through the research that are based on literature, field survey, urban planning map and local experts consulting, it found out suitable sites for development such as a deteriorated residential zone or a unused site and it was deduced eight development types from analyzing the characters of developable sites. And then it is suggested various housing forms which were applied to eight development types of developable sites. The consequences of this research are summarized as follows.
