Background and Objectives: Radioactive iodine therapy (RAI) is an important treatment modality of Graves' disease (GD), but there is still not a consensus on the optimal dosage regimen. We studied the treatment success rate of different RAI doses, and examined which clinical markers were useful for determining the optimal RAI dosage for successful therapy in Korean patients. Materials and Methods: We retrospectively studied 123 patients with GD treated with RAI between 2004 and 2014 at Chonnam National University Hwasun Hospital. The responder group was defined as patients who developed hypothyroidism requiring levothyroxine replacement following RAI, regardless of the RAI dosage. Results: A total of 54 patients (43.9%) became hypothyroid after the first dose, and 31 needed two to four additional doses to achieve hypothyroidism. In the responder group as a whole (85 patients), the mean total dose of RAI was $15.5{\pm}7.0mCi$ and the mean thyroid volume (TV) was $35.4{\pm}23.4mL$. When divided into low dose (<15 mCi, n=46) and high dose (${\geq}15mCi$, n=39) responder groups, TV was significantly lower in the low-dose responder group ($25.7{\pm}11.4$ vs. $48.4{\pm}31.3$, p<0.001). The optimal cut-off TV for the low-dose responder group was <32.37 mL (sensitivity 80.9%, specificity 76.7%). Conclusion: TV had significant effects on the outcome of RAI in GD patients. The optimal fixed RAI dose for Korean GD patients with a large goiter (${\geq}33mL$) should be at least 15 mCi to achieve the best outcome.
Firstly, we compared the two staining techniques, Giemsa and Acridine orange, to determine micronuclei on samples of cultures of five healthy human peripheral blood lymphocytes after ${\gamma}-irradiation\;(^{137}Cs)$ in dose ranges of 0 to 800cGy. It was found that the Acridine orange staining method gives more reliable results than the usual Giemsa staining method in micronucleus tests. Moreover, the frequency of micronuclei in cytokinesis-blocked human B-lymphocytes was studied after in vitro irradiation in dose ranges of 0 to 50cGy. After setting and separating the B-lymphocytes, the frequency of radiation-induced micronuclei were observed as the end-point markers for the low-dose radiation dosimetry after staining with Giemsa and Acridine orange dyes. The micronuclei frequency in B-lymphocytes was significantly elevated from 10 to 30cGy ${\gamma}-irradiation$. The determination of micronuclei in B-lymphocytes after staining with Acridine orange was higher than that of Giemsa. The frequency of micronuclei in B-lymphocytes was observed to be at least two times higher than those of T-lymphocytes Giemsa in dose increasing. Therefore, the determination of low-dose radiation-induced micronuclei in B-lymphocytes after staining with Acridine orange is likely to have the greatest potential in the estimation of low dose radiation exposure.
This study aims to present new chest AP examination exposure conditions through a study on the effect on image quality and patient dose by applying high tube voltage and scatter ray post-processing software during chest AP examination in digital radiography equipment. This study was used a human body phantom and in the chest AP position, the dosimeter was placed horizontally at the thoracic spine 6. The experiment was conducted by dividing into a low tube voltage (70 kVp, 400 mA, 3.2 mAs) group and a high tube voltage (100 kVp, 400 mA, 1.2 mAs) group. The collimation size (14″× 17″) and the source to image receptor distance(110 cm) were same applied to both groups. Radiation dose was presented to dose area product and entrance surface dose. Image quality was compared and analyzed by comparing the difference between the signal-to-noise ratio and the contrast-to-noise ratio of the image according to the application of the scatter ray post-processing software under each condition. The average value of the entrance surface dose in the low and high tube voltage conditions was 93.04±0.45 µGy and 94.25±1.51 µGy, which was slightly higher in the high tube voltage condition, but the dose area product was 0.97±0.04 µGy and 0.93±0.01 µGy. There was a statistically significant difference in the group mean value(p<0.01). In terms of image quality, the values of the signal-to-noise ratio and the contrast noise ratio were higher in the high tube voltage than in the low tube voltage, and decreased when the scattering line post-processing function was used, but the contrast resolution was improved. If there is a scatter ray post-processing function during chest AP examination, it is helpful to actively utilize it to improve the image quality. However, when this function is not available, I thought that applying a higher tube voltage state than a low tube voltage state will help to realize images with a large amount of information without changing the dose.

In this study, a dose assessment was conducted on the exposure dose of thyroid, breast and sexual gland using a personal dosimeter in multiple CT examinations currently being conducted in health examinations. The dose assessment was measured by attaching TLD and EPD to the locations of the thyroid, breast and sexual gland during CT examinations of Brain, Brain + C-S, Brain + Low lung, Brain + L-S among CT items. The generated dose of equipment, CTDIvol and DLP, was measured. The study found that effective doses were rated 41.7% higher for thyroid TLD in Brain + C-S CT examinations than for the general public, 156% higher for EPD, 10% for breast EPD in Brain + Low Lung CT examinations, 124.4% higher for reproductive TLD and 339.8% higher for Brain + L-S CT examinations. The CTDIvol and DLP analysis results showed that C-S CTDIvol values were higher than the diagnostic reference levels at 0.6%, Low Lung CTDIvol values at 5.7%, DLP values at 11.8% and L-S CTDIvol values at 1.2%. In order to reduce the exposure dose of patients, indiscriminate examination is reduced and dose limit setting is needed in health examination.

Purpose : Concurrent chemoradiotherapy(CCRT) with 3 weekly cisplatin is the standard treatment of locally advanced head and neck cancer(HNC). The aim is to evaluate the efficacy and toxicities of low-dose weekly cisplatin-based CCRT, which was devised to reduce the toxicity of CCRT. Method : We retrospectively analyzed HNC patients who received low-dose weekly cisplatin-based CCRT between 2008 and 2010. Cisplatin 35mg/$m^2$ was weekly given to all patients during radiotherapy. The efficacy was evaluated by the degree of clinical response, treatment failure and survival. The toxicity was evaluated by hematologic toxicities and oral mucositis. Results : A total of 27 patients were analyzed and median age was 59(range 31-81). The ratio of administered dose of radiotherapy and cisplatin to planned dose were 0.98 and 0.93, respectively. Complete remission and partial remission were 73% and 23%, respectively. Treatment failure was observed in 8(30%) patients. 1-year survival rate and 1-year disease free survival rate were 82% and 59%, respectively. Overall survival and progression-free survival did not reach median time. Grade 3/4 anemia, neutropenia, thrombocytopenia and oral mucositis were observed in 11%, 19%, 7% and 32% of patients, respectively. In terms of administered cycles, however, only 1-3% of grade 3/4 hematologic toxicities occurred among total 190 cycles. Severe oral mucositis were statistically associated with old age(p=0.003). Treatment failure had no statistical relation with age, pathology, primary site and stage. Conclusion : Low-dose weekly cisplatin-based CCRT seemed to deliver enough dose of cisplatin and to show low drop-out rate and good efficacy with low hematologic toxicities.

Korean nuclear power plants (NPPs) have various radiation protection programs to attain radiation exposure as low as reasonably achievable (ALARA). In terms of ALARA, this paper provides a comprehensive overview of administrative dose control for occupationally-exposed workers in Korean NPPs. In addition to dose limits, administrative dose constraints are implemented to resolve an inequity of radiation exposure in which some individuals in NPPs receive relatively higher doses than others. Occupational dose constraints in Korean NPPs are presented in this paper with the background of how those values were determined. For pressurized water reactors, 80% and 90% of the annual average limit for an effective dose, 20 mSv/y, are set as the primary and secondary dose constraints, respectively. Pressurized heavy water reactors (PHWRs) have also established the primary and secondary dose constraints corresponding to 70% and 80% of the effective dose limit, and additional constraints for tritium concentration are provided to control internal exposure in PHWRs. Follow-up measures for exceeding these administrative dose constraints are also introduced compared to exceeding the dose limits. Finally, analysis results of dose distributions show how the implementation of administrative dose constraints impacted the occupational dose distributions in Korean NPPs during the years 2009-2018.
Two approximate methods for a cosmic radiation shielding calculation in low earth orbits were developed and assessed. Those are a sectoring method and a chord-length distribution method. In order to simulate a change in cosmic radiation environments along the satellite mission trajectory, IGRF model and AP(E)-8 model were used. When the approximate methods were applied, the geometrical model of satellite structure was approximated as one-dimensional slabs, and a pre-calculated dose-depth conversion function was introduced to simplify the dose calculation process. Verification was performed with mission data of KITSAT-1 and the calculated results were also compared with detailed 3-dimensional calculation results using Monte Carlo calculation. Dose results from the approximate methods were conservatively higher than Monte Carlo results, but were lower than experimental data in total dose rate. Differences between calculation and experimental data seem to come from the AP-8 model, for which it is reported that fluxes of proton are underestimated. We confirmed that the developed approximate method can be applied to commercial satellite shielding calculations. It is also found that commercial products of semi-conductors can be damaged due to total ionizing dose under LEO radiation environment. An intensive shielding analysis should be taken into account when commercial devices are used.
This study was designed to assess the distribution of cytochrome P450 2E1 (CYP2E1) polymorphism among Korean patients on warfarin therapy. CYP2E1 polymorphism was analyzed at 5' flanking region of CYP2E1 gene using restriction fragment length polymorphism method. Patient characteristics including the measured internal normalized ratio (INR) were also evaluated. Based on the warfarin dose and the bleeding cases, the patients were grouped as the regular dose control, the regular dose bleeding, the low dose control, and the low dose bleeding. Total 96 patients were evaluated for both Pst I and Rsa I loci of the CYP2E1 gene and the results showed that both loci were tightly linked. Thirty-three patients(34.4%) were heterozygotes and 4 patients(4.2%) were homozygote. There was no significant difference in patient characteristics in the dose and bleeding case groups. CYP2E1 polymorphism showed a little difference among the groups but was not statistically significant, however, lower INR value was observed in homozygote genotype groups. It was also revealed that genotype allele frequencies of CYP2E1 in Korean was close to other Asian groups but was significantly different from other Caucasian and African-American populations.
In a previous study, a relatively high dose of gamma radiation (1 kGy) did not fully induce typical SOS genes such as sulA, recA, recN, and din in Salmonella Typhimurium (S. Typhimurium) (Lim et al. 2008, Gene expression profiles following high-dose exposure to gamma radiation in Salmonella enterica serovar Typhimuium. J. Radiat. Ind. 3:111-119). In this study, we examined changes in the transcriptional repertoire of S. Typhimurium after a dose of 10 Gy using DNA microarrays. It was found that more than half (~65%) of the 26 up-regulated genes belong to the SOS regulon: ten genes are typical SOS genes, and seven genes are Salmonella prophage genes, which are known to be activated by LexA cleavage. Among 29 down-regulated genes, the function of five genes with the most decreased expression is associated with carbohydrate transport and energy production. This suggests that upon exposure to gamma radiation cells may cease growing by reducing the metabolic activity, and repair DNA damage using a DNA repair system such as the SOS response system. The difference in expression of the SOS genes between a high (1 kGy) and low (10 Gy) dose of radiation shows the possibility that cells may opt for one of multiple regulatory circuits in response to the specific gamma radiation dose.

Hemorrhagic complications associated with aspirin use occur primarily at skin or gastrointestinal sites but can occasionally occur in the central nervous system. In particular, spontaneous spinal epidural hemorrhage (SSEH) associated with aspirin is very rare. We report a case of low-dose (100 mg daily) aspirin-related SSEH that was successfully treated with medical management. Our case indicates that low-dose aspirin could induce SSEH and that conservative treatment with close observation and repeated imaging studies should be considered in cases with neurological improvement or mild deficits.
