Objectives: The purpose of this study is to investigate the effect of the aerobic exercise and weight training combined with VLCD(very low calorie diet) and chegameuyiin-tang(體減薏苡仁湯) on the change of the body composition during 15 days of hospitalization treatment. Methods: Twenty four female obese patients were observed. Exercise regimen was composed with aerobic exercise and weight training program, and the intensity of aerobic exercise were 50% HRmax(maximal heart rate) twice time a day and weight training were 50% 1RM(one repetition maximum) a day. All subjects diet regimen was VLCD of 600kcal/day. Results & Conclusions. 1. There was no significant difference between the aerobic exercise and the weight training groups in the change of body weight, fat mass, and percent body fat, waist-hip ratio, body mass index and resting metabolic rate. 2. In the comparison of the change rate of FFM(fat free mass), weight training group preserved FFM better than aerobic-exercise-only-group with no statistical significance.

To investigate effects of ethanol and dietary fat on growth and bichemical indices of liver tissue and blood in rats 40 male rats of Sprague-Dawley wtrain weighing about 160g were divided into 5 groups (low-fat diet group ethanol-administered low-fat diet group high-fat diet group ethanol-administered high-fat diet group and commercial diet group) and fed expe-rimental diets for 8 weeks. Ethanol-administered groups consumed ethanol corresponding to 22 cal% which was considered as moderate drinking. Neither the ethanol intake nor the dietary fat level affected calorie intake. Nonetheless the low-fat diet group with ethanol had the lowest growth rate and 2-fold increase in the concentration of plasma triglyceride. There was no effect of ethanol and dietary fat level on contents of protein lipid and lipid composition of liver tissue. The level of lipid peroxide of liver tissue tended to be increased by ethanol intake but the increase was statistically insignificatnt. The low-fat ethanol group had lowered hepatic mitochondrial respiration rate and deformed structure of mitochondria of hepatocytes.
This study was conducted to investigate the effect of a 3 week low calorie diet (LCD) and a 9 week of behavior modification (BM) program on the weight loss, mineral and vitamin status in 22 obese women. The subject were healthy, obese (PIBW> $120\%$) women aged 20 - 50 Yr and not taking any medications known to influence body composition, mineral or vitamin metabolism During the LCD program, subjects were provided commercial liquid formulas with 125 kcal per pack and were instructed to have a formula for replacement of one meal and at least one regular meal per day within the range of daily 800 - 1200 kcal intake. During the BM program the subjects weekly attended the group nutrition counseling session to encourage themselves to modify their eating behavior and spontaneously restrict their energy intakes. The BM program focused on stimulus control, control of portion sizes and modification of binge eating and other adverse habits. The initial mean energy intake of subjects was 2016.9 $\pm$ 129.8 kcal ($100.8\%$ of RDA) and dropped to 1276.5 $\pm$ 435.7 kcal at the end of a 3 week of LCD program and elevated to 1762 $\pm$ 329.3 kcal at the end of a 9 week of BM program. Carbohydrate, protein and fat intakes were significantly decreased at the end of the LCD but carbohydrate was the only macro nutrient that showed significant decrease (p < 0.05) at the end of the BM program compared to baseline. Calcium and iron intakes decreased significantly (p < 0.01, respectively) with no significant changes in other micronutrients at the end of the LCD. The mean weight of the subjects decreased from 73.8 $\pm$ 8.0 kg to 69.2 $\pm$ 7.7 kg with LCD and ended up with 67.7 $\pm$ 7.1 kg after 9 weeks of BM. The 3 weeks of LCD reduced most of the anthropometric indices such as BMI, PIBW, fat weight, wast-to-hip ratio and subscapular and suprailiac skinfold thickness. The 9 weeks of behavior modification showed slight change or maintenance of each anthropometric measurements. Weight loss and decreased WHR with the diet program induced significantly decreased systolic blood pressure. SGOT, SGPT and serum insulin levels with improved serum lipid profiles. Biochemical parameters related to iron status such as hemoglobin, hematocrit were significantly decreased (p < 0.01) at the end of the LCD. But their mean values were within normal range. The mean serum 25 (OH) vitamin $D_3$ level significantly increased after whole diet program. Serum folate level significantly decreased after 12 weeks of diet program. In conclusion 3 weeks of LCD brought 4.6 kg reduction in body weight without risk of iron, zinc or vitamin D deficiency and 9 weeks of the BM was effective to maintain nutritional status with slightly more weight reduction (1.5 kg). However calcium intake and serum folate should be monitored during the LCD and BM because of increased risk of deficiencies.
Objectives : The weight reduction program with very low calorie diet(VLCD) for women generally reduce bone mineral density(BMD). So many treatments were done to prevent side effect of the weight reduction program and herbal diet meditation were often used in oriental medicine. This research were done in order to study the effect of VLCD and herbal diet meditation, Chegamuiyiintang on BMD. Methods : This clinical research carried out 22 cases of pre-menopausal outpatients with obesity during 1 month. To evaluate the effect Chegamuiyiin-tang with VLCD on BMD, the weight, BMI, percent body fat, WHR, BMD(T-score) were estimated in pre-treatment and post-treatment. Results : The mean weight reduction after 1 month was $6.87{\pm}1.54\;Kg$(10.28 % of initial weight), BMI reduction was $2.35{\pm}0.65\;Kg/m^2$, percent body fat reduction was $3.40{\pm}1.01%$, WHR reduction was $0.04{\pm}0.01$. But BMD(in T-score) had significant increase. Conclusions : Generally VLCD were estimated to reduces BMD, but VLCD with herbal diet meditation, Chegamuiyiin-tang has positive effect on BMD in pre-menopausal women during short term(1 month).

Objectives: The objective of this study was to analyse the methods being used to control food intake and physical activity in RCTs of human obesity. Methods: A total of 21 randomized controlled trials (RCTs) were investigated. Nine of which were domestic studies from "http://oasis.kiom.re.kr" and the other of which were foreign studies from systematic reviews of RCTs on herbal medicine for treatment of human obesity. Results: According to domestic studies, "low calorie diet" were recommended in five cases of the domestic studies, "maintain current dietary habit" were recommended in two and no information on diet was two. Considering the seven cases where the information on diet was available, patients' food intake were checked at every visit in six cases. Only two cases among the six had been dropped owing to the violation of dietary habit by patients. Exercises were prohibited in two cases, "maintain current level of phisical activity" were recommended in three cases and, from the rest, no information was available. The level of physical activity were not strictly controlled by any means hence no drop out. According to foreign studies, "low calorie diet" were recommended in two cases, "very low calorie diet (less than 700 kcal/day)" in one case, "maintain current dietary habit" in two cases, "do not eat fat" in two cases and no information was available in the rest five cases. Exercises which concerns spending about 300 kcal/day was recommended in one case, "moderate exercise" were recommended in three cases, "maintain current level of physical activity" were recommended in three cases and no information available in the rest five cases. Conclusions: In order to improve the accuracy of RCT, for the dietary side, researchers should record patient food intake at every visit by means of 24-hour dietary recall methods. This can be supplemented by multiple choice survey that are designed to help patients to diagnose themselves more accurately leading to less bias. For the exercise side, it is highly recommended to confine the exercises to walking only so as to quantify the amount of physical activity more easily by using pedometer.
To study effect of dietary polyunsaturated fatty acid and $\omega$-tocopherol content on lipid peroxidation in rat liver, rats were fed for 3, 6 and 9 weeks with normal tocopherol diet added 40mg of DL-$\alpha$-tocopherol/kg of diet (PF group), high tocopherol eit 200mg of DL-$\alpha$-tocopherol/kg of diet(PFE group), low tocopherol diet without addition of DL-$\alpha$-tocopherol to diet(PFO group), and control diet added 40mg of DL-$\alpha$-tocopherol/kg of diet(control group). Each diet group supplied 45% of total calorie from corn oil except control group which supplied 12% of total calorie from corn oil. After each feeding period, lipid peroxide and tocopherol contents were measured in the liver as well as activities of glutathione peroxidase and superoxide dismutase. PF group had almost the same contents of liver peroxide, tocopherol contents and activities of glutathione peroxidease and superoxide dimutase as control group, while PFE group had higher tocopherol content and glutathione peroxidase activity and lower lipid peroxide contents and superoxide dismutase activity than control group. On the other hand, changes in the values of PFO group were opposite to those of PFE group. Differences in the values among groups were more pronounced as feeding period became longer.
Effects of alchohol and fat content in a balanced diet on chemical composition and morphology of liver were investigated in growing rats. Fourth eight male rats of Sprague-Dawley strain weighing about 160g were divided into 4 groups ; high fat diet group, alcohol-administered high fat diet group, low fat diet group and alcohol-administered high fat diet group, low fat diet group and alcohol-administered low fat diet group. High and low fat diets supplied 30% and 12%, respectively, of total calorie intake from fat, and alcohol was given by adding ethanol in drinking waster at 10%. Diets contained adequate amounts of all nutrients required for rats, including lipotrpoic agents(choline and methionine) to minimize effects of factors other than alcohol on liver damage. Ratios of liver weight to body weight were statistically different among groups. Liver/dody weight ratios alcohol-administered rats were significantly higher than those of non-alcohol groups after 6 weeks treatment. Although total lipid and triglyceride per gram liver were increased in alcohol-administered rats, especially low fat diet fed rats, the values were not significantly different. Opticmicroscopical observation revealed increase in cell size and no change in morphology of liver. Examination of hepatocytes by electron microscopy showed that fat droplets were observed in all groups but enlarged in the alcohol-administered low fat diet fed rat. Contents of protein, cholesterol and phospholipid were not affected by alcohol consumption. The level of lipid peroxide was significantly lower in the livers of alcohol-administered rats than in the livers of non-alcohol groups. The results of this study indicate that even moderate alcohol drinking and dietary fat content did not affect any significant change in composition and morphology of liver until 6 week treatment but that even moderate alcohol drinking caused some signs of steatosis of liver.
This study was performed to report the effectiveness of modified fasting therapy using very-low-calorie-diet on a patient with obesity. The patient was diagnosed as severe obesity and participated in a 14-day fast with the Signature, a liquor which is made from fermented medical herbs and carbohydrates (intake <600 kcal/day) accompanying 7 prefasting relief days, and 16 days with stepwise reintroduction of food. The fasting patient also received intensive oriental medical treatments such as acupuncture, moxibustion, cupping therapy and was encouraged to take more than two hours' of walking exercise a day. We have evaluated the efficacy of treatments by measuring the changes of body composition and blood chemistry according to each period of fasting therapy. After treatment, Body Fat Mass of the patient was decreased more than Muscle Mass and the level of AST, ALT was decreased to normal range. These results suggest that the modified fasting therapy was effective on the patient with obesity.
Obesity is an excess of body fat frequently resulting from an imbalance between energy intake and expenditure, raises the risk of morbidity from chronic diseases such as hypertension, dyslipidemia, type 2 diabetes, coronary heart disease, stroke, fatty liver and osteoarthritis. Recently we experienced a 51 years old obese woman who was accompanied with hypertension, dyslipidemia and osteoarthritis. Through weight reduction program(very low calorie diet, Chegameuiintang, exercise) during 3 months, total weight loss was 17.0kg, her condition was improved.

Effects of wild plant extract (Lak) based on Korea traditional prescription on maximal exercise performance and endurance were evaluated using calorie-restriction animal model. In acute forced swimming test with 10% body weight attached to tail, dietary Lak supplementation increased exercise performance endurance by increasing concentrations of ATP and insulin-like growth factor-1 (IGF-1) under calorie-restriction condition, and decrement of blood lactic acid concentration and increment of muscle ATP content were observed. These results suggest Lak is very effective for decreasing side-effects of obesity therapy using very low calorie diet.
