제 2형 당뇨 환자의 저혈당지수 영양교육이 혈당관리에 미치는 영향 (Effects of Low Glycemic Index Nutrition Education on the Blood Glucose Control in Patients with Type 2 Diabetes Mellitus)

  • This study was intended to assess the effects of low glycemic index (LGI) nutrition education on dietary management and glycemic control of patients with type 2 diabetes mellitus. The subjects were 48 sex-matched patients with type 2 diabetes mellitus, aged $66.5\;{\pm}\;6.2$ years, visiting a public health center. They were divided into two groups: the control group (males 10, females 14) and the educated group (males 10, females 14). The educated group was provided with a LGI nutrition education program for 7 weeks. The control group was educated only one time for general diabetic education. Anthropometric indices, knowledge and perception of efficacy of low glycemic index carbohydrates, dietary glycemic index (DGI) and glycemic load (DGL), fasting blood glucose, and HbA1c were assessed. In the educated group body weight, body mass index and systolic blood pressure (from $138.0\;{\pm}\;18.9\;mmHg$ to $130.6\;{\pm}\;15.0\;mmHg$) were significantly reduced after the nutrition education (p < 0.05). The scores of knowledge and perception of efficacy of low glycemic index carbohydrates increased significantly in the educated group. Dietary glycemic index and glycemic load of the educated group decreased significantly from $103.4\;{\pm}\;67.6$ to $45.4\;{\pm}\;27.1$ (p < 0.001), and from $173.3\;{\pm}\;135.9$ to $66.8\;{\pm}\;50.4$ (p < 0.001), respectively. Also fasting blood glucose and HbA1c levels of the educated group significantly decreased from $124.5\;{\pm}\;28.8\;mg/dL$ to $96.7\;{\pm}\;21.6\;mg/dL$ (p < 0.001) and from $7.1\;{\pm}\;1.3%$ to $6.4\;{\pm}\;1.2%$ (p < 0.05), respectively. The score of knowledge and perception of efficacy of low glycemic index significantly correlated with fasting blood glucose and HbA1c levels negatively. DGI, DGL and duration of diabetes significantly correlated with HbA1c level positively. From stepwise multiple linear regression analysis, DGI, DGL and the duration of diabetes were extracted as factors influencing HbA1c level of the subjects. The results of this study suggest that low glycemic index nutrition education programs is an effective intervention measure for the glycemic control in type 2 diabetic patients.

Effect of Wheat Flour Noodles with Bombyx mori Powder on Glycemic Response in Healthy Subjects

  • Recent trial results suggest that the consumption of a low glycemic index (GI) diet is beneficial in the prevention of high blood glucose levels. Identifying active hypoglycemic substances in ordinary foods could be a significant benefit to the management of blood glucose. It has been hypothesized that noodles with Bombyx mori powder are a low GI food. We evaluated GI and changes in postprandial glucose levels following consumption of those noodles and compared them with those following consumption of plain wheat flour noodles (control) and glucose (reference) in healthy subjects. Thirteen males (age: $34.2{\pm}4.5years$, body mass index: $23.2{\pm}1.1kg/m^2$) consumed 75 g carbohydrate portions of glucose and the 2 kinds of noodle after an overnight fast. Capillary blood was measured at time 0 (fasting), 15, 30, 45, 60, 90, 120, and 180 min from the start of each food intake. The GI values were calculated by taking the ratio of the incremental area under the blood glucose response curve (IAUC) for the noodles and glucose. There was a significant difference in postprandial glucose concentrations at 30 and 45 min between the control noodles and the noodles with Bombyx mori powder: the IAUC and GI for the noodles with Bombyx mori powder were significantly lower than those for glucose and plain wheat flour noodles. The wheat flour noodles with Bombyx mori powder could help prevent an increase in postprandial glucose response and possibly provide an alternative to other carbohydrate staple foods for glycemic management.

Blood Triglycerides Levels and Dietary Carbohydrate Indices in Healthy Koreans

  • Objectives: Previous studies have obtained conflicting findings regarding possible associations between indices measuring carbohydrate intake and dyslipidemia, which is an established risk factor of coronary heart disease. In the present study, we examined crosssectional associations between carbohydrate indices, including the dietary glycemic index (GI), glycemic load (GL), total amount of carbohydrates, and the percentage of energy from carbohydrates, and a range of blood lipid parameters. Methods: This study included 1530 participants (554 men and 976 women) from 246 families within the Healthy Twin Study. We analyzed the associations using a generalized linear mixed model to control for familial relationships. Results: Levels of the Apo B were inversely associated with dietary GI, GL, and the amount of carbohydrate intake for men, but these relationships were not significant when fat-adjusted values of the carbohydrate indices were used. Triglyceride levels were positively associated with dietary GI and GL in women, and this pattern was more notable in overweight participants (body mass index [BMI] ${\geq}25kg/m^2$). However, total, low-density lipoprotein and high-density lipoprotein cholesterol levels were not significantly related with carbohydrate intake overall. Conclusions: Of the blood lipid parameters we investigated, only triglyceride levels were positively related with dietary carbohydrate indices among women participants in the Healthy Twin Study, with an interactive role observed for BMI. However, these associations were not observed in men, suggesting that the association between blood lipid levels and carbohydrate intake depends on the type of lipid, specific carbohydrate indices, gender, and BMI.

식단의 당부하량에 따른 20대 성인의 체중 감량 효과 연구 (Effects of a low glycemic load diet on body weight loss in overweight or obese young adults)

  • 본 연구에서는 한국인이 평소 섭취하는 일반식사와 유사한 수준의 당부하량을 가진 high GL 식사와 당부하량이 낮게 설계된 low GL 식사를 섭취한 후 체중, 체지방, 혈압 및 혈액 내 생활습관병 위험 지표 등의 변화를 분석 및 비교하였다. 20대 성인남녀를 대상으로 체중 감량을 위해 wash-out 기간인 중간 2주를 제외하고 개인 필요 열량의 30%를 줄인 low GL 식사와 high GL 식사를 각 2주간 제공하였다. Low GL 식사와 high GL 식사의 효과를 비교한 결과 low GL 식사가 high GL 식사에 비해 체중 (p < 0.001), 체지방량 (p = 0.024), 체질량지수 (p < 0.001)에서 유의적으로 감소량이 더 컸다. 하지만 혈압, 혈액 내 비만 질환 위험 지표에서는 유의한 차이가 없었다. Low GL 식사의 전·후를 비교한 결과 체중, 체지방량, 체질량지수, 체지방률, 이완기 혈압, 중성지방, 총콜레스테롤, HDL-콜레스테롤, LDL-콜레스테롤, 공복혈당, C-peptide HOMA 지수가, high GL 식사에서는 체중, 체지방량, 체질량지수, 총콜레스테롤, HDL-콜레스테롤, 공복혈당, C-peptide, C-peptide HOMA 지수에서 각 식사 전보다 후에 유의적으로 감소되었다. 또한 실험 식사 섭취 전과 섭취 후를 비교한 결과 체중, 체지방량, 체질량지수, 체지방률, 혈압 (수축기, 이완기), 총콜레스테롤, HDL-콜레스테롤, LDL-콜레스테롤, 공복혈당, 당화혈색소, C-peptide, C-peptide HOMA 지수가 모두 유의적으로 감소되었다. 본 연구 결과 체중감량 및 생활습관병 위험을 줄이기 위해서는 단순히 섭취 열량을 낮추기보다는 열량과 함께 당부하량을 고려하여 섭취하는 것이 체중을 감소시키는 데에 더 효과적일 것으로 사료된다.

반응표면분석을 이용해 개발한 황금과 대두콩/서리태 분말 혼합 쿠키가 정상인의 혈당반응에 미치는 영향 (Effect of cookies made with soybean/seoritae and Hwanggum using response surface methodology on the blood glucose response in healthy adults)

  • 본 연구에서는 반응표면분석을 이용하여 개발한 대두/서리태 분말, 황금, 아이소말토 올리고당 첨가 쿠키가 정상인들의 혈당반응에 미치는 영향을 살펴보고자 하였다. 반응표면분석의 중심합성 계획에 따라 독립변수 대두/서리태 분말, 황금, 아이소말토 올리고당 3가지에 대한 반응변수로는 전반적인 기호도로 설정하였다. 반응모형의 결정계수는 대두 쿠키가 0.923, 서리태 쿠키가 0.927로 1에 근접하였으며, 전체적인 모형이 0.05보다 낮아 적합한 반응모형으로 나타났다. 대두쿠키는 안장점을 나타내고 있어 능선분석을 실시한 결과 최고점을 만족하는 각 변수의 조건은 대두분말 68.7%, 황금 분말 2.5%, 이소말토 올리고당 75.2%로 결정되었다. 서리태 분말 쿠키 또한 능선분석 실시 결과 서리태 분말 56.5%, 황금 분말 3.8%, 이소말토 올리고당 56.2%에서 최대 값을 보여주었다. 최적배합비에 따라 제조한 대두/서리태 쿠키의 조직감 측정 결과 단단한 정도와 바삭거리는 정도는 유의적으로 낮아졌고, 전반적인 기호도는 대조군보다 대두/서리태 쿠키의 전반적인 기호도가 유의적으로 향상되었다. 그리고 최적배합비로 개발된 쿠키가 정상인의 혈당반응에 미치는 영향은 대조군보다 대두쿠키 섭취군에서 혈당반응면적과 당지수가 유의적으로 낮아졌으며, 당부하지수 계산 결과 대두쿠키 19.9, 서리태쿠키 21.9로 나타났다. 당부하지수 20이하는 저당지수 식품에 해당이 되므로 대두쿠키가 저당지수 식품에 포함되는 것을 확인할 수 있었으며, 본 연구를 통해 혈당상승을 지연시킬 수 있는 쿠키 개발 가능성을 제안할 수 있었다.

  • Milk intake is widely recommended for healthy diet, not only for bone growth and maintenance, but also as a protein, calcium and magnesium sources as part of an adequate diet. Many research suggest that milk and dairy products are associated with a lower risk of type 2 diabetes mellitus (T2DM). Milk and dairy products are low Glycemic index (GI) and Glycemic load (GL) foods. The GI and GL are useful tools to choose foods to help control blood glucose levels in people with diabetes. The GI and GL of milk are 32~42 and 4~5, respectively, and which are about 1/2 and 1/5 of boiled rice. The mechanisms underlying the effects of dairy on T2DM development includes the calcium and vitamin D content in dairy foods and the possible positive effect of high milk and calcium intake on weight control. The role of dairy products on reducing the risk of diabetes can be inferred from the reports that lower serum IGF-1 levels were positively associated with diabetes and the girls with low milk intake had significantly lower IGF-1. Accumulating data from both patients and animal models suggest that microbial ecosystems associated with the human body, especially the gut microbiota, may be associated with several important diseases, such as inflammatory bowel disease, obesity, diabetes and cardiovascular disease. It was thought that fermented milk containing lots of probiotics can be useful for controling blood glucose levels and preventing complication of diabetes, but sucrose in commercial yogurt should be substituted. There are some reports of oligosaccharide, xylitol, and stevia as a potentially useful sweetener in the diabetic diet.

생식조성이 정상성인의 혈당반응에 미치는 영향 (Effect of Saengshik on Blood Glucose Response in Healthy Subjects)

  • 본 연구는 생식을 정상성인에게 섭취시켰을 때 혈당반응에 미치는 영향을 연구하고, 생식의 혈당지수를 구하여 당뇨병 환자의 혈당 조절식으로 이용가능성을 조사하기 위하여 시판중인 생식 4(ES, HS, BS, SS)종을 섭취 전, 섭취 후 120분 동안 15분 간격으로 채혈하여 혈당을 측정하였다. 생식 섭취 후 혈당수치는 포도당 섭취시보다 전반적으로 낮게 나타났으며, ES는 $15{\sim}75$분, SS는 $15{\sim}75$분, HS는 $15{\sim}45$분, BS는 15분 후의 혈당이 포도당보다 유의적으로 낮게 나타났다. 식이 섭취 후 측정한 시간에 따른 혈당값을 이용하여 혈당반응 곡선의 면적을 구한 결과, 생식 섭취군은 포도당 섭취군보다 면적이 적은 것으로 나타났으며, 상기 면적을 이용하여 GI를 산출한 결과, ES는 $43.1{\pm}12.6$으로 Low GI(55이하) 범위에 속하였으며, 그 외 생식은 Medium GI($56{\sim}69$) 범위에 속하였다. 생식의 소화, 흡수 형태를 알아보기 위하여 상기에서 구한 혈당반응곡선 면적을 일반적인 최고 혈당값 나타내는 시간인 30분을 전후로 하여 LAR(혈당상승기)과 RAR(혈당감소기)을 구한 결과, 생식은 혈당 상승과 감소를 완만하게 해주는 것으로 나타났으며, 생식섭취 후 최고 혈당값을 포도당을 섭취했을 때보다 유의적으로 감소시켰으며, 최고 혈당값을 나타내는 시간은 생식 ES에서 연장되는 경향을 나타내었다. 위의 결과들을 종합해 볼 때, 생식의 섭취는 체내 당의 소화와 흡수 과정을 천천히 진행시키며, 식후 최고 혈당수치를 감소시켜 전반적인 혈당반응을 원만하게 하는 것으로 나타났으며, GL은 ES(14.1)이 가장 낮았으며 HS, BS, SS($19.2{\sim}19.5$) 수준으로 GL 역시 낮은 수치를 나타내었다. 위의 연구결과 생식은 당뇨병 환자의 식후 혈당상승을 조절하여 유익한 영향을 미칠 수 있을 것으로 사료되며, 생식의 조성에 따라 그 효과는 차이를 보이며, 사용하는 당의 특징과 곡류군의 조성, 식이섬유의 종류, 과일소재의 첨가 등에 따라 달라지므로 추후 심화된 연구가 필요할 것이다.

Low HDL cholesterol is associated with increased atherogenic lipoproteins and insulin resistance in women classified with metabolic syndrome

  • Both metabolic syndrome (MetS) and elevated LDL cholesterol (LDL-C) increase the risk for cardiovascular disease (CVD). We hypothesized that low HDL cholesterol (HDL-C) would further increase CVD risk in women having both conditions. To assess this, we recruited 89 women with MetS (25-72 y) and LDL-C ${\geq}$ 2.6 mmol/L. To determine whether plasma HDL-C concentrations were associated with dietary components, circulating atherogenic particles, and other risk factors for CVD, we divided the subjects into two groups: high HDL-C (H-HDL) (${\geq}$ 1.3 mmol/L, n=32) and low HDL-C (L-HDL) (< 1.3 mmol/L, n=57). Plasma lipids, insulin, adiponectin, apolipoproteins, oxidized LDL, Lipoprotein(a), and lipoprotein size and subfractions were measured, and 3-d dietary records were used to assess macronutrient intake. Women with L-HDL had higher sugar intake and glycemic load (P< 0.05), higher plasma insulin (P< 0.01), lower adiponectin (P< 0.05), and higher numbers of atherogenic lipoproteins such as large VLDL (P < 0.01) and small LDL (P<0.001) than the H-HDL group. Women with L-HDL also had larger VLDL and both smaller LDL and HDL particle diameters (P<0.001). HDL-C was positively correlated with LDL size (r=0.691, P<0.0001) and HDL size (r=0.606, P<0.001), and inversely correlated with VLDL size (r=-0.327, P<0.01). We concluded that L-HDL could be used as a marker for increased numbers of circulating atherogenic lipoproteins as well as increased insulin resistance in women who are already at risk for CVD.
