Effect of Low Sulphur Fuel Oil Use on Korean Container Shipping Companies by IMO Environmental Regulations (IMO 환경규제에 따른 저유황 연료유 사용이 국적 컨테이너 선사에 미치는 영향)

  • This study examined the background of IMO environmental regulations and countermeasures of domestic and foreign liner shipping companies and also analyzed the additional cost and profitability of container shipping companies by implementation of IMO 2020, when the fuel oil is converted to low sulfur oil. The results show that the national flag carrier would incur significant additional costs when using low sulfur oil, which would worsen its profitability in all scenarios. These analysis results suggest that for regional shipping companies need reasonable BAF setting standards, which is flexible to the price of fuel oil and also seek ways to co-exist through mutual cooperation among oil refiners, shippers and shipping companies.

The ice features of Very Low Luminosity Objects (VeLLOs): Unveiling their episodic accretion history through the spectroscopic observation of AKARI IRC

  • Although mass accretion from the disk to the central protostar is a key process of low mass star formation, the accretion mechanism is still poorly understood. To investigate "episodic accretion", which has been suggested as an accretion mechanism in low mass star formation, we have carried out near-infrared spectroscopic observations of three very low-luminosity objects (VeLLOs) and one background source, using InfraRed Camera onboard the AKARI space telescope. The ice absorption features of $H_2O$, $CO_2$, and CO were detected around the wavelengths of 3.0, 4.26, and $4.67{\mu}m$, respectively. In addition, we revealed the XCN ice feature, which is attributed to high energy UV photons produced by the episodic burst accretion. The comparisons of the ice abundances of our targets with those of other YSOs observed previously with AKARI IRC imply that the three VeLLOs had experienced burst accretions although they are now in a very quiescent phase.

Immunohistochemical Differentiation between Urothelial Papillomas and Papillary Neoplasms of Low Malignant Potential of the Urinary Bladder

  • Background: Urothelial papilloma and non-invasive papillary carcinoma are common neoplasms of the urinary bladder. Distinguishing papillomas and papillary carcinomas, especially the low grade type, is often debatable on the basis of histological features alone. Materials and Methods: We investigated immunohistochemical expression of cytokeratin 20 (CK20), p53, and Ki-67 in a group of 20 urothelial papilloma cases and 30 noninvasive papillary neoplasms of low malignant potential (PNLMP) of the urinary bladder. Whole tissue sections were examined. Results: Among the 30 carcinoma cases, 12 (40%) showed strong reactivity for the whole panel, 16 (53%) reacted positively for two markers, and 2 (7%) reacted just to one of them. Ki-67 was considered positive in 27 cases (90%) and p53 in 24 (80%), CK20 showed positive reactivity in 21 cases (70%). Only small percentages of papillomas were positive, and then only weakly. Conclusions: We concluded that the intense positivity of suspicious cells for at least one of these markers would confirm the presence of malignant changes and favours the diagnosis of carcinoma.

The Effects of Hip Mobilization on Pain, Trunk Flexibility, Function for Chronic Low Back Pain Patients (고관절 가동술이 만성요통환자의 통증, 체간 유연성 및 기능에 미치는 영향)

  • Background: This study aimed to the effects of hip mobilization on pain, trunk flexibility, function for chronic low back pain patients. Methods: Patients were randomly assigned to control group (n=15) and experimental group (n=15). Both groups received conventional physical therapy, three times a week for four weeks. Experimental group was performed additional hip mobilization, three times a week for four weeks. All measurement of each subject were measured at pre-intervention and post-intervention (after 4 weeks). Results: Assessed items included the visual analog scale score (VAS), Korean version of the Oswestry disability Index (KODI) a significant reduction was observed post intervention compared to pre-intervention values in both group (p<.01). Trunk flexibility was significant increased post intervention compared to pre-intervention values in both group (p<.01). There were significant difference between two groups in VAS (p<.01), trunk flexibility (p<.05) and except KODI. Conclusions: Our results indicate that conventional physical therapy with the hip mobilization may be useful for improving chronic low back pain patient.

A Study on the Risk Factors of Elder Abuse and Social Support System in Rural Area (농촌지역의 노인학대 위험요인과 사회지원체계)

  • Research method was a case study as narrative method through in-depth interviews around 21 sufferers of elder abuse in rural area and the researcher interviewed them directly. Conclusively, the majority of sufferers of elder abuse were physically very low in the ADL(activities of daily living) and had been bereaved with no spouse. The relations of abused parents and the adult children were much disagreed and divided as unrecovered. Their economic reliance and physical reliance were much overlapped and the majority of them were low educational background and low income level. In case of the abused old people, they had severe low self respect and melancholy, so they considered severely suicide so much as invaluable as their existing value. In spite of over 80.5 years old average, the abuse action had continued for 6 months to 50 years long and the intensity of the abuse was more strengthened. The majority of abused old people refused mostly the entrance of social welfare facilities for elders and also they desired to live in present living area and their own houses.

Immunohistochemical analysis of the effect of low power GaAlAs laser treatment on the expression of proliferating cell nuclear antigen (PCNA) in full-thickness excisional wound of rat skin (CaAlAs 저출력 레이저 자극이 흰쥐의 피부 전층결손 절제 창상의 치유시 proliferating cell nuclear antigen(PCNA)발현에 대한 면역조직화학법적 분석)

  • We evaluated the effect of low power GaAsAl laser on re-epithelization in full-thickness excisional wound of rat skin. Two full-thickness excisions were made on the back of the experimental animals. Low power laser applications with 10mW intensity were treated experimental animals twice a day for 7 days. On the seventh postoperative day the quantitative analysis of re-epithelization was performed using immunohistochemical staining for proliferating cell nuclear antigen (PCNA). The majority of PCNA immunoreactive cells was observed at epithelial cells in the margin of full thickness excisional wound. The low power laser treatments significantly increased the number of PCNA immunoreactive cell as compared to that of non treated animal group (p<0.01). The shape of PCNA immunoreactive cell appeared as small dark, round to ovoid structures. Most PCNA immunoreactive cells exhibited a high intensity of staining that contrasted sharply with the surrounding background. In conclusion, these findings suggest that GaAlAs laser treatments effectively enhance the epithelial wound healing by the stimulating cell proliferation. Furthermore, the majority of cell proliferation occurred in the margin of full thickness excisional wound.

Development of the Driving Pump for the Low Noise Large Cavitation Tunnel (저소음 대형 캐비테이션 터널 구동 펌프 개발)

  • It is reported to develop the driving pump for the Low Noise Large Cavitation Tunnel(LOCAT) which is under construction at Maritime & Ocean Engineering Research Institute(MOERI). For low background noise condition of the LOCAT, it is crucial not only the best pump efficiency but also no cavity occurrence at any operating conditions. Design condition of the pump is determined by considering the required pump headrise, flow quantity, shaft rotation velocity and pump diameter. Performance analysis of the pump is conducted using commercial CFD codes ($BladeGen^+$, CFX-10), and the predicted results are verified by a series of model tests. Cavity was not observed at any operating condition in the model test, which were conducted at the midium cavitation tunnel of MOERI. The optimum pump for LOCAT, named as LP-11, was successfully developed through a series of pump design processes composed of blade design, performance analysis and model test.

  • Objective: The Purpose of this study is the factive and descriptive research to identify the factors affecting the functional disability of the chronic low back pain patients. We investigated the effect of ODI by the Questionnaire and questioned an 70 patients who were back pain unit at G university hospital in D city from March to May, 2007. The functional disorder in this study were based on the Oswestry Disability Index(ODI) lower back pain disability questionnaire by fairbank(1980), and these data were proceeded by using SPSS/WIN version 10.0. Result: 1. The functional disorder in accordance with age, gender, academic background, occupation, diagnosis, pain cause, pain experience, treatment duration had no significantly similar difference statistically(p>0.05). 2. The functional disorder in accordance with duration of painful generation had statistically significance(P<0.05). Conclusion: we could verify that duration of painful generation affect on the factors affecting the functional disability of the low back pain patients.

Acceptance versus catastrophizing in predicting quality of life in patients with chronic low back pain

  • Background: The aim of this study was to investigate the relationship between pain catastrophizing, acceptance, and quality of life in relation to chronic low back pain in Jakarta, the capital city of Indonesia. We also analyze the effect of personality in catastrophizing and acceptance. Methods: A total of 52 chronic low back pain patients were enrolled as participants from 2 hospitals in Jakarta (43 females, 9 males, mean age 54.38 years). Participants completed a set of self-reported questionnaires: the NEO Five Factor Inventory (NEO-FFI), Chronic Pain Acceptance Questionnaire-Revised (CPAQ-R), Pain Catastrophizing Scale (PCS), and Pain Discomfort Module (PDM). Results: This study showed that acceptance increased the patient's quality of life by giving physical relief from pain. In contrast, pain catastrophizing decreased the quality of life, and increased the patients' tendency to get frustrated, irritated, and anxious about the pain. From a personality perspective, the trait neuroticism may lead to a higher level of pain catastrophizing. Conclusions: This study showed that catastrophizing, compared with acceptance, had a greater impact on the patient's life by reducing its quality.

Safety of low-dose anticoagulation in extracorporeal membrane oxygenation using the Permanent Life Support System: a retrospective observational study

  • Background: Bleeding and thrombosis are major complications associated with high mortality in extracorporeal membrane oxygenation (ECMO) management. Anticoagulant therapy should be adequate to reduce thrombosis. However, related studies are limited. Methods: We retrospectively reviewed all patients supported with ECMO at a single institution between January 2014 and July 2022 and included those on all types of ECMO using the Permanent Life Support System. Patients were classified into two groups according to their measured mean activated partial thromboplastin time (aPTT) during ECMO management: a high-anticoagulation (AC) group (aPTT, ≥55 seconds; n=52) and a low-AC group (aPTT, <55 seconds; n=79). The primary outcome was thrombotic or bleeding events during ECMO. Results: We identified 10 patients with bleeding; significantly more of these patients were in the high-AC group (n=8) than in the low-AC group (15.4% vs. 2.5%, p=0.01). However, thrombus events and oxygenator change-free times were not significantly different between the two groups. Four patients in the high-AC group died of bleeding complications (brain hemorrhage, two; hemopericardium, one; and gastrointestinal bleeding, one). One patient in the low-AC group developed a thrombus and died of ECMO dysfunction due to circuit thrombosis. Conclusion: Heparin did not significantly improve thrombotic outcomes. However, maintaining an aPTT of ≥55 seconds was a significant risk factor for bleeding events, especially those associated with mortality.
