AMP-activated protein kinase (AMPK) is an important cellular fuel sensor. Its activation requires phosphorylation at Thr-172, which resides in the activation loop of the ${\alpha}1$ and ${\alpha}2$ subunits. Several AMPK upstream kinases are capable of phosphorylating AMPK at Thr-172, including LKB1 and CaMKK${\beta}$ ($Ca^{2+}$/calmodulin-dependent protein kinase kinase${\beta}$). AMPK has been implicated in the regulation of physiological signals, such as in the inhibition of cholesterol fatty acid, and protein synthesis, and enhancement of glucose uptake and blood flow. AMPK activation also exhibits several salutary effects on the vascular function and improves vascular abnormalities. AMPK is modulated by numerous hormones and cytokines that regulate the energy balance in the whole body. These hormone and cytokines include leptin, adiponectin, ghrelin, and even thyroid hormones. Moreover, AMPK is activated by several drugs and xenobiotics. Some of these are in being clinically used to treat type 2 diabetes (e.g., metformin and thiazolidinediones), hypertension (e.g., nifedipine and losartan), and impaired blood flow (e.g., aspirin, statins, and cilostazol). I reviewed the precise mechanisms of the AMPK activation pathway and AMPK-modulating drugs.
Renin-Angiotensin계는 정상 및 질병시의 혈압조절에 매우 중요한 역할을 담당하고 있음이 밝혀지면서, 이 조절계의 특정단계를 간섭함으로써 새로운 고혈압치료제를 개발하려는 연구가 일찍부터 시도되었다. (그림 1). 그 중에서 내인성 생리활성물질인 Angiotensin II의 합성을 차단하는 ACE 저해제는 임상적으로 고혈압 및 심부전치료제로서 유용성이 인정되어 현재 널리 사용되고 있다. ACE 저해제는 종종 마른기침, 발적과 같은 부작용이 나타나므로 이러한 부작용을 극복하려는 연구가 많이 있었으나 이는 작용기전에서 기인되는 것으로 해결에 한계를 보여왔다. 그런데 1982년 일본의 Takeda사의 연구진은 S-8307, 8308이라는 효과가 매우 약하기는 하지만 Angiotensin II 수용체를 선택적으로 차단하는 비펩타이드성의 AII길항물질을 특허 출원하였다. 미국의 Du Pont사는 AII길항약물이 효능은 그대로 유지하면서 ACE 저해제들의 부작용을 해결할 수 있을 것으로 예상하고 Takeda 화합물을 모핵으로하여, 많은 유도체들을 합성하면서 구조-활성 연구를 수행한 결과 비펩타이드성길항제인 Dup 753(Losartan, Cozaar$^{R}$) (2-N-butyl-4-chloro-5-hydroxymethyl-1-(2'-(1H-tetrazole-5-yl)biphenyl-4-yl) imidazole, potassium salt)을 발견하게 되었다. 이 Dup 753은 특별히 AII수용체중 혈압조절과 관련이 있는 AT1 수용체를 선택적으로 차단하는데, 효력은 ACE 저해제인 captopril과 유사하며, 경구흡수가 잘되고 지속시간이 길어 하루에 한번 먹는 경구제제로 개발되고 있는 것으로 알려져 있다. 이 Dup 753의 지속시간이 긴 것은 그 대사물인 Exp 3174에 기인하는 것으로 알려져 있으며, 대사체가 Dup 753에 비해 효력도 훨씬 더 높고 지속시간도 길어서, Dup 753은 일종의 prodrug적 개념이 들어있는 약물이라 할 수 있다.
To investigate the mutual relationship between angiotensin II (Ang II) and nitric oxide (NO) in paraventricular nucleus (PVN), Ang II receptor type Ia $(AT_{1A}),$ type Ib $(AT_{1B}),$ endothelial constitutive nitric oxide synthase (ecNOS), and neuronal constitutive nitric oxide synthase (ncNOS) mRNA levels of rat PVN were measured after unilateral carotid artery ligation. $AT_{1A}$ and $AT_{1B}$ mRNA levels were markedly elevated 6 hrs after unilateral carotid artery ligation. Losartan injection $(10\;{\mu}g/0.3\;{\mu}l)$ into the PVN augmented of the increment of $AT_{1A}$ and $AT_{1B}$ mRNAs It also increased ecNOS gene expression. In addition, $AT_{1B}$ mRNA levels increased after N-nitro-L-arginine methyl ester (L-NAME) injection $(50\;{\mu}g/0.3\;{\mu}l)$ into the PVN. These results suggest that Ang II and NO in the rat PVN may interplay, at least in part, through regulation of gene expression of ecNOS and $AT_{1B},$ respectively.
Angiotensin II (Ang II), a key mediator of hypertensive, causes structural changes in the arteries (vascular remodeling), which involve alterations in cell growth, vascular smooth muscle cell (VSMC) hypertrophy. Ang II promotes fibrotic factor like IGFBP5, which mediates the profibrotic effects of Ang II in the heart and kidneys, lung and so on. The purpose of this study was to identify the signaling pathway of IGFBP5 on cell proliferation and migration of Ang II-stimulated VSMC. We have been interested in Ang II-induced IGFBP5 and were curious to determine whether a Pitavastatin would ameliorate the effects. Herein, we investigated the question of whether Ang II induced the levels of IGFBP5 protein followed by proliferation and migration in VSMC. Pretreatment with the specific Angiotensin receptor type 1 (AT1) inhibitor (Losartan), Angiotensin receptor type 2 (AT2) inhibitor (PD123319), MAPK inhibitor (U0126), ERK1/2 inhibitor (PD98059), P38 inhibitor (SB600125) and PI3K inhibitor (LY294002) resulted in significantly inhibited IGFBP5 production, proliferation, and migration in Ang II-stimulated VSMC. In addition, IGFBP5 knockdown resulted in modulation of Ang II induced proliferation and migration via IGFBP5 induction. In addition, Pitavastatin modulated Ang II induced proliferation and migration in VSMC. Taken together, our results indicated that Ang II induces IGFBP5 through AT1, ERK1/2, P38, and PI3K signaling pathways, which were inhibited by Pitavastatin. These findings may suggest that Pitavastatin has an effect on vascular disease including hypertension.
막증식성 사구체 신염은 메산지움의 증식과 사구체 모세혈관의 비후를 동반하는 진행성 신질환으로 병리학적 소견에 따라 1, 2, 3형으로 분류할 수 있다. 이 질환은 일차성 사구체신염의 약 2%가량을 차지하는데, 대개의 경우 만성적으로 진행하는 경과를 보이며 다른 사구체신염에 비하여 신기능 소실로 진행하는 경우가 많지만 효과적인 치료법이 정립되어있지 않고 다양한 치료가 시도되고 있다. 학교 집단신체검사에서 소변검사가 포함되기 시작하면서 제1형 막증식성 사구체신염은 수차례 보고되었고 이들에 대한 조기 치료가 신질환의 진행을 늦추는데 효과적인 것으로 알려져 있는데 반해, 제2형 막증식성 사구체신염이 학교 집단신체검사에서 발견된 예는 현재까지 국내에서 보고된 바 없기에 문헌고찰과 함께 보고하는 바이다.

Objectives: Angiotensin II (AngII), a major product of renin-angiotensin system (RAS) has important role in induction of hypertension and antihypertensive effect of several medicinal plant was mediated by effect on this agent. Therefore, this study examined the possible effect of hydroalcoholic extract of Crocus sativus (C. sativus) on hypertension induced by AngII. Methods: Six groups (n = 6) of rats were used as follow: 1) Control, 2) AngII (300 ng/kg), 3) Losartan (Los, 10 mg/kg) + AngII and 4-6) C. sativus extract (10, 20 & 40 mg/kg,) + AngII. The femoral artery and vein were cannulated for recording cardiovascular parameters and drugs administration, respectively. All drugs were injected intravenously (i.v). Los and all doses of C. sativus injected 10 min before AngII. Systolic blood pressure (SBP), mean arterial blood pressure (MAP) and heart rate (HR) were recorded throughout the experiment and those peak changes (${\Delta}$) were calculated and compared to control and AngII. Results: AngII significantly increased ${\Delta}MAP$, ${\Delta}SBP$ and ${\Delta}HR$ than control (P < 0. 01 to P < 0.001) and these increments were significantly attenuated by Los. All doses of C. sativus significantly reduced peak ${\Delta}MAP$, ${\Delta}SBP$, and ${\Delta}HR$ than AngII group (P < 0. 05 to P < 0.001). In addition, peak ${\Delta}MAP$, ${\Delta}SBP$ in doses 10 and 20 were significant than Los + AngII group (P<0.05 to P< 0.01) but in dose 40 only MAP was significant (P<0.05). Peak ${\Delta}HR$ in all doses of C sativus was not significant than Los+ AngII. Conclusion: Regarding the improving effect of the C. sativus extract on AngII induced hypertension, it seems that this ameliorating effect partly mediated through inhibition of RAS.
Objectives: Nephrotic syndrome is a kidney disorder, which is characterized by proteinuria, edema (swelling), and hyperlipidemia. Maydis Stigma (Corn silk) has been widely used in Asia as a traditional medicine and is known to have a diuretic effect and is used for the treatment of edema and indigestion. Methods: The aim of this study is to investigate the improvement effect of Maydis Stigma in treating nephrotic syndrome induced by puromycin aminonucleoside. Sprague-Dawley rats were intravenously injected with 75 mg/kg/day puromycin aminonucleoside, then treated with either Losartan or 200 mg/kg/day Maydis Stigma for seven days. Results: Maydis Stigma significantly decreased ascites and proteinuria level. Plasma levels of blood urea nitrogen (BUN) and plasma creatinine reduced significantly by Maydis Stigma. In addition, treatment with Maydis Stigma attenuated histological damage. Treatment with Maydis Stigma also restored podocin expression and reduced inflammation markers such as intracellular adhesion molecules (ICAM-1), monocyte chemotactic protein-1 (MCP-1), tumor necrosis factor alpha (TNF-α) and high-mobility group box-1 (HMGB1). Conclusions: Maydis Stigma ameliorates kidney injury in nephrotic syndrome rat models. Maydis Stigma exerts a renoprotective effect owing to its anti-inflammatory effects and reductions of ascites and proteinuria. Thus, these results indicate that Maydis Stigma is likely to be a promising agent in the treatment of nephrotic syndrome.
연구배경 : Angiotensin II가 폐포상피세포의 세포사멸을 유도하고 폐섬유모세포에서 TGF-${\beta}$등의 발현을 증가시켜 폐섬유화증을 촉진시킬 수 있다고 알려져 있어 angiotensin II receptor의 차단이 폐섬유화증을 감소시키는 효과가 있을 것으로 예상하고 특발성 폐섬유화증 환자에게 angiotensin II receptor antagonist(AGIIRA)를 투여하여 치료효과를 알아보고자 하였다. 방 법 : 저자들은 가천의대 길병원에서 특발성 폐섬유화증으로 진단된 13명의 환자를 대상으로 하였다. 이들 중 8명 의 환자에게는 angiotensin II type 1 receptor antaginist인 losartan(cozaar$^{(R)}$) 을 투여하였고 나머지 5명의 환자에게는 losartan을 투여 하지 않았으며, 치료 직전과 치료 l년 후에 모든 환자에게 폐기능 검사와 호흡곤란 지수의 정도의 변화를 측정하여 그 결과를 비교하였다. 결 과 : AGIIRA 복용 군에서는 폐기능이 전체적으로 약간의 호전을 보였으며, 미 복용 군에서는 DLco%가 5%로 증가하였으나 TLC가 14%로 감소하는 등 전반적으로 페기능이 감소하는 소견을 보였다. 폐기능 변화가 두 군 모두 통계적으로 유의성이 없었고 두 군간의 변화율의 변화에서도 통계적 유의성은 없었다. 호흡곤란 지수는 AGIIRA 복용한 군에서만 통계적으로 유의하게 더 호전을 보였다. 결 론 : 특발성 폐섬유화증의 치료에 AGIIRA가 일부 환자들에게 임상적 안정화에 도움을 줄 수 있을 것으로 사료된다. 차후 더 많은 환자를 대상으로 오랜 기간의 연구와 함께 폐섬유화에 있어서 anglotensin II와 그 receptor에 대한 기전과 역할에 대하여 더 많은 연구가 필요할 것으로 생각된다.
The only compounds with antagonistic activity via AT$_1$receptor, one of two subtypes of angiotensin II (AII) receptor, have been demonstrated to block the vasoconstriction effects of AII and thereby provide therapeutic potential. This initiated the search for compounds with high specific affinity to AT$_1$receptor and their effective screening methods. The radioligand binding assay for the AII receptor is regarded as the primary method for the evaluation of AT$_1$receptor antagonists for their activity. In this paper, we characterized the liver AT$_1$receptor and describe the efficient method of the radioligand binding assay using rat liver as a source of AT$_1$receptor. Equilibrium binding studies with rat adrenal cortex, adrenal medulla, liver and bovine adrenal showed that the specific bindings of [$^3$H] AII were saturable in all tissues and the Scatchard plots of those data were linear, suggesting a single population of binding sites. Hill slopes were very near to the unity in all tissues. Kinetic studies of [$^3$H) AII binding in rat liver homogenates yielded two association rate constants, 4.10$\times$10$^{7}$ M$^{-1}$ min$^{-1}$ and 4.02$\times$10$^{9}$ M$^{-1}$ min$^{-1}$ , with a single dissociation rate constant, 7.07$\times$10$^{-3}$ min-$^{-1}$ , possibly due to the partial dissociation phenomenon. The rank order of inhibition potencies of [$^3$H] AII binding in rat liver was AII>Sarile>Losartan>PD 123177. Rat liver homogenates revealed to have very high density of homogeneous population of the AT$_1$receptor subtype, as the specifically bound [$^3$H] AII was not inhibited by PD 123177, the nonpeptide antagonist of AT$_2$. The results of this study demonstrated that the liver homogenates from rats could be the best receptor preparation for the AT$_1$receptor binding assay and provide an efficient system for the screening of newly synthesized candidate compounds of AT$_1$receptor antagonist.
Method using liquid chromatography coupled with tandem mass spectrometry (LC-MS/MS) was developed and validated for the determination of pregabalin in plasma samples. Acquisition was performed by monitoring the transitions: m/z 160.1${\rightarrow}$142.2 for pregabalin and m/z 423.2${\rightarrow}$207.1 for losartan (as an internal standard). After cold acetonitrileinduced protein precipitation of the plasma samples, separation was performed with C18 column by isocratic mobile phase consisted of 10 mM ammonium acetate and acetonitrile (15:85, v/v). Results were linear over the concentration ranged from 0.1 to $10{\mu}g$/mL and the correlation coefficients (r) were $\geq0.99$. Intra- and inter-day precisions were $\leq6.02$ and $\leq11.04%$, respectively, and intra- and inter-day accuracies were 96.60-101.09 and 98.10-102.60%, respectively. This validated method was successfully applied to a bioequivalence study of two formulations of pregabalin, Daewoong pregabalin capsule (Daewoong Pharm. Co., Ltd.) and Lyrica$^{(R)}$ capsule (Pfizer Korea Ltd.) in twenty eight healthy Korean volunteers. The subjects received a single oral dose of each formulation (150 mg as pregabalin) in a randomized $2{\times}2$ crossover study and plasma samples were obtained from each subject at predetermined time intervals. Then, the pharmacokinetic parameters ($AUC_{0-t}$, $C_{max}$ and $T_{max}$) were calculated and statistically analyzed to assess the differences between two formulations. The 90% confidence intervals for the log-transformed data were acceptable range of log 0.8-log 1.25 (e.g., log 1.0048-log 1.0692 for AUC0-t, log 0.9142-log 1.0421 for $C_{max}$). Thus, $AUC_{0-t}$ and $C_{max}$ met the criteria of the Korea Food and Drug Administration (KFDA) for bioequivalence test indicating that Daewoong pregabalin capsule was bioequivalent to Lyrica$^{(R)}$ capsule.
