Zygomaticomaxillary complex fracture after two-jaw surgery

  • Orthognathic surgeries often utilize rigid fixation for stabilization of the osteotomy site. The longterm fate of rigid fixations is still under investigation, and whether they should be routinely removed is under debate despite their low complication rates. Here, we report a case where a 26-year-old man suffered high-velocity trauma to his face 7 years after a two-jaw surgery. Computed tomography examination revealed a zygomaticomaxillary complex fracture, and open reduction and internal fixation was performed along with anterior maxillary wall reconstruction using absorbable mesh. Intraoperative examination revealed a broken L-shaped titanium plate near the fracture site with multiple bony fragments near each titanium screw. The rigid titanium system may have caused comminution of the fracture pattern, worsening the severity of the fracture.

  • Vascular stenting has a great attention as a treatment for coronary arteries diseases as compared with percutaneous balloon angioplasty. In-stent restenosis and thrombosis are side effects resulting from using bare metal stent (BMS). Employing platelet therapy allowed to reduce the rate of thrombosis, however, the rate of restenosis remains a major problem. In 2002, drug-eluting stents (DESs) were introduced as an effort to reduce the restenosis. The commercially available DESs continue to suffer from coating defects that might lead to a series of adverse effects. Most importantly, multiple concerns remain regarding the polymer coating integrity on metal surfaces or the relation of polymer irregularities to longterm adverse events.

  • Weight change in the postpartum period is a dominant concern of new mothers recently. Postpartum weight retention has important public health implications as well, because retention of gestational weight can be a significant contributor to longterm obesity and associated health risks. As traditional medical practitioners have managed some problems from postpartum weight retention, use some terms translated in Korean like as postpartumedema, obesity. But those have been not given any discuss objective and with evidences for nomenclature. Now reports review normal postpartum weight loss patterns, as well as risk factors for weight retention, ancient literatures, so practitioners can assist their patients in achieving a healthy postpartum weight.

Peritoneopericardial diaphragmatic hernia in a immature dog (미성숙견에서 선천성 Peritoneopericardial diaphragmatic hernia 발생례)

  • A 5-month-old, 5kg, intact male mixed dog was admitted to the Veterinary Medical Teaching Hospital of the College of Veterinary Medicine Cornell University because of severe systolic murmur. On physical examination, the dog appeared to be thin and slightly small for his age. Radiographic studies with barium sulfate clearly showed the presence of loops of intestines in the pericardium. Based on these findings peritoneopericardial diaphragmatic hernia was diagnosed. Many abnormalities were found in dog's heart : ventricular septal defect, pulmonic stenosis with moderate pulmonic valve insufficiency, and mild mitral and tricuspid valve insufficiency. Puppy's longterm prognosis was fair. His intestines were removed from his pericardium and the defect in his diaphragm was closed. The surgery went smoothly and puppy recovered very nicely from anesthesia.

  • 본 연구의 목적은 SWAT 모형에서 적용되는 하도유출추적 방법인 변동저류추적법과 Muskingum 방법에 대한 문제점 및 한계점을 제시하고 이를 개선하고자 하는데 그 목적이 있다. 이를 위해 충주댐 상류유역에 SWAT 모형을 적용하였으며 기존 두 가지 방법에 대한 한계점을 제시하였다. 또한 새로운 하도추적 방법으로 연속방정식과 Manning 식으로부터 비선형 저류방정식을 구성하여 적용하였으며 주하도 유입량에 대한 세가지 방법을 비교한 결과 기존 방법보다 본 연구에서 제시한 방법이 일 단위 장기유출모의에서 합리적인 유출량 결과를 제시하는 것으로 나타났다. 이러한 결과는 SWAT 모형의 적용성을 향상시킬 뿐만 아니라 유출 및 수질모의에도 상당한 영향일 미칠 것으로 판단된다.

Recent Advances in Nanozyme Research for Disease Diagnostics (질병진단을 위한 나노자임 연구의 최근 동향)

  • Nanomaterial-based artificial enzymes (Nanozymes) have attracted recent attention because of their unique advantageous characteristics such as excellent robustness and stability, low-cost production by facile scale-up, and longterm preservation capability that are critically required as an alternative to natural enzymes. These nanozymes exhibit natural enzyme-like activity, and they have been applied to diverse kinds of detection methods for disease-associated biomolecules such as DNAs, proteins, cells, and small molecules including glucose. To highlight the progress in the field of disease diagnostics using nanozyme, this review discusses many nanozyme-based detection methods categorized by the types of target biomolecules. Finally, we address the current challenges and perspectives for the widespread utilization of nanozyme-based disease diagnostics.

  • The purpose of this study is to suggest optimum size of residents' bedrooms in nursing homes. Bedroom is an essential part of nursing homes because nursing home residents spend most of their times in bedrooms and occupy the largest part of facility area. Therefore it is necessary to set an adequate standard related to resident room area in nursing facilities, which have a great effect on planning longterm care facilities for the elderly. The body dimensions of elderly people and care givers have been collected from the previous study. Based on the body dimensions data, the behaviors of elderly as well as care givers have been analysed. The minimum areas-exclusive of toilets, closets, vestibules and so on - have been proposed from the results of the behavior analysis.

Development of a Cold Rolling Oil with Lubricity and Mill Cleanness Property (윤활성 및 압연기 청정성을 겸비한 냉간 압연유의 개발)

  • The purpose of this study is to develop a cold rolling oil with lubricity and mill cleanness property under the rolling conditions of high reduction ratio and high rolling speed. Six kinds of oil samples were blended. Evaluation of lubricity and anti-seizuro property of the samples were carried out with a laboratory scale rolling mill, where the contact conditions between work roll and strip are very close to actual cold rolling mill. Laboratory evaluation for dispersion, contamination, anti-oil stain property and residual carbon, etc. were carried out with several testers such as longterm circulation tester, Coulter counter and Conradson tester. A new high speed cold rolling oil with good lubricity and mill cleanness property was developed compared with the previously developed rolling oil.

Effect of $1-{\alpha}(OH)D_3$ on Steroid Induced Bone Loss in Frequently Relapsing Childhood Nephrotic Syndrome (빈회재발형 소아 신증후군에서 스테로이드에 의한 골다공증에 미치는 $1-{\alpha}(OH)D3$의 효과)

  • Steroid induced bone loss is a serious problem in frequently relapsing nephrotic syndrome especially in growing children. In order to evaluate whether $1-(OH)D_3$ (IL Sung Pharma.Co.) is effective in preventing steroid induced bone loss, we gave $0.5{\mu}g$ of $1-(OH)D_3$ for one year to forty patients with frequently relapsing nephrotic syndrome, receiving longterm prednisolone therapy (mean duration $50.12{\pm}29.40$ months). We checked the following markers before and after $1-(OH)D_3$ therapy. i.e. bone mineral density(BMD) using dual energy X-ray absorptiometry(DEXA) at the 2nd to 4th lumbar spine, serum calcium, phosphorus, parathyroid hormone(PTH), osteocalcin and urine pyridinoline(U-PYD). BMD($g/cm^2$) was increased even steroid therapy from $0.71{\pm}0.0\;to\;0.73{\pm}0.0$ (p<0.05). Lumbar spine BMD is a sensitive marker for evaluating steroid induced bone loss in children receiving longterm corticosteroid therapy and that $1-(OH)D_3$ appears to be effective in treating and preventing steroid induced bone loss.

Analysis of Radiologic Factors Affecting Longterm Surgical Results of Anterior Cervical Fusion Patients (경추 전방 유합 환자의 장기 수술 결과에 영향을 주는 방사선학적 요인 분석)

  • Object : Anterior cervical discectomy and fusion is accepted as appropriate surgical intervention for disease processes of the ventral cervical spine. In some cases, however, improvement of symptom following operation develop myelopathy or rhizopathy at longterm follow-up. We studied to clarify the correlation between clinical results and plain radiologic findings at long term follow-up. Methods : A total of 86 patients who underwent anterior cervical discectomy and fusion were grouped into deteriorated and good recovery group. These two groups were compared in lateral functional roentgenograms on the cervical kyphosis and disc height at fused level, sagittal plane rotation and dynamic sagittal canal diameter at adjacent level. Results : The presence of cervical kyphosis and disc height at fused level do not correlate with long term followup results. Sagittal plane rotation of more than 20 degrees were identified in 36% of the cases in deteriorated group, whereas the same findings were identified in 15% of the cases in good group. Sagittal diameter of less than 12mm were identified in 48% of the cases in deteriorated group, whereas the same finding were identified in 8% of the cases in good group. Preoperatively, sagittal plane rotation of more than 20 degrees at adjacent level were identified in 28% of the cases in deteriorated group, whereas the same findings were identified in 13% of the cases in good group. Preoperatively sagittal diameter of less than 12mm at adjacent level were identified in 40% of the cases in deteriorated group, whereas the same finding were identified in 7% of the cases in good group. Conclusion : Large sagittal plane rotation and small dynamic sagittal diameter at adjacent level were factors that might be associated with later clinical deterioration after cervical anterior fusion.
