  • Background: The purpose of this study was to identify the risk factors influencing cognitive function and life satisfaction in elderly living alone. Methods: It was cross-sectional descriptive study by using secondary data from the 2014 national survey of Korean elderly. The study sample consisted of 2,249 subjects who live alone, aged 65 years old and over. Data were analyzed frequency analysis, ${\chi}^2$-test, t-test, ANOVA and Logistic regression analysis. Results: The risk factors that significantly influenced cognitive function were old age, bereavement, lack of education, no regular exercise and low socio-cultural activity satisfaction. The risk factors that significantly influenced life satisfaction were no bereavement, lack of education, no work, negative evaluation of health status, diagnosis of depression and no regular exercise. Also experience of emotional and economical abuse, neglect of financial support and low cognitive function were identified risk factors of life satisfaction. Conclusions: The elderly living alone with risk factors of cognitive decline should be interested. To prevent or delay cognitive impairment, regular exercise and socio-cultural activities were recommended. It was suggested that the promotion of cognitive function, appropriate work and exercise, and social efforts to prevent abuse and neglect can contribute to improving life satisfaction of elderly living alone.

  • Objectives: This study aimed to confirm the association between cognitive function and oral health status in the elderly. Methods: In this study, 5,794 respondents, aged 55 or older, who had participated in the 7th aging research panel survey were used. Statistical analyses were performed using a complex samples cross-tabulation analysis, complex samples general linear model, and a complex sample multinomial logistic regression analysis. Results: After analyzing the cognitive function of the elderly according to their oral health conditions, it was found that suspicions of dementia (19.6%) and cognitive function decline (25.9%) were more common for those who wore dentures than for those who did not wear dentures (p<0.001). Compared to those who did not wear dentures, those who wore dentures were linked to a 1.665 times higher suspicion of dementia (p<0.001). In comparison, cognitive decline was 0.964 times lower when the number of natural teeth increased by 1, and it was 0.941 times lower when the oral health evaluation index for the elderly increased by 1 point (p<0.001). Conclusions: As a result of this study, it was confirmed that cognitive function and oral health status are related in the elderly. Therefore, systematic measures for oral health management and oral health promotion should be prepared in tandem with awareness of possible cognitive decline in the elderly.

  • Generally, Global Climate Models (GCM) cannot be used directly due to their inherent error arising from over or under-estimation of climate variables compared to the observed data. Several bias correction methods have been devised to solve this problem. Most of the traditional bias correction methods are one dimensional as they bias correct the climate variables separately. One such method is the Quantile Mapping method which builds a transfer function based on the statistical differences between the GCM and observed variables. Laux et al. introduced a copula-based method that bias corrects simulated climate data by employing not one but two different climate variables simultaneously and essentially extends the traditional one dimensional method into two dimensions. but it has some limitations. This study uses objective functions to address specifically, the limitations of Laux's methods on the Quantile Mapping method. The objective functions used were the observed rank correlation function, the observed moment function and the observed likelihood function. To illustrate the performance of this method, it is applied to ten GCMs for 20 stations in South Korea. The marginal distributions used were the Weibull, Gamma, Lognormal, Logistic and the Gumbel distributions. The tested copula family include most Archimedean copula families. Five performance metrics are used to evaluate the efficiency of this method, the Mean Square Error, Root Mean Square Error, Kolmogorov-Smirnov test, Percent Bias, Nash-Sutcliffe Efficiency and the Kullback Leibler Divergence. The results showed a significant improvement of Laux's method especially when maximizing the observed rank correlation function and when maximizing a combination of the observed rank correlation and observed moments functions for all GCMs in the validation period.

  • Objectives According to previous studies, the Chromogranin B (CHGB) gene could be an important candidate gene for schizophrenia which is located on chromosome 20p12.3. Some studies have linked the polymorphism in CHGB gene with the risk of schizophrenia. Meanwhile, smooth pursuit eye movement (SPEM) abnormality has been regarded as one of the most consistent endophenotype of schizophrenia. In this study, we investigated the association between the polymorphisms in CHGB gene and SPEM abnormality in Korean patients with schizophrenia. Methods We measured SPEM function in 24 Korean patients with schizophrenia (16 male, 8 female) and they were divided according to SPEM function into two groups, good and poor SPEM function groups. We also investigated genotypes of polymorphisms in CHGB gene in each group. A logistic regression analysis was performed to find the association between SPEM abnormality and the number of polymorphism. Results The natural logarithm value of signal/noise ratio (Ln S/N ratio) of good SPEM function group was $4.19{\pm}0.19$ and that of poor SPEM function group was $3.17{\pm}0.65$. In total, 15 single nucleotide polymorphisms of CHGB were identified and the genotypes were divided into C/C, C/R, and R/R. Statistical analysis revealed that two genetic variants (rs16991480, rs76791154) were associated with SPEM abnormality in schizophrenia (p = 0.004). Conclusions Despite the limitations including a small number of samples and lack of functional study, our results suggest that genetic variants of CHGB may be associated with SPEM abnormality and provide useful preliminary information for further study.nwhile, smooth pursuit eye movement (SPEM) abnormality has been regarded as one of the most consistent endophenotype of schizophrenia. • This study suggested a novel approach of estimating the optimal probability density function (OPDF) of the annual maximum rainfall time series (AMRT) combining the L-moment ratio diagram and the geographical information system. This study also reported several interesting geographical characteristics of the AMRT in Korea. To achieve this purpose, this study determined the OPDF of the AMRT with the duration of 1-, 3-, 6-, 12-, and 24-hours using the method of L-moment ratio diagram for each of the 67 rain gages in Korea. Then, a map with the Thiessen polygons of the 67 rain gages colored differently according the different type of the OPDF, was produced to analyze the spatial trend of the OPDF. In addition, this study produced the color maps which show the fitness of a given probability density function to represent the AMRT. The study found that (1) both L-skewness and L-kurtosis of the AMRT have clear geographical trends, which means that the extreme rainfall events are highly influenced by geography; (2) the impact of the altitude on these two rainfall statistics is greater for the mountaneous region than for the non-mountaneous region. In the mountaneous region, the areas with higher altitude are more likely to experience the less-frequent and strong rainfall events than the areas with lower altitude; (3) The most representative OPDFs of Korea except for the Southern edge are Generalized Extreme Value distribution and the Generalized Logistic distribution. The AMRT of southern edge of Korea was best represented by the Generalized Pareto distribution.

  • An investigation was undertaken of the optimal discriminant model for predicting the likelihood of insolvency in advance for medium-sized firms based on the technology evaluation. The explanatory variables included in the discriminant model were selected by both factor analysis and discriminant analysis using stepwise selection method. Five explanatory variables were selected in factor analysis in terms of explanatory ratio and communality. Six explanatory variables were selected in stepwise discriminant analysis. The effectiveness of linear discriminant model and logistic discriminant model were assessed by the criteria of the critical probability and correct classification rate. Result showed that both model had similar correct classification rate and the linear discriminant model was preferred to the logistic discriminant model in terms of criteria of the critical probability In case of the linear discriminant model with critical probability of 0.5, the total-group correct classification rate was 70.4% and correct classification rates of insolvent and solvent groups were 73.4% and 69.5% respectively. Correct classification rate is an estimate of the probability that the estimated discriminant function will correctly classify the present sample. However, the actual correct classification rate is an estimate of the probability that the estimated discriminant function will correctly classify a future observation. Unfortunately, the correct classification rate underestimates the actual correct classification rate because the data set used to estimate the discriminant function is also used to evaluate them. The cross-validation method were used to estimate the bias of the correct classification rate. According to the results the estimated bias were 2.9% and the predicted actual correct classification rate was 67.5%. And a threshold value is set to establish an in-doubt category. Results of linear discriminant model can be applied for the technology financing banks to evaluate the possibility of insolvency and give the ranking of the firms applied.

  • Objectives: The aim of this study was to evaluate the oral health status and behavior in Korean diabetic adults. Methods: The study subjects were 11,840 adults who participated in the fifth Korea National Health and Nutrition Examination Survey(2010-2012). Diabetic status was defined by doctors and fasting blood sugar(FBS) level. Oral health status was assessed by decayed-missing-filled teeth(DMFT), community periodontal index(CPI), periodontal disease, denture needs, limitation of oral function, and chewing difficulty. The oral health behavior was evaluated by oral examination within a year, brushing times a day, and use of auxiliary oral product. The data were analyzed by descriptive analysis, chi-square tests and multiple logistic regression analyses. Results: The prevalence rate of diabetes mellitus diagnosed by doctor and FBS was 8.3% and 10.2%, respectively. The prevalence rate of periodontitis was 25.4%. The proportions of $DMFT{\geq}20$, $CPI{\geq}3$, periodontal disease, denture needs, oral function limitation and chewing difficulty in the confirmed diabetic group by doctor were significantly higher than those of the non-diabetic group(p<0.05). In multiple logistic regression analysis, the adjusted odds ratio(aOR) for periodontal disease(aOR=1.73, 95% CI=1.41-2.12), presence of denture needs(aOR=1.40, 95% CI=1.06-1.84), limited oral function(aOR=1.43, 95% CI=1.15-1.78) and chewing difficulty(aOR=1.41 95% CI=1.13-1.77) in diabetic subjects were significantly higher than those of the non-diabetic subjects. There were similar associations between diabetes defined with FBS and oral health. In oral health behavior, diabetic subjects had significantly lower odds ratios for oral examination(aOR=0.76, 95% CI=0.60-0.98), brushing time ${\geq}2$ times(aOR=0.73, 95% CI=0.57-0.93), and auxiliary oral products(aOR=0.74, 95% CI=0.59-0.94). Conclusions: There was a significant relationship between oral health status and behavior in Korean diabetic adults. Further study is needed to evaluate the underlying mechanisms between diabetes mellitus and oral health status.

  • Purpose: This purpose of study was to investigate the relationships among functional state, self-efficacy, and life satisfaction in the elderly with decreased visual acuity. Methods: The subjects were 162 elderly people from the G university hospital. Functional state was measured by Late-Life Function and Disability Instrument (LLFDI) and Minimum Data Set-Home Care version 2.0 (MDS HC 2.0). Self-efficacy and Life satisfaction were measured by the tool of Rho & Lee (2011) and Yoon (2007). Data were analyzed using t-test, ANOVA, Pearson's Correlation Coefficient, and logistic regression. Results: The daily life function was significantly associated with self-efficacy and vision decrease. The regression model with these two variables explained 35.6% of the variance of daily life function. IADL was significantly associated with vision decrease, age, gender, and self-efficacy. The regression model with the three variables explained 52.9% of the variance of IADL. Life satisfaction is significantly associated with self-efficacy, daily life function, vision decrease and IADL. The last regression model with the four variables explained 51.8% of the variance of life satisfaction. Conclusion: The levels of functional state, self-efficacy and life satisfaction in the elderly with decreased visual acuity were low. Self-efficacy was an important factor that influences on the functional state and life satisfaction. Therefore, nursing interventions that can enhance the self-efficacy are required in order to increase the functional state and life satisfaction in the elderly with decreased visual acuity.

  • Objectives This study aimed to examine differences in the hepatic function disorders and prevalence rates of liver diseases in accordance with the Sasang constitutional type, and to analyze whether Sasang constitution is a significant risk factor for fatty liver. Methods A total of 1211 patients who underwent health check-up at the Ilsan Hospital of Dongguk University and had their physical constitutions diagnosed by a specialist in Sasang constitutional medicine from October 31, 2011 to September 28, 2012 were used as the subjects of this study. Presence of hepatobiliary diseases was analyzed from the results of the hepatic function test, lipid test and viral hepatitis infection marker test in the biochemistry tests, and abdominal ultrasonography. Results & Conclusion Subjects of the study were composed of 691 males and 520 females and, in terms of Sasang constitution distribution, 550 Taeeumin, 343 Soeumin and 318 Soyangin. As the results of analysis of prevalence rates in accordance with Sasang constitutions, significant differences were observed in the prevalence rates of abnormal groups in the cases of AST, ALT, GGT, Triglyceride, HDL-cholesterol and LDL-cholesterol, and all of their prevalence rates were in the descending order of Taeeumin, Soyangin and Soeumin. Soeumin displayed significantly higher positive rate for HBs Ab in comparison to Taeeumin and Soyangin. In the results of abdominal ultrasonography, the prevalence rates of fatty liver displayed significant differences and were in the descending order of Taeeumin, Soyangin and Soeumin. And as the result of logistic regression analysis in order to find the risk factors of fatty liver, the Sasang constitution was found to be a significant risk factor for fatty liver. The odds ratio of Taeeumin in fatty liver was found to be 1.634 higher than Soeumin and 1.773 higher than Soyangin

  • Purpose: The authors performed this study to investigate the risk factors for predicting stent failure and to evaluate its impact on prognosis. Materials and Methods: Between January 2002 and March 2017, we retrospectively reviewed 117 consecutive patients who underwent retrograde ureteral stenting and exchanging at least once every 3 months for malignant ureteral obstruction. The patients were classified according to their pre-stenting chronic kidney disease (CKD) stage. The factors affecting stent failure were analyzed using a logistic regression model. Overall survival (OS) was estimated, and the prognostic significance of each variable was estimated using Cox proportional-hazards regression modeling. Results: Before stenting, 91 patients were CKD stages 1-3 and 26 patients were CKD stages 4-5. These two groups differed significantly only in pre-stenting estimated glomerular filtration rate (eGFR), bilateral obstruction, and pre-stenting pyuria. Among the 117 patients, stent failure occurred in 30 patients (25.6%), and there were no differences between the groups. Pre-stenting pyuria and post-stenting complications were significant predictors of stent failure. There were 79 deaths in total, including 56 in the CKD stages 1-3 group and 23 in the CKD stages 4-5 group. In the multivariate analysis predicting patient OS, pre-stenting eGFR and post-stenting disease progression were significant factors. Conclusions: Internal ureteral stenting was effective for maintaining renal function in malignant ureteral obstruction. However, it did not restore renal function, which is related to the prognosis of the patients. Therefore, to improve patients' renal function and prognosis, patients who require stenting must be quickly recognized and treated.
